- (j) A review of administration of medication forms in the Wobbler/Toddler room were made available to the inspector. Four records were available from January 2026 to date of inspection. Of the four records available, one record did not detail the signature of a witness. This posed a risk that incorrect medication may be administered if not witnessed by another staff member. This was found as a non-compliance on previous inspection on 01 October 2025
Provider's corrective action:
- The registered provider reminded staff the importance of completing all paperwork when administering medication. This will be monitored going forward

- (i) The registered provider did not have a roster in place despite the policy on staff absences stating that a roster would be available and maintained. It is acknowledged that there were staff sign in and out records available for review

- (j) Three if the seven medication administration records reviewed were not completed correctly, • One record did not have parent signatures requesting the medication to be administered, parents/guardians did not sign for the medication post administration, and the record did not indicate a second staff member had checked the medication. • A second record did not have a second staff signature on the record indicating they checked the medication, • The third record did not have the date, time of administration or the dose of the medication administered recorded. This was at variance with the services policy on the administration of medication which indicated safe practices to follow in the administration of medication
Provider's corrective action:
- (j) Staff have been reminded to ensure all forms are completed correctly to include parent and staff signatures, dates of birth and the full details of the medication being administered. Staff have been informed in writing, and a review will be carried out weekly by a named staff member

- (1) (k) Two of the three accident and incident reports reviewed were not completed fully with no parent/guardian signature indicating that the report had been reviewed by them. There was a written note to state the parent/guardian had been informed. Accurate records are required to ensure parents are informed of any incident or accident that occurs in the service to enable them to provide continuous care at home

- (1) (k) The accident and incident reports have been updated, and both have been signed by parents/guardians. A commitment has been received that all paperwork will be completed in a timely manner

- (i) The registered provider did not keep a record of staff rota, hours or break cover on file. The inspector reviewed staff sign in records, which were not completed from the beginning of September 2025. Due to the service not maintaining a record of staff sign in and out times or lunch cover, the inspector was unable to determine if adult to child ratios were maintained at all times
Provider's corrective action:
- The registered provider created a group messaging chat to include weekly updates with the staff rota. The registered provider has assured the weekly updates of the staff rota will be sent in to the group for all staff members

- (a)There was no curriculum vitae on file for 1 adult present in the service on day of inspection. (k) There was an accident and incident record book in the service. The services agreed practice was to have parents receive a duplicate of accident forms once these were signed, however this practice was not being followed. On review of the accident and incident records since the last inspection dated 25 November 2024, the following was noted • There were 6 accident and incident written records that the parent or guardian had not received a duplicate copy of the incident. • There were 2 of the 6 accident and incident records incomplete as the respective parent/guardian had not signed the accident and incident record. The service is not following the Accident and Incident policy and procedure of the service
Provider's corrective action:
- In the corrective and preventative response (CAPA) received from the named person in charge the following was stated:
- (a)A copy of the curriculum vitae was received and as a completed curriculum vitae is part of the vetting process. Going forward, curriculum vitae will be obtained prior to a person being appointed, assigned or allowed access to, or contact with, a child attending the service. (k) • The 6 accident and incident written records , that on day of inspection , that the respective parent or guardian had not received a duplicate copy of the written incident that occurred to their child while in the service. The person in charge stated,these parents/guardians have now all received a written copy of the accident and incident that occurred to their child while in the service. • The 2 accident and incident records not signed by parent/guardian at time of inspection are now signed and a duplicate copy has been given to the respective parent/guardian

- (1) (i) The staff roster available on the day of inspection did not include the staff breaks or start and finish times for the staff. Not having transparent records of what staff are caring for children and the number of staff available may potentially lead to not being able to assess that there were adequate adults available to care for the children throughout the operation of the service

- (1) (i) The staff roster has been updated and now includes start and finish times and breaks being taken. The roster is kept on file and is displayed in the service

Réaltaí Óga Pre-School, Cloyne · Regulation 16 — Record in relation to pre-school service
- 1. Signed parental consent had not been obtained for staff to administer the medications that may be required by two of the children, who attended the setting. This was at variance with the service’s medication management policy, which stated that “Parents/guardians must fill in the medicine consent form of the service, authorising the administration of medicine to their child.” It was acknowledged that written parental consent was in place for staff to administer medication to another child. 2. Staff members were unclear in relation to documenting the administration of medication, should it be required. During discussion, one staff member was unsure where to record the administration of medication and the inspector was advised by a second adult that provision to record the administration of medication was unavailable. The inspector subsequently found that a template, entitled record of medicine given, was available within the medication management policy. However, this confusion posed a risk that medication administered to a child may not be appropriately recorded
Provider's corrective action:
- 1. The written reply stated that signed medication consent forms have been completed. These forms relate to the two children in question. A form has been added to the service’s registration forms, which includes the five R's of administering medication and a consent form, to be signed by the parent. Going forward, this form will need to be completed for all children who have been prescribed medication, that may be required to be administered on the premises. 2. Following staff training, staff are now aware where to locate the administration of medication record and are aware of the five R’s, in relation to the administration of medication. The administration of medication record is now clearly visible above the desk in the Caterpillar classroom. The right child, right medication, right dose, right route and right time are documented. The record is signed by a witness and the person who administers the medication

- The written response outlined that a new record of administration of medication, which includes provision for a signature and the signature of a witness, has been created and printed. This is now used daily. The registered provider will ensure that the record of administration of medication is in line with the policy and that it includes the two signatures

- (k) The accident and incident records were reviewed and of the four records reviewed there was information not recorded on all four records: 1. The first record did not have the second staff name or signature and the details of the treatment given recorded. 2. The second record did not have a date of the incident, full name and date of birth of the child , no witness signature and no treatment described. 3. The third record did not have the full name and date of birth of the child recorded. 4. The fourth record did not have a full name and date of birth of the child or a parent signature recorded. Not ensuring accident and incident records are completed may hinder the continuity of care delivered to a child following an accident or incident
Provider's corrective action:
- (k) The registered provider has developed a new procedure on completing accident and incident forms and this has been discussed at a staff meeting

- (j) There were no written records available in relation to the daily medication that was administered to one of the children, who attended the service. This was confirmed during conversation with the person in charge. The documenting of this practice is required in order to support the provision of safe and correct procedures when administering medication to a child

- An administration of medication record has been added to the child’s file and is signed daily by the child’s parent. Going forward, all children who require any form of medicine will have an administration of medication record on file

In addition, the break times of each staff member were not recorded, and the roster had been pre-populated for the week. In conversation, the manager explained that the deputy was currently in a sister service and a new member of staff had commenced employment on the day of inspection

- The following statements were received from the manager; (i) The staff roster was amended to reflect the staff in the service on the day of inspection. A new staff break roster is now in place detailing staff breaks, timing of breaks and cover, this will be reviewed daily and amended if necessary

Clonakilty Montessori Pre-School, Clonakilty · Regulation 16 — Record in relation to pre-school service
- (h) Some of the services’ records assessed by the inspector during the inspection were noted not to be maintained accurately. During the service overview, the registered provider informed the inspector that 4 of the 5 adults were working between 9am and 12 mid-day and that 1 adult was working 8.30am to 1.30pm. This was the same as what the inspector was told by 4 other adults that were interviewed - they were due to finish work at 12mid-day on 04 June 2024 and that the registered provider would remain alone on the premises with 4 children between 12midday and 1.30pm. However, on assessing the records it was recorded that 2 adults would remain on the premises between 12 mid-day and 1.30pm. At 12 mid-day it was observed that 1 of the adults, that had earlier informed the inspector that she would be finishing work at 12 mid-day, remained on the premises as a second adult until 1.30pm. Also, the records demonstrated that 2 adults were on the premises between 8.30am and 9am, however when 1 of these adults was interviewed, she informed the inspector that she began work at 9am. This adult was requested by the inspector to correct the inaccurate record regarding her arrival time. It was noted that the proposed departure time of each child from the service were all carried out by 1 of the adults at 11am while the children were participating in their circle time activities. The records maintained each day were not accurately capturing the actual arrival and departure times of the children and adults to and from the premises. It was also noted that some of the records were blotchy and indistinct
Provider's corrective action:
- The registered provider stated that two staff now remain on site until 1:30 each day, and that rostered submitted show the correct times of staff arriving and departing

- (k) A staff meeting was held on 28 May 2024. Staff agreed that a detailed account of care given will be included on each form in the future. Two staff members will check to ensure that all care details are included. The new details have been included in the first aid policy
