Ballymote Community Childcare, Ballymote · Regulation 16 — Record in relation to pre-school service
- On the day of inspection, inspectors observed that a person trained in first aid for children was available during the hours of operation. However, the record of the staff roster was incorrect and did not highlight the availability of staff members trained in first aid for children for the duration of the operation of the service
Provider's corrective action:
- The roster has been amended to ensure there is someone available with first aid training at all times. Training has been arranged for six staff members

- (1) (k) A sample of 20 accident and incident forms were reviewed. The following information was omitted on four of the forms. • No management signature on the four forms to indicated that the accident had been reviewed by management

- Circulated accident/ Incident policy to staff, clearly outlining all reports need to be co-signed

- (1) (k) A sample of 13 accident and incidents records involving a pre-school child were reviewed for this regulation. The following information had been omitted from the forms. • The signature of the parent was not present on two forms. • The second name of the child Was not present on four forms
Provider's corrective action:
- 1. Accident records were reviewed, and any incomplete forms were followed up to ensure that required child information and parental signatures were obtained. Procedures for completing accident forms have been reinforced with staff to ensure all required sections, including child details and parental signatures, are completed in full at the time of the incident or collection. The person in charge will ensure that accident forms are fully completed with all relevant details recorded before signing off on the report. Any issues will be addressed immediately to support consistent practice

- (1) (h) On the day of the inspection, attendance was not adequately recorded in the wobbler room, as at 10:40 there were eight children present, but only five were signed in. (1) (k) A sample of accident and incidents records involving a pre-school child were reviewed for this regulation. The following information had been omitted from the forms. • The signature of the parent was not present on one form. • The manager’s signature was omitted from five of the forms
Provider's corrective action:
- (1) All Staff have been informed to sign all children in as they come to creche. (2) All Staff have been informed to make sure all accident report forms are signed by parents and managers

Once Upon a Time Montessori, Ballinfull · Regulation 16 — Record in relation to pre-school service
- (16) 1 h) Details of each pre-school child's attendance were not maintained daily with actual arrival times. On the day of inspection, children were not signed in upon arrival at the service. i) A staff roster was in place; however, on the day of inspection, it did not accurately reflect the staff members present. As a result, the roster did not provide a reliable record of staffing arrangements for that day
Provider's corrective action:
- 1. A meeting has been held with all staff to reinforce the importance of maintaining daily attendance records. The staff roster on the day in question was incorrect due to an emergency, and this has been addressed during the meeting. To improve accuracy, one staff member has now been assigned the responsibility of welcoming children each day and ensuring they are signed in correctly. 2. A daily staff roster is now in place, and any changes will be updated on the same day to reflect the correct staffing

Little People's Playworld Creche, Sligo · Regulation 16 — Record in relation to pre-school service
- (1) (k) A recent incident in was not documented in the accident and incident book. This is in contradiction to the service policy where it states that: ‘…records are kept of significant incidents to include, child’s name, time and location of incident, events leading up to the incident, nature of incident, others involved, witness, how the situation was handled, whether restraint was used, what form of restraint, consequences and parent /guardian’s signature…’
Provider's corrective action:
- In a written response the registered provider stated: (1) (k) The recent incident was recorded in the behaviour log and was updated to the accident and incident book after the inspection. A discussion with staff took place to remind them to record all incidents in the accident and incident book as outlined in the service policy
