- 1(i) On arrival to the service there was no staff roster available to determine what hours and days staff members were working
Provider's corrective action:
- A staff roster is now in place for the service. The registered provider will ensure that the staff roster is maintained on a daily basis

Sandys Creche and Montessori School, Dunshaughlin · Regulation 16 — Record in relation to pre-school service
- (1)(j) Not all medication administration forms maintained in the service were complete. A sample of 16 medication administration records were reviewed on the day of the inspection. Out of the 16 forms reviewed, 8 forms did not include a parent initial to show written parental consent. Examples of this practice included forms maintained in relation to medication administered in the service on 09/07/2025, 11/07/2025, 15/08/2025, 03/02/2026, 03/02/2026, 04/02/2026, 04/02/2026, 05/02/2026. In addition, out of 16 of the forms reviewed, 7 forms did not include a second staff member’s signature to show that the medication had been appropriately checked, and the procedure undertaken by 2 staff members. Examples of this practice included forms maintained in relation to medication administered in the service on 19/05/2025, 20/05/2025, 21/05/2025, 09/07/2025, 01/12/2025, 01/12/2025, 05/02/2026. A non-compliance identified under Regulation 16 in relation to medication administration forms was identified at the last inspection on the 29 April 2025. The registered provider provided written assurances in the corrective and preventative actions that procedures had been put in place to prevent a recurrence of the non-compliance; however these were insufficient to ensure that the non-compliance did not re-occur
Provider's corrective action:
- Corrective Action (1)(j) After inspection a staff meeting was held, this topic was discussed. Each staff member was told how to fill in the medicine book appropriately and that all documentation was to be filled in accordingly both staff and parent. Preventive Action (1)(j) Room leader or management will do weekly checks

Happy Days Creche & Pre School, Rathmoylon · Regulation 16 — Record in relation to pre-school service
- (i) The staff roster available did not reflect the staff present on the day of inspection. A staff member was rostered to be present and was on statutory leave for a number of weeks previous, confirmed by the manager. The roster did not reflect the month or the days of the week, it had names and times beside staff but no confirmation of dates, therefore it was not transparent which staff were present each day
Provider's corrective action:
- The service acknowledges that the staff roster available at the time of inspection did not accurately reflect the staff present. It also did not clearly indicate the relevant month or days of the week, and included a staff member who was on statutory leave. The staff roster has been fully reviewed and updated. A new rostering format has been implemented which clearly outlines the full date, including month and day, as well as the names, roles, and working hours of all staff scheduled to be on the premises. Staff who are on leave are no longer included on the active roster for the relevant period. An accurate, up-to-date roster is now maintained on site at all times

Lámha Beaga, Stamullen · Regulation 16 — Record in relation to pre-school service
- (1) (h) It was observed upon the inspectors arrival at 9.28am that there were 16 children present in Seomra Dearg. Upon review of the attendance register at 10.24am the inspector found that there were only 15 children’s details completed in the attendance register. The inspector notified the person in charge at this time that there were only 15 children recorded on the attendance register but 16 children were present. The person in charge reviewed the records and found that one child had not been recorded in the attendance register and confirmed that the child had commenced in the service on 01 September 2025
Provider's corrective action:
- Missing name from attendance register was added immediately, along with parent details etc. We explained to inspector that this child only confirmed their return to pre-school after we had completed the attendance register details, which would explain the over sight. Register will be checked by more than 1 member of staff before we commence our year start every August

Oak Childcare Ltd, Stamullen · Regulation 16 — Record in relation to pre-school service
- (h) Records reviewed did not adequately reflect the attendance of each child on the day of the inspection. One child in the Back Preschool room had not been signed in upon their arrival. It is acknowledged that when the inspector brought this to the attention of staff at 10.30am, the record was updated
Provider's corrective action:
- Registered provider and person in charge reviewed the signing in process with the team. It was acknowledged that internet issues can at times cause issues with signing in. Internet booster ordered to ensure team can use the tablets for sign in more efficiently. Deputy manager assigned to reviewing attendance records every half hour in the morning to ensure all children are checked in on time. Deputy manager has taken on responsibility for additional checks on attendance as part of morning operational procedures. Internet booster in progress for installation to mitigate any technical issues. Internal review of digital application to see if this is fit for purpose moving forward

Little Scholars Creche, Dunboyne · Regulation 16 — Record in relation to pre-school service
- (1)(i) Three staff members had not signed in on arrival to the service. The last sign in recorded for 2 staff members was 27/09/24 and for 1 staff member 18/10/2024.Staff members must sign in and out on a daily basis
Provider's corrective action:
- (1)(i) All staff members attended a staff meeting and where informed that they must sign in and out when entering and leaving the service. All staff members were given a copy of the staff handbook to read and to sign off on. Staff are now using a roll book to sign in and out. The manager will ensure that this practice is implemented on a daily basis
