- Safe sleep: 1. Ten-minute sleep checks were not recorded to observe the colour, position, breathing patterns of one child aged 16 months from the Ladybugs room who slept from 11.30am to 11.50am. This is not within safe sleep guidelines. Infection Control: 2. The steps in the services infection control policy were observed not to be followed as evidenced by the following: • The children attending the Sunshine room or the Ladybugs room did not wash their hands before they had their morning snack. • The practitioner or the children’s hands were not always washed following nappy changing. • Practitioners were observed wiping children’s noses without washing their hands afterwards. 3. The children attending the Bumblebee room were brought to the nappy changing area in pairs, while one child was being changed the other child was playing on the floor. This routine is not in keeping with good infection control practice. 4. In the Bumblebee room the required 50cm distance was not maintained between the children’s beds when the children were sleeping. Action submitted by the Registered Provider Corrective Action Safe Sleep: 1. Staff were reminded of the importance recording the children sleeping, their positions, breathing and body colour. Infection Control: 2., 3., 4. All staff have been remined on the importance of hygiene and infection control. Preventive Action Safe Sleep: 1. Staff were told the importance of completing the record sheets. Also new sleep diagrams have been completed in order to ensure all sleep beds are at least 50cm apart at all times. Infection Control: 2., 3., 4. Visual aids have been put in place to ensure staff are using correct measures to prevent infection spreading and resent policies and procedures stating the importance of hand washing. Summary Comment The inspectors have reviewed the actions and evidence submitted. - General Safety: 1. Garda vetting was available for 1 adult. However, one vetting disclosure was not dated within the previous three years in adherence to the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. Action submitted by the Registered Provider Corrective Action General Safety: 1. The identification verification stage of the vetting process was completed on 25 August 2025, and due to a misunderstanding that arose from changes to the vetting process, the process was not completed in a timely manner. The registered provider has since clarified the process for staff. Preventive Action 1. As above, the preschool service has ceased operating permanently. If continuing in service, the registered provider would ensure that future vetting processes completed without delays. - General Safety: 1. On the inspectors arrival to the service the front door was unlocked which increased the risk of unauthorised entry to the service and to children leaving the service unsupervised. Infection Control: 2. In both care rooms the children’s snacks which they brought with them from home and contained meat and dairy produce were stored in their bags and not stored under refrigerated conditions. - General Safety: 1. There was no safety lock on two presses containing hazardous cleaning materials and as a result these products were potentially accessible to children. It is acknowledged that once the risk was brought to the attention of the person in charge the cleaning products were removed and a new safety lock was installed by 2pm on the day of inspection. Safe Sleep: 2. Safe sleep practices for children under the age of two sleeping in cots were not carried out in line with Tusla safe sleep guidance or service policy which posed a risk to children’s safety. Three children were observed to be sleeping with comforters present in cots for a prolonged period which is not in line with safe sleep guidance. Action submitted by the Registered Provider

- General Safety: 1. All hazardous cleaning materials were removed from the lower storage presses and relocated to a higher shelf inaccessible to children. In addition, a safety lock was installed on the storage unit. These corrective actions were completed by 2:00pm on the day of the inspection. Regular health and safety checks will continue to include verification that all storage units containing hazardous materials are appropriately secured. Safe Sleep: 2. The finding was brought to the attention of the Person in Charge during the inspection. The comforters were removed immediately from the cots of the children concerned and staff were reminded of the requirements of the service's Safe Sleep Policy and Tusla Safe Sleep Guidance. Safe sleep practices have been added as a specific item to room supervision checks and management audits. The Person in Charge will continue to monitor compliance through regular observations and supervision to ensure ongoing adherence to the service's Safe Sleep Policy and Tusla Safe Sleep Guidance

- General Safety: 1. A fire extinguisher located high on the wall in Room 2 appeared to be loose and at risk of falling. Infection Control: 2. - General Safety: 1. The registered provider confirmed that the loose bracket was removed and remounted in another area of the room. The fire safety equipment will be checked daily and included in monthly health and safety inspections. Infection Control: 2. The registered provider confirmed that the hot water switch had been turned off on the day of inspection; The staff have been shown where the switch is and have agreed that it will not be switched off. It is confirmed that temperatures will continue to be checked and recorded daily. 3. The registered provider confirmed that the storage box was immediately removed from the area. - General Safety: 1. Grapes within a child’s lunchbox were observed not to be cut in half, increasing the risk of choking to the child. It is acknowledged the staff member implemented measures to mitigate the risk when brought to their attention. 2. An emergency medication stored in the yellow room had passed its expiry and remained available for use, this posed a potential risk of delayed medical treatment in the event of an emergency. 3. The inspectors observed that the full height glazed doors and windows within the service did not have visibility strips or safety markings positioned at the children’s eye level. The absence of these markings reduced the visibility of the glass panels and increased the likelihood of children inadvertently colliding into the glass surface. Infection Control: 4. In the orange room a low-level cupboard storing cleaning chemicals was not secured by a lock and was accessible to children. 5. Mouthed toys were observed not removed from play once children had placed them in their mouths. These toys were left on the floor or table and observed to be accessible to other children present. This was not in accordance with the service’s infection control policy. Action submitted by the Registered Provider

- The registered provider confirmed that findings on inspection were brought to the attention of staff. Assurances were given by management that regular checks and audits will take place to prevent reoccurrence. Staff will further be periodically reminded of procedures through staff meetings and supervisions. General Safety: 1. All employees were reminded that grapes present in children's lunchboxes from home must be cut in half. Communication was sent to all current ECCE parents requesting that grapes be cut prior to including in lunchboxes. To prevent reoccurrence, grapes will be prohibited in the service beginning with the September intake of new ECCE children. 2. The emergency medication identified in the Yellow Room was removed from the service following confirmation from the Childs parents that it was no longer required. 3. Visibility safety markings have been applied to all glass panels. Infection Control: 4. The cleaning chemicals stored in the low-level cupboard in the Orange Room were removed and relocated to the high-level cupboard. Daily room safety checklists have been updated to reflect safe storage of chemicals. 5. Staff were reminded of the Infection Prevention and Control Policy in relation to mouthed toys

- General Safety: 1. The inspector noted a wooden slab located on the shed porch in the outdoor area was broken and uneven, creating a raised edge and unstable surface. The inspector observed children playing in the area, the damaged slab presented a potential trip and fall hazards to children. 2. Rubber grip handles from a children’s metal trike were not present, posing a risk of harm to children. 3. An outdoor heat pump was accessible to children due to the lock on the door and wooden fence surrounding it being left open. Infection Control: 4. Children in the Senior Discoverers room did not have their hands washed before eating snack. This is at variance with the service policy on infection control. Fire Safety: 5. At sleep time in the Senior Discoverers room, a floor bed with a sleeping child was observed blocking the designated fire exit in the room. Designated fire exits should be clear of any obstructions during the operation of an early years’ service to ensure the safe evacuation of staff and children in the event of an emergency. Safe Sleep: 6. The inspector observed that a child who had been placed in the cot room was not physically checked from 14.50 to 15.40pm, despite the sleep monitoring record indicating checks had occurred every 10 minutes throughout the sleep period. This constituted a failure to ensure adequate supervision arrangements and compromised the reliability of safeguarding records maintained in the service. 7. The sleep room located outside of the Discoverers room required ambient lighting to be able to monitor sleeping children safely and easily. Sleeping areas should not be fully darkened for day-time naps. 8. A sleep plan was not available for a child in Senior Discoveries who was observed to be under 2 years of age and sleeping on a sleep mat. 9. The inspector observed that a child requiring sleep was placed on a sleep mat rather than being provided with age-appropriate sleep equipment. The use of a sleep mat for this child was not in accordance with Tusla’s Guidance for the Early Learning and Care Sector on Sleep. Action submitted by the Registered Provider

- The registered provider confirmed that staff meetings were held; the following topics were discussed and reviewed: Risk management policy, Hand washing policy and procedures, Safe sleep guidance and policy, including sleep plans and the Sleep and Rest Policy. Items identified on inspection have been added to the daily safety checklist to prevent reoccurrence. General Safety: 1.The wooden slab on the shed has been replaced. A safety audit of the building was overseen by management to ensure the maintenance logs are up to date. Management will continue to oversee the maintenance log. 2. The bike handles were replaced. Additional rubber handles have been purchased to ensure prompt replacement to prevent reoccurrence. 3. A combination lock has been added to the outdoor heat pump unit to ensure it remains inaccessible to the children. Infection Control: 4. Handwashing signage reviewed to ensure correct handwashing and supervision of same takes place. Management will ensure and regularly review policy with staff. Fire Safety: 5. A floor plan for the cocoon beds and sleep mats has been completed. The floor plan is displayed on the wall of the room for reference. Safe Sleep: 6. To prevent reoccurrence, a weekly roster has been implemented for ten-minute sleep checks. Timers are present and signage has been put in place also. Support and supervision meetings were held to discuss safe sleep procedures. Management will oversee weekly audits of sleep checks. 7. Dimmable lighting has been installed in the cot room to allow soft lighting where children sleep. 8. A sleep plan has been revised and implemented. Management will oversee an audit of current sleep plans and regularly review to prevent reoccurrence. 9. Management have retrained on ‘Sleep and Rest Policy’ and sleep provisions for children

- General Safety: 1. The service had not completed or maintained comprehensive indoor and outdoor daily risk assessments to identify, assess and manage potential hazards within the environment. During the inspection, a number of safety risks were observed that had not been identified or addressed by the provider, including: • A large storage shelf in a care room containing equipment was unanchored and unstable. • The outdoor slide was cracked and broken on the upper surface of the slide posing a risk of injury to children. • Radiator dial covers were missing in both the Sun Room and Moon Room, exposing sharp edges to children. • Cleaning agents were stored beside the nappy changing pad and were accessible to children. • Medication was stored improperly on an open shelf accessible to children. Infection Control: 2. The service did not have sufficient cleaning schedules and monitoring systems in place to ensure that all areas of the service were maintained in a clean and hygienic condition. Records reviewed during the inspection were incomplete and not updated regularly, with cleaning records for the baby room last documented on 6 March. Observations during the inspection identified a number of areas that had not been cleaned or maintained appropriately, including: • Ventilation ducts in the sanitary facilities contained a heavy build-up of dust. • Toilets were observed to be unclean with visible limescale accumulation. • Limescale build-up was also present in some sink areas. • Excessive clutter around sinks in the baby room and nappy changing room prevented effective cleaning and appropriate hand-washing practices. 3. Thermostatically controlled warm water was not available for hand washing at the wash hand basins in the sanitary accommodation in all rooms other than the baby room. The water in both the hot taps and the cold taps felt cold to touch. Cold water does not support children to effectively wash their hands. This is a reoccurring noncompliance as evidenced in the inspection held on the 1 November 2025. 4. In both the Sunroom and the Moon room children’s snacks provided by the parents, some of which contained meat and dairy produce, were stored in the children’s school bags at room temperature on the day of inspection. This increased the risk of bacteria multiplying in the perishable food items. This is a reoccurring noncompliance as evidenced in the inspection held on the 1 November 2025. - General Safety: The unit in question has been secured to the back wall using L brackets. Outdoor slides have been replaced Radiators caps have been replaced. Staff have been reminded of the importance of storing cleaning agents safely. This will be part of daily risk assessment. Parents and staff have been reminded to not leave medications in a child bag or indeed a nappy changing box. All medication must be recorded in each morning and placed in the fridge with each child’s name on the box. A risk assessment is in place for items like these and this will be monitored going forward. Infection Control:
- While a cleaning system is in place within the crèche and cleaning checklists are readily available to all staff, employees have been reminded of the importance of maintaining high standards of cleanliness throughout the day. All areas have been cleaned and addressed. Monthly checks are in place and all risk assessment forms and cleaning forms are now being collected at the end of each month for examination
- The plumber has attended the service and hot water available in some sinks. Service is working together with the landlord to ensure that the hot water system is fully operational and functioning correctly on a permanent basis going forward
- A refrigerator has been placed in the upstairs kitchen area for the storage of children's snacks. This arrangement will be implemented going forward and will also apply to all new children enrolling from September 2026

- General Safety: 1. Garda vetting was available for 35 staff members. However, one of these vetting disclosures was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. Locks were not always present on cupboards containing cleaning items which were within reach of children. The inspector brought this to the attention of a staff member, and items were removed from the area. Fire Safety: 3. A large window to the side of the premises in the upstairs area had been screwed shut. This could impede or delay an evacuation of the premises in the event of an emergency. It is acknowledged the roof windows present were fully openable. Action submitted by the Registered Provider

- General Safety: 1. Garda Vetting renewal was submitted, however there is currently a delay with the vetting department. We now have changed our procedure to renew Garda Vetting for staff every 2 years. This process will ensure that all Garda Vetting disclosures always remain current and within the required 3-year period, in line with Tusla requirements. 2. Safety locks are not fitted to any cupboards where art material is kept, for children to be able to access them. Staff have been informed that washing up liquid should be stored on the high shelf, out of reach of children. Staff have been reminded that our risk assessments are to be always adhered to. - Infection Control: 1. Infection control measures that were outlined in the services infection control policies were not always followed as evidenced by the following: • Some of the children’s hands were observed not to have their hands washed following nappy changing. • The nappy changing mat was not always cleaned in between changing children’s nappies. • The children attending Room 3 did not have their hands washed before they ate their morning snack. • A staff member was observed not to wash their hands after cleaning a child’s nose. 2. The water in the hot taps at one of the children’s wash hand basins in the sanitary accommodation in Room 5 was recorded as 13.6°C by the inspector. Cold water does not support pre-school children to effectively wash their hands. 3. A pedal bin was not available for disposal of waste and contaminated items in the sanitary accommodation in Room 1. Safe Sleep: 4. The sleep forms that were available were not comprehensively completed with the necessary information that may be required to support the children’s individual sleep needs and preferences. Seven sleep plans were reviewed, 6 of the sleep plans had sections of the plans that were not completed. Action submitted by the Registered Provider Corrective Action Infection Control: 1. The staff received an updated copy of their Hand Hygiene and Nappy Changing policy to revise. Age appropriate and clearly visible hand-washing posters are now clearly displayed at all sinks and nappy changing areas to guide both staff and children. Visual reminders are now provided to prompt staff to remember to sanitise the nappy changing mats in between changing each child’s nappy and to remind staff and children to wash their hands before they eat all their meals and regular increments throughout the day. There are now also visual cues to remind staff of correct hand hygiene practices like washing their hands after they clean the children’s noses. 2. Their plumber fixed the problem that had occurred with the tap in room 5 so now the water is the correct temperature for the children to support the preschool children to effectively wash their hands. 3. A new pedal bin was provided for the bathroom in room 1. Safe Sleep: 4. All of the children’s sleep plans have been reviewed and correctly filled out and manager has ensured they are now comprehensively completed. A new checklist has been created for new wobblers/children starting to ensure all relevant documentation is complete and returned prior to start date. Preventive Action Infection Control: 1. Regular checks will be done to ensure staff are maintaining a high standard of hygiene and following their infection control policy. The visuals cue permanently there to prompt the staff and children to ensure that they remember and use the highest standard of hygiene. 2. They will carry on with their weekly water checks to ensure that all the taps are always at the correct temperature for all of the taps in the building to ensure that the staff and children can both effectively wash their hands at all times. 3. Regular checks to ensure all of the bins are the correct pedal bins at all times. Safe Sleep: 4. Ensure that when a child’s sleep forms are always comprehensively completed when one is filled out for each child. Ensure new checklist is completed and returned before or on child’s start day. Tigers Childcare, Bettystown · Regulation 23 — Safeguarding health, safety and welfare of child
- General Safety: 1. A cord from a telephone was hanging down at a low level in the Explorers' room which posed a risk of injury to a child. It is acknowledged that when brought to the attention of staff, it was moved out of the children's reach. Infection Control: 2. Hand washing practices were observed during the inspection and it was noted that there were occasions where they were insufficient to reduce the risk of cross contamination. These are outlined as follows • Four of the children in the Junior Discovery room were observed to move directly from playing outdoors, and in the care room to the table to have their dinner without washing their hands. It is acknowledged that the four children who had their nappy changed after outdoor play had their hands washed by staff before they had their food, however, those that did not have their nappy changed did not have their hands washed before eating. • No handwashing was observed before children were served their afternoon snack in the Junior Discovery room. • The children in the Senior Discovery room were observed to move directly from playing or sleeping to having their afternoon snack. No handwashing was observed before children were served their food. • During nappy changing in the Senior Discovery room, a staff member dampened a paper towel to clean the child's hands after they had their nappy changed. Safe Sleep: 3. Sleep room checks were being carried out by staff at 10-minute intervals as required however they were unable to record details of the checks in the electronic devices due to the number of children in the care room, the number of staff needing to use the devices which delayed the recording of sleep information. - General Safety: 1. The telephone cord was immediately repositioned and secured out of children’s reach on the day of inspection and then replaced. Following the inspection, educators conducted an immediate review of all rooms within the service to identify and remove or secure any similar hazards that could pose a risk of injury to children. Educators have been reminded of their responsibility to identify and address potential hazards immediately and to record them on our daily safety checks. Educators will also inform management of any risks observed Infection Control: 2. Handwashing practices were reviewed with all educators to ensure children wash their hands appropriately before all meals and snacks, after outdoor play and after toileting/nappy changing. Management shared the infection control policy with educators, and the team signed off agreeing they have read and reviewed the policy. We have also introduced a “When to wash my hands poster” in our Junior Discovery room. The nappy changing procedure was reviewed with educator on the day of inspection. Educators were reminded that children’s hands must be washed with soap and water following every nappy change in line with infection prevention and control procedures. Management will complete regular observations and spot checks within care rooms to ensure procedures are followed consistently. Infection prevention and control procedures will continue to be discussed during staff meetings and supervision sessions Safe Sleep 3. We have requested a second tablet for the senior discovery room in the meantime the service now uses both a tablet and the centre mobile phone to record sleep checks, which has improved staff access to electronic devices and reduced delays in documentation. Management will carry out regular audits of sleep records to ensure checks are completed and documented accurately at the required intervals

The Village Preschool, Moynalty, Kells · Regulation 23 — Safeguarding health, safety and welfare of child
- General Safety: 1. The surface temperatures of two radiators in the Big room were recorded between 49.9°C and 50.8°C. Hot surfaces over 50°C pose a burn risk to a child. It is acknowledged that these were located behind armchairs and a painting unit and when brought to the staffs attention, they adjusted it immediately which reduced the surface temperatures to 49.4°C and lower. 2. The edging of a low partition wall between two areas in the Big room was broken and had a nail exposed, which posed a risk of injury to a child. 3. As the service is located in a village with adjoining gardens, a daily outdoor risk assessment would be required to ensure no hazardous items were thrown into the outdoor area. As part of the corrective and preventive action plan submitted following the last inspection on the 05/11/2025 the registered provider gave assurances that risk assessments would be carried out on a daily basis and had submitted a risk assessment template. It was evident that the corrective and preventive actions were not implemented. Infection Control: 3.The pedal bin for soiled nappies was broken as the lid had to be opened by hand which poses a risk of cross contamination. The corrective & preventive action plan submitted following the inspection on the 05/11/2025 indicated that a new bin was provided, however it was found that the bin was broken. It was evident that the assurances given by the registered provider were inadequate. Administration of Medication: 4.There was no prior consent from parents for the administration of emergency medication although this practice was stated in the Medication Policy. As part of the corrective and preventive action plan submitted following the last inspection on the 05/11/2025 the registered provider gave assurances that consent for emergency medication is now included on the children’s admission form however this consent was not obtained. It was evident that the corrective and preventive actions were not implemented. Fire Safety: 5.There was no fire drill conducted for the month of February 2026. Following the last inspection on the 05/11/2025 the registered provider submitted proof that fire drills were being completed on a monthly basis as a corrective and preventive action. However, it was evident that the action previously stated by the registered provider did not prevent the non-compliance from re-occurring. - General Safety: 1. A Garda vetting disclosure that was available for 1 staff member was not dated within the previous three years in adherence with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. A large in-ground trampoline was located in the large outdoor play area to the side of the service in addition to 1 small portable trampoline which was available for the children to use in the outdoor play area to the rear of the Senior room. The area in which the large inground trampoline was located was not in use on the day of inspection due to the ground being wet underfoot. Children were observed using the small portable trampoline. Trampolines are inappropriate equipment in an early years setting as they are not recommended for children aged less than 6 years due to safety risks. This noncompliance was observed at the last inspection of the service on the 29/04/2025. The registered provider provided written assurances in the corrective and preventative actions that procedures had been put in place to prevent a recurrence of the non-compliance however these were insufficient to ensure that the non-compliance did not re-occur. 3. On discussion with staff members regarding the procedure for taking photographs of the children to share with parents through a smartphone messenger application two staff members reported that they use their own personal mobile phones to take photographs of the children which are subsequently sent to parents. This practice is at variance with the services policy which stated that the service’s mobile phone should only be used to take photographs. The practice of staff members using their own personal mobile phones to take photographs of children in the service posed a potential risk to the children. This noncompliance was observed at the last inspection of the service on the 29/04/2026 and the actions stated by the registered provider in the corrective and preventative did not prevent the noncompliance from recurring. Infection Control: 4. The steps in the services nappy changing policy were observed not to be followed as evidenced by the following: • Staff members were observed not to wash their hands after nappy changing. • Some of the children were not assisted to wash their hands after nappy changing. 5. Practices observed during nappy changing were unhygienic and increased the risk of cross contamination. Children from the Junior Room 1 were observed to be brought to the nappy changing area in pairs during nappy changing time. On one occasion, whilst the staff member was changing one child, another child was sitting on the bathroom floor whilst waiting in this area. This practice does not support effective infection control practice. 6. Inadequate space was left between some of the floor beds set up in the Junior room 1 for the children to sleep after dinner. Spacing between some beds was found to be less than the recommended distance of 50cm apart. The inadequate space could potentially delay staff from accessing children in the event of an emergency and also increased the risk of cross infection. 7. There was an unlidded bin observed in the Junior Room 1 used for the storage of waste including food waste. This practice does not support effective infection control practice. Safe Sleep: 8. One of the cot mattresses in the sleep room used by the children attending the Toddler room was soft and did not provide a firm comfortable base for the children to sleep on. This is at variance to safe sleep guidelines. Action submitted by the Registered Provider Corrective Action General Safety: 1. The person in question has agreed to take annual leave until vetting has returned. 2. A cover is in the process of being made, we will forward proof on as soon as its completed. the trampoline is now not in use. 3. This was discussed again at the staff meeting all staff have been informed that no personal phones are to be used to take any pictures of the children. The only phone that is allowed is the creche mobile which is available at all times. Infection Control: 4. This was outlined in the staff meeting, the importance of practicing good Hygiene and how the standard need to be met. 5. Each child is now taken to the nappy area one by one. 6. There is now 50cm placed between each bed to prevent cross infection and ensuring a staff member has enough room to aid a child if needed. 7. A new bin has now been purchased to replace the old bin. Safe Sleep: 8. Mattress was replaced on day of inspection with a new one. Preventive Action General Safety: 1. All garda vetting doubled checked and now up to date. 2. Trampoline not in use. 3. A verbal warning will be given if this is not adhered to. Infection Control: 4. Monitored at all times. 5. Monitored at all times by management. 6. Staff have been informed and this will be monitored at all rest times by management. 7. Room leader assigned to inform management when new equipment is required. Safe Sleep: 8. The chairs provided in the Toddler room were not age appropriate for all of the children present. The Wobbler room 2 children moved to the Toddler room after dinner time. A child from the Wobbler room 2 group was observed falling off the chair due it’s unsuitability. Infection Control: 3. In both the Wobbler room 1 and the Toddler room the children’s afternoon snack of buttered brown bread was placed directly on to the table; no plates were used. This presents a risk of cross contamination. 4. Handwashing was observed not to occur at the following times: • The children in the Wobbler room 1, Wobbler room 2 and the Toddler room did not have their hands washed before they had their afternoon tea. • In the Wobbler room 1 staff and children were not always observed to wash their hands after nappy changing. • Staff members were observed not to wash their hands after cleaning children’s noses. Administration of Medication: 5. Two children for whom medication was available to be administered in the service in the event of an emergency, to include an inhaler, did not have a completed medical care plan with parents’ signature to accompany this in advance of potential administration. Safe Sleep: 6. In the Toddler room a staff member was observed to conduct a sleep check on 5 children while standing at the sleep room door. The children were not individually checked. All children must be individually checked while sleeping to include the children’s colour, breathing and position. Action submitted by the Registered Provider Corrective Action General Safety: 1. Trampoline has been removed and is no longer available for children to use. 2. Smaller chairs are now available in Toddler room for when smaller children are present. Infection Control: 3. Snack is now brought into the room on plates rather than on trays. 4. Registered provider purchased more cloths so that children can have hands cleaned this way rather than transitioning out of the room to the bathroom area to clean hands as this can cause upset, particularly at mealtimes. All staff have been given nappy-changing policy to revise and sign off on and reminded of the importance of hand-washing. Hand sanitiser clearly labelled in room for staff use after cleaning noses. Administration of Medication: 5. In relation to the medicine’s discussed – one was a temporary inhaler which was only used for a short period of time and was mistakenly not sent home as the parent advised they did not need it. The inhaler has now been sent home with the parent for disposal. The second medication was an antihistamine which was sent in along with Calpol should the child need it. Said medication has since been sent home with the child and parents have been advised that any such medication will require a doctor’s note and medicine form. Safe Sleep: 6. Staff have been advised that it is not sufficient to simply investigate the room during sleep checks. Policy has been provided for review. Preventive Action General Safety: 1. Trampoline has been removed from premises and information on trampoline use included in policy. 2. Clearly label smaller chairs for Wobblers so they do not sit on bigger chairs. Infection Control: 3. As above. 4. Posters up in rooms to remind them of the importance of hand-washing before meals, before and after nappy changes and after cleaning noses. Administration of Medication: 5. Staff have been provided with medicine administration policy to review and sign off on. All medicine entering the building must now go through the manager to ensure appropriate documentation. All relevant staff have been retrained in nappy changing and disposal procedures with an emphasis on infection control. 3. Staff were reminded to store all perishable items in the childrens lunch boxes in a fridge at or below 5°C upon arrival to the service. The temperature of the refrigerator will be checked and recorded on a daily basis to ensure compliance with food safety standards. A food storage policy was implemented requiring all perishable items to be refrigerated immediately on arrival to the service. 4. All children’s play equipment was immediately removed from the staff toilet and transferred to a clean designated storage area away from sanitary facilities. Any items stored were disinfected immediately to eliminate any possible contamination. A specific storage area has been assigned for children’s play equipment ensuring it is clean, dry and separate from toilet and cleaning areas. The gate that leads into the outdoor area at the rear of the premises, where staff advised that parents drop and collect their children from, had a high-level turn handle. Although the turn handle was out of the reach of children, it did not prevent or restrict unauthorised persons from gaining access to the premises. The care rooms located directly off the outdoor areas were not secured to prevent unauthorised access. 3. Three electrical leads from plug sockets of a temperature device, a fan and a cooling unit were accessible to children in the cot room, the leads were located next to 2 cots, which poses a risk of injury to a child. Infection Control: 4. The children in the Toddler room were observed to come directly in from outside to have their lunch. No handwashing was observed before children were served their food, which poses an infection control risk. Safe Sleep: 5. Sleep checks reviewed did not adequately record sleep checks every 10 minutes as per the services safe sleep policy. On dates reviewed (19th, 22nd and 25th August 2025) the sleep checks were recorded on occasion at 13, 14, 15 and 20 minute intervals. 6. Sleep equipment provided for children under 2 was not suitable. Two children aged 1 year 9 months and 1 year 11 months were observed to sleep on floor mats. Children aged 15 months upwards can sleep on a floor bed, once a care plan has been completed and the bed is fitted with a mattress of at least 6cm in thickness. There were no care plans available for these two children to demonstrate consultation with parents/guardians for the transition of their children from a cot to a floor bed

Infection Control: The following practices were observed on the day of inspection which increased the risk of cross contamination: 2. The children attending the Nursery room and the Toddler room did not have their hands washed after they had their nappies changed or before they ate. 3. Handwashing was not always carried out by staff in between nappy changing of children. 4. The nappy changing mat was not cleaned using disinfectant in between nappy changing of children. Safe Sleep: 5. Staff members in the Nursery room were observed conducting the children’s sleep checks at variance with the services safe sleep policy. Staff members stood at the sleep room door to conduct the sleep checks and viewed the children through the glass panel on the door. All sleeping children must be physically checked every 10 minutes to include checking the child’s colour, breathing and sleeping position. Fire Safety: 6. Two children’s tables and chairs in addition to play materials were stored in the corridor between the Nursery room and the Toddler room which could potentially impede staff and children’s exit from the service in the event of an emergency. Upon inspection, the staff member, who has been employed for three years and was initially Garda vetted, had already submitted their Garda vetting renewal application. They are currently awaiting the updated vetting disclosure from the Garda Vetting Bureau. It has been advised by the Early Childhood Ireland Garda Vetting team that additional processing time is required for applicants who have lived in EU countries outside of Ireland (up to 10 extra working days) or the UK (up to 20 extra working days), which both are applicable to this staff member. This will include implementing a tracking system to monitor staff vetting expiration dates and initiating the renewal process a minimum of two months in advance of the expiration date. This will allow for sufficient time to complete the process and prevent any lapse in vetting status. Infection Control: 2. Management will ensure staff members receive regular refresher training on the handwashing and nappy- changing policies. Some of the nappy changing practices observed during the inspection posed a risk of cross-infection as demonstrated in the following examples: ➢ Babies hands were not always washed after nappy changing. ➢ A staff member was observed to wear gloves for nappy changing but not to wash her hands before changing her gloves between each nappy change. This posed a risk of cross-contamination when removing the used gloves. ➢ Aprons were not worn for by staff members for all nappy changing procedures observed during the inspection. Aprons are required to protect staff members’ outer clothing from contamination. Safe sleep: 2. Inadequate space was left between 3 of the 9 cots in the sleep room. Spacing between these 3 cots was found to measure between 27cm and 30 cm which was less than the recommended distance of at least 50cm apart. This increased the risk of cross infection and also posed a risk that a child who was awake in one of these cots could make physical contact with another child in the neighbouring cot

The renewed garda vetting disclosure, processed on 13/06/2024, was emailed to the inspector on the following day. This Garda Vetting disclosure had not been renewed before the previous disclosure expired. 2. There was a trailing electrical lead hanging down from a radio on a shelf above one of the cots in the Baby room’s sleep room. This lead was accessible to a child placed in this cot and posed as a health and safety risk. Infection Control: 3. Thermostatically controlled warm water was not available for hand washing at one of the two the wash hand basins in the sanitary accommodation adjoining the Junior Pre-school room. The water in the hot taps and the cold taps on this wash hand basin felt cold to touch. Cold water does not support pre-school children to effectively wash their hands

- Corrective & Preventive Action General Safety: 1. The staff's Garda vetting was received and promptly forwarded to the inspectors the following day. 2. The radio was promptly removed from the sleeping area's shelf. Preventive Action: 1. Each staff member's Garda vetting will be renewed within the 3-year timeframe to comply with the regulatory notice from the Early Years Inspectorate. 2. A radio should never be placed on a shelf without the electrical cord being covered to mitigate potential health and safety hazards, particularly for children sleeping in cribs. Infection Control: 3. The thermostat has been adjusted to warm the water in the junior preschool, ensuring optimal temperature for effective hand washing. Preventive Action: 3. - General safety: 1. One of the blind cords at the door leading from the Toddler room to the outdoor play area was loose and not completely secured to the wall, which potentially a child could get caught in. Infection Control: 2. The paper hand towels in the nappy changing area located between the Toddler room and the Explorers room not hygienically dispensed. The rolls of paper were stored on worktops and were subjected to repeated handling; this was inadequate for infection control purposes. 3. During sleep time in the Toddler room the recommended distance of 50cm was not maintained between the floor mats. This is inadequate for infection control purposes. - General Safety: 1. The blind cord is attached the wall and no longer hanging down. Infection Control: 2. New batteries have been put in the hand towel dispensers and the blue/white roll is placed in them to avoid multiple people handling it. 3. Children are distanced 50cm apart at sleep time. Staff are aware to report any safety issues to management who in turn will ask maintenance team to fix. We have put up a notice to ensure staff are aware of the distance required between bed. Summary Comment The registered provider has provided evidence of the non-compliance’s addressed, the blind cord no longer hanging down, the hand towel dispenser working and the sleeping children 50cms apart. The non-compliances in Regulation 23 have been addressed

- General Safety: (1) A large protruding nail was noted in the steering wheel in the wooden bus in the outdoor area which posed a safety hazard. (2) Children less than 3 years of age were noted on a high climbing frame that was not age appropriate which posed a safety hazard. Corrective Action: 1. During the inspection a protruding nail was noted on the steering wheel of the outdoor truck. This was tightened on the day of inspection and inspector was sent photographic evidence. Preventative Action 1. The protruding nail will remain tightened going forward. Corrective Action: 2. As a number of the children in the green room are aged 3 and are permitted to climb the climbing frame a risk assessment will be carried out prior to allowing the children to climb to ensure they are capable. Preventative Action: 2 Children below 3 years of age, that are not capable of climbing the frame will be provided with alternative age-appropriate climbing equipment instead.
