  • Corrective & Preventive Action General Safety: 1. Electrical cords have been secured higher on the wall using cable clips to ensure they remain out of children’s reach. Although the risk is assessed daily, the charging cord has also been relocated to the kitchen as an additional control measure to minimise access by children. To prevent non-compliances from recurring, all electrical wires have been permanently secured to the wall, and the charging station has been relocated to the kitchen to ensure it remains out of children’s reach. Additional supervision has been implemented in the outdoor area, with particular attention given to areas where the artificial grass is lifting, until it is removed and replaced with a new surface. Grass that has lifted has also been pinned back down for the time being to further eliminate this risk. Administration of Medication: 3. Two medicine administration forms that had not been signed were reviewed and have now been completed with the required signatures. Management has reinforced the importance of ensuring that medicine administration forms are signed daily by parents/guardians. Ongoing monitoring will be carried out to ensure compliance with this

  • Corrective & Preventive Action General Safety: 1. In the Zebra room Maintenance replaced the existing thermostat with a new thermostat control valve and adjusted the system to ensure that hot water temperatures at children's handwashing facilities do not exceed the recommended maximum temperature of 43°C. Staff in the Zebra Room have been reminded that water temperatures at children's handwashing facilities must not exceed 43°C. During routine water temperature checks, staff are required to immediately report any temperature reading above the recommended limit to management so that prompt corrective action can be taken. Ongoing monitoring and recording of water temperatures will continue to ensure compliance and maintain a safe environment for children. A new floor supervision form has been created to check all compliance on the floor such as water temperature are being checked by the management throughout the day. 2. Immediately following the inspection, all cleaning chemicals were removed from areas accessible to children and relocated to secure, locked storage. New safety latches were installed on storage units within the Tigers and Zebra rooms to prevent children from accessing any potentially hazardous items. As a preventive action A memo was sent to all staff reminding them of their responsibility to ensure that cleaning chemicals, pest control products, and any other hazardous substances are stored securely and remain inaccessible to children at all times. A sign has been created for the cupboards to remind staff that such items should not be kept there. A new maintenance officer has been appointed in the centres where staff can report issues to. The officer checks the rooms every Tuesday. The room and health and safety check are checked by the manager each day. Infection Control 3. The swing-top bin in the Zebra room was replaced with a hands-free pedal-operated bin to comply with infection prevention and control requirements and reduce the risk of cross-contamination. As a preventive action regular environmental health and safety checks will include verification that appropriate waste bins are in place and functioning correctly. Any non-compliant equipment identified during routine inspections will be reported and replaced immediately. Management will continue to monitor maintenance compliance with the floor supervision documents

  • Corrective & Preventive Action General Safety: 1. This particular staff member no longer works for the service. The registered provider will check all staff vetting is up to date by checking staff files on an annual basis. 2. Hot water boiler has since been checked and water temperature is recorded. Water temperature is recorded twice daily and signed by manager/person in charge. 3. Corner cushioning has been added to all sharp edges in the care rooms. Cushioning will be maintained and secured regularly. Infection Control: 4. Manager/person in charge to ensure the hot water is switched on daily. 5. Staff have been advised to wear aprons during nappy changing. Manager/person in charge to ensure there is a supply of disposable aprons on site, nappy changing policy has been updated. 6. All perishable food items have been labelled with child’s name and stored in fridge. Manager /person in charge to ensure all perishable items are stored daily in the fridge. 7. Children are supervised whilst washing their hands upon returning from garden and before snack times. 8. Staff have been reminded to ensure tables and surfaces are cleaned appropriately before and after snacks. Food items are placed on individual plates during snack times. 9. Nappy bins have since been replaced with pedal bins and bin liners. Safe Sleep: 10. Staff are now aware to remain in the room during sleep time. Staff updated on the sleep policy and regulations so that staff are aware they remain in the room whilst children are napping. 11. Staff preparing children for sleep, have since been made aware to ensure the monitor is also in the sleep room before the children have been put to bed. Person putting the children to sleep have been made aware to ensure room is set up properly for sleep including the monitor is in the room. 12. New sleep documentation books have since been purchased and the manager has ensured staff are aware on how to fill in and complete this book appropriately and sign properly. Fire Safety: 13. The non-compliant mattress was removed immediately and replaced with a new mattress that meets the required safety standards and includes the appropriate labelling. All sleep equipment has been reviewed to ensure compliance with safety standards and appropriate labelling. Regular equipment audits will be conducted by management to ensure ongoing compliance with Tusla requirements. 2. The low-level electrical lead was removed immediately, and cable covers were installed to eliminate access to electrical hazards. A full environmental safety check will be conducted to identify and eliminate accessible electrical hazards. Staff have been verbally reminded to include electrical safety checks in daily risk assessments. Ongoing monitoring and regular safety Checks and Hazards Identification Audit will be conducted to ensure compliance with Tusla standards. 3. The broken chalkboard was removed immediately. The identified hazards, including protruding nails, were addressed to eliminate risk to children. A comprehensive inspection of the outdoor area has been completed to identify and rectify any additional hazards. A routine outdoor safety checklist has been scheduled daily. Staff have been reminded to report and act on hazards promptly. Regular maintenance checks will be carried out to ensure continued compliance with Tusla requirements. 4. Maintenance staff addressed the hazard immediately by fitting a metal piece to securely cover the gap. All outdoor surfaces have been reviewed to identify potential trip hazards. The manager will ensure that the maintenance log is used daily to ensure timely repair of identified issues. Regular inspections will be conducted to ensure that outdoor play areas remain safe and compliant with Tusla standards. 5. Safety corner covers were installed immediately on all low-level units to reduce the risk of injury. A more robust approach has been implemented to ensure all sharp edges are consistently covered and maintained. All rooms have been re-assessed for similar risks. Management will carry out frequent spot-checks to ensure corner protectors remain securely in place. The effectiveness of control measures will be reviewed regularly to prevent recurrence and ensure compliance with Tusla requirements. 6. The lights and associated plug were removed immediately to eliminate the hazard. All electrical equipment within children’s reach has been reviewed and removed or made safe as appropriate. Daily safety checks and periodic audits will ensure continued compliance with Tusla standards. Infection Control: 7. Infection control training was delivered to all staff during a staff training day on 02/04/2026, with a focus on correct nappy changing procedures, appropriate use of PPE, and hand hygiene practices. Additional Nappy Changing training focusing just on this specific area has been booked for all staff. The nappy changing procedure has been reinforced with all staff, including clear step-by-step guidance in line with best practice. Ongoing supervision and spot checks will be carried out by management to ensure adherence to procedures. Infection control will be a standing item on staff meeting agendas, and regular refresher training will be scheduled to ensure continued compliance with Tusla requirements. 8. Infection control training was provided on 02/04/2026, and the importance of handwashing after outdoor play was reinforced at a subsequent staff meeting. A clear handwashing routine has been discussed, requiring all children to wash their hands after outdoor play. Staff have been assigned the responsibility to supervise and ensure compliance. Visual reminders have been introduced where appropriate, and management will monitor adherence through regular observations to ensure compliance with Tusla standards. 9. Staff were immediately reminded of the correct procedures for storing perishable food items and milk bottles, ensuring all such items are stored in appropriate refrigerated conditions. Food storage procedures have been reinforced with all staff. Regular checks will be carried out to ensure compliance with safe food storage practices. Clear guidance has been reissued regarding the handling and storage of children’s food and bottles, and compliance will be monitored through routine supervision in line with Tusla requirements. 10. A new, appropriate pedal-operated nappy bin with a fully functioning sealed lid was purchased and put into use immediately. Infection control procedures, including correct waste disposal practices, were reinforced during the staff training day using practical scenarios. All waste disposal equipment has been reviewed to ensure suitability and proper functioning. Staff have been instructed on the correct use of pedal bins to avoid hand contact. Ongoing monitoring and refresher training will be implemented to ensure adherence to infection control procedures and compliance with Tusla standards. 11. Staff were immediately reminded to ensure that no toys or play items are brought into nappy changing areas and to supervise children closely during transitions to prevent such occurrences. This was also discussed with staff during the Staff Training Day on 02.04.2026. Clear procedures have been reinforced to ensure that nappy changing areas remain strictly separate from play items. Staff supervision during nappy changing routines has been strengthened. Regular reminders and monitoring will be implemented to ensure adherence to infection control practices and compliance with Tusla requirements

The registered provider had a plumber fix the water in order to ensure it was reaching the correct temperature. Initially it was a temporary fix however, the piping and units have been replaced to ensure it is working correctly at all times. This will be monitored going forward

  • Corrective & Preventive Actions 1. General Safety 1. The temperature of the running water in the sanitary areas have been checked. A risk assessment on the temperature of the hot water in the hand wash sinks has been carried out. See evidence Safety Checks example 1 and 2. Staff have been instructed to ensure that each wash-hand basin is equipped so that the user has access to, running cold and hot water and that the hot water temperature should be no more than 43°C.Daily and Monthly risk assessments carried out in each room. Staff have been instructed on the potential hazard of the hand wash sinks hot water and are to inform Management of any issues identified. 2. The 2 corner protectors have been replaced. Staff have been trained on the safety hazard that sharp corners can pose. Our service keeps a supply of corner protectors in stock at all times in order that protectors can be renewed immediately Management will ensure that all aspects of the service are reviewed and assessed to ensure the safety of all the children attending. Daily and Monthly Risk Assessments carried out in each room. 3. Staff have been reminded that after use CD players are to be wrapped up and put safely away out of reach from the children. Our service always keeps spare nappy change mats in stock. Daily safety and risk assessments are carried out. 7. Staff have been reminded of the correct hand-washing procedure including the use of warm running water. Staff training and Supervision on the correct implementation of the handwashing policy. Ongoing review of Infection Control procedures and policies including the handwashing policy Safe Sleep 8. Physical safe sleep observations were carried out every 10 minutes and documented as required according to the services safe sleep policy and best practice guidelines. Sleeping children are supervised at all times by; a staff member remaining in the room where children are sleeping; or a staff member going into the sleep room at least every 10 minutes and observing each child (only where children are sleeping in standard cots). Physical checks of sleeping children (at least every 10 minutes) are recorded in accordance with the service’s policy on safe sleep. 9. A room thermometer is now in place in the Foxes room for sleep time. Staff training and supervision on the correct implementation of our Safe Sleep Policy. 10. There is low level dim lighting in the Foxes room during sleep time. The staff member on sleep duty sits right next to the children and every 10 minutes (or as required e.g. if a child wakens) gets up and moves around the individual children to check on their breathing and colour. Regular safety audits are carried out to ensure health and safety hazards are minimized. Our Safe Sleep Policy has been reviewed to explain in detail the requirements involved in observing children during sleep / rest times. Management and Room Leaders will regularly review all staff practices to ensure compliance with health and safety and the Safe Sleep Policy

  • Corrective & Preventive Action General Safety: 1. The updated Garda vetting disclosure was submitted and as a preventive action the registered provider has manually recorded expiry dates, these have been added to a digital calendar which will send a reminder two months prior to renewal date. Administration of Medication: 2. The registered provider stated that there are robust plans in place for children with severe allergies or medical conditions where medication is needed to prevent any serious health risks, however, this was not done for children who have allergies or illnesses requiring no medication. A review of all records has been carried out and after discussions with parents a plan is in place for all children should they be exposed to any allergens. This includes signs and symptoms, what to do in the case of exposure. These have been signed by parents and discussed with staff. A new template is in place, and any issues will be discussed with parents and plans drawn up at enrolment stage, these plans will be reviewed at various stages ie. transitions, new terms or should the condition change. Safe Sleep: 3. A staff member will remain with children while sleeping on stackable beds to ensure children are correctly supervised to avoid children leaving their beds. A review of the policy has taken place, and this has been communicated to all staff. 4. & 5. While all staff are aware of our sleep safe policy, they acknowledged on the day of inspection the correct procedure was not followed. A review of the policy has taken place to include the changes highlighted above and all staff have been made aware of the correct procedure to follow and its importance. Staff in the toddler room now remain in the sleep area for nap times, checks will be carried out and documented by this staff member. Spot checks from manager and senior staff will be carried out at sleep times to ensure staff remain vigilant with this procedure. 6. As children are offered rest in the ECCE room where they are in full view of staff, this document was not completed. The process or recording checks every 10 mins is now in place. All staff were updated of changes during a recent policy review

  • Corrective & Preventive Action General Safety: 1. The registered provider stated that the non-compliance was brought to the Board of Directors attention and discussed. The Finance sub-committee have approved the upgrade of the playground. Quotes are currently being obtained, and a tender process will be implemented. Work will be carried out as soon as possible. As a preventive action it was stated that once the playground is completed a scheduled annual maintenance and health and safety audit will be conducted of the playground. 2. The toys stored under the climbing frame have been removed and are now stored in the shed. Staff will monitor the area for pooling water and block off area at times that it is unsafe for children to play in. This monitoring has now been added to our daily risk assessment to remind staff to check the area. 3. The Garda Vetting for the staff member in question was submitted to the Inspectorate. This was an oversight and a Garda Vetting list will be maintained in a password protected file and checked regularly to ensure this doesn’t occur again

  • Corrective & Preventive Action General Safety: 1. To correct the surface temperature of the radiator area on the ground floor, the registered provider has completed the following: A wooden radiator cover was installed to prevent the children from being exposed to high-temperature heating. Risk assessments of the surface are being carried out on a daily basis to ensure the correct temperatures. 2. The registered provider turned down all the hot water thermostat to ensure the correct temperatures. Risk assessments have been carried out to prevent the hot water temperature from rising at any time. 3. The registered provider turned off the radiator in the sensory room and a daily risk assessment will be carried out. The radiator in the sensory room has been turned off because the space is small and sufficient heat flows in from the Toddler Room to maintain the recommended room temperature. Infection Control: 4. The registered provider has spoken to the staff member who returned to a care room without removing the disposable apron following changing a child’s nappy and has stated ‘this will not happen again’. Staff have received a copy of the policies regarding Infection Control and in-house training will commence next month. 5. The cloth towel has been removed and staff have been spoken to by the registered provider. Staff have been advised not to use cloth towels to dry children’s hands and that the blue roll must be used to prevent cross contamination. 6. All nappy bins have been replaced in all changing areas. Going forward, all staff will use pedal labelled bins to dispose of nappy waste. 7. The blue paper towel bin and staff are using the pedal to open the bins. In-house training will commence in a month in relation to infection control measures within the services policies. Staff have been advised to use the pedal bin to dispose of blue paper towels

  • Corrective & Preventive Actions The response from the registered provider stated the following 1. The temperature valve has been fitted to the radiator which can be adjusted by staff to achieve a suitable safe temperature. A risk assessment check list is now in place and this will be checked daily by room leader and will report to management should there be any issues. 2. The hot water tap in the nappy area in the wobbler room has been repaired. Staff have been instructed to contact management when they notice any weakness or inefficiency in the mechanics of the taps, so that preventative maintenance may be carried out to avoid complete breakdown. 3. New mattresses are in place. Staff have been instructed to check the mattresses thoroughly daily, should they see any wear and tear this must be reported to Management immediately. This check is now included on the risk assessment sheet to be signed off by management 4. Staff have been instructed to adhere to the risk assessment and check their paper towel stock, if they are running low they need to arrange to restock

  • Corrective & Preventive Action General Safety: 1. The wooden toy microwave has been taken down from the top of the toy fridge and removed from the classroom. It will only be re-introduced when a suitable solution is found to include the toy microware safely. Infection Control: 2. The new child’s sink in Room 3 has a new soap dispenser and disposable hand towels placed in an area on the wall, that they can now reach. All rooms have been checked and all have adequate soap and towel dispensers available at children’s height level. 3. Following a call to the plumber who arrived and fitted the replacement part, the hot water supply is now working effectively. The registered provider will continue to check the hot water temperature as standard. 4. The registered provider has introduced new cleaning policies to all staff. Reminders of the cleaning policies are on February staff memo. The current mouth toy cleaning procedure to change the wording from ‘wash’ the toys to ‘wash in hot soapy water’’. The service will continue to follow the mouthed toy cleaning procedure and do mock inspections in the rooms with some of these focusing on the cleaning policies

The manager has recorded the dates in advance for the next inspection. Infection Control: 2. A staff meeting was held and the importance of hand washing was discussed at length. Manager has overseen washing hands to ensure it is being done correctly. Random spot checks to take place during hand washing times by manager. 3. Staff have been reminded to visually check lunch bags in the morning for perishable foods, any perishable foods must be stored in the fridge with the child’s name on them. Once the Medical care plan had been returned to our service the parent and teacher discussed the information provided. One copy of the care plan was placed in the child’s file, and another was placed in a visible spot in the child’s classroom where all staff have access to it. The care plan was sent to all staff so we all have the information, and we would all know what to do if a situation arises. Going forward any child who presents with a medical condition will be given the new template to fill in. Staff will liaise with the child’s parents to see if there is an update regarding their child’s care plan. The care plan will be displayed in the child’s room and communicated to all staff, the medication will be stored in a press in the child’s room out of reach, dates on medication will be checked each term. If there are any changes with the child’s care plan management will be informed, and they will then notify all staff members. The child’s care plan will be updated and signed by the parents. Infection Control: Some of the children’s snacks provided by parents were not in a refrigerator - to correct this we have given each class a box, any snacks brought from home will be placed in the box with the child’s name clearly marked on it. On the day of inspection, once the temperature of the water was brought to the attention of management, the registered provider immediately closed off the wash hand basin adjoining the Green Room and contacted a plumber. The plumber made arrangements to call to the service on 04/02/2025 to address the temperature of the water. On that day the plumber fitted a thermostat to the sink and adjusted it to the maximum temperature of 42°C. Preventive Action 1. All staff have been retrained to know that we first wash in hot soapy water and then sterilise using Milton/Sterilising product made using cold water as per product instructions. The typing error in the policy has been corrected to read as per these instructions. Staff have been retrained and the policy has been reissued. Manager/Person in charge is carrying out unannounced spot checks to ensure the practice is adhered to. 3. On the day of inspection, the bin lid was incorrectly fitted after the bin had been emptied. This was corrected on the day. All staff have been trained on how to assemble the bin properly. Manager/person in charge will carry out spot checks to ensure this practice is in place. 4. The paperwork is now completed by the person who performs the physical check and not the other staff member in the room. All staff have been informed that the person who performs the check is to complete the paperwork

  • Corrective & Preventive Actions Safe Sleep: 1. It was communicated to the staff that using symbols instead of completing written documentation is not acceptable. All staff were informed of this policy. 2. During the staff meeting, management reminded everyone that pre-populated temperatures are not allowed and will be monitored regularly. General Safety: 3. In Playgroups 1 and 2, all sharp corners and edges were covered with foam for safety. Foam protectors were also placed on the sink unit in both Playgroups. The registered provider checked all the rooms for sharp edges and covered them with foam for added protection. Units are being replaced in all rooms over the coming weeks. 4. The CD player was removed from the room immediately. On 24/4/23, a meeting was held with all staff to discuss areas that required attention. It was emphasized that all staff should ensure that records are kept correctly going forward. Summary Comment The registered provider submitted details of the corrective and preventative actions taken in the service to address the non-compliances. Based on this information the non-compliances under Regulation 23 have been addressed. Implementation of the corrective and preventative actions will require assessment at the next inspection

  • Corrective & Preventive Action The registered provider engaged a plumber who checked and adjusted water thermostat controls. The staff will monitor the hot water supply and keep in contact with plumber and have him re check regularly. In-house training was given to this member of staff. The registered provider will spot check this staff member regularly to ensure that the nappy changing procedures are being followed. The staff will be observed to ensure that hands are washed between nappy changes, that the nappy changing mat is wiped down and that used nappies are bagged separately and disposed of in bin. Staff training was provided, and room leader and assistant is responsible for setting up and maintaining the position of the slide. they are responsible for setting up area and ensuring slide is in the recommended manufacture instructions. Checks of the position of the slide will take place. I have reviewed the manufacture safety instructions
