Cookies' Early Learning Centre Ltd, Cellbridge · Regulation 16 — Record in relation to pre-school service
- (i) While it is acknowledged that a staff roster was available, it was not adequately detailed to evidence planning for sufficient cover to maintain the required staffing levels. The roster did not include four staff members who were present on inspection. This non-compliance was present on the last inspection on 14 January 2026. The corrective and preventive actions as stated by the registered provider did not prevent recurrence of this non-compliance. (k) A sample of 10 accident and incident records were reviewed and had been completed. However, an incident form had not been completed for an incident where a child exited the premises unsupervised on 11 May 2026. The person in charge and deputy person in charge stated that they were unaware that an incident form was required for this incident. This is at variance with the accident and incident policy in place which stated that all accident and incidents are recorded in an accident record sheet
Provider's corrective action:
- (i) Following the inspection, the Registered Provider completed a review of rostering procedures and identified that the daily roster did not accurately reflect all staff members present within the service. The roster template has been revised to include all employees, relief staff, students, agency staff and any other adults working within the service. Management reviewed current staffing records and ensured that daily rosters accurately reflect staff attendance and room allocations. Staff responsible for preparing and maintaining rosters have been reminded of the requirement to ensure that rosters accurately reflect staffing arrangements and are updated where changes occur. To ensure compliance with Regulation 16(1)(i), the Registered Provider has implemented enhanced roster management procedures. A revised Daily Staff Roster Template has been introduced which records all adults present in the service, including permanent staff, relief staff, students, agency staff and management. The daily roster will identify staff room allocations, working hours, break cover arrangements and staffing deployment throughout the day. The Person in Charge or designated responsible person will verify the accuracy of the roster each morning and update it immediately if staffing arrangements change during the day. Monthly audits of staff rosters will be undertaken by management to ensure compliance and identify any discrepancies. Roster compliance will be included as a standing item during management reviews to ensure continued oversight and adherence to regulatory requirements. (k) Following the inspection, the incident involving a child leaving the premises unsupervised on 11 May 2026 was formally reviewed and an incident record was completed and retained on file. The Registered Provider reviewed accident and incident recording procedures with the Person in Charge, and all staff to ensure clarity regarding the requirement to record all accidents and incidents in accordance with the service policy. A review of recent accident and incident records was completed to ensure that all reportable incidents had been appropriately documented. To ensure compliance with Regulation 16(1)(k), the Registered Provider has strengthened accident and incident reporting procedures throughout the service. Refresher training has been provided to management and staff on accident and incident reporting requirements, including examples of incidents that require formal documentation. The Accident and Incident Policy has been reviewed with all staff and signed acknowledgement records will be maintained on personnel files. An Accident and Incident Reporting Checklist has been introduced to support staff in determining when an incident record is required. All accident and incident records will be reviewed weekly by the Person in Charge to ensure incidents have been appropriately documented and followed up. Accident and incident reporting will be discussed during staff meetings and supervision sessions to reinforce understanding and ensure consistent implementation of procedures. Quarterly audits of accident and incident records will be completed by management to monitor compliance and identify any gaps in recording practices. Staff refresher training completed by July 17th (in full), they began week of June 22nd with KCCC and Better Start week of June 15th for Quality Development- whole setting. Weekly reviews commenced immediately. Quarterly audits ongoing thereafter. An Accident and Incident Reporting Guidance Leaflet has been developed and issued to all staff. The leaflet clearly defines what constitutes an accident and an incident, outlines reporting responsibilities and provides examples of events that require formal documentation. - (1) (j) Although records were available detailing medicines given to children, signed parental consent was not always available. Medication records were sampled in the Wobbler room. There was no signed parental consent available in relation to medicines that had been administered to four children dated from October 2025 to April 2026. There was no consent available for medicine stored in the Pre-school 1 room for a child who may require it due to an underlying medical condition. One of the managers was unaware of the requirement to obtain written parental consent prior to administering medication. (1) (k) It is acknowledged that records were kept of accidents and incidents. However, the inspectors observed that the risk management section of records which was completed for two incidents that occurred on Day 2 of the inspection was not adequate. Details regarding the incidents are documented under regulation 19(1)(b) – non- compliance number 1. (a). The inspectors observed the incidents likely occurred due to overtiredness and slipping while dancing in socks. The measures documented to reduce the risk of reoccurrence was “We talk to her that we need to be careful when we jump” and “We talk to him to be more careful and not to play in the bed”. Failure to identify risks and take appropriate mitigating measures places the children at an increased risk of accidents and injuries
Provider's corrective action:
- (j) All our services were instructed on 29 April 2026 to replace medical consent forms with the new one which includes a section for signed parental consent. (1)(k) A sample accident was given to all staff for them to see what we should be putting onto Accident/Incident forms and signed by all staff. The service manager and assistant manager will monitor this is being completed

- (i) The staff roster was available on the day of inspection but did not reflect the adults working in the service. This was found non-compliant on the last inspection dated 24th March 2025. The corrective and preventive actions taken by the registered provider did not prevent the non-compliance from re-occurring
Provider's corrective action:
- The service promptly reviewed and updated the roster after staff swapped shifts due to an emergency hospital appointment, ensuring the documentation accurately reflected all adults present on the day of the inspection. A standard daily roster is in place which includes all staff, students, agency staff, relief staff, and management personnel present in the service. The weekly roster is emailed to staff every Friday and will be updated immediately to reflect any changes due to emergencies or staff sickness. The person in Charge is responsible for reviewing and signing the roster twice daily. We have now added a section on the back of the roster for changes and additional notes. A compliance tracker and monthly management audit system have been introduced to monitor and adherence to roster requirements. Failure to maintain accurate roster records will now be addressed through staff supervision and performance management processes where necessary. The register provider will review the effectiveness of these measures over a 3- month period to ensure sustained compliance and prevent further recurrence

- (1)(k) Whilst records demonstrated that the registered provider was compliant in regulation (1) – (3) it was not evident that parents/guardians were always informed when accidents and incidents occurred in relation to their child. Fifteen accident and incident records were sampled for inspection dated between January and March 2026. Three of the records did not include a parent/guardian signature contrary to the service’s accident and incident policy which stated that parents/guardians will be asked to sign off on the accident/incident report and will receive a copy. Failure to communicate accidents/incidents with parents may impact the continuation of safe and appropriate care for the child
Provider's corrective action:
- In-person training on the accident and incident reporting policy has been completed with all staff. Staff have been reminded of the importance of completing all sections of the accident/incident form thoroughly, ensuring all relevant information is recorded in a timely manner. Accident/incident and parent handover training cards have been completed, signed, and are retained in staff files. Ongoing supervision will be maintained through regular checks by local management to ensure all accident/incident forms are completed and signed appropriately. The policy will be reinforced with staff through periodic trainings and though staff meetings

- (4) Local management have been reminded of the importance of informing parents/guardians of significant incidents in a timely manner. The parents were informed the following day and have been kept up to date with all relevant information. A meeting was offered to the parents to discuss the incident and any concerns. Local and Regional Management will ensure ongoing compliance will be monitored through supervision, regular internal audits and management oversight of all significant incident reports to ensure timely communication and adherence to regulatory requirements

The registered providers and manager conducted a thorough review of the service’s policies and procedures in the aftermath of the incident. Provider's corrective action:
- All parents complete a registration form, on the registration form there is a section were parents complete the medication administration including permission to administer medication, whether medication can be given in an emergency. Our procedure before a child is given medication, the parent is contacted and informed, the parent is asked if the child has had any other medication and if they consent to the child being given the medication. This is followed up on the APP and the administration of medication form is completed. Therefore, the parent is contact in real time, and the information is then recorded and acknowledged by parents digitally and then with paperwork on collection. We feel that this method ensures that parents and all staff can see medication given and when, it is acknowledged and recorded both digitally and in paper format and ensures that it is available at all times to refer back to easily. Parents are contacted by the manager before medication is administered to ensure no other medication has been given and to ensure it is safe to do so

- (j) Our Medication administration form was updated to state name of medication being administered. All team members have received training on the new form and understand this requirement. The centre management team collect and file the medication forms weekly and will ensure that each form is thoroughly checked to ensure they are being completed correctly

- (i) Although the staff roster was available on the day of inspection it did not accurately reflect the adults working in the service on the day. An additional staff member arrived subsequent to the inspectors’ arrival which was not recorded on the roster. (j) Four of the medication administration records reviewed did not have the child’s full name recorded; this lack of detail had the potential to cause confusion as to which child had received the medication particularly when the child was being picked up from the service as the parent’s signature was required
Provider's corrective action:
- (i) Staff Rosters are supplied to staff on a weekly basis. We have now put in place that all staff rosters need to be updated throughout the day reflecting the changes that happen if staff ring in sick and cover staff arriving. Staff rosters have to reflect the day when changes happen as they are not accurate if they do not get changed. (j) Medical consent forms have been amended and all staff are required to check the child’s full name, date of birth, medicine name, dose, time due and date antibiotic was prescribed. It is then up to staff to complete the record of medicines administered and sign and witnessed and then get the Parent/Guardian signature to show medication has been given. Medical Consent forms must be filled in correctly to make sure all Staff and Management know the requirement of the dosage and time due and for which child. Staff are required to get Parents/Guardians to sign so Parents/Guardians know when their child has received medication to know when the next dose is due
