General Safety: Perishable food items from the children’s snack and main meal were not stored in a fridge. They were kept in the children’s bags and stored at the room temperature of 19.5 °C taken at 12:50 pm on the day of inspection. Children were in attendance in the service from 9:00 am and the perishable food items were served at 12:45 pm. This is longer than the recommended limit of two hours unrefrigerated. This posed a risk to the safety of the children. Action submitted by the Registered Provider

Provider's corrective action:

General Safety: A new fridge has been purchased and will be used in ECCE 2 for the storage of children’s perishable food items. The children from ECCE 1 will have their perishable food items stored in the fridge in the kitchen. The healthy eating policy has been updated to reflect this change and all staff have been informed of this update

Little VIP's Pre-school, Castlebar · Regulation 23 — Safeguarding health, safety and welfare of child

General safety: 1. A large area of small particles of flaking paint and rust were visible on a radiator in the children’s sanitary area posing a safety risk to the children in the sanitary area. Infection control: 2. No nappy changing unit was available for the children who required nappy changing. The registered provider stated that children are changed on a mat on the floor of the sanitary area, posing a risk of cross contamination. 3. Three vinyl sofas in use in the rest/quiet area had foam exposed from loose stitching making them difficult to clean. Action submitted by the Registered Provider

General safety: 1. The radiator has been sanded and painted to ensure the safety of the children. Infection control: 2. A nappy changing unit has been ordered for staff to change children on to prevent cross contamination. 3. The sofas are in the process of being repaired. Management will conduct visual checks to ensure that the soft furnishings are fit for purpose

Louisburgh Childcare CLG., Louisburgh · Regulation 23 — Safeguarding health, safety and welfare of child

General Safety: 1. Three highchairs in use in the creche room had the shoulder straps removed, so children were not safely secured into the highchairs. Another child was in a highchair where the shoulder straps were available but only the waist strap was used. Infection Control: 2. Seven children in the creche room did not have their hands washed before eating their dinner. A baby wipe was used to wipe hands, but this is not an effective method of handwashing. General Safety: 1. New straps have been purchased and fitted on to the three highchairs in use in the creche room. Children will be fastened securely into the highchairs using the correct straps. Staff have been informed of this practice. Infection Control: 2. Handwashing before meals will be carried out for all children. Staff have been reminded of the policy on infection control and a written notice is now in place in the care rooms

The following non compliances may pose as a safety risk to children: General Safety: 1. There were two bolts on the fire emergency door out of the Toddler room on the ground floor. 2. A baby gate was blocking the emergency fire exit from the Junior room which may pose as a safety risk to children. The following may pose as a risk of cross infection for a preschool child: Infection Control: 3. At 10:35 in the nappy changing area off the Toddler playroom there were 2 training potties with urine in each of them. This was brought to the attention of the adults by the inspectors. 4. Two of five adults were observed at nappy changing and did not wash children’s hands after nappy changing. 5. In the Toddler room nappy changing area three barrier creams were not individully labelled with a child’s name and were in use. Safe Sleep: 6. During sleep time in the designated sleep area in the Toddler room the area was dark at the back of the sleep area. There was insufficient ambient light to allow staff to properly conduct the required physical sleep checks on sleeping children. This may present as a safety risk to sleeping children. General Safety: (1) Following the inspection the bolts were removed immediately from the door in the toddler room. The fire officer visited on the 27TH of May 2026 and assessed all fire safety arrangements. Staff have been reminded of emergency evacuation procedures and the importance of maintaining clear unobstructed emergency exits at all times. (2) The baby gate was removed immediately. Tir Na Nog Crèche & Montessori School Limited, Belmullet · Regulation 23 — Safeguarding health, safety and welfare of child

General Safety: 1. The staff inflated a small bouncy castle in the outdoor area during the inspection, and 3 to 4 children were observed to play on it. When the registered provider was informed of potential safety concerns the staff immediately removed the small bouncy castle from the area. 2. A window restrictor was absent on one of the windows in the early start room and could pose a potential safety concern. 3. The was no press lock under the sink in the early start room which could pose a potential access/safety risk as there were room sprays stored in these presses. 4. An electric cable from a stereo was within child reach in the sleep room and the early start room which could pose a potential safety concern. Infection Control: 5. The foot operated pedal bin was broken in the first friends’ room and required replacement, and a flip top bin was provided in the early start room which could pose infection control risk. 6. The nappy changing mat in the dedicated nappy changing area was ripped in places which could prove difficult to effectively clean. The nappy changing unit was not maintained in a clean condition. 7. There were numerous baskets of children’s play equipment to include plastic blocks, playdough accessories and wooden equipment stored on a high shelf in the children’s sanitary accommodation posing a risk of cross contamination. General Safety: The registered provider submitted a written response to state 1. The small bouncy castle was removed. Immediately and the service will not use bouncy castles in future. 2. A lock has been fitted to the window in the early start room, and this will be checked regularly. 3. New press locks have been purchased and fitted on the presses, and these will be checked regularly. 4. No plugs are used in these sockets any more we are now using wireless speaker for the relaxing music for the children to fall asleep to. Infection Control: 5. New bins purchased and these will be checked regularly to ensure they are in proper working order. 6. There was an extra changing mat in the changing area on the day, and it was replaced immediately. The changing mat will be checked regularly. 7.The storage baskets have been removed from the sanitary area and are now stored in the attic space

General Safety: 1. Garda vetting was available for the 6 staff members and the students on work experience. However, three of these vetting disclosures were not dated within the previous three years and were not in adherence to the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. A wooden plant box and a wooden pull along toy in the outdoor area were broken, a lath of wood was not secured on the wooden fence, and a wooden post had a rough surface and could pose risk of injury to a child. 3. General Safety: The registered provider submitted a written response to the office of the early years inspectorate to advise 1. Garda vetting has been reapplied for the three staff members, and these will be sent to the office of the early years inspectorate once processed. Regular checks will be carried out on the staff files to ensure vetting’s are in date. 2. The wooden plant box and the pull along toy have been removed from the garden area. The lath of wood was secured to the fence, and the wooden post was sanded off and is now smooth We will ensure that all outdoor areas are checked regularly for safety and security. 3. General Safety: 1. A staff member was observed entering the Toddler room in an abrupt or forceful manner. The door into the playroom was opened by a staff member without taking due care that the small toddlers could have been standing behind the door. This posed a risk of injury to the children if they were standing or sitting within reach of this door. 2. The front door was not secure when the inspectors went to leave the building. This posed a risk of a child exiting the building unsupervised or unauthorised entry to the building. 3. A trailing cord for a monitor was plugged in adjacent to a cot in the sleep room. The trailing cord beside a cot posed a risk of strangulation. The monitor was immediately unplugged and removed by the inspection team. 4. A drop rail on one of the cots was broken which allowed for the rail to be moved up and down by a child and could pose a risk of injury to a child. Action submitted by the Registered Provider

General Safety: 1. Signs have been put on all care room doors reminding staff and visitors to look before entering. Staff have been reminded of the need for caution when entering care rooms. 2. There is a large sign on the door asking for the door to be kept locked at all times. Staff have been reminded not to leave the door on the latch for any reason and to ensure it is always shut properly when they come in. 3. The monitor has been put on a high shelf with all cables out of reach. The risk assessment for the sleep room has been amended to include this. 4. The cot was removed and a replacement cot has been ordered. All staff have been reminded to advise management if they notice anything is damaged or broken and the room leader will check all the cots weekly as per the sleep room risk assessment

General Safety: • It is acknowledged that the outdoor area play areas used by the service were undergoing redevelopment at the time of inspection. This included the development of a sheltered enclosed sand and mud kitchen play area and a separate enclosed outdoor play area for the Naíonra. However, a section of the boundary wall in the outdoor play area next to a field was low and measured 73cm in an area posing a potential risk. General Safety: 1. Garda vetting was available for eighteen staff members. However, the vetting disclosure for one staff member was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. A gate in an outdoor area not used by the children during the inspection required additional measures to ensure security of the gate. An additional securing mechanism was placed on the gate by the designated person in charge during the inspection. 3. A cupboard lock was broken in the Waddler room which could pose a potential access/safety risk as there were paints in this press. 4. On the day of inspection at 11.38hrs, there was a temperature reading of 46.8°C, from the warm water tap at the wash hand basin, used by children in the nappy changing area. At 11.50hrs, there was a temperature reading of 47.3°C. These water temperatures were above the recommended 43°C upper safety limit to prevent potential risk of scalding. 5. The foot operated pedal bin in the Toddler room was broken, and a flip top bin was observed in hallway. Action submitted by the Registered Provider The service advised in writing that:

General Safety: 1. All staff files were reviewed on 11 March 2026 to identify expired or soon‑to‑expire Garda Vetting. Renewal of Garda vetting has been added to the mandatory annual staff file audit. A Garda Vetting tracking log has been created to record vetting dates and renewal deadlines for all staff. A risk assessment form has been created. 2. A securing mechanism was placed on the gate on 11 March 2026 to ensure it remains closed at all times during operation. Staff were informed of the updated procedure and reminded to check the gate as part of daily safety checks and during the day. A monthly premises safety audit will check that all gates are functioning correctly with any issues reported immediately to management and logged. 3. The broken cupboard lock was replaced with a secure child‑proof locking mechanism.. Staff were reminded immediately of the requirement to ensure all hazardous materials are always inaccessible to children and report any issue to management immediately. A monthly premises safety audit will now include inspection of all locks, gates, and storage areas containing hazardous materials. 4. The sink taken out of use on the day of inspection. The thermostatic controls were adjusted by the maintenance staff and will be inspected and serviced every 6 months or sooner if issues arise. Staff have been reminded of the requirement to test water before children use sinks. Any temperature outside the safe range must be reported immediately and recorded in the maintenance log. 5. The bins were replaced and staff were reminded that only foot‑operated bins are permitted in children’s areas for hygiene and infection control. Staff have been instructed to report any damaged or unsuitable equipment immediately using the maintenance log. A stock of spare foot‑operated bins will be kept on site to allow immediate replacement if needed

Infection Control: • The water from the warm water taps in both sinks in the children’s sanitary area used by the Preschool playroom was not warm and was recorded at 19.7°C at 11:00hrs where children were washing their hands at snack-time. The water temperature had increased to 23.1°C and 23.5°C when rechecked at 13:52hrs. This was a repeat non-compliance from the most recent inspection on 4 September 2025. General Safety: 1. A section of the flooring was absent in the sanitary accommodation off the junior preschool room which could pose a tripping hazard. Infection Control: 2. The Mulberry Tree (Kilmore), Ballina · Regulation 23 — Safeguarding health, safety and welfare of child

General Safety: 1. A trailing wire was accessible to the preschool children in the junior pre-school sessional room which could pose risk of injury to a child. 2. A fluorescent light cover in the senior preschool sessional room was observed to be cracked which could pose risk of injury to a child. 3. During the inspection, two bait boxes used for pest control were observed under the sinks in the Full day care junior room and in the full day care senior room. These boxes were accessible to the preschool children and could be a potential safety concern. 4. There were no step ups provided for the children to access the nappy changing units in the full day care junior sanitary areas which could be a potential safety concern necessitating the adults to lift the child onto the units which could pose a potential risk of injury. Safe Sleep: 5. On the day of the inspection, it was observed that measures to maintain a safe sleeping temperature between 18–22°C were not in place. The full day care junior room used for children's rest did not meet the recommended temperature range during the inspection period. The following room temperature recordings were taken on the day of the inspection: At 13.01 hrs, the room temperature reading was 22.3°C.and four children were sleeping At 13.17hrs the room temperature reading was 23°C and four children were sleeping. It is acknowledged, that on the day of the inspection, when staff were advised by the early years inspector of the high temperatures, the staff opened windows and the doors to enhance cross ventilation to reduce the temperature of the room during the sleep period. The registered provider submitted a written response on 8th of September to state 1. The trailing flex has been secured to the wall in the junior preschool sessional room and this has been added to the manager biannual checklist. 2. The fluorescent light cover has been replaced, and the light covers have been added to the managers biannual checklist. 3. Locks have been fitted to the cupboard doors in all of the preschool rooms and the locks on the cupboards have been added to the biannual checklist. 4. New step ups have been purchased to access the nappy changing units provided and these have been added to the managers biannual checklist. Safe Sleep: 5.Temperature checklists have been introduced, and the temperatures are checked at sleep/rest time. A new air con unit has also been purchased

General Safety: Perishable food items within the children’s lunches were stored at room temperature and not in the service fridge. The children entered the service with their lunch provided from their parents/guardians at 9am. The lunch was consumed at 11.35am. This exceeded the 2-hour time limit for the consumption of perishable food items not stored in a fridge set by the Food Safety Authority of Ireland. Infection Control: Liquid soap was not available in the children’s sanitary area. The use of liquid soap supports effective handwashing. A shared hand towel was used by a number of children to dry their hands. This poses a risk of cross infection. Action submitted by the Registered Provider

General Safety: Children’s perishable lunch items will be stored in the service fridge. Staff members will ensure that all perishable items are stored in accordance with the Food Safety Authority of Ireland. General Safety: 1. Whole grapes were present in a child’s lunch box. Whole grapes are a choking risk to a preschool child. 2. General Safety: 1. Whole grapes were removed from the lunchbox and chopped for the child. The service has revised the safe healthy eating policy and parents informed. Staff will conduct a lunch box check on arrival to the service. 2. The Healthy Eating Policy has been revised to not permit perishable food items in the children’s lunches. General Safety: 1. Garda Vetting was applied for immediately after the inspection for the staff member whose vetting had expired. All staff certificates and vetting are to be checked yearly at end of the school year and calendar reminders for updates to be set accordingly. The staff will keep a document checklist. Infection Control: 2. A basket is now left on the counter at the entrance area so parents and children can place the perishable food items into it. Once all children are in the service, this basket is placed straight into the fridge. Parents were also reminded to put any perishable food items into labelled clear plastic bags. At the start of the school year the staff will inform parents of the process and rationale for storing perishable items in the fridge. 3. The green couch with the tear was removed and a new one ordered. The staff will make sure to remove and replace any torn or broken toys/equipment. General Safety: 1. Garda vetting available for one staff member was not dated within the previous three years and not in adherence to the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. It is acknowledged that a copy of the required garda vetting renewal was submitted to the Inspectorate by the service on the 27 May 2025. 2. Water temperatures in a sink in the ECCE room measured at 46°C, this was not used as a children’s hand wash basin. This was brought to the attention of the person in charge on the day and the temperature was reduced. Infection Control: 3. Powdered formula feeds for children in the Wobbler playroom were reconstituted in the service with cool boiled water provided by parents. This was contrary to the current food safety guidance and was a repeat non-compliance from the most recent inspection on the 19 September 2022. 4. The were tears in the covers of seating in rest areas in the Wobbler, ECCE and Senior Toddlers room with foam exposed posing potential difficulties with cleaning leading to infection control risks. Administration of Medication: 5. A review of a sample of medication administration records found that they had not always been signed by two staff contrary to the service’s policy on medication management. It was unable to be confirmed if two staff had been present and checked the medication prior to administration to the child. General Safety: 1. The staff member’s garda vetting has been updated. Files will be reviewed at the end of each month to ensure that garda vetting is in date. 2. Information has been sent out to parents about sending in bottles pre-prepared for the children attending the service. Children’s bottles of formula will only be accepted in the service if it has been prepared. 4. General Safety: 1. Garda vetting was available for the nine adults. However, one of these vetting disclosures were not dated within the previous three years and were not in adherence to the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. A child had whole grapes which posed a potential choking hazard in their lunch food provided from home in one of the playrooms. When this was highlighted to the staff by the inspector, the grapes were removed, chopped and returned to the child. 3. Several pieces of equipment for the children were damaged with potential pinch points that could cause an injury to a child. These included a water table and a wooden play table in the outdoor area used by the Blooms playroom and a storage unit with a broken drawer runner in the Little Buds playroom. It is acknowledged that the service removed the tables on the day of the inspection. 4. The door to the emersion heater storage area with electrical controls and wiring was unsecured during the inspection with the potential for a child to access the area. Infection Control: 5. The waste bins provided in the large children’s sanitary area and staff sanitary area were push top lid operated and not the required foot pedal operated bins. 6. A small couch in the Blooms playroom had tears in the covering exposing foam posing difficulties with effective cleaning. Administration of Medication: 7. Medication in the Little Buds playroom was stored in a child’s bag at a low level with the potential for a child to access the medication. General Safety: 1. One of the windows in the upstairs playroom was not secured and presented a potential risk to children in the playroom. It is acknowledged that when this was brought to the attention of the designated person in charge that the window was immediately secured. 2. There was a rodent bait box was a sign stating that it contained rodenticide and it was accessible to children in the upstairs sanitary unit. 3. A number of play items in the outdoor area including a plastic tree house and a playhouse were damaged and had pinch points that posed potential risks of injury to the children. One of the playhouses was positioned over a drain cover that was not securely anchored in the ground posing a hazard to children playing in it. 4. There were gaps between some of the impact resistant ground cover tiles in the outdoor area and posed a potential tripping hazard to the children and adults using the area. This was also non-compliant on the most recent inspection of the 25 January 2022. 5. There were several old broken toys discarded in the grass section of the outdoor area. The tree house has been repaired and a playhouse has been disposed of. General Safety: 1. Garda vetting was available for five staff members. However, these vetting disclosures were not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI- RN12.3 Renewal of Garda Vetting’. 2. The door to the heating boiler in the outside area had rusted and was not fitted correctly in place with the potential for a child to access the boiler and controls. It is acknowledged that the registered provider refitted the door prior to the inspector leaving the service. 3. The securing mechanism on the gate leading to a section of the outdoor area no longer in use by the children was broken and required repair/replacement. Infection Control: 4. There were rips/tears in the soft seating in the rest area in the Full Day Care playroom exposing the foam posing difficulties for effective cleaning leading to a potential infection control risk. 5. The waste bins in the Preschool playroom and Preschool sanitary areas were not the required foot pedal operated waste bins. 6. A cot used for a child to sleep in had a stained cot mattress and did not have a waterproof cover in place. 7. Cleaning records for the main children’s sanitary area had not been completed by staff since the 16 April 2024. General Safety: 1. Garda vetting renewals have been applied for the staff members. The service is still awaiting return of garda vetting renewals for two staff members. The registered provider will forward email copies of the renewals soon as they are received. The staff will regularly check garda vetting to ensure that it is in date for all staff. 2. The door on the boiler has been refitted and has been painted with rust proof paint. The staff will regularly inspect the outdoor area for wear and tear and respond as needed. 3. The gate leading to the small outside play area has been repaired. The staff will regularly inspect outdoor area for wear and tear and respond as needed. Infection Control: 4. The soft seating that was ripped in the Full Day Care playroom has been removed. The staff will regularly inspect indoor area for wear and tear and respond as needed. 5. The waste bins in the Pre-school areas have been replaced with pedal bins. The staff will make sure all bins in the service are pedal operated bins. 6. The mattress in this cot has been removed. The service has not replaced it as the cot is not now being used. The staff will replace the mattress with a new mattress it if in time it is needed to be used cot again. The staff will carry out regular check all cots for stains and make sure they are fitted with mattress protectors. 7. Cleaning records are up now up to date. The staff will check that cleaning sheets are filled out every evening before closing

General Safety: 1. The recycled microwave positioned on the wooden mud kitchen in the outdoor area was not adequately secured and posed a potential safety to children. Infection Control: 2. The metal waste bin in the children’s sanitary unit was rusty in places posing difficulties for effective cleaning and required replacement. 3. A number of ride on toys and a stool in the outside area were rusty posing difficulties for cleaning the equipment effectively. 4. Sweeping brushes and mops used to clean the playroom area and staff belongings were stored in the nappy changing area and posed potential infection control risks. General Safety: 1. The recycled microwave has been moved to the ground where it does not pose a potential safety to children. Staff will carry out risk assessments of materials in the service to ensure they are safe for all children to use. Infection Control: 2. The metal bin in the children’s sanitary unit has been replaced. Bins will be replaced as needed and well maintained. 3. Three bikes with rust on them have been removed to be fixed over the summer and the stool with rust on it has been disposed of. Outdoor equipment is subject to rust, but the service will look at getting a preventative measure to ensure equipment lasts longer. 4. Sweeping brushes are now placed in the hall when not in use, out of reach of children. The mop handle is also stored there, and the mop head is replaced after each use so is not considered a potential infection risk. Staff will place their belongings in their car or in the kitchenette if needed

General Safety: 1. One of the entrance gates to the outdoor area was observed to be not adequately secured when the children were playing outdoors. It is acknowledged that this was addressed immediately when brought to the attention of the registered provider. 2. There was a rodent bait box accessible to children in the grass area of the outdoor space used by the children on the day of the inspection. 3. A large number of toys and equipment were stacked high on top of each other in the playroom and posed a potential safety risk to children. 4. The roof of a plastic playhouse in the outdoor area was damaged with sharp edges and was a potential safety hazard to the children. General Safety: 1. On the day the bolt was not in the lock properly. The staff secured this immediately once made aware of this. The registered provider checks the bolt every morning to ensure it is in the locked position. 2. The pest control company had visited the Tuesday previously and had left the bait box in view of children. The staff immediately moved the box out of view and reach. The designated person in charge rang the company that day to address the issue with them and ask them to ensure all boxes are out of reach and sight. The designated person in charge will also check this after each visit by the company. 3. Some of the afterschool boxes of equipment had been stacked too high. The staff immediately removed them and placed them in a safe area. All boxes will be stored in a low safe manner in future. 4. The plastic house was discarded immediately

General Safety: 1. The outdoor areas at the rear of the premises, that the staff advised the inspector were used by the children, led to the front of the school and had two open gates leading out to a busy road. The staff advised the inspector that before children used the outdoor areas, the staff undertake a risk assessment of the area to ensure that the gates are secured. However, there was no written risk assessment on file or written records to record that the gates are checked by the staff for security prior to use. It is acknowledged that the children did not play outside during the inspection due to icy conditions outside. 2. There were two rodent bait boxes accessible to children in the tarmacadam section of the outdoor area. Infection Control: 3. There were two waste bins stored in the tarmacadam section of the outdoor area and were potentially accessible by children when using this space.
