  • (k) The immediate action taken by the service to address the non-compliance included obtaining the correct dates for the children's records where details were missing and ensuring that the incident records were signed by both the manager and the parent/guardian. In addition, the remainder of the incident record book was reviewed and completed accurately, ensuring that all required information was recorded correctly and in full. To prevent this non-compliance from reoccurring, all staff attended a refresher session on the correct completion of the Incident Record Book on 3 July 2026. Staff were reminded of the importance of recording all required information accurately and were given the opportunity to complete a practice incident record to ensure they understood the correct procedures

In addition, an administrator will be recruited to support the management team with the organisation and monitoring of documentation and record-keeping systems.   • (k) In response to the non-compliances the registered provider has stated that all previously missing signatures and dates for the incident report books have now been completed. A revised process has been introduced to ensure that all forms have been completed accurately. Additionally, we have noticed that signatures are not transferring clearly to the second page when parents are given the first page. We will write firmly and check that the signature has transferred correctly before handing the top copy to the parents. A staff meeting was held to reiterate the importance of promptly informing the manager of all accidents and incidents and ensuring that all relevant details are accurately recorded on the board and in the accident books.   • Corrective Action (1)(i) The service acknowledges that staff sign -in records were not consistently maintained on the day of inspection with eight out of nine staff members not recorded as signed in. Immediately following the inspection, the service reviewed attendance documentation and reinforced the requirement for all staff to sign in and out on arrival and departure. A staff sign-in record has now been fully implemented and is being actively used to ensure accurate daily recording of staff attendance. Preventive Action (1)(i) The service has strengthened its procedures regarding staff attendance recording to ensure full compliance going forward. A mandatory staff sign-in/sign-out system has been introduced, and all staff have been informed that completion of the attendance record is a requirement at the start and end of each shift . These signed forms have been placed on each individual staff members file. To prevent a reoccurrence of this non-compliance, medical forms have been updated to include management as the second signatory needed on file when administering medication.   • (k) Corrective Action: Immediately after the Inspection; • The Service purchased new Accident & Incident Books. Their aim is to have a book in each class for ready access. However, the layout of the forms in new Acciden & Incident Books is slightly different to the old Accident and Incident forms and therefore it was decided to hold off on putting new books into the classes until training took place (Status: completed 21 November 2024). • Between the time that the Tusla Inspection took place and the receipt of the Tusla Inspection report on 13/12/24, the service placed particular emphasis and attention on the management of our Accident and Incident system and initiated daily manager checks of the Accident & Incident Book from 25 November 2024 for the weeks commencing 25 November, 2 December, 9 December and 16 December 2024. This also allowed us to identify particular areas of weakness. • From week commencing 16 December 2024, a new Accident & Incident Book was placed in each class, to allow for ready access by staff when needed (Status: completed 16 December 2024). • A sample Accident & Incident form was completed, with notes highlighting where particular care was needed when filling in the form. This sample form was used for training on 19 December 2024 and was stapled into the front of the Accident & Incident Book for continuous ease of reference by staff. (Status: completed 19 December 2024). • Signage was placed in each class to act as a reminder/prompt to staff about ensuring all required data filled in. (Status: Completed 19 December 2024). • Staff in-house training took place on 19 December 2024 where each staff member was shown how to fill in the form, the importance of each field of information, the need to ensure that date of birth is filled in for the child as the form is being filled out and that all relevant parties sign off and date the form on the day of the accident/incident. Staff signed and dated the Sign off sheet confirming they had received this training. (Status: Completed 19 December 2024). (k) Preventive Action: • The manager, and in their absence the deputy manager, will carry out daily and weekly checks of the Accident and Incident Book, to make sure that the forms are being fully completed and accurately (Status: Ongoing) • The service has raised internal awareness of our Accident and Incident Reporting and Management by making it an actionable area on our Core Funding Quality and Inclusive Practical Plan (QIPP). We submitted our QIPP on 12 December 2024 and as part of our QIPP we will: Focus on the role of the manager in ensuring that o ALL accident & incident forms are completed 100% correctly. o ALL parents sign and date the accident & incident forms on day of accident/incident. o ALL sheets are signed off by manager or in her absence deputy manager. o Training will be initiated with staff immediately if it is found in the course of daily inspection that any of the above are not being achieved.   • Corrective Action (1)(g) The service inclusion policy wasn’t on the premises at the time of the inspection as it was being reviewed as part of the Quality Improvements processes. (k) Staff were re-trained in the completion and maintenance of accident / incident reports and the importance of completing them correctly and having parents signing them was highlighted. This is now being done through the service management system using a software package. Preventive Action (1)(g) Staff and Managers have been informed that Copies of the Policy Documents cannot be removed from the premises/facility. If people need a copy for work outside the premises a request should be sent by email and a copy of the document will be sent.   • Corrective Action (h) The registered provider had a meeting with the team post inspection the importance of accuracy when roll taking was spoken about. Particularly in relation to Fire & evacuation. The attendance roll must reflect the correct number of children on the premises at all times. After the meeting each staff member re-read the points of the policy and were given an opportunity to ask any questions. Once everyone was of agreement and they understood this policy it was signed and dated by each staff member. (i) The registered provider is now back on ratio in Pre-School room and this will be reflected on roster going forward. (j) Post inspection the service reverted back to using physical medicine administration books as the electronic application enables too many inaccuracies. The Service found a new Medicine Administration book and an Accident/Incident book in basement so started using these straight away. The service has also purchased a further two new books to have at hand if needed. Staff were all re-introduced to these books and as a team they all went through what was needed to be filled out in the best interest of the child’s health and safety and also what would be needed to keep up with compliance. (k) The service has reverted back to physical Accident & Incident record form books as it will provide more accurate detail that was previously missing. Parents will be notified on the electronic application that an accident/incident occurred and they will be provided with a copy of the physical form at pick up time. After parent is satisfied with all information on it, it will be signed and dated by both parent and manager. Parents can then take their copy home with them and a copy will be left in the book for service files. Preventive Action (h) The Manager will periodically check numbers in rooms on daily basis against roll on electronic application to ensure numbers are correct and there are no oversights with roll taking. Continue to train staff on ‘Children’s Roll policy’ on a regular basis. (i) The deputy manager will run weekly roster by the registered provider to ensure they are added to it each day/week before posting to notice board. (j) Now with use of physical books the registered provider can more easily check that all dates and signatures are in place before the final sign off of forms. This will eliminate any missing information on forms going forward. Staff training on the correct procedure for filling out such forms will be addressed at each Team meeting going forward. Following feedback from parents, the service have decided to use a hard copy administration of medication record book. The pre consent will be signed at drop off if parent brings in medication that needs to be administered. The parent will sign the acknowledgement of medicine administered when they collect their child. If a child needs anti febrile medicine during the day they will call the parent and they will need to sign pre-consent on the online software application system, and it will be noted in the medication record book. Then the parent will sign the acknowledgement of anti-febrile medicine at collection time. The service also maintains pre-consent for anti-febrile medicine on all children’s child records forms that attend the service. Preventive Action The service have briefed the staff on the use of the hard copy administration of medication book. They sent out a document to parents and staff regarding the change from online software to a physical book. Room heads and management will ensure pre-consent and administration are signed by staff and parents
