Sugradh Creche, Carrick-On-Suir · Regulation 16 — Record in relation to pre-school service
- (j) Signed medical consent was not obtained on all medication used for individual children. This posed the risk that parents were not informed of the medicine administered. (k) It is acknowledged that a parent’s signature was obtained on the majority of accident and incident forms. However, there was no parents signature on one accident and incident form to confirm that the parent had been informed of the reported incident
Provider's corrective action:
- (j) Medical forms are signed on the service’s electronic application. A form was uploaded to the services electronic application to ensure all parents provide the following information; dosage, times, medication required and signature. (k) All staff are now informed of the importance of obtaining parents signatures

- One adult employed in the service did not have a written record of their qualifications and experience available. Not having this record does not allow for adequate assessment for the suitability for the role
Provider's corrective action:
- The following response was received from the registered provider: Corrective and Preventive Action Evidence of staff files are now with the proposed new registered provider. Quarterly checks will be completed on staff files and a check list will be completed

Little Einstein’s Preschool & Afterschool, Roscrea · Regulation 16 — Record in relation to pre-school service
- (1)(k) The service did not record every incident that occurred in the service on the accident and incident forms. Staff stated that incidents were usually communicated to parents verbally at collection time but that a written record was not maintained. For example, if a child ate or drank something that they were not allowed due to allergies or cultural reasons, there was no record that this had occurred, that parents had been informed or that measures were put in place to insure against a re-occurrence
Provider's corrective action:
- The service has an incident book placed in each room. Any incidents at all will be monitored and parents will be informed. These are separate to our accident book for the service

Cahir Bears Childcare Centre, Cahir · Regulation 16 — Record in relation to pre-school service
- (j) There were 11 records of medication administration reviewed. No parental signature was obtained on four of the forms. Without parental signature, there was no evidence parents were aware of the medication being administered in the service. This posed a safety risk to the children
Provider's corrective action:
- The following statement was received from the registered provider: Corrective and Preventive Action Parents have now signed the consent forms, and a staff meeting was held to discuss the medication administration policy and procedures to be followed. The lead educator will check medication administration books to ensure they are signed in the future

Little Scholars Carrick Playschool Ltd, Carrick-On-Suir · Regulation 16 — Record in relation to pre-school service
- (j) A sample of 12 accident and incident records were reviewed from the date of the last inspection 07 November 2022 to the inspection date. Five of the records did not have parental signatures on the forms. Without signatures on the document the inspector was unable to determine that the parents were informed of the incident. This posed a possible risk to the child
Provider's corrective action:
- Parents are notified of accidents and incidents on a digital application. The team will ensure that parental signatures are obtained on the accident and incident forms

- (k) On the day of inspection, the service did not have previous accident, injury or incident records available for review. This is at variance to the services policy which described how the information would be recorded. The inspector could not ascertain that incidents that may have occurred were recorded correctly, and parents/guardians informed
Provider's corrective action:
- The registered provider purchased an accident and incident book and will ensure it is available in the service at all times

- : On review of staff records, the inspectors observed that there was no evidence of photographic identification or record of past employment history, on file for each of two staff members

- submitted by the Registered Provider: CORRECTIVE ACTION Photographic Identification and record of past employment history for two staff members are now on file. PREVENTIVE ACTION Staff member will check all staff records yearly. EVIDENCE SUBMITTED Copy of photographic identification and of past employed history submitted.
