- Safe Sleep: 1. The inspector observed the sleep checks in the ‘seedlings room’ between 11:15 and 12:10. Children were checked through a viewing panel. However, no physical sleep checks were completed throughout this time. Contrary to the written sleep record where it was recorded at 11:25, 11:35, 11:45 and 11:55 the position, breathing pattern and the temperature to touch of the three children in the sleep room. - Safe Sleep: 1. Staff have been retrained in how to monitor children while they are sleeping including the 10-minute physical checks. - General Safety: 1. Detergents and a bait box were stored in an unsecure cupboard accessible to preschool children in the toddler sanitary area posing a risk of harm if accessed by preschool children. 2. Flexes and chords from air conditioning unit was loose and accessible to the preschool children in the toddler room. 3. The radiator cover in the upstairs preschool room was not secured to the wall, posing aa risk to the preschool child if it fell. 4. A large wooden toy car in the outdoor play area had worn in spots resulting in exposed sharp edges that could cause injury to a child. Infection Control: 5. The service had a system of storing soothers in individual containers, however inspectors observed on the day that a number of soothers were not in the individual containers and posed a risk of cross contamination. 6. Two mattresses in the baby sleep room were torn with exposed foam. These mattresses are unable to be cleaned effectively and pose an infection control risk. 7. Perishable food items were stored in the children’s lunch boxes on the shelf in the upstairs preschool room. 8. The walls in the downstairs preschool room were flaking making them unable to be cleaned efficiently. Administration of Medication: 9. Medication was stored in an unlocked fridge in the upstairs pre-school room, which was accessible to the preschool children which posed a risk of overdose if ingested. - General Safety: (1) An Immediate Action Notice (IAN) was issued to the registered provider on 13 March 2026 in relation to regulation 23 general safety, where two external fire doors were not appropriately secured to prevent children from exiting the service unsupervised. A written response was received on the 20 March 2026 to the IAN deemed adequate by the inspector. Following the inspection, a referral was made to the Chief Fire Officer in relation to the unsecured exits and the corrective action taken. (2) An Immediate Action Notice (IAN) was issued to the registered provider on 13 March 2026 in relation to regulation 23 general safety, in respect of water temperature of 66.3° in the sanitary area and accessible to the preschool children. A written response was received on the 15 March 2026 to the IAN stating that the water temperature was checked daily and within acceptable levels. This was deemed adequate by the inspector. (3) In the sanitary area exposed pipes and a water heating system was accessible to the preschool children. Infection Control: (4) Foot operated pedal bins were not in operation throughout the service This increases the risk of cross infection. (5) An accumulation of dust and cobwebs had developed in the sanitary area. Regular cleaning of sanitary areas can reduce the risk of cross contamination. Action submitted by the Registered Provider
- General Safety: (1) A referral was made to the fire officer, and any recommendations will be but into place. (2) Thermostatically controlled water system in place. Water temperatures will be checked daily, and a record will be kept. (3) The hot water pipes have been covered, and the water heating system is encased in a box and so no longer accessible to the children - the pipes that are on display are cold water pipes. - General Safety: (1) In preschool room 4 there was a Bait box accessible to the pre-school children which posed a risk of safety to the preschool child. (2) The Nappy changing room beside preschool room 4 was being used to store cleaning materials which were accessible to the preschool children being changed. (2) A new lockable storage cupboard has been built in this room to store all cleaning materials all the cleaning materials will be stored in the new lockable cupboard. (3) We have turned the windows into display boards due to the children consistently removing the visibility strips. This makes it more noticeable. Infection Control: (4) We have purchased 14 new waterproof cot mattress covers. All educators in the room have been reminded to report any need for new mattress protectors due to wear and tear, or failure to be returned by our laundry provider

- General Safety: 1. An Immediate action notice was issued to the registered provider on the day of the in relation to unsecure blind cords accessible to the pre-school children in the sanitary areas. On the day of inspection, the person in charge closed off the sanitary area as there was sufficient alternative sanitary facilities on the premises. Information received on 19 February 2026 confirmed that the blind cords were removed from sanitary area. 2. The sensory table in the outdoor area was left uncovered when not in use, posing as a risk of infection should vermin access it. 3. Low hanging wires were accessible to children from fish tank in the sensory room. These posed as a risk to children. 4. A radiator cover in the kitchen was broken, leaving exposed sharp wooden edges accessible to the children. This presents a risk of injury, such as cuts or splinters. 5. Large plastic boxes containing toys were stored on an insecure base in the kitchen accessible to the children. This creates a crushing or tipping hazard if the boxes or base were to shift. Infection Control: 6. On the day of inspection, after nappy changing, the hands of the babies were observed to be not consistently washed, posing a risk of infection. Gloves and aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care activity of placing on a clean nappy, followed with redressing the child. This could lead to a risk of cross infection. 7. Foot-operated pedal bins used for soiled nappies were not sealed, resulting in a strong malodour in the nappy changing area. This lack of containment poses an infection control risk 8. The fridge where children store their lunches was not clean. This poses a risk of food contamination and the spread of infection. - General Safety: 1. The bathroom was closed immediately on the day of inspection and evidence submitted to TUSLA the following morning that the blinds have been removed. The Service will ensure blinds are checked regularly for wear and tear. 2. The Tuff tray covered had recently ripped; we have removed it from use until the new cover arrives. All staff will be Remind that TUFF trays are to be removed from use temporarily if their cover becomes damaged 3. The fish tank has been removed from the area. (4) The windows in the bluebell room did not have restrictors in place and could pose a risk of injury to the children if they accessed the window and fell out. (7) No foot operated bins in the bathroom in the blue bell room which can cause a risk of cross contamination if the children do not have an appropriate facility to dispose of used paper-towels. Fire Safety: (8) A fire door in the bluebell nappy changing area, was observed to be held open with a wooden door stop. - General Safety: (1) An immediate action notice was issued on the day of inspection as two hand wash basins observed to be in use had water temperatures exceeding 43 Celsius. In the Willow sanitary accommodation, the temperature was recorded at 46.6˚ Celsius, and 45.1˚ Celsius in the Blossoms sanitary accommodation which posed a risk of scalding to the preschool child. - General Safety: (1) The registered provider issued a response to the immediate action notice issued on the 11 February 2026 which outlined the immediate steps taken to ensure that the taps were not in use until the issue was resolved. All areas have been fitted with thermostatic mixing values to regulate hot water and ensure that they are always at an appropriate level for the children to use. All of the hot water taps for wash basins and children’s sanitary areas are all fitted with these values now to ensure the safe use of them for the children

- General Safety: (1) Garda vetting was available for 10 staff adults. However, 2 vetting disclosures were not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. (2) On the day of inspection, it was observed that rodent control boxes were positioned in areas accessible to children in the preschool room. This presents a safety risk due to the potential for accidental ingestion or physical injury. Infection Control: (3) On the day of inspection, it was noted that the water temperature at the wash hand basins was not sufficiently warm for effective hand hygiene. The temperature was reading at 14.1◦. This presents a potential risk of cross-contamination. Action submitted by the Registered Provider
- General Safety: (1) Renewed garda vetting have now been submitted for the two staff members concerned, and confirmation will be retained on file. A tracking checklist has now been implemented to monitor Garda vetting expiry dates to ensure renewals are completed within the required timeframe going forward. (2) The rodent control boxes were immediately relocated to areas inaccessible to children. All bait stations are now secured in tamper-resistant containers and placed in approved locations in accordance with safety regulations. A full inspection of the facility was conducted to ensure no additional pest control devices are accessible to children. Proper placement and monitoring of pest control equipment has been discussed with staff. Our pest control company has been notified and instructed to coordinate with administration prior to placing any future devices. Infection Control: (3) A new water heater has been installed in the room to ensure an adequate supply of suitably warm running water at the wash hand basins. - General Safety: 1. In the Toddler room, a blind cord was unsecured. It is acknowledged that it was out of the reach of children. Infection Control: 2. Hand hygiene was observed not to be completed routinely in the following situations to prevent the risk of spreading infection: • Staff in the baby room were not observed to wash hands prior to nappy changes. • Children in the baby room did not have their hands washed after having their nappy changed. • Children in the baby room did not have their hands washed prior to mealtimes. 3. In the Toddler room a number of items posed a risk of contamination as follows: • Cushions in the rest area were stained and in need of cleaning. • The soft foam mats underneath the play slide were stained and in need of cleaning. 4. At sleep time in the Toddler room and tweenies floor beds and stackable beds were not spaced 50cm apart as required posing a risk of infection control. Safe Sleep: 5. Contrary to the service sleep policy cot mattresses were not checked regularly for signs of damage and replaced. Three mattresses were observed to be torn with exposed foam posing a risk of safety to the preschool child 6. In the toddler sleep room, the time and position of the sleeping child was recorded on the sleep chart. However, the colour and breathing pattern of the sleeping child was not checked or recorded on the sleep record. When asked Staff confirmed to the inspector that colour and breathing pattern is not checked. This is contrary to the services own Safe sleep policy which outlines the steps for carrying out sleep checks. 7. In the Toddler room during sleep time, the room was dark which did not allow for accurate sleep checks to be carried out. This is at variance with the services own sleep policy which states that there should be ambient lighting in the sleep room when children are asleep. 8. Seven children under the age of 2 years were sleeping on floor beds, without the required sleep plan and written parental permission. Action submitted by the Registered Provider
- General Safety: In a written response the registered provider stated: 1. The blind chord has been secured; these will be checked regularly. Infection Control: 2. Room meeting held with staff to remind them of the importance of hand washing 3. Cushions and foam mats were replaced as the stains proved impossible to remove. Regular cleaning will be completed to prevent stains reoccurring. 4. Toddler/Tweenie floor beds will be positioned 50cm between each bed to ensure appropriate spacing. Safe Sleep: 5. Three new mattresses have been ordered. Mattresses will be checked regularly. 6. The sleep chart has been adjusted and now records the colour and breathing pattern of the sleeping child. - General Safety: 1. The door of the sluice room was observed to be opened which contained chemicals that were accessible to the preschool children. 2. The radiator cover in the blossom room was broken with sharp wooden edges that were accessible to the children posing a risk of injury. 3. In the sensory room leads and cables were accessible to the preschool children posing a risk of injury. Infection Control: 4. On the day of inspection, after nappy changing, the hands of the babies were observed to be not consistently washed, posing a risk of infection. 5. Gloves and aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care activity of placing on a clean nappy, followed with redressing the child. This could lead to a risk of cross infection. 6. Two Children in the sunflower room were observed using soothers, it was noted that the children were sharing soothers. Additionally, when soothers fell on the floor, children were observed placing them back into their mouth without been cleaned or sanitized. 7. Waste bins in both the sunflower and bluebell rooms were broken, children were observed using their hands to open and close the bin lids, posing a risk of cross contaminations. 8. There was an insufficient supply of hot water across all preschool rooms. The water temperature in the bluebell bathroom was measured at 13.1◦C which is inadequate for effective handwashing. 9. A child-sized couch in the sunflower room and adult- sized couch in the blossoms room had exposed foam and could not be cleaned effectively. Action submitted by the Registered Provider
- General Safety: (1) Once this non-compliance was brought to the attention of the management at the inspection the manager ensured the door was fully secured and locked. The manager then proceeded to each care room that evening and the following morning to verbally remind each staff member the strict importance of ensuring that the Sluice room door is secured and locked at all times. During the staff team meeting held the following evening 04/11/2025, the manager reiterated to the staff Team the significance and importance of ensuring that the sluice room door is securely closed and locked at all times. Domestic/kitchen staff have taken on the role of ensuring the sluice room door is shut at all times these staff members are across the hall and are not bound to be in the rooms. All staff members assured management at the staff meeting to take extra care and vigilance to nsure the same.New signage was immediately stuck on the door as a reminder. The manager’s daily checklist now includes the sluice room door being securely locked. (2) The radiator cover was removed the next morning 04/11/2025.During staff meeting the staff were reminded to include such damages to radiator covers on the daily risk assessments so these can be addressed swiftly adressed and shown the importance of completing the daily risk assessment as these inform any potential risk to the children and to also verbally advice management of same. (3) Trailing lights and cables were removed from the sensory room until the electrician was available. Staff were reminded and shown the importance of completing the daily risk assessment as these inform any potential risk to the children and to also verbally advice management of same. Manager implemented an induction template for lead educator to complete with newly appointed educators. Peer mentoring template for nappy changing procedure has also been introduced to reinforce good practices in line with Little Buddies policies and procedures. This is to eradicate forgetfulness or oversight when completing these tasks which fall under these procedures. (5) At the staff meeting the nappy changing procedure including removal of ppe after each nappy change was reinforced via retraining, infection control was also included in this session of the staff meeting. Manager implemented an induction template for lead educator to complete with newly appointed educators. Peer mentoring template for nappy changing procedure has also been introduced to reinforce good practices in line with little buddies’ policies and procedures. (6) The Infection control policy was outlined and retrained to staff during a staff meeting and focus was on the procedures to follow when children share and drop their soothers. It was also looked at times when soothers should not be offered to the children, e.g. when playing. Lead educator Induction Template newly introduced includes Supervision of the children and infection control topics to be observe and monitored by the Lead educator. (7) Age-appropriate bins were purchased and supplied in the Sunflower Room and the Blubell room. During the staff meeting staff team were reminded of the importance of ensuring the daily risk assessment to include items such as the bins being broken. (8) Plumber and electrician were already sourced to fix this problem with water temperature. Plumber reduced the water thermostat 19th November 2025. Daily risk assessment informs and highlights any irregularities with the water temperature. (9) Child sized couch in sunflower room and adult couch in blossom room were both removed. Dress up area was extended into the space of the adult couch, which is demonstrated in the photographic evidence provided. The child sized couch in the sunflower room was replaced with beanbags until finances ensure replacement child sized couches. Manager reminded staff to please use the daily risk assessment to inform of risk management systems and outlined the importance of this to children’s safety and wellbeing. Management will closely monitor the completion of these risk assessments during the weekly sign off

- General Safety: 1. The radiator covers in the toddler room were broken posing a risk of injury to the preschool child. 2. Garda vetting was available for sixteen staff members. However, six of these vetting disclosures were not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 3. Medication in the toddler room was stored in a fridge in the room with no lock therefore accessible to the preschool child, posing a safety risk. 4. The mechanical ventilation in the wobbler and toddler rooms was not working correctly on the day of inspection. It was confirmed by the registered provider at closing meeting that they were broken and due for maintenance. Infection Control: 5. Children’s hands were not routinely washed after nappy changes as observed the wobbler and toddler room. This poses a risk of spread of infection. 6. - General Safety: 1. Cleaning products in the upstairs bathroom were not stored in a secure, locked location. These substances were readily accessible to preschool children, posing a significant risk of accidental ingestion or chemical injuries. It is noted that the press did have a lock on it but was not in use at the time of inspection. 2. A loose piece of timber with an exposed nail was identified within the upstairs bathroom. As this area is accessed by preschool children, the exposed sharp hardware presents an immediate risk of physical injury or puncture wounds. Infection Control: 3. - General Safety: 1. An Immediate Action Notice was issued on 04th November 2025 for unsecure blind cord that was accessible to the preschool children in the discovers room. A response was received from the service on the 05th of November with photographic evidence of the unsecure blind cord removed. 2. Garda vetting was available for 1 staff member. However, this vetting disclosure was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 3. The door of the kitchen with chemicals stored was not locked and children could potentially access the chemicals inside. 4. In the navigator’s room Children had access to pest control monitoring point in unlocked presses, material for baking and sensory play was also stored in the press, 5. Plastic storage boxes in the navigator room for storing toys were observed to be broken with sharp edges which posed a risk to the preschool children. 6. There was an exposed pipe in the sanitary accommodation area between Explorers and discoveries room posing a potential risk to children if they fell against it. This was observed on last inspection, and a wooden protective cover was put over it, however this was not securely attached to it, and the pipe was accessible to the preschool children. 7. In the sanitary area of adventure room 1, it was observed that a wall mirror was missing, leaving sharp tiles which could cause injury to the children. 8. A number of wooden items that the preschool children had access to in the outdoor play area were found to be in poor condition, this included the picnic table, the wooden border surrounding the mud kitchen and a wooden bracket fixed to the wall. Theres items showed visible deterioration including splintered wood which can cause injury to the children. In addition, weeds were significantly overgrown and encroaching from the neighbouring field into the children play area at the mud kitchen. Infection Control: 9. A child-sized couch in adventure room 1 was torn with foam exposed. This damage prevented the surface from being cleaned or disinfected effectively, posing an infection control risk. 10. Handwashing and personal protective equipment (PPE) protocols were inadequate to prevent the spread of infection. Staff did not routinely wash children’s hands following nappy changes. Additionally, staff were observed maintaining the same gloves and aprons throughout the entire process, including dressing the child and cleaning the changing area for the next child, rather than disposing of them and sanitizing between tasks. 11. It was observed that the walls behind the bins in the care rooms and sanitary areas were visibly dirty with a significant build-up of residue. As children have direct access to these spaces, this presents a risk of cross- contamination. Action submitted by the Registered Provider
- General Safety: 1. The blind cord was secured on the day of the inspection. A risk assessment was carried out that we could not satisfactorily prevent the cord from coming loose again. As a result, we determined the safest course of action was to remove the cord from the blind. Blind safety checks were conducted in all other rooms. All other cords were secured with no visible deterioration of means of security mechanisms. Blind cord safety has been reinforced with the staff team and is monitored as part of daily risk assessment of care rooms with any issues addressed immediately to maintain a safe environment. 2. A review of staff vetting records was undertaken. Re-vetting had already been applied for and has since been obtained for the relevant staff member. Records have been updated accordingly. A vetting and re- vetting tracking system has been introduced to ensure all staff vetting is monitored and renewed within required timeframes. Responsibility for oversight has been assigned to management, with vetting status reviewed as part of on-going staff record checks. Staff vetting records are scheduled to be reviewed by management on a quarterly basis, with upcoming re-vetting requirements identified in advance to support timely completion and continued compliance. 3. An adult-height safety latch was installed on the kitchen door to prevent unauthorised access by children. Clear signage was also put in place to remind staff that the door must be secured when the kitchen is not in use. Procedures for securing the kitchen door have been reinforced with staff and clarified via internal communication memo to ensure consistent use of the safety latch. Signage remains in place as a visual prompt, and responsibility for checking that the door is secured has been clarified to support safe practice. 4. The cabinet was secured using child-safe press locks to prevent access by children. Sensory play materials are no longer stored in the press. A supply of additional child-safe press locks has been purchased and is kept on site to allow for prompt replacement if locks become damaged. Cabinet security is monitored through daily risk assessments any issues identified reported to management and addressed promptly to ensure on-going child safety. 5. Broken storage toy boxes were removed from use and replaced to ensure safe and appropriate storage of play materials Staff have been reminded to visually check the condition of storage boxes as part of daily risk assessments. Broken boxes are to be immediately removed. A supply of spare storage boxes is now kept on site to allow for prompt replacement should damage occur in order to maintain child safety. 6. The pipe boxing was re-secured to the wall using fixings to strengthen its security in place, eliminating access to the exposed pipe. A daily check of the pipe boxing has been added to the sanitary area risk assessment to ensure any loosening or deterioration of the seal is identified promptly and addressed before the boxing becomes loose again. Any issues identified are reported to management and addressed promptly to maintain a safe environment. 7. Tile edging was fitted to cover the exposed sharp edge and eliminate the risk of injury. The condition of this tile edging has been included and is monitored through daily sanitary area risk assessments. Any issues identified addressed promptly to maintain a safe environment. A surplus of this edging strip is available to facilitate prompt replacement if needed in order to prevent the tile becoming exposed again. 8. The damaged wooden equipment was removed from the garden immediately to eliminate any risk to children. Overgrown weeds were trimmed back to ensure the outdoor area was safe and accessible for children. A system of regular outdoor risk assessment has been implemented, with room leaders assigned a designated day to check the garden environment. Any risks identified are documented and reported to management for review and action. With issues addressed promptly to ensure children’s safety. A hedge trimmer has been purchased to support prompt cutting back of future regrowth and prevent overgrowth. Any regrowth or overgrowth identified will be documented and reported as part of the garden risk assessment process. Infection Control: 9. The torn areas of the couch were initially repaired using waterproof vinyl patches to restore a wipe able surface and enable appropriate cleaning. However, after the patches were in place for some time the edges began to lift which impeded through cleaning. As a result, the couch has been removed and a new one ordered. Soft furnishings within playrooms are now checked as part of daily risk assessments to ensure surfaces remain intact, wipeable and suitable for effective cleaning. Any damage identified will be addressed promptly to maintain appropriate hygiene standards. 10. The service identified that although staff were familiar with the policy, there was insufficient visual guidance at the point of practice and a lack of structured monitoring to consistently verify implementation of the nappy-changing procedure across all rooms and staff. To address the non-compliance, a step-by- step nappy-changing procedure poster has been developed and displayed at all nappy-changing stations to support consistent practice. All staff involved in nappy changing have been re-briefed on the procedure, with emphasis on infection control, supervision, and maintaining children’s dignity. An internal communication memo circulated utilised to confirm staff understanding of and commitment to following the procedure. To ensure sustained compliance, an observation system has been implemented to monitor nappy-changing practice. Spot checks will be carried out on a regular and unannounced basis and reviewed by management to ensure sustained compliance, using a standardised observation form. Any issues identified will be addressed promptly through feedback, support, and additional training where required. Records of spot checks and actions taken will be retained as evidence of on-going management oversight and quality assurance. This area of non-compliance was also identified during the previous inspection. The service acknowledges that earlier actions were insufficient to fully embed consistent practice. The current measures strengthen governance through clear visual guidance, staff accountability, and structured management oversight. 11. The affected areas were initially cleaned; however, as this was not deemed sufficient, the walls were subsequently painted to restore an appropriate standard of cleanliness. A wipe-able protective wall coating has been applied behind bin areas to support effective cleaning and prevent future build-up, in line with manufacturer guidance for easy-clean, hygienic surfaces. Cleaning schedules have been updated to ensure these areas are included as part of routine environmental hygiene practices, and staff responsibilities have been clarified to support consistency

- General Safety: (1) In the fenced off swing area the wooden fence panels had exposed sharp edges and an exposed nail. This was accessible to the preschool children and may cause injury. (2) In the second outdoor area there was a damaged Wooden Bench. The bench has a hole and cracked wood, creating sharp edges and an exposed nail which may cause injury to a child.. (3) Broken decorative panel the yellow submarine-themed decorative board is chipped and broken at the edges, leaving sharp points that could cause injury to a children. Infection Control: (4) Children’s hands were not routinely washed after nappy changing, posing a risk of cross-infection, and staff were observed leaving aprons on after completing nappy changes, which could cause an infection control risk. Administration of Medication: (5) On discussion with staff, four children were identified to the inspector as requiring an individual care plan. It is acknowledged staff present were aware of each Childs requirements, however no documented care plans were available on the day of inspection. - General Safety (1) Children’s access to the fenced-off swing area was restricted on identification of the hazard. Once the damaged fencing panels were replaced, the children regained access to the area. The damaged fence panels were replaced, and the outdoor risk assessment checklist was updated to include weekly inspection of all fencing to ensure early identification of hazards. All staff have been reminded to report any damage or safety concerns immediately to management. (2) The damaged wooden bench was removed from the second outdoor area. The area was checked for any loose nails or debris, and children regained access to the area once all was deemed safe. All staff have been reminded to report any sightings of damaged or broken equipment or outdoor furniture. The bench was removed from the area until the cracked wooden panel has been replaced. The outdoor risk assessment checklist has been updated to identify damage, wear and tear, or hazards such as cracks, splinters, and exposed fixings. In the staff meeting, staff were reminded to report any sightings of hazards immediately to management. Any equipment or furniture that appears to be damaged will be removed from use within the same day. (3) Upon identification of the hazard the play area was made inaccessible to the children, once the damaged yellow submarine-themed decorative panel was removed and the new fencing panels were fitted the children regained access to that section of the play area. The outdoor risk assessment checklist was updated to include the inspection of outdoor displays, fixtures, and decorative panels to identify signs of damage, chipping, or wear and tear. All staff have been informed to notify management of hazards identified, which will be responded to appropriately on the same working day. Infection Control: (4) Upon identification of the non-compliance, all staff were reminded of the service’s nappy changing procedure. Staff were made aware of the need to wash children’s hands with soap and warm water after each nappy change and were reminded to remove and dispose of aprons immediately after completing the nappy- changing process. The step-by-step nappy-changing procedure has been updated to highlight the need to wash children’s hands with soap and warm water after each nappy change and for staff to dispose of aprons after each nappy change. The updated step-by-step routine is now on display in the nappy changing area. Administration of Medication: (5) Following the inspection, individual care plans were completed and documented for all four identified children.Copies of each care plan were placed in the children’s individual files and placed on display in the communal.kitchen area where relevant medication is stored, and in the children’s individual care rooms. All staff were informed of the contents of each care plan, including medical needs, emergency procedures, and location of medication, during the staff meeting. Care plans will be reviewed at least every six months or immediately following any change in a child’s medical or care needs, as informed by parents. For all new children attending Little Meadows parents are asked to declare any medical or additional care needs on the child’s record form. This will trigger the need to complete an individual care plan before the child begins to attend the service

On the day of the inspection, chemicals were observed stored on the fire alert panel in the main entrance hall and were not locked away, posing a potential risk to children's safety. 2. Whole grapes were observed in the preschool room without being cut, posing a potential choking hazard. It is acknowledged that when the inspector brought this to the attention of the staff members the grapes were cut. 3. Low level visibility strips were not present in the glass doors in the wobbler room posing a safety risk for the young preschool children. 4. A wooden kitchen in the wobbler outdoor area with a broken door, was posing as a risk to children. 5. In the outdoor area of the wobbler room there was a broken part of the gate and there was exposed sharp edges and posing a risk of injury to the preschool child. 6. In the rest area of preschool one soft furnishings were ripped with exposed foam which if ingested could injure the preschool child. Infection Control: 7. Hand hygiene was not completed for children after nappy changing, which could lead to cross- contamination. 8. The wooden tables in the preschool room were observed to be worn and in poor condition, making them unsuitable for effective cleaning. Administration of Medication: 9. No evidence of parents being informed of administration of prescribed medication which may lead to overdose. 10. Medical risk assessment had instructions indicating that a care plan should be in place. However, staff were unable to locate the care plan when requested. While it is acknowledged that the staff member present was aware of the steps to be taken should the emergency medication be required. This could pose a risk with unfamiliar staff. - General Safety: 1. The blinds in both the baby sleep room and the toddler/preschool room were not securely fixed to the wall. While the blind cords were out of reach of children, they had not been secured as required to eliminate potential safety risks. 2. A one-year-old child was observed being fed while seated in a tilted baby rocker rather than in a highchair, which does not comply with the required feeding practices for children of that age. 3. A plug socket accessible to preschool children in the baby room was observed without the required safety covering, presenting a potential hazard. 4. Low hanging wires were accessible to children from baby monitor equipment in the baby room. 5. In the baby room bathroom, the pipes to the boiler were exposed and accessible to the preschool children. 6. The flooring in the toddler room was worn and had visible holes, creating a potential trip hazard. 7. Several items in the baby/toddler outdoor area were in poor condition and required replacement. Two slides were cracked and broken, exposing sharp plastic edges, and had been temporarily held together with masking tape, posing a safety risk. Additionally, a basketball hoop had a large hole at the top. 8. A fence panel accessible to preschool children was broken, with exposed sharp edges, posing a potential safety hazard in the outdoor area. 9. Several items in the preschool children's outdoor area required replacement. The mud kitchen had eroded wooden sections with sharp edges exposed, posing a safety risk to children. 10. Two trampolines were observed to be in use in the preschool room which are prohibited as they were not prescribed as a therapeutic intervention for a child in attendance. 11. A rodent box accessible to the preschool children was observed in the Preschool toddler room. 12. The sand pit in the outdoor area was left uncovered when not in use, posing as a risk of infection should vermin access it. Infection Control: 13. There was no warm water in the baby changing unit as the boiler had broken, children were observed washing their hands at 11.41 with a water temperature of 12.7◦. 14. Children’s hands were not routinely washed before food or after outside play. Staff were observed cleaning children’s hands with baby wipes. 15. Foot operated pedal bins were not in operation in all the preschool bathrooms or preschool rooms; staff were observed using their hands to open and close flip top bins. 16. On the day of inspection, after nappy changing, the hands of the babies were observed to be not consistently washed, posing a risk of infection. 17. Aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care activity of placing on a clean nappy, followed with redressing the child. This could lead to a risk of cross infection. 18. In the baby room, mattresses did not have waterproof coverings as required, and one mattress was visibly discoloured. 19. Soothers were not routinely cleaned before use. The inspector was informed that children bring their soothers daily in their bags. Soothers were observed being taken directly from changing bags without covers, handed to children from a shelf, and used outdoors Safe Sleep: 20. A child aged 15months was observed being put to sleep in a baby rocker, this is contradiction to the service safe sleep policy where it states that all children under the age of two will have access to a cot. 21. Staff were checking and recording children's sleep every 15 minutes, which is not in line with the service’s own Safe Sleep Policy that states children should be checked every 10 minutes. Fire Safety: 22. Documentation reviewed during the inspection evidenced that preschool children were not consistently checked in and out of the service as required. At 11:23 on the day of inspection, no sign-in was available for any of the children in the Preschool room. The absence of accurate check-in and check-out practices may hinder the safe evacuation of children in the event of an emergency. Action submitted by the Registered Provider

The fence panel has been fixed and sanded to remove any sharp edges. The outdoor area will now be checked daily for any damage to the perimeter. 9. All items, including the mud kitchen, have been removed from the outdoor area to clear the space. We will check the area regularly and replace play items immediately as needed. 10. I have put individual equipment use plans in place for the trampolines, which were recommended by an audiologist to help calm specific children. These are stored out of reach, and we now require a signed parental consent form before any child uses them. 11. The rodent box has been moved to a new location that is completely out of reach of the children. It is checked by staff every morning before the children enters the room. 12. The sandpit is now covered after every use. A staff member also checks the sand for any vermin or debris before the children are allowed to play in it. Infection Control: 13. The boiler has been repaired to ensure warm water is available at all times. We now take a water temperature reading every morning to make sure it is working correctly. 14. All children's hands are washed with water after play and before eating. Staff will adhere to the handwashing policy at all times, and the policy is clearly displayed in the rooms. 15. All bins have been replaced with foot-pedal bins in all rooms. Only pedal-operated bins will be used throughout the service to maintain hygiene. 16. Children and babies will have their hands washed after every nappy change. Staff will follow the handwashing policy at all times, which is clearly displayed in the changing areas. 17. Disposable aprons will be removed and binned after every nappy change, before a fresh nappy is applied or the child is dressed. Staff have refreshed their training on nappy-changing procedures to ensure this is followed every time. 18. All mattresses have been fitted with waterproof protectors. These are checked frequently before use to ensure they remain in good condition and that the mattresses stay protected. 19. Soothers are now stored individually in their own containers on the shelf or in the child’s bag. They are sterilized before every use. Children are only given a soother if it has come from a clean container and has been sterilized before use. Safe Sleep: 20. Children are no longer put to sleep in baby rockers; all children are now placed in a suitable bed or cot for sleep. Staff have been briefed on the safe sleep policy, which includes checking on children every 10 minutes. The safe sleep policy is also clearly displayed for staff to reference. 21. Staff have been made aware of the safe sleep policy and children are checked every 10 mins in Ine with the policy. The safe sleep policy is on display. Fire Safety: 22. Children are checked in and out, and this is recorded daily. All children must be checked in and signed for before each session begins

- General Safety: 1. Garda vetting was available for one staff member. However, this vetting disclosure was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. The windows in the service did not have restrictors in place. In the ground floor preschool, furniture was placed directly under the window, which was a potential hazard as a child may use this furniture to climb up and out the window. 3. Visibility strips were required on the glass patio doors in the baby room in the event children did not recognise the glass and therefore a potential injury risk. This was brought to the attention of management at the closing meeting on the day of inspection. 4. Two of the spring ride-on bikes in the outdoor play area were found to have visible rust on them, posing a potential safety risk to the children Infection Control: 5. The hand hygiene and nappy changing procedures for preschool children were not consistently conducted in a manner that mitigated the potential risk of cross-contamination, as observed during the inspection Children’s hands were disinfected before and after meals using alcohol-based rubs and wipes. This practice contradicts the service’s infection control policy, which states that “hand sanitisers or alcohol- based rubs are not a substitute for hand washing if soap and water is available. Children’s hands were not washed after each nappy change, increasing the risk of spreading infection. 6. Children were put directly on the plastic changing table with no mat underneath them. 7. Although foot-operated bins were available in the nappy changing area, staff did not use them. Instead, nappies were disposed of in a bin that was not leak-proof, sealable, or easy to clean. Furthermore, toilet trained children had access to the contents of this bin, posing a significant infection control issue. 8. Children’s snack foods brought from home, which included meat and dairy ingredients, were stored in their bags at room temperature. While some lunches were packed in cooler bags, the day was exceptionally hot, raising concerns about the adequacy of temperature control for perishable items. Safe Sleep: 9. On the day of inspection, children under the age of 2 years were observed to be sleeping on a stackable bed. These beds are not suitable for children under two years. In accordance with best practice guidance a sleep plan is required for children under 2 years of age that do not sleep in cots. On discussion with staff no sleep plan was available for these children. 10. During the inspection, room temperature readings were taken at various intervals to assess compliance with safe sleep guidelines, which recommend maintaining a temperature between 16°C and 20°C to reduce the risk of overheating. Staff were proactive in their response, opening windows to improve ventilation, removing covers from sleeping children, and conducting constant monitoring to ensure the safety and comfort of the children. However, the temperature did not reduce below the recommend guidance. The recorded temperatures were 22.6°C at 01:01 PM and 22.1°C at 01:28 PM. Action submitted by the Registered Provider

During a staff meeting, the infection control policy was discussed and reinforced with all staff to ensure it is always adhered to. 6. All nappies will be disposed of in enclosed pedal bins, placed away from others and emptied regularly. 7. A mat has been placed on the changing unit for use during nappy changes, and the use of pedal bins will be maintained daily. 8. Children’s snacks for those attending sessional days are kept in cooler bags, while snacks for full-day attendees are stored in the fridge. In periods of high temperatures, all snacks will be placed in the fridge to ensure food safety Safe Sleep: 9. On the day of the inspection, a trial day was held for younger children to assess how they would manage a full day. A tyre in the outdoor area was secured to the ground with concrete and not covered in a shock absorbing surface material to protect children from injury. 4. A window restrictor has been ordered from the hardware shop but has not come into stock yet. All perishable items such as yoghurts should be stored in a fridge at the correct temperature. Infection Control: 5. Floor covering in the sanitary accommodation of both the children and adults’ sanitary area was flaking with large amounts of the floor cover worn away. Therefore, making cleaning ineffective and posing an infection control risk. 6. The sofa in the rest area of the room was torn and damaged with exposed foam, making the sofa difficult to clean effectively posing a risk of infection control. - General Safety: 1. Room temperature in the wobbler room was recorded at 24.3°c at 12:14, when brought to the attention of staff, windows and doors were open in an attempt to reduce the room temperature. 2. The sanitary area in the wobbler room was equipped with a ladder to assist the child to climb up to the nappy changing area. On observation the ladder provided was unsuitable for the age range of the children as they could not manage to independently climb up or down the ladder. 3. The door of the storeroom containing cleaning products next to the stairs used by the children from the Little learners care-room was found to be inadequately secured at on the day of the inspection. This was also a non-compliance on the most recent inspection of the 27 June 2022. 4. There was a large unsecured wooden branch with small metal hooks fixed into it positioned at a low level in the mud kitchen area. Several other small metal hooks were also positioned at low levels in the space and posed risks of injury to children. 5. Pieces of bark had come away from a wooden piece of equipment in one of the outdoor play areas leaving splinters exposed posing a risk of injury to children. Infection Control: 6. Infant formula for children was provided from home in powder form resulting in a risk of cross contamination. 7. Nappy changing procedure were observed not to be in line with infection control guidance or the service policy. Gloves were not routinely removed after changing a soiled nappy and used to dress the child posing an infection control risk. Staff did not routinely use the foot pedal of the waste bin, using their hands to open and close the bin posing a risk of cross contamination. 8. Spacing between cots in the wobbler sleep room did not meet the required 50cm spacing between four cots, measuring at 34cm and 36cm. This poses an infection control risk. Fire Safety: 9. Portable toilets had been positioned outside in front of the emergency exit doors to the rear of the building located at the bottom of one flight of stairs leading from the first floor. These portable toilets posed a potential hazard for people in the event of an emergency evacuation using this emergency exit. 10. The sleep room had a quantity of equipment including boxes of toys, spare waste bins and cardboard stored at the bottom of the sleep room which could potentially pose a fire risk. - General Safety: 1. Staff will open windows and doors regularly. The fan extractor above the door has been turned to the cool air and another fan was also provided. Staff will check the room temperature in the morning and open windows and doors regularly. 2. New steps were provided to assist the children to climb up to the changing area safely. The steps will be maintained. Staff will also support and assist the children using the steps. 3. The overhead close shutter on the door in the storeroom has been tightened, with the door now closing securely. The door of the storeroom will be checked and maintained regularly. 4. The unsecure wooden branch and small metal hooks have been removed. All toys, equipment and natural material will be secured safely and appropriately. 5. The wooden piece of equipment has been sanded down to remove loose pieces of bark. Natural material and equipment will be checked and maintained regularly and will be removed and replaced when necessary. Infection Control: 6. Parents/guardians have been asked not to bring in baby formula from home. If for any medical reasons bottles need to be made regularly parents will be asked to state it in writing. This will be made up in line with best practice. Registration forms and parent packs will be updated to include information on baby formula. 7. The correct procedure for nappy changing were discussed at a staff meeting and staff were provided with a copy of the policy and procedures. Staff will read and familiarise themselves with the nappy changing policy and procedure. 8. The cots have been measured allowing 50cm spacing in between each cot. The 50cm spacing in between each cot are marked to ensure the cots remain the correct width apart. Fire Safety: 9. The portable toilets outside an emergency exit to the rear of the building has been removed. The service will ensure fire exits are always kept clear of obstruction. 10. Equipment including toys, spare waste bin and cardboard stored in the sleep room has been removed.
