Immediate action notice. • An Immediate Action notice was issued to the service on the 27 March 2025 in relation to Regulation 25- First Aid. On the 27 March 2025 the registered provider responded to the immediate action notice with the measures implemented within the service to mitigate the risk. See body of report for details. • Following submission of their corrective and preventive actions on the 2 May 2025, additional information was requested from the service which was received on the 6 and 12 May 2025 and is included in the report.
Regulation 9 — Management and recruitment
(2)(a)(b) Two validated written references were not available for one emergency contact person. (3) The registered provider did not ensure that the procedures specified in paragraph (2) were carried out in respect of a person being appointed, assigned or allowed access to the early years’ service

(2)(a)(b) In response to the non-compliance the service has stated that references have been reviewed and updated for the emergency contact. (3) Going forward files will be checked on a yearly basis to verify documents are compliant or any training that needs to be updated
Regulation 16 — Record in relation to pre-school service
(k) On review of two accident and incidents records that were available for inspection the following was observed. • A parental signature was not obtained following one incident that occurred to a child within the service. • The surname and date of birth of a child was not recorded on one incident record. These non-compliances were identified on the last inspection

(k) The registered provider has stated that accident forms have been updated, and staff will sign and check to ensure parent signs also. Registered provider will check weekly records to ensure accidents/incidents are recorder correctly
Regulation 23 — Safeguarding health, safety and welfare of child
Infection Control: 1. The children’s perishable food items from home such as cheese and yogurts were not refrigerated on arrival to the service. All the children’s food was stored in their bags within the care room. This increased the potential risk of food borne infection. This non-compliance was identified on the last inspection. 2. Handwashing practices within the service were not observed to be in line with the service infection control policy or best practice guidelines. For example. • Children were not encouraged or prompted to wash their hands before eating their snack or after using the toilet. • Children were assisted to wash their hands before going home with cold water from the sink in the outdoor area. This is not an effective way to wash hands. Fire Safety: 3. Yearly maintenance certificates were not available for the fire extinguishers and smoke alarms in the service. Fire extinguishers were last serviced on the 9 January 2023 and the available documentation stated that smoke alarms were serviced on the 8 July 2021. Infection Control: 1. In response the service has stated that staff will ensure each child removes lunch boxes from school bag on arrival and places in fridge. 2. To address the non-compliance children will now wash their hands before eating and using the toilet in warm water in hand basin in preschool. Water will be checked on a daily basis to ensure at right temperature. Fire Safety: 3. In response the service has stated that the registered provider will check to ensure fire extinguishers and smoke alarms are serviced yearly. Fire extinguishers and smoke alarms have been serviced
Regulation 25 — First aid
(1) The registered provider did not ensure that a staff member with the First Aid Responder (FAR) training or paediatric first aid was available to the children during the session. This non-compliance was identified on the last inspection. • An immediate action notice was issued to the service on the 27 March 2025. In response to the immediate action notice the registered provider responded that they have placed an order with a training company to update their Paediatric First Aid
Provider's corrective action:
1. In response to the non-compliance the registered provider has stated that they will be permanently closing the service on the 27th of June and therefore would not be completing a 3 day FAR course. However, the register provider has updated their Paediatric First Aid certificate
Regulation 16 — Record in relation to pre-school service
(1)(h) The daily attendance of the children to the service was not recorded on arrival at 9:15am but at the later time of 10:20am. (j) The parental signature following the daily administration of a medication to a child was not obtained to ensure that parents were aware of the administration. (k) A parental signature was not obtained following two incidents that occurred to a child within the service

(1)(h) Roll call is changed to 9:15am daily. Roll book is moved nearer to preschool door for sign in. All paperwork is up to date and checked daily. (j) Daily administration of medication has been discontinued. Going forward parents will sign for medication. (k) Going forward parental signatures will be obtained following incidents
Regulation 23 — Safeguarding health, safety and welfare of child
General Safety: 1. The external gate and door of the service and care room were not secured to reduce the risk of a child exiting the service unsupervised and the unauthorised entry of an adult. 2. The children had access to a cleaning product in an unlocked cupboard. 3. The surface temperature of an accessible radiator was recorded as 52 .80C at 10.13am which exceeded the recommended maximum temperature of 500C and increased the potential risk of burning a child. Infection Control: 4. The children’s perishable food items from home such as cheese and yogurts were not refrigerated on arrival to the service. All the children’s food was stored in their bags within the care room. Fire Safety: 5. The last recorded fire drill in the service was the 23 October 2022, no fire drills took place in November 2022. 6. Yearly maintenance certificates were not available for the smoke alarms and fire extinguishers in the service. The smoke alarms were last serviced on the 8 July 2021 and the fire extinguishers on the 30 November 2021. Action submitted by the Registered Provider

General Safety: 1. A new sign has been erected to ensure gate is always closed by staff and parents. 2. Cleaning products removed and put on high shelf with other cleaning products and sign erected to ensure children’s safety. Cleaning products on high shelf away from children for safety. 3. Heating in radiator now been adjusted to temp of 50 degrees. Will continue to monitor. Infection Control: 4. Memo to staff reminding them to put children’s lunches in fridge on arrival to service. Fire Safety: 5. Date of planned fire drill put on calendar and reminder put on centre phone. Recent fire drill 6th January 2023. 6. Message sent regarding servicing of fire equipment
Regulation 25 — First aid
(1) The registered provider did not ensure that a staff member with the First Aid Responder (FAR) training was available to the children during the session. However, it is acknowledged that both staff had a valid certificate for basic first aid

(1) We have contacted a provider of FAR training and are currently awaiting a response regarding upcoming dates
Regulation 32 — Complaints
(2)(b) The service did not have documentary evidence to demonstrate that a complaint received was dealt with according to the service complaints policy. For example: • The service did not acknowledge or engage with the complainant to inform them of how the complaint would be dealt with and the expected time frame. • The service did not inform all parties of the investigations undertaken and the outcome of the complaint. • The service did not document the outcome of the complaint and any details of recommendations, changes to practice, policy, or statement. • The service did not give the complainant a full explanation in writing of the outcome of the investigation
