- General Safety: 1. There were no visibility strip/stickers on the glass door in the full day care room one which could be a potential safety concern. 2. A rodent bait box was in the children’s toilet in the outdoor play area which could prove risk of injury to a child. 3. A section of the wooden fencing in the outdoor area was worn and uneven with potential risk of injury to a child. 4. There were trailing wires from decorative outdoor lighting within child reach which could pose risk of injury to a child. 5. Three sections of the linoleum flooring in the full day care room 1 were worn and in poor condition which could pose a potential tripping hazard. 6. It was observed that one child’s medication was out of date and required replacement. Infection Control: 7. The soft seating in the toddler room and sensory room was ripped and frayed and the picnic table in the outdoor area had chipped paint and could prove difficult to effectively clean. 8. A flip top bin was in the sensory room for waste disposal. This could pose a risk of cross contamination. 9. A newly fitted wall mounted nappy changing unit in full day care room 2 posed a potential infection control issue as it was located over the children’s toilet. This unit must be relocated to an alternative space as the toilet was also in use by the children. Action submitted by the Registered Provider
Provider's corrective action:
- General Safety: The manager of the service submitted a written response to the office of the early year’s inspectorate to advise. 1. Appropriate visibility decals have been fitted to the glass door at suitable heights to ensure the glass is clearly visible to both children and adults. Glass doors will be included in routine health and safety checks. Visibility markings will be inspected regularly and replaced promptly if damaged, removed or no longer sufficiently visible. 2. The rodent bait box was immediately removed from the children’s toilet area and relocated to the adjacent boiler house which is inaccessible to children. All pest-control devices and locations will be frequently reviewed with our pest-control provider to ensure they are inaccessible to children. Pest-control points will also be incorporated into routine safety checks. 3. The affected section of fencing has been replaced. Outdoor fencing and boundaries will be included in documented outdoor-area safety checks. Any deterioration will be recorded in the maintenance log and prioritised according to the level of risk. 4. The decorative lights were removed immediately. Staff will ensure that any new decorative lighting or electrical equipment is installed with cables appropriately secured and inaccessible to children before use. 5. The affected sections were immediately assessed and temporarily secured to remove the tripping risk while arrangements are made for professional replacement of the flooring. Flooring condition will be incorporated into routine premises inspections. Damage will be recorded immediately in the maintenance log, risk assessed and escalated for repair/replacement according to priority. 6. The expired medication was no longer in use or required by the child and was disposed of. Medication expiry dates will be checked and recorded at regular scheduled intervals and when medication is received. Parents/Guardians will be contacted in advance where medication is approaching its expiry date to allow sufficient time for replacement. Any medication which is left in the service and no longer required by the child will be safely disposed of. Infection Control: 7. The damaged soft seating in the toddler and sensory room was removed and replaced. The picnic table was removed. We will ensure furniture and equipment are frequently checked, with any damage or defects being reported. 8. The flip-top bin was removed and replaced with a pedal-operated, hands-free waste bin. Suitable waste- disposal bins will be maintained in relevant areas and frequently checked. 9. The changing unit was removed and full day care room 2 use an alternative changing area located in a separate sanitary area away from any toilets. Nappy changing facilities will be reviewed frequently by management to ensure their location and use are appropriate

- General Safety: 1. There were low level branches from a large tree in the grassed area of the outdoor area which could pose a risk of injury to a preschool child. It is acknowledged that this area was not utilised on the day of the inspection as the grass required cutting. 2. There was a water play table in the outdoor play area with evidence of rust and flaked paint which could pose a potential safety concern. Infection Control 3. The lining on the base of sand play area was ripped and could provide difficult to effectively clean. A cover is also required for the sand play area to ensure the sand is protected when not in use. - General Safety: The person in charge submitted a written response to the office of the early years inspectorate to state; 1. Immediately following the inspection, the low-level branch on the large tree in the grassed outdoor play area was tied up with some rope, ensuring that it was safe for the children to play in the grass area. It was then cut back to eliminate the risk of injury. 2. Immediately following the inspection, the water play table was removed from use. The rust on the metal legs was thoroughly sanded down, any loose and flaking paint was removed, and the legs were repainted using a suitable, durable paint to restore the equipment to a safe condition. The table was inspected to ensure there were no remaining sharp edges, rust, or peeling paint before being returned to use. And to prevent a recurrence, a regular outdoor safety inspection has been incorporated into health and safety checks. Trees, shrubs, play equipment, and the overall outdoor environment will be routinely monitored for potential hazards, with any identified risks addressed promptly. This ensures the outdoor environment remains safe and compliant. Infection Control: 3. The damaged lining at the base of the sand play area was removed and replaced with a new, durable lining that can be effectively cleaned and maintained. A cover was also made and is now used to cover the sand play area whenever it is not in use, protecting the sand from contamination by animals, debris, and the weather and to ensure ongoing compliance, the sand play area has been incorporated into the regular health and safety checks. Staff will check the condition of the lining, the cleanliness of the sand, and that the cover is securely in place at the end of each use. Any signs of damage or wear will be addressed promptly to ensure the area remains safe, hygienic, and suitable for children's use

- General Safety: 1. The door to the stage in the main hall had no system of security posing a potential access/safety risk. 2. There were stackable chairs stored in the main hall which could pose a potential safety risk. Infection Control: 3. The children from the seomra beag room stored their full lunch bag in the refrigerator which could pose a risk of cross contamination. 4. Children’s snacks that were provided from home including perishable food items such as cheese, and yoghurts were observed to be stored in the main hall with an air temperature recorded at 16.8°C and therefore not kept below 5°C as recommended by the Department of Health and Children. Food and Nutrition Guidelines for Pre-School Service. 5. The pressure of water from the hot water taps in the children’s sanitary area was very weak with a temperature reading of 15.8°C which could prove difficult to enable effective handwashing. 6. The large digging area was not covered in the outdoor area which could pose a risk of exposure to animal excrement and infection to a preschool child. Action submitted by the Registered Provider

- General Safety: The person in charge submitted a written response to the office of the early years inspectorate to state; 1. A suitable locking mechanism has been purchased and installed on the stage door. The door is now secured when the stage is not in use, preventing unauthorised access by children and removing the identified safety risk. 2. Following the inspection, all stackable chairs were removed from the children's play area and stored safely underneath the stage to eliminate the potential risk straight away. Staff and all relevant community stakeholders who use the hall outside of childcare operating hours have been informed of the importance of storing chairs safely after use. Staff have been instructed to include the main hall as part of the daily operating health and safety checks, It has also been added to the services operating app so staff will check it on a daily basis and mark off its been completed daily. Any chairs left in the hall by external users will be removed and stored safely under the stage before children have access to this area. Community groups have also been reminded via the chairperson of the community centre, the importance of returning chairs to their designated storage area after use. Infection Control: 3. After the inspection, the practice of storing children’s full lunch bags in the fridge has stopped. The policy within the policies and procedures on healthy eating has been updated. The updated policy and procedures manual will be communicated to all parents and staff and will have to be signed off. Children are now asked to come in with their lunches inside labelled sealable bags before refrigeration. Staff have been informed to ensure this procedure is followed daily and to monitor compliance as part of the daily health and safety routine. This practice was implemented with immediate effect to ensure compliance with infection prevention and control requirements in the seomra beag and Fain Spéir room. 4. After the inspection a dedicated refrigerator has been purchased and installed for the Faoin Spéir outdoor room, which is kept in the main building. This is to ensure that perishable food items brought from home are stored below 5 degrees Celsius in accordance with the food and nutrition guidelines for preschool services. Children’s food is now stored in clearly labelled sealable bags, as per the updated policy for healthy eating policy. Staff have been instructed to refrigerate all lunch items when the children arrive for the day, each morning. The policies and procedures have been updated. Parents will be reminded on a regular basis via our services operating app. The routine will be monitored to ensure compliance with food safety guidelines. 5. The hot water system has been reviewed following the inspection. Significant improvement works had previously been completed, including the installation of solar panels, an EDDI machine and a water pressure booster pump, to improve both the hot water availability and water pressure throughout the service. Following the inspection, staff have been instructed that the first staff member on-site each morning will activate the hot water system to ensure adequate water temperature is reached before children arrive. This procedure was implemented with immediate effect. The revised opening procedure has been incorporated into the daily opening routine, and all staff have been informed of their responsibilities. Water temperature will be monitored as part of the daily health and safety checks to ensure warm running water is available to support effective handwashing. The registered provider will continue to review the effectiveness of the existing systems and explore any further improvements that may become available to the premises. 6. After the inspection a heavy-duty, UV resistant rot proof tarpaulin was purchased to cover the digging area when not in use. The cover is securely fastened each evening using the dedicated eyelets using hooks attached to the fence. The cover is securely fastened each evening after the children have finished playing and remains in place overnight to protect the area from contamination by animals and the weather. Staff roll back the tarpaulin each morning and secure it to the fence before the children access the digging area. The daily opening and closing procedures have been updated to include covering and uncovering the digging area. Staff have been informed to the revised procedure and are responsible for ensuring the tarpaulin is securely in place at the end of each day and removed prior to the children use each morning. The area will be monitored as part of the routine environmental safety checks to ensure it remains clean, safe and suitable for children’s play

- General Safety: The registered provider submitted a written response to state 1. The bin has been replaced in the preschool room. The service has updated cleaning sheets to include toys/equipment check. This will be signed off on a daily basis by the staff team/ member responsible. Equipment that needs replacing will be removed and replaced by the manager. We are aware of the hazards/injuries that can be caused by broken toys and equipment, so all staff will be reminded that if they come across any broken toys or equipment on a daily basis, they must be disposed of straight away and replaced or repaired promptly. Also risk assessments are carried out on a regular basis to ensure all outdoor equipment is checked to safeguard children’s safety. 2. The spray was removed straight away and is now stored in a press away from where children have access to it. The Service will review Safeguarding, Health, Safety and Welfare Policy, which specify the procedure for managing hazardous sprays for example - always keep them out of the reach of children etc. Staff have been informed of the Health and Safety Policy to prevent it from happening again

- General Safety 1. There were sharp edges on the corners of two of the raised planting beds, posing a potential safety risk. 2. The goal post in the outdoor play area was not anchored, and the netting was ripped which could pose a potential safety risk. 3. A storage press containing cleaning products in the junior ECCE room was not secured and a cleaning spray was stored within child reach in the Junior ECCE full day care room posing a potential safety risk. 4. There were four wall mounted chalk boards in the outdoor area which were broken/cracked and could pose a potential safety risk. 5. A play washing machine and plastic sink from the play kitchen in the junior ECCE room were broken with sharp edges exposed which pose a potential risk of injury to a child. It is acknowledged these items were removed on the day of inspection. The door on the play kitchen in the junior ECCE room was not secure. 6. An electric cable in the hallway between the junior ECCE room and the senior ECCE room was not securely attached to the wall and could pose a potential safety concern. 7. There was a black cable on the artificial grassed area, posing a tripping hazard. 8. In the senior toddler room, a section of the roof on the doll’s house was broken 9. A bag of coal was used to cover a drain in the outdoor area which could pose a potential safety concern. Infection Control: 10. The water temperature taken from the hot tap was only 12.5°C in the junior ECCE full day care room. 11. A section of the child sized couch in the senior toddler room was ripped which could prove difficult to effectively clean. Action submitted by the Registered Provider

- General Safety: The registered provider submitted a written response to advise 1. The planting beds have been rounded and they are sourcing covers to cover the planting areas 2. Specially purposed sandbags have been attached to the goalposts adding weight to prevent movement. Goal post nets have also been attached to the goals posts and made secure. Added goal posts to the outdoor risk assessment – sand level and nets to be checked during the outdoor assessment 3. Labels have been placed on presses containing cleaning products and door locks have been purchased and fitted to ensure they are inaccessible to the children at all times. Spoke with staff about the importance of safe storage and dangers of cleaning products – memo also sent to all staff with reminder to keep cleaning presses locked and secure. 4. Cable/fence area added to outdoor risk assessment for continual checks to take place and ensure grass area free of debris 8. The section of roof on the dolls house in the senior toddler room has been repaired. Cleaning sheets adapted to include reporting of broken/damaged play materials to manager 9. A drain cover has been fitted to the downpipe and secured to the wall, the bag of stones have been removed. Infection Control: 10. Temperature corrected at mains switch; water running at warmer and more appropriate temperature. 11. The couches have been repaired, and machine washable covers have been added to the couch also and this can be removed for cleaning. - General Safety: 1. An immediate action notice was issued in relation to blind cords in the toddler room, butterfly room and the preschool room. These were not secured and potentially accessible to the children. 2. There was a broken bench in the outdoor area with potential pinch points posing potential safety risks to a child. 3. A lath of wooden was missing from a wooden playhouse and a sharp plastic edge was exposed on a tricycle and posing a potential safety risk to children in the toddler playroom outdoor play area. 4. A large amount of plumber’s expandable foam had been used as part of repair works under a sink in the Preschool sanitary area and was accessible by children posing a potential safety risk to children. 5. A cupboard storing cleaning products under the sink in the toddler room and under the sink in the nappy changing area were not adequately secured posing a potential safety risk to children. 6. The tap on a play kitchen was loose, and a plastic toolbox as broken with potential pinch points posing potential safety risks to a child. Infection Control: 7. There were three couches in the dining area with large tears in the covering exposing foam and three couches in the butterfly playroom which were frayed posing difficulties in cleaning and potential infection control. 8. The waste bin in the nappy change area was not the required foot pedal operated bin and posed an infection control risk. 9. There was no soap provided at the wash hand basin in the nappy changing area to enable effective hand washing. 10. There was evidence of staining on the stainless-steel sink in the nappy changing area. Safe Sleep: 11. The cot mattresses on two cots in the dedicated toddler sleep room were propped with cushions underneath. Action submitted by the Registered Provider

- General Safety: 1. On the 28 November 2025 the person in charge advised the Inspectorate that the three blind cords have been secured and provided photographic evidence. 2. The wooden bench has been repaired. 3. The playhouse has been repaired. 4. The foam has been levelled off. 5. Locks have been fitted on all accessible presses in the care rooms. 6. The tap on the play kitchen has been secured, and the plastic toolbox has been disposed of. Infection control 7. The couches have been removed. 8. A new foot operated pedal bin has been ordered. 9. Five of the radiator covers in the preschool room 1 and 2 and one radiator cover in the dining area were in poor condition and damaged with the potential risk of injury to a child. 2. The sensory board was removed immediately due to rusted areas and put into storage with the intent of repurposing. The service will use durable, weather-appropriate materials going forward. Infection Control: 3. The nappy changing mat has been replaced. Changing mats included in daily morning risk assessments and a spare mat will be kept on site to allow for same day replacement. 4. The mechanical ceiling ventilation covers have been vacuumed and cleaned, and the ventilation covers will be included in our biweekly deep clean. 5. The frayed couch has been removed until a new cover has been sourced. Only wipeable materials that allow for effective cleaning will be used going forward. A wall mounted chalk board in the outdoor area was broken and could pose a potential risk of injury to a child. 3. Two radiator covers in preschool room 1 and preschool room 2 were not secured to the wall and could prove risk of injury to a child. Infection Control: 4. Children’s snacks that were provided from home including perishable food items such as ham, cheese and yoghurts were observed to be stored in the hallways with an air temperature recorded at 20.6°C and 20.7°C and therefore not kept below 5°C as recommended by the Department of Health and Children. Food and Nutrition Guidelines for Pre-School Service (2013) This was also a non-compliance on two previous inspections. 5. The large newly constructed sand play area was not covered in the outdoor area which could pose a risk of animal excrement and infection to a preschool child. Fire Safety: 6. It was noted that the last fire drill was carried out on 29th of May 2025.Fire drills must be carried out monthly to ensure the children are familiar with the procedure in event of an emergency. - General Safety: 1. On the day of inspection, at 13.08hrs the water temperature from a hot water tap at a sink in the playroom used by children was recorded at 65°C. This water temperature was above the recommended 43°C upper safety limit to prevent potential risk of scalding. It is acknowledged on the day that the registered provider contacted the plumber to turn off the hot water supply to this sink until the temperature could be adjusted to a maximum of 43°C. 2. A goalpost in the outdoor grass area, was not securely anchored posing a potential safety risk. 3. The plastic underlay on the gravel area in the outdoor area to the rear of the premises, close to the climbing frame, slide and playhouse was exposed and uneven and posed a potential tripping risk. 4. A section of the wire fencing in the outdoor area to the rear of the premises was uneven and not secured and posed a potential tripping hazard. Infection Control: 5. It is acknowledged that on the second day of the inspection the floor mats in preschool room two had been deep cleaned. 9. There were three soft toys in preschool room two which were heavily stained. It is acknowledged these were removed on the day of the inspection. 10. Children’s spare clothes were stored on the radiator cabinet in the children’s sanitary accomodation which could prove risk of cross contamination. 11. A large bag of natural crystal salt, and a wooden play kitchen unit was stored in the staff sanitary accommodation which could prove a risk of cross contamination. A cover on a four-inch pipe on the footpath in the outdoor area was not securely fixed and could pose a potential tripping hazard. 3. The window frame of the playhouse was broken with a sharp edge exposed which could pose a safety risk 5. The children were observed consuming popcorn which could pose a risk of choking. 6. - On 12th and 18th of March and 24th of April; the registered provider advised the office of the early year’s inspectorate General Safety: 1. The plumber has adjusted the system to limit the hot water temperature to a maximum of 42°C. and the energy diverter has been turned off, and the thermostat on the immersion heater has been adjusted. 2. A new metal cover has been installed. 3. The timber on the garden gate has been replaced. 4. The little green window on the playhouse has been removed. 5. A letter has been sent to parents to advise them not to send popcorn in the children’s lunchboxes and lunch boxes will be checked to ensure there is no choking hazards such as popcorn and it will not be provided as a party treat by the service. 6. The roof ridge tile has been removed. Infection Control: 7. A new goalpost and donut swing has been purchased. and we have added the above non-compliances into our Daily Health & Safety Checklist, so that we can carry out necessary repair and replacement in time if it’s required in the future. We will continue to monitor the solar panel system and keep a record of the water temperature to ensure the hot water temperature does not exceed 43°C

The door from the preschool room to the registered providers main residence had no system of security posing a potential access/safety risk. 3. The surface coating of handbasin waste in the children’s sinks were worn and sharp and could pose a potential safety concern. Outdoor area: There were safety risks in the outdoor play area to the side of the premises including 4. The artificial grass at the climbing frame and swing set was ripped and could pose a potential trip hazard. 5. A play wheelbarrow had evidence of rust with paint flaking, chipped, and worn requiring repair/ repainting. 6. A lath of wood was missing on one of the wooden gates and a section of wood was broken on the second gate in the outdoor area which could pose a potential safety concern. 7. A wall mounted chalk board was in poor condition with two sections missing posing a potential safety concern. Infection Control: 8. Perishable foods were stored in the children’s lunch boxes within the preschool room with the temperature recorded at 18.1°c. Perishable foods must be stored at a temperature below 5°c. 9. In the designated home area, a child sized couch had evidence of being frayed and torn, thereby preventing effective cleaning. 10. The nappy changing mat was torn and in poor condition proving difficult to effectively clean. 11. In the outdoor play area, a play sand was uncovered resulting in potential access by vermin /rodents. 12. - The registered provider submitted a written response to the office of the Early Years Inspectorate to advise General Safety: 1. The garda vetting forms have been updated and the staff files will be checked on a regular basis to ensure they are within date 2. A latch has been fitted on the door from the preschool room to the main residence and daily checks will be carried out to ensure it is secure. 3. The handbasin plugs have been removed and they will be replaced with new ones. Daily safety checks will be carried out on the premises and will include the sanitary accomodation. Outdoor area 4. The artificial grass will be replaced over the easter holidays when the children are not in attendance. The artificial grass has been included on the daily safety checks. 5. The rusty wheelbarrow has been removed and disposed of and the outdoor equipment will be checked daily and removed if rusty or broken. 6. The laths of wood have been replaced on the two gates, and these are included on the daily safety checks. 7. The chalk boards: have been removed from the garden and will do daily outdoor checks for any wear and tear of equipment Infection control 8. Perishable foods are now stored in fridge. 9. Couch ripped which has now been removed and will do daily checks of all school equipment to make sure it’s clean and free of any wear and tear 10. Nappy mat torn I have removed torn nappy mat and replaced with a new one and will endeavour to do daily checks on all school equipment for any wear and tear 11. The registered provider will ensure that the sand pit is covered when not in use. 12. Awaiting plasterer to re-plaster the area and will forward the evidence once complete
