In addition, a safety lock was installed on the storage unit. These corrective actions were completed by 2:00pm on the day of the inspection. Regular health and safety checks will continue to include verification that all storage units containing hazardous materials are appropriately secured. Safe Sleep: 2. The finding was brought to the attention of the Person in Charge during the inspection. The comforters were removed immediately from the cots of the children concerned and staff were reminded of the requirements of the service's Safe Sleep Policy and Tusla Safe Sleep Guidance. Safe sleep practices have been added as a specific item to room supervision checks and management audits.   • The registered provider confirmed that findings on inspection were brought to the attention of staff. Assurances were given by management that regular checks and audits will take place to prevent reoccurrence. Staff will further be periodically reminded of procedures through staff meetings and supervisions. General Safety: 1. All employees were reminded that grapes present in children's lunchboxes from home must be cut in half. Communication was sent to all current ECCE parents requesting that grapes be cut prior to including in lunchboxes. To prevent reoccurrence, grapes will be prohibited in the service beginning with the September intake of new ECCE children. 2. The emergency medication identified in the Yellow Room was removed from the service following confirmation from the Childs parents that it was no longer required. 3. Visibility safety markings have been applied to all glass panels. Infection Control: 4. The cleaning chemicals stored in the low-level cupboard in the Orange Room were removed and relocated to the high-level cupboard.   • The registered provider confirmed that staff meetings were held; the following topics were discussed and reviewed: Risk management policy, Hand washing policy and procedures, Safe sleep guidance and policy, including sleep plans and the Sleep and Rest Policy. Items identified on inspection have been added to the daily safety checklist to prevent reoccurrence. General Safety: 1.The wooden slab on the shed has been replaced. A safety audit of the building was overseen by management to ensure the maintenance logs are up to date. Management will continue to oversee the maintenance log. 2. The bike handles were replaced. Additional rubber handles have been purchased to ensure prompt replacement to prevent reoccurrence. 3. A combination lock has been added to the outdoor heat pump unit to ensure it remains inaccessible to the children. Infection Control: 4. Handwashing signage reviewed to ensure correct handwashing and supervision of same takes place. Management will ensure and regularly review policy with staff. Fire Safety: 5. A floor plan for the cocoon beds and sleep mats has been completed. The floor plan is displayed on the wall of the room for reference. Safe Sleep: 6. To prevent reoccurrence, a weekly roster has been implemented for ten-minute sleep checks. Timers are present and signage has been put in place also. Support and supervision meetings were held to discuss safe sleep procedures. Management will oversee weekly audits of sleep checks. 7. Dimmable lighting has been installed in the cot room to allow soft lighting where children sleep. 8. A sleep plan has been revised and implemented. Management will oversee an audit of current sleep plans and regularly review to prevent reoccurrence. 9. Management have retrained on ‘Sleep and Rest Policy’ and sleep provisions for children

Parents and staff have been reminded to not leave medications in a child bag or indeed a nappy changing box. All medication must be recorded in each morning and placed in the fridge with each child’s name on the box. A risk assessment is in place for items like these and this will be monitored going forward. Infection Control:
  • While a cleaning system is in place within the crèche and cleaning checklists are readily available to all staff, employees have been reminded of the importance of maintaining high standards of cleanliness throughout the day. All areas have been cleaned and addressed. Monthly checks are in place and all risk assessment forms and cleaning forms are now being collected at the end of each month for examination
  • The plumber has attended the service and hot water available in some sinks. Service is working together with the landlord to ensure that the hot water system is fully operational and functioning correctly on a permanent basis going forward
  • A refrigerator has been placed in the upstairs kitchen area for the storage of children's snacks. This arrangement will be implemented going forward and will also apply to all new children enrolling from September 2026

  • General Safety: 1. The telephone cord was immediately repositioned and secured out of children’s reach on the day of inspection and then replaced. Following the inspection, educators conducted an immediate review of all rooms within the service to identify and remove or secure any similar hazards that could pose a risk of injury to children. Educators have been reminded of their responsibility to identify and address potential hazards immediately and to record them on our daily safety checks. Educators will also inform management of any risks observed Infection Control: 2. Handwashing practices were reviewed with all educators to ensure children wash their hands appropriately before all meals and snacks, after outdoor play and after toileting/nappy changing. Management shared the infection control policy with educators, and the team signed off agreeing they have read and reviewed the policy. We have also introduced a “When to wash my hands poster” in our Junior Discovery room. The nappy changing procedure was reviewed with educator on the day of inspection. Educators were reminded that children’s hands must be washed with soap and water following every nappy change in line with infection prevention and control procedures. Management will complete regular observations and spot checks within care rooms to ensure procedures are followed consistently. Infection prevention and control procedures will continue to be discussed during staff meetings and supervision sessions Safe Sleep 3. We have requested a second tablet for the senior discovery room in the meantime the service now uses both a tablet and the centre mobile phone to record sleep checks, which has improved staff access to electronic devices and reduced delays in documentation. Management will carry out regular audits of sleep records to ensure checks are completed and documented accurately at the required intervals

  • Plan (CAPA) were insufficient to prevent the non- compliances from re-occurring. 2. Risk assessments were not conducted for the outdoor area. As the service is located in a village with adjoining gardens, a daily outdoor risk assessment would be required to ensure no hazardous items were thrown into the outdoor area. As part of the corrective and preventive action plan submitted following the last inspection on the 05/11/2025 the registered provider gave assurances that risk assessments would be carried out on a daily basis and had submitted a risk assessment template. It was evident that the corrective and preventive actions were not implemented. Infection Control: 3.The pedal bin for soiled nappies was broken as the lid had to be opened by hand which poses a risk of cross contamination. The corrective & preventive action plan submitted following the inspection on the 05/11/2025 indicated that a new bin was provided, however it was found that the bin was broken. It was evident that the assurances given by the registered provider were inadequate. Administration of Medication: 4.There was no prior consent from parents for the administration of emergency medication although this practice was stated in the Medication Policy. As part of the corrective and preventive action plan submitted following the last inspection on the 05/11/2025 the registered provider gave assurances that consent for emergency medication is now included on the children’s admission form however this consent was not obtained. It was evident that the corrective and preventive actions were not implemented. Fire Safety: 5.There was no fire drill conducted for the month of February 2026. Following the last inspection on the 05/11/2025 the registered provider submitted proof that fire drills were being completed on a monthly basis as a corrective and preventive action. However, it was evident that the action previously stated by the registered provider did not prevent the non-compliance from re-occurring. Corrective & Preventive Action submitted by the Registered Provider Corrective & Preventive Action General Safety: 1.Cleaning products are now stored up high and not accessible to children. 2. A daily risk assessment is now conducted in the outdoor area. The registered provider will ensure that all areas accessible to children are safe on a daily basis. Infection Control: 3.The bin has been replaced with a new pedal operated bin.The risk assessment for the sanitary area now includes the bin. The registered provider will ensure that the pedal operated bin for soiled nappies is functional and available on a daily basis. Administration of Medication: 4. Consent for emergency medication has now been obtained for all children attending the service. The admission form for the service has now been updated to include this information. The registered provider will ensure all admission forms are fully completed with all relevant information before children start in the service. A food storage policy was implemented requiring all perishable items to be refrigerated immediately on arrival to the service. 4. All children’s play equipment was immediately removed from the staff toilet and transferred to a clean designated storage area away from sanitary facilities. Any items stored were disinfected immediately to eliminate any possible contamination. A specific storage area has been assigned for children’s play equipment ensuring it is clean, dry and separate from toilet and cleaning areas. Regular inspections will be conducted by the supervisor to ensure compliance with storage and hygiene requirements

This cot has now been taken out of the sleep room and is kept in a storage area to ensure the correct space between all cots in the sleep room can be maintained at all times throughout the day.
