- (a) The registered provider did not ensure there was a record of experience for one staff member
Provider's corrective action:
- The registered provider stated that the staff details have been complied and assembled in the staff folder. The manager and deputy manager will, going forward, collaboratively assemble and review staff folders on a fortnightly basis

- The registered provider stated that the required documents for the staff have been files in the personnel files. These requirements have been added the updated staff file checklist

- (a) There were no records available to demonstrate the registered providers had considered the previous work experience for one staff member
Provider's corrective action:
- The registered providers stated that they have filed a copy of the staff members curriculum vitae (CV) as evidence of reviewing their previous work experience. A recruitment checklist has been developed to ensure the required documents are on file before a new staff begins their employment

Our Little Treasures LTD, Waterford City · Regulation 16 — Record in relation to pre-school service
- (1) (j) On a review of the medication records, there was no written consent from parents for the administration of medication other than an anti-febrile medication in the case of an emergency. The person in charge stated that parents gave verbal permission. However, this is not in line with the service’s administration of medication policy which requires prior written consent to administer any medication
Provider's corrective action:
- The registered provider stated that they have developed a consent form for antibiotics/cough bottles/ inhalers etc for parents to sign to give consent and there is now a separate medicine book for these for each room. Each teacher is aware of this form and the requirement for any new parent joining the service to complete it. There is now a medication book in each room for this with a separate book for antifebrile medication

- The registered provider stated that the staff files have been updated to include C. V.’s and a recruitment policy had been updated to ensure an up to date and comprehensive C.V. is available to the employer

- (1) (j) On the day of the inspection, there were three medication administration forms available for review; of these three, none were completed correctly, as identifying details of the children were not included on the forms. This creates a risk of identifying the correct child in case of an emergency. (k) Of the 10 accident and incident report forms reviewed, 8 were not completed fully, with children's identifying details not included on the forms
Provider's corrective action:
- The registered provider stated that all staff have been informed of the requirement for children’s complete identifying information to be included on all forms. Management will check the accident and incident records and the administration of medication forms to ensure the required details are included

- The inspector reviewed four accident and incident reports, and of these, three were not completed fully. A second staff signature was omitted from the forms

- The registered provider stated that going forward accident and incident reports will be completed as per the requirements on the forms

Villierstown Montessori School, Cappoquinn · Regulation 16 — Record in relation to pre-school service
- (j) The inspector reviewed a sample of four medication administration forms dated from the 8th of January 2024 to the 3rd of March 2025. It was noted that these forms were not fully completed as per the services policy; for example, the dates, written parental permission, and staff signature were not included on the forms. The registered provider stated that communication with parents was verbal or on a messaging application. (k) The service did not follow the accident and incident policy regarding the recording of these events. Through discussion, a staff member stated that minor accidents and incidents were not formally recorded in the services accident and incident book. The last record was dated the 4th of November 2019. They said that an account of the accident or incident was noted in the child's file, and a text message was sent to the parent via the online application the service uses to communicate with parents. This practice was not in line with the services policy, which stated that the accident and incident book would be completed, counter signed by the manager and shared with the parent on their arrival
Provider's corrective action:
- (j)(k) The registered provider stated that administration of medication records and accident and incident forms were reviewed to include the accurate information and relevant signatures. During a staff meeting correct recording procedures were discussed and staff were reminded of this practice

Scallywags Lismore Community Childcare Centre Ltd., Lismore · Regulation 16 — Record in relation to pre-school service
- (j) A sample of 12 administrations of medication forms was reviewed, and of these, 5 had not been completed as they did not include the parents' signatures as required by the services' medication policy, which stated that completed forms must have two staff signatures and a parental signature. (k) A sample of accidents and incident digital and paper reports were reviewed, and they were found not to include necessary information. For example, the children’s details were not completed fully, and parental or staff signatures were absent. The services' accident and incident policy indicated that records of incidents or accidents must have the child’s full name, be signed by the staff member, the person in charge and the parent of the child when they have been informed of the incident or accident
Provider's corrective action:
- The registered provider stated that a decision has been made that records will not be stored on the digital application. Medication forms and records of accidents and incidents will be completed on paper going forward and will include the relevant information and required signatures. This decision has been added to the agenda for the next staff meeting, and the correct completion of these forms will be discussed

- Records forms of the children's accidents and incidents were not completed fully. Five forms dated between 19th June 2024 and 4th February 2025 were reviewed and two did not have the required signatures as required by the services accident and incident policy
Provider's corrective action:
- The registered provider stated that the importance of recording all details of children’s accident and incidents was included in a recent staff meeting. Staff reviewed old forms and completed them as required. They stated that all incidents or accidents forms will be reviewed at the end of each day and discussed at weekly meetings if required

- (k) A sample of the accident and incident records were reviewed. While it is acknowledged that the records included details of the accident and/or incident, the following was noted: 1. The records did not demonstrate that it was shared with the child’s parent and/or guardian. 2. The date of the accident was not consistently recorded. 3. The full name of the child was not consistently recorded
Provider's corrective action:
- (k) The accident and incident book has been updated and the parents have signed the required pages. The service will check the book at the end of each day and make sure that any accidents or incidents recorded are completed correctly

- Details of children's accidents or incidents were not fully recorded in writing. The inspector reviewed 10 accident and incident forms, and of these, 5 did not have the required signatures
Provider's corrective action:
- The registered provider has stated the outstanding signatures have been completed on the service’s online application forms. Staff have been reminded that all forms must be completed by the relevant persons and the deputy person in charge will check these forms weekly to ensure compliance

Provider's corrective action:
- The registered provider has stated that they have created a new procedure for the completion of forms for the administration of medication and the recording of accidents and incidents. The staff team are now required to read and agree with these procedures. They stated that a copy of these new procedures is on display in the care rooms. The registered provider stated that a member of the management team will be responsible to review all reports at the end of each week
