  • In a written response the Registered provider started:
  • A new book has been introduced for accidents and incidents that records one incident at time. We will use this book going forward and parents will only have access to information pertaining to their child

Provider's corrective action:
  • (h) The child was not on rota on inspection day as they were settling in. An error that has been rectified immediately. Every child will be in the register, whether they are in for 1 or 10 hours going forward. (i) A member of staff failed to sign in on arrival. Registered provider has reminded staff member to ensure they sign in as soon as they arrive. This includes me, the service provider. Who has made a reminder notice for staff to ensure no recurrence of this. (k) Service records every incident in our incident report book. Although this particular child is quite prone to minor incidents as you will see on report attached. We now record all incidents minor or not

  • The register was updated by the end of the preschool day. This was an oversight as the child in attendance came in late. It is not a normal occurrence. After a staff meeting following the inspection, we were all reminded of the importance of updating the registers and regular checks

  • The parent was informed verbally by management of the incident on the day it happened at collection time (Friday 11th April 2025). The incident report form was completed the following Monday (14th April 2025) and the child was off for the two weeks of Easter holidays and did not return until Tuesday 29th April 2025. The parent has read the report form but will not sign it for us. Staff have been verbally reminded that parents must sign accident/incident report forms and be given a copy of the report in a timely manner

Provider's corrective action:
  • 1. When a child is on antibiotics the first dose is always administered by the parent. Staff continue the administration when the child returns to the service. Written consent to administer medication is obtained on the child’s registration form. If the child becomes unwell while attending, staff can then administer anti febrile medication. Additionally, form 2 is used, this is completed on all occasions noting type of meds dose time and route given, (as per label that must include the child’s name and date) and signed by the parents. However, Form 2 was not used on these two occasions. Staff are always reminded to complete form 2 on all occasions noting type of meds dose time and route given, (as per label) and signed by the parents. 2. Medication is always appropriately given and witnessed just on that occasion the staff member did not sign the form to show they witnessed this. The second consent form other than the daily one will also be filled out. Staff have been reminded to sign as a witness at all times. Summary Comment The registered provider has given assurances that medication will be consented by parents and administration witnessed by two staff members. This has been accepted by the inspectorate. This has addressed the non- compliances in Regulation 16
