(d) In the children's sanitary area on the ground floor, the laminate countertop between the sinks was damaged with dampness in multiple places, exposing an inner porous material and preventing the surface from being effectively cleaned. (e) There were six toilets and five sinks available to children on the day of inspection. This number of sinks and toilets does not meet the regulatory requirement of one toilet and one sink for every eleven children attending. There are 73 children in attendance on the busiest day as demonstrated through a review of records and discussion with staff. This was found non-compliant on two previous inspections dated 12 November 2024 and 13 December 2021. The corrective and preventive actions submitted by the registered provider did not adequately address the same non-compliance found on two previous inspections

Provider's corrective action:

(d)(e) The registered provider stated that children's sanitary areas are currently being refitted with additional and replacement toilets, sinks and surrounds. There will seven toilets and sinks available to children on completion of the works and going forward we will aim to complete necessary building works in a timely manner

Ringsend Creche Ltd, Dublin 4 · Regulation 29 — Premises

(d) The registered provider did not ensure the premises was cleaned, maintained and repaired as required. The following was observed: 1. Rubbish and debris was observed in the outdoor play area and the route from the main building to the Preschool room to the rear of the premises. The outdoor risk assessment had not been completed since the week ending 22 May 2026. 2. The door to the Preschool building appeared to be damaged and was sticking. In the playgroup room the following was observed: 3. The plaster on the walls was damaged with holes evident; leaving an unfinished surface. 4. The paint on the windowsills was peeling and chipped; leaving an unfinished surface. In the sanitary area the following was observed: 5. There was a build-up of what appeared to be dead insects in the light fitting. 6. There was a build-up of dust on extractor van on the left side of the room. 7. Sections of the radiator were rusted, leaving an un-wipeable surface. This was identified as a non- compliance on the last inspection and the actions submitted failed to prevent a recurrence. (e) The water temperature in the wash hand basin used by the children in the Toddler 2 room exceeded the recommended temperature of 43oC. A temperature of 44oC was recorded by the inspector at 11.12am

Provider's corrective action:

(d) 1. The outdoor area is now clear of rubbish and debris. Staff were reminded to ensure they complete the outdoor risk checks which will be reviewed by management. 2. The service report that the door was repaired and is now closing effectively. This will be monitored. 3. Management report the walls have been sanded and will ensure monthly inspection will be carried out on walls, skirtings and window sills. 4. Management report the sills have been painted and will ensure monthly inspection will be carried out on walls, skirtings and window sills. 5. The management company of the building were informed of this issue, and evidence was submitted that a pest control company has been engaged. The service report they will fit light fittings with insect proof seals and the pest control company check will now monitor for and prevent pest entry. 6. Management report the ventilation unit was cleaned and will ensure weekly inspection will be carried out. 7. Management report the radiators have been sanded, painted and treated and will ensure monthly inspection will be carried out. (e) Thermostatic Mixing Valve connected to the wash and hand basin in Toddler 2 room was adjusted and recalibrated to restrict the maximum hot water output. The service will ensure the water is checked by staff

(d) Throughout the premises, areas were found not maintained and repaired as required as detailed below: 1. In the sanitary unit upstairs, holes were observed where plaster was missing. 2. In the Pre Montessori room, paint was chipped off the pillar and a mesh beneath was exposed. The radiator cabinet was warped and chipped in parts. Holes in the walls were observed where plaster was missing. 3. In the Baby room, the windowsill was chipped and in need of repair and painting

Provider's corrective action:

Following the inspection, a painter attended the service to assess the required work and commence preparatory repairs. The painter is scheduled to return on 20 July 2026 to fill all remaining holes and complete the repainting of the entire setting. This work will ensure that the premises are maintained in a good state of repair, are clean and well maintained, and continue to provide a safe, suitable, and welcoming environment for children. The person in charge will continue to monitor the condition of the premises through daily environmental checks, regular health and safety inspections, and ongoing risk assessments. Any maintenance issues identified will be recorded, prioritised, and addressed promptly to ensure the environment remains safe, clean, and well maintained. A planned maintenance schedule will also be maintained to support the timely completion of repairs and to ensure continued compliance with regulatory requirements

(d) • Wash hand basins are now repaired. Landlord to replace during the summer months. • Cable tie has been removed to the tap is back in working order. Management will check the taps each morning to ensure they are in working order

(c) The air vent of the mechanical ventilation in Honeybees required cleaning for it to work correctly. This was completed. (d) A new part was put onto the tap by the maintenance team to ensure the tap was easier to turn on for the children. The air vents and taps will be monitored by adding them to the daily health and safety list, to ensure that they remain in working order

(d) During the inspection it was observed that the premises was not cleaned and maintained as required. The following was observed: • A rusted and damaged metal bar was present on the walkway from the garden to the ground floor. • There were damaged radiator covers in the Montessori room. • The vent in the Montessori sanitary area was heavily dusted. • There was a build-up of dirt and debris long the floor edges of the Montessori room. • Fixtures on the wall in the cot room had a heavy layer of dust. • There is chipped and damaged paint in the Montessori room. • There is damaged skirting board in the Montessori room

Provider's corrective action:

(d) • The metal bar was removed and all the area cleaned. A section of plastic was fitted to protect any risk it can be there for the children’s safety. • The damaged radiator cover was removed and replaced. This needs to be painted. • All the vents in the Montessori area and the rest of the creche were clean. • The floor for Montessori including the floor edges were cleaned. • The cot room was clean to protect the children’s safety and health. • Montessori class will be painted the weekend of July 18th. • We fix it the damage skirting board in the Montessori Class and this will be painted the weekend of July 18th. The room assessment and risk assessment have bee updated to check for dust and damage daily

(d) The outdoor area was not maintained and repaired as evidenced by; 1. The wooden fence panel separating the play areas was in a poor state of repair. Paint was chipped off multiple panels, splintered wood exposed and loose fence panels were inappropriately secured with masking tape, posing a potential hazard. 2. The wooden picnic table was not adequately maintained. The table surface had visible cracks in the wood and the bench seating showed signs of significant wear and deterioration. 3. The wooden panel marked as the fire assembly point was damaged at the base, and a visibly gap was present posing a safety risk

Provider's corrective action:

(d) 1. An imme diate review of the outdoor garden was conducted. The damaged panels separating the play areas have been identified for replacement and any loose panels will be securely fixed and damaged areas replaced where necessary. All exposed and splintered wood will be repaired and repainted where needed. The outdoor area will continue to undergo daily risk assessments to identify any further damaged fencing, panels or wooden areas. 2. The wooden picnic table will be removed and replaced with a new one . Materials have been delivered and maintenance have scheduled plans for building a new table. To prevent this from happening again staff have been asked to notify the compliance manager if there are any repairs needed when carrying out risk assessments in the garden. Any repairs noted will be but in the handyman’s book for prompt attention. 3. The damaged fire assembly has been repaired and the gap at the base has been eliminated to eliminate the safety risk. The outdoor area will be inspected to ensure all hazards are removed. To prevent this from happening again any maintenance issues identified will be addressed promptly

(d) The premises was not maintained and repaired as required as evidenced by the following examples: 1. In the garden, a slide and wooden structure were propped on a steel sink and not appropriately secured. 2. Wood was rotting and splintered on equipment and materials for children’s use in the outdoors to include the playhouse, kitchen and a wooden pallet. 3. Paint was chipped on the wooden windowsills in Infancy and Montessori 2 rooms. 4. Paint was chipped in multiple places on the architraves around the doors in Infancy room

The are registered providers stated that 1 & 2 - The items noted in points 1 and 2 were removed except for the playhouse which will be renovated over the summer. New outdoor play equipment was ordered. 3 & 4 - The school is painted each year over the summer holidays and all chips in wood will be repaired when the painting work is complete. Going forward the painter will be brought in during a break in the school year to ensure the building is maintained when needed

Areas of poor maintenance and repair were observed in the service, as follows: • In the sanitary facility upstairs, one toilet had no toilet seat and the area behind this toilet was damaged by water causing presence of mould and an unfinished surface which cannot be effectively cleaned. • In the Toddler room, parts of the wall beside the windows, was in poor repair with exposed plaster board

• Toilet seat has been replaced, and water damaged area has been treated and painted. • Exposed plaster board beside windows has been fixed and painted. More regular maintenance checks will be completed on the premises

(d) The registered provider did not ensure that the premises and outdoor play area of the service was cleaned, maintained and repaired as required as evidenced by. 1. In the sanitary area used by the Baby and Toddler room a tile was missing from the wall and the soap dispenser was broken off the wall. 2. The floors in the Baby and Toddler rooms were visibly dirty, with marks and food crumbs present. 3. The windows in the care rooms were observed to be heavily smeared and covered with visible dust and dirt. 4. Parts of the outdoor playground were observed not to be maintained to an appropriate standard of cleanliness and safety. There was a build-up of moss and dirt increasing the risk of injury. 5. In the outdoor play area the big slide was observed to be in poor condition. The wooden panels leading up to the slide were broken, and masking tape had been applied to the top bar of the wooden frame as a form of support. Additionally, the wooden steps appeared rotten and were visibly deteriorating. This posed a potential safety hazard. 6. In the outdoor play area, the child size picnic table had a large hole in the centre of the table and the wood was chipped around the edge of the table. The paint was chipped off the bench seating. 7. Four large wooden armchairs located in the outdoor area were not maintained in a safe and appropriate condition. The paintwork was observed to be heavily chipped and one arm had a visible hole in the armrest, indicating a deterioration of the furniture. 8. The wooden covering on top of the chalkboard in the outdoor play area was observed to be broken, with visible wooden splinters present. It’s acknowledged the person in charge provided a copy of an email to confirm a playground service had been contacted to address the issues in the outdoor area

Provider's corrective action:

(d) 1. Wall tile is still missing service is currently sourcing a tiler to replace in short term a laminated piece of paper has been placed over the missing tile until a tiler can fix this. 2. A cleaning company was brought in to carry out a deep clean of the whole service. The floors have been professional cleaned. Staff are aware of the need to sweep and clean the floor daily and cleaner will ensure the rooms are mopped each evening to maintain the cleanliness. 3. All windows have been professional cleaned images attached. Window cleaning sprays have been added to each room to ensure these are maintained. 4. Outdoor area has been power hosed and weeds/bushes cut back and removed. 5. Slide has been removed and new play equipment has been added. 6. Picnic table has been disposed of. 7. Armchair has been removed and disposed of. 8. Chalk board has been removed from wall. Management and Registered Provider will ensure checks are carried out on the building to prevent weeds/dirt/damage. Risk assessment checks will be carried out regularly by staff and management and any issues will be reported and rectified immediately going forward

(d) The registered provider did not ensure that the premises was being maintained in an appropriate condition as evidenced by the following: • A hole was present in the wall adjacent to the window and behind the door in the Preschool room. • A hole was present in the wall of one of the children’s toilets on the first floor of the service. • The casing covering on low level wiring in the Preschool room was cracked with the wires exposed. • The paint was peeling off the wall and there was wall damage at the sides of the gated entrance into the sanitary accommodation in the Baby room. • There were two holes in the wall with plaster exposed at both the top and bottom of the gate situated in between the Baby room and the cot room

The Village Day Care & Montessori School Ltd, Dublin 22 · Regulation 29 — Premises

(d) 1. A visible build-up of dust and debris in crevices, toyboxes, on surfaces, flooring and shelving units throughout the premises was observed on the days of inspection. This is at variance with cleaning schedules which were completed for the days of inspection and previous days in Montessori 1 and the Toddler room. 2. A damp mark with evidence of mould spores was visible on the ceiling in the Náionra room. 3. Three toilet seats were missing and /or damaged in the two children’s sanitary areas. 4. A sink had become loose from the wall, upstairs in the children’s sanitary area. 5. Parts of the wood in the playhouse in the garden area appeared to be rotten. 6. Lino was worn with concrete exposed in the Montessori 1 room and the Toddler room. 7. Lacquered surfaces on a dolls house and on two shelving units in the Toddler and Montessori rooms were worn away. 8. One blind cord which was not in reach of the children was not secured in Montessori 1. 9. A lock on a small gate in the outdoor area which separates the different age groups of children was broken and in need of repair

Provider's corrective action:

1. All the skirting boards radiator covers and window sills have been freshly painted and the rooms have had a deep clean. The manager will review rooms on a monthly basis. 2. The service engaged a painter who treated this area and painted the entire ceiling. The manager will review rooms monthly and will organise maintenance where required. 3. Toilets seats are replaced. The manager will inspect toilets as part of their daily morning inspection risk assessment. 4. The sink is now secure. The manager will check toilets as part of the daily morning inspection. 5. The play house has been replaced with a heavy-duty plastic playhouse and the manager will inform the owner when items need to be replaced. 6. The lino has been replaced in Montessori room 1 with a wooden floor. Due to finances the Toddler room floor was not replaced but it was given a deep clean. The three toy units were replaced the service intends to replace this floor in October or sooner if finances allow. The owner will take monthly inspections of the rooms and premises. 7. Dolls houses have been replaced. This was discussed at the staff meeting regarding the risk assessment. 8. The cord has been fixed. This was discussed as part of the risk assessment checklist. 9. The lock on the gate has been repaired. Staff have been made aware the staff meeting if they see anything in the back garden that would be a risk to the children’s safety the staff sign an outdoor checklist

Bright Minds Childcare and Montessori School Ltd, Dublin 16 · Regulation 29 — Premises

(d) Throughout the premises, areas of the building were found not to be cleaned, maintained and repaired as required as detailed below: 1. A deep gap was observed between the outside building and the ground surface. This created a large foothold that was accessible to children. 2. Throughout the garden, uneven paving slabs were observed to be loose and lifting. This created a trip hazard for children and staff. 3. The ramp leading from the conservatory area into the sessional Montessori room was loose and broken in parts. This posed potential tripping hazard. 4. The door leading from the toddler room to the outdoor area was off the runner making it difficult to open and close. This posed a potential risk to children as it was the only means of entry from the garden into the room. 5. In the Montessori room the paint on the edges of the shelving units were worn away exposing porous wood, this prevented effective cleaning and posed a risk of cross contamination to children. 6. In the Montessori room, the laminate surface on the three tables used by the children at mealtimes and during tabletop activities was observed to be worn exposing porous wood. In addition, a section of one of the tables had sticky tape holding down a paper shape. The tape was coming away leaving a sticky residue with debris accumulated on it. The corrective and preventive actions submitted by the registered provider following the last two inspections of the service on 02 September 24 and 05 June 2025 did not prevent the re-occurrence of the non-compliance under Regulation 29 on points 1,2 and 3 above

Provider's corrective action:

The registered provider has stated: 1. The gap between the outside building and the ground surface has been temporally filled with sand. The outdoor area will be landscaped in July. This gap will be filled with a pipe, and a grate will be laid on top. 2. The garden will be resurfaced in July; this will include laying grass on the ground instead of paving slabs. 3. The ramp has been covered by an anti-slip tape surface; this will be maintained to ensure it is not a tripping hazard. 4. The track for the sliding door was cleaned to ensure the door opens and closes easily; a company has been appointed to replace the door. 5. The shelves in the Montessori room have been sanded and repainted. The service will carry out regular checked to ensure the shelves are maintained. 6. New tables have been ordered for the Montessori room, these tables will be monitored and maintained

(d) Throughout the premises, areas of the building were found not to be maintained and repaired as required posing a potential risk of injury to a child. 1. In the sanitary area in the Montessori 1 room, pipes and electrical cables were exposed and accessible to children. The pipes and cables were connected to a water heater. A laminated page was taped to the top of the heater to restrict children’s access to the controls; however, the controls were still accessible to children. In addition, there was no hot water available in this sanitary area on the day of inspection. 2. In the Montessori 1 room, the toilet seat was loose and falling off, staff confirmed it is tightened regularly but continuously becomes loose. 3. In the Montessori 3 room a safety protective cover was missing from a ceiling light

The registered provider has stated: 1. The water heater in Montessori 1 has been removed and relocated to the press under the sink in the kitchen area where it is better located to heat the water for Montessori 1, 2 and the kitchen area. The area of the wall where wires were visible through a gap has been repaired by the carpenter with a wooden panel. 2. The toilet washers were used to tighten the toilet seat to avoid future loosing. 3. New protective cover has been fitted to the light in the Montessori 3 room

(d) The inspection included a review of maintenance works previously identified, including those the registered provider had committed to completing over the Easter period in 2025. However, during this inspection, it was found that these works had not been completed despite assurances from the registered provider in the corrective and preventative action submitted following the last inspection on the 29 January 2025. In addition, further areas of non-compliance in relation to the premises were identified, as outlined below. 1. The gaps remained present on the flooring in both the Junior and Senior Montessori rooms and the floor covering was visibly worn. 2. The stair gate was broken outside the Senior Montessori room. 3. In the Toddler room the paint was chipped beneath the window and around the skirting boards. This was identified on last inspection. 4. In the Senior Montessori room, the heater provided was broken and held together with a cable tie. It was visibly covered with dust and dirt. 5. In the Junior Montessori room, the window was not fitted properly and was stuffed with paper towel to prevent a draft. There were cobwebs visible around the window and the window sill was visibly stained with dirt. 6. In the Junior Montessori room, a cracked remained on the wall beneath the window. This was identified on last inspection. 7. In the Junior Montessori room, the walls were visibly stained and chipped. This was identified on last inspection. 8. In the Junior Montessori the paint was chipped off the pillar and mesh beneath was exposed. 9. There were water marks on the ceiling in the Senior Montessori room. 10. In the Senior Montessori room there were two holes in the wall. The paint was chipped off the window sills and evidence of water damage was visible on the wooden panelling frame around the window. There was a heavy layer of dust visible around the opening of the window. 11. In the outdoor area outside the Junior Montessori room the artificial grass surface was coming away from the wall

Provider's corrective action:

1. Following the identification of gaps remaining present in the flooring in both the junior and senior Montessori rooms, as well as the visibly worn condition of the existing floor covering, immediate action has been taken to address these issues. The service provider has engaged a specialist safety flooring company to attend the service and carry out a full replacement of the flooring. Confirmation of works has been secured, and a deposit has been paid to initiate the project. Photos with the new flooring in both, junior and Senior Montessori rooms will be emailed to Tusla inspectors in due course. Approved contractors specialising in safety flooring will be engaged for all future works. 2. Both locks have been replaced at the stair gate outside Senior Montessori Room and outside Junior Montessori room. The maintenance personnel will carry out monthly inspections to identify any issues on the premises. 3. In the Toddler room the skirting boards and the wood beneath the two windows have been freshly painted. The maintenance personnel will carry out monthly inspections to identify any issues on the premises. 4. In the Senior Montessori room, the heater was cleaned of all the dust and dirt, and the maintenance personnel repaired it and it is now in good condition. The maintenance personnel will carry out monthly inspections to identify any issues on the premises. 5. In the Junior Montessori Room, all windows have been thoroughly dusted, cobwebs removed and thoroughly cleaned. The windowsills in the junior Montessori room were thoroughly freshly painted. The window in the junior Montessori is now fully closed and lock has been fixed. The maintenance personnel will carry out monthly inspections to identify any issues on the premises. 6. In the Junior Montessori Room, the crack on the wall beneath the window has been filled and repaired. The maintenance personnel will carry out monthly inspections to identify any issues on the premises. 7. The Junior Montessori Room was fully repainted. The toilet seat was missing from one of the toilets in the Butterfly sanitary area. 4. Three sinks were blocked. These included an adult sink and a child sink in the Toddler room and another adult sink in the Butterfly room

1. The painter has been booked to paint the Toddler room on April 25th 2026. The painter has verbally confirmed. The painter is going to use a different paint that will reduce the possibility of paint chipping. They are also going to create a blackboard section so children can do mark making on this section of the wall. 2. The landlord has made arrangements for the ceiling tiles to be replaced in the Baby and Toddler rooms. Management will have to find suitable maintenance repair personal moving forward, who can be contactable on a regular basis. 3. The manager bought and replaced the toilet seat. 4. A company carried out an initial inspection and maintenance of the pump below the Toddler room sink, which connected to the two other sinks on the 24th of February. They determined the pump was broken and need to be replaced. The pump was order immediately and the work was completed on March 6th. Visible dust, debris, and dirt were present. Staff advised that the ball pool is cleaned on a weekly basis; however, the cleaning policy requires daily cleaning. This practice is not consistent with the service’s stated procedures and presents a potential infection control risk. 2. In the sanitary area of the Butterflies, Appledrops and Rascals rooms the laminate surface on the cupboards was peeling off, creating areas that could not be cleaned effectively and therefore posed a potential infection control risk. 3. In the Appledrops room the loft playhouse and was not maintained as required paint was chipped off the banister and ladder. 4. The Rascals room was not cleaned and maintained as required as evidenced by. • In the sanitary area there was a build-up of dirt and debris under the nappy changing mat. There was evidence of grime around the taps in handwash basins used by the children. The walls, skirting boards, the lid of the pedal bin and doors were heavily stained with visible dirt. In the corners of the floor in the nappy changing room there was a heavy layer of dust visible and tissues discarded on the floor. • There was a sticky residue from tape observed on the window sill. • The door to the kitchenette and nappy room were chipped and heavily stained with dirt and food residue. There was a build-up of dust on top of the skirting board. • There was a build-up of dust, dirt and debris and debris behind furniture and equipment around the care room. • Some of the children’s chairs were heavily stained and discoloured. 4. In the Jellytots room the partition at the edge of the changing area and the area around handwash sink was chipped and could not be cleaned effectively. 5. In the sanitary area used by the Jellytots room the hand wash basin was found to be clogged and not draining properly

1. Ball pool was emptied, hoovered and cleaned. Ball pool will be emptied and hoovered and wiped out every second day. 2. (c) 1. Parts of the premises were not adequately heated. During the inspection, the room temperature in T3 was 15.9°C at 11:26 which is below the required range of 18-22°C. (c) 1. The heating system is fully operational, and staff have full access to temperature controls to adjust heating levels as required. The nature of the programme includes high levels of physical activity (e.g., sensory circuits, trampoline use and movement-based activities), and room temperature is often adjusted in response to children’s activity levels. Following the inspection, heating levels were immediately increased to ensure the room temperature was maintained within the required range. 3 room thermometers have been ordered so we are able to continuously measure room temperature. (d) 2. A full deep clean of the premises took place following inspection. Deep clean have been scheduled for every break, i.e. midterms, easter, Christmas, summer. Alongside our usual cleaning schedules/ Rotas. 3. We are awaiting a quote from a painter and have planned to schedule works for the two-week break over Easter holidays. Painter is coming to quote on the 24th with works due end of March. 4. The area of flooring had an extra layer of lino in T3, which was glued to original floor and acted as a tripping hazard was removed. The glue was removed from the floor, the floor remains discoloured, in comparison to the rest of the floor but is clean and there is no risk of tripping. 5. The sink unit has been replaced. 6. The mirror has been fixed. The registered provider and management will complete a thorough check of the building before every school holiday break, so if anything needs to be repaired it can be done so with minimum disruption to the children. (c) Heating settings were reviewed and adjusted immediately. Room temperatures were monitored following adjustment to ensure they reached the required range. The matter was addressed with staff on the day of inspection and expectations regarding maintaining appropriate room temperature were clarified. (d) Following inspection findings, the entire service was repainted to restore wall surfaces to an appropriate standard. A full deep clean of flooring and skirting boards was carried out in the Toddler and Baby Rooms to remove any build-up of dirt, dust and debris. The play kitchen in use in the Baby Room was repaired. The worn covering was removed and replaced with a washable surface to ensure effective cleanability. Laminate covering on shelving and room partitions that had worn away was removed and replaced to ensure that all surfaces can be effectively cleaned

(d) Throughout the premises, there was a heavy accumulation of dirt and dust, despite a cleaning schedule being in place. This was observed on walls, floors, shelving units and behind toys and in the sanitary facility. This was a non-compliance on the last inspection 23 March 2023, the corrective and preventative actions submitted by the registered provider have failed to address the non-compliance. (e) The registered provider confirmed the service does have sufficient sanitary facilities for the 22 children who attend the service, but the service only uses one toilet and sink as the second toilet and sink is located outside the main Montessori room. The registered provider confirmed the children have never used this 2nd sanitary facility as it has insufficient lighting for the children and is used by a community group when the service is closed. Thirteen children were observed queuing to use the toilet and/or wash their hands, prior to lunchtime This took approximately fourteen minutes to complete. Children were observed leaving the queue during this time and did not return to wash their hands

Provider's corrective action:

(d) The class room is cleaned twice daily and deep cleaned every Monday, Wednesday and Friday -steamed mopped, disinfected, dusted etc. The service will continue to do this but unfortunately it is an old building so for the amount of cleaning that is done it doesn’t reflect. There is a possibility that renovation work will be conducted this summer or in the very near future, which will help greatly. The service will continue daily and deep cleaning and will schedule a meeting with the community group in the new year to discuss all. (e) Maintenance of the entrance toilet will be carried out over the Christmas break. Handwashing has gone back to table by table, ensuring all children wash their hands and the task is carried out smoothly. No supporting documentation was submitted. Summary Comment The actions as stated by the registered provider address the non-compliance. The regulatory requirement has been met. Regulation 29 d & e will be reassessed on the next inspection

A nappy changing unit has been ordered. This will be put in the toilet block as the designated nappy changing area to maintain the children’s privacy and dignity as well as promoting a higher standard of hygiene, safety and efficiency as advised. The existing policy has been updated to incorporate most of the issued raised in the inspection to use until the unit is delivered. This updated policy has been read, understood and signed off by all members of staff. The person in charge developed a new policy and procedure to be used when the new unit is delivered. (c) On the day of inspection at 12:10 there was a malodour in two units of the sanitary accommodation in the main hallway and in the nappy changing room closest to the staff room. While these toilets appeared clean in appearance there was no visible sound from the mechanical ventilation servicing them. This was brought to the attention of one of the managers who agreed to check this out. At 12:45 she advised that a maintenance engineer was due to arrive shortly to assess same. While it is acknowledged that the maintenance engineer arrived onsite promptly and increased the ventilation setting, the malodour remained. At 14:15 and 15:50 the malodour was still present. This malodour was also noted on the previous inspection on 2nd October 2023 and the corrective action submitted by the registered provider failed to address this non-compliance

Provider's corrective action:

A full investigation was carried out to identify the source of the malodour in the two affected sanitary units in the main hall. The sewerage and drainage systems were inspected and confirmed to be clear and fully operational. The toilet areas underwent a deep clean by the cleaning team, including steam cleaning of all tiled surfaces to remove any lingering odours. Air fresheners have been installed in the affected toilet units to help maintain a pleasant environment. Weekly checks of the toilet areas are now conducted by the manager/deputy manager, and any issues are reported immediately to the regional manager

Areas of the service were found to be in need of maintenance on the day of inspection as follows: 1. In the children’s nappy area upstairs, the lacquered surface of the nappy changing table had worn away exposing the porous wood beneath which could not be effectively cleaned. 2. The linoleum flooring at one of the doors into the Toddler room was lifting exposing the flooring underneath and creating a trip hazard. 3. The lacquered surface of one of the radiator covers beside the children’s toilet in the rear building had worn away exposing the porous wood beneath which could not be effectively cleaned. 4. The gate leading into the outdoor play area was broken and could not be secured. The gate and the wooden fence attached to it were broken in a number of places, exposing rusted nails and sharp splintered wood posing a risk of injury to a child. 5. In the outdoor play area, gaps and holes in the shock absorbent surfacing created an uneven surface posing a risk of injury to a child

Provider's corrective action:

1. The upstairs nappy changing table with a worn lacquered surface has been newly wrapped with vinal wrapping to ensure the surface can be cleaned properly. This ensures the surface is hygienic and suitable for daily use. All furniture in changing areas is now checked monthly as part of the maintenance schedule. Any signs of wear will be reported and addressed immediately. 2. The damaged linoleum at the door to the Toddler room has been repaired, and the flooring has been secured to remove the trip hazard. The area was made safe on the same day. Weekly checks are now carried out on all door entry flooring to identify and fix any lifting or damage early, preventing similar risks in future. 3. The damaged radiator cover near the rear toilet has been sanded and sealed, and painted with washable paint All radiator covers have been reviewed for wear and are now part of the bi-monthly maintenance inspection. Any worn surfaces will be promptly refinished or replaced. 4. The broken outdoor gate and its attached wooden fence were removed and replaced with a secure removable, child-safe gate. All broken wood, splinters, and rusted nails were safely disposed of. Outdoor structures will be inspected weekly by staff and formally checked monthly by management to ensure all equipment remains secure and hazard-free. 5. The gaps and holes in the shock-absorbent surfacing have been filled and repaired, creating a smooth, even surface. The play surface is now checked daily before outdoor time and included in the monthly facility audit. Repairs will be arranged immediately if damage is found

The window glass was replaced, and they will ensure that such cracks etc are reported and replaced as soon as possible. 2. Both containers under the sinks were removed. The Montessori sanitary area was deep cleaned and was discussed with the cleaners. A deep cleaning in these will be added to cleaning records. 3. Cleaning sheets have been updated and implemented. They will be signed by cleaners daily and checks completed by the manager. 4. The skirting board was repaired and discussed with staff the importance of reporting any hazards. 5. A new table was purchased discussed with staff the importance of reporting any repairs. 6. The step has been repaired and checking the stairs has been added to the risk assessment. 7. Hand dryer -Staff have been advised to use a certain cleaning product when required if stains are hard to remove. Flooring- Staff have been advised to use a certain cleaning product and a scouring pad to help remove the tougher staining on floor. Surfaces and storage in the classrooms have been more detailed on the cleaning sheet and staff are required to clean more regularly on a daily basis and a deeper clean on a Friday. Drink bottles that were in a tray will be placed in a different area of classroom and the wooden table has been removed. Classroom fridges are used daily, and the staff has been advised to keep a closer eye on the cleanliness of the fridges throughout the day. Management have reviewed the cleaning and risk assessment sheets for the rooms and have gone through these changes with the staff, to ensure all areas are covered in both classroom and bathroom

One panel of the fence detached from the supporting posts, creating a gap large enough for a child to pass through. An area of damaged plaster was observed exposing a hole in the wall next to the skirting board in the care room. The plaster had uneven edges and a visible build-up of dirt and dust. The ambient temperatures in the Little Folks room were not maintained of 18-22°C for children aged 1 year and older, as detailed in table 1.1 below. These temperatures posed a risk to the safety and comfort of the children in attendance. It is acknowledged that, when asked by the inspectors, the staff member accurately described the safe air temperature range and had already implemented and continued to implement control measures seeking to achieve this through opening windows and doors to enhance ventilation in the room, additionally the staff member brought the children to the garden while the room was cooling. The laminated surfacing on some shelving units and children’s tables in the Toddler and Preschool 1 rooms were heavily worn and coming away in parts, exposing an unfinished wooden surface which could not be effectively cleaned. 2. The children’s coat hook in the hallway of the Toddler bathroom was coming off the wall. 3. The door of a press under the sink in the Toddler room and a storage press in the Preschool 1 room where broken and coming away from the hinges. 4. One of the radiators in the Preschool 1 room was coming off the wall and needed to be secured. 5. The linoleum flooring was coming away from the wall in the hallway of the Preschool 1 bathroom. 6. There was a sticky residue on the surface of some shelves, radiators, and presses in the Toddler and Preschool 1 rooms which was accumulating dirt and debris. 7. Two rugs in the Toddler and Preschool 1 room were lifting and unravelling in parts and needed to be replaced. 8. Large areas of paint on the walls in the Toddler and Preschool 1 rooms were heavily chipped in parts exposing the plastered wall beneath. 9. Three wooden structures in the outdoor play area with fabric cushioned tops were heavily stained and soiled. The fabric coverings were worn and frayed and there was evidence of damp and mildew on the cushioned surface. 10. Although cleaning schedules were documented as being completed in the service, some of the care rooms, children’s bathrooms, and nappy changing areas were observed to be heavily soiled and required a deep clean as follows; - Areas of the flooring, skirting boards, nappy units, ledges, and shelving had heavy accumulations of dust, dirt, and debris in crevices and on surfaces. - There were heavy accumulations of cobwebs in areas of some ceilings and corners of the care rooms. - Two fabric cushions on the couch in the Toddler room were stained and required cleaning. - Fabric baskets used to store toys in the Toddler room were heavily stained and soiled

Provider's corrective action:

1. Two new shelving units have been purchased and the worn ones replaced. These will be added to the weekly risk assessments in the rooms. 2. This coat hook was tightened. These will be added to the weekly risk assessments in the rooms. 3. The hinges on the press were tightened. The hinges on all of the presses in the building were tightened. 4. The radiator cover was tightly secured to the wall. The radiator covers will be checked as part of the weekly risk assessments. 5. The lino has now been re-secured to the wall. The lino will be checked as part of the weekly risk assessments. 6. The sticky residue from adhesive tape has been thoroughly cleaned. Adhesive tape will no longer be used to stick artwork. 7. (c) An Immediate Action Notice was issued on the 31 January 2025 as the registered provider did not maintain an ambient room temperature of 18-22°C. On the day of Inspection, the temperatures in the care room were: • 11:12am room temperature measured as 12.1 °C, small heat source turned on. • 11:25am room temperature measured as 12.3°C, small heat source remains on. • 11:50am room temperature in the care room measured 16.7°C within the area surrounding the heat source, dropping to 13.1°C in the centre and corner of the care room. Radiators were observed to be cold to touch. Staff were aware of the low temperatures as temperature risk assessments had been completed by staff noting a temperature of 13.3°C on the day of inspection. The manager of the building was informed of the lack of heating, no rational was given for why this was not on. Children were visibly cold with one child presenting with cold extremities, red nose and pale. This impacts a child’s ability to regulate body temperature and maintain normal body functions

A similar non-compliance was also found during the last inspection on 18/10/2021. Additionally the narrow space between the two garden sheds was cluttered with tubing, timber, sacking and tents which also posed a risk of harbourage for pests

The registered provider stated the following corrective actions, and preventive actions have been undertaken: Corrective Action (d) 1. The registered provider has ordered a strong coloured plastic cover to seal off the underneath of the raised decking area from children. The registered provider will send on photographs when the work is complete. The registered provider will seal it off in the meantime with a temporary cover. The registered provider has cleared out the narrow gap between the two sheds. Preventive Action (d) 1. (c) The registered provider did not ensure that the premises was adequately heated and ventilated. 1. The room temperature in Ms M and Ms C’s room was 16 degrees Celsius at 10:35 and one of the staff members was observed wearing a coat. The heating was not turned on in the care room at this time and staff when questioned confirmed one of the heater’s was broken and they were not familiar how the other heater worked. 2. In Ms M and Ms C’s room black marks were observed on the wall just beneath the ceiling. (d) The registered provider did not ensure the service was cleaned and maintained as required as evidenced by the following. Many of these non-compliances were identified on the last inspection on the 15 November 2022. The corrective and preventive actions submitted by the registered provider did not prevent the reoccurrence of these non-compliances: In Ms C and Ms G’s room: 1. Paint was peeling off the skirting boards onto the floor. The mat at the door was heavily worn and coming away at places. 2. Pieces of the lacquer on a cupboard beside the rest area were peeling off and exposed the porous wood beneath. 3. Areas of the flooring were heavily chipped and the concrete flooring underneath exposed. 4. The wood around the sink units in the care room were stained and damp and could not be cleaned effectively. In Ms M and Ms C’s room: 5. The wooden cover on the radiator was broken and parts of the broken cover were on the floor. 6. One of the doors on the cupboard beneath the sink unit was broken off its hinge. 7. The skirting boards were covered in dust. 8. Cobwebs were visible around the windows, ceiling and light fittings. 9. The tile flooring was worn with cracks and some holes present. In Ms O’M room: 10. Dust was visible on the skirting boards and the paint was heavily stained. 11. The floor covering was ripped with visible holes and the concrete flooring underneath was exposed. This non- compliance was found on the last inspection 15 November 2022. The Corrective Action submitted at the time stated a new floor would be laid. Inspectors confirmed on this inspection that this action had not be completed by the registered provider. In the sanitary area: 12. Two holes were present in the flooring of the children’s sanitary area where a rectangular cover was missing
(c) 1. Ms. C and Ms. M’s room has a storage heater on one wall and another heater at ceiling height which was installed to address previous issues with the room temperature. The thermostat was reset to raise the temperature. There is also a radiator in this room which we have been having issues with and was not working on the day of the inspection. The heating has since been repaired. 2. Mould has been treated and the area has been repainted. (d) In Ms C and Ms G’s room: 1. Skirting boards have been painted. The doormat has been replaced. 2. The damaged shelving has been removed. 3. New floor covering to be installed over Easter. 4. A new sink area has been installed. 5. Radiator cover replacement has been completed. 6.The damaged shelving has been removed. 7., 8. The manager has arranged for all teachers to conduct a deep cleaning of their classrooms including all skirting boards. 9. New floor covering. Ms. O’M’s, Ms. C & G, and the bathroom floors will be replaced over Easter 2025. In Ms O’M room: 11. The manager has arranged for all teachers to conduct a deep cleaning of their classrooms including all skirting boards. 12. New floor covering. Ms. O’M’s to be replaced over Easter 2025. The registered provider did not ensure the following: (d) The flooring in Room 3 was not sealed to the wall and there was a build up of dirt and debris in behind it as a result. This was identified as a non-compliance on the last inspection held on the 15 November 2022 and actions put in place failed to prevent a recurrence. (e) The following issues were identified in the sanitary facilities: o In the sanitary accommodation used by Rooms 2, 3 and 4; the pressure of the water when the mixer tap was turned to warm water was too high, causing the water to splash out of the sink onto the floor. Two urinals were also co-located in the sanitary facilities but access to the toilets is restricted if the urinals are being used. One toilet and wash hand basin must be provided for every 11 children. The service is registered to cater for up to 55 children

Corrective Action The service has now secured interim access to another child's toilet through the Sand Room. The additional sanitary accommodation will be available exclusively for staff and children of Mead Day Care centre and a key to this facility will be held by all staff members in Mead Day Care Centre. Preventive Action The service is planning on removing the two urinals present in the onsite bathroom which will then provide for privacy and complete use of the child's toilets in that area. In addition, the service plan on installing a fifth toilet in this bathroom which will address the non-compliance. Management hope to have this work completed within 3 months

(c) 1. There was a malodour present in the children’s bathroom which was also the nappy changing room throughout both days of inspection indicating the ventilation in place was inadequate. (d) 2. The linoleum flooring in the Caterpillars, Treehouse, and Montessori room was heavily stained in parts with areas of black discolouration, indicative of a potential presence of mould. 3. There was no toilet seat on one of the children’s toilets in use. A second toilet seat was loose and moving while children were trying to use the toilet. The corrective and preventive actions submitted following the previous inspection of the 11th January 2023 did not prevent the reoccurrence of the non-compliance under point 2 above

The registered provider submitted the following response: Corrective and Preventive Action 1. Instructions have now been placed on nappy changing sheet to remind staff each morning to ensure bathroom window open. 2. The linoleum floor in the caterpillar room will be completely replaced. The dark areas of discoloration on the linoleum in the treehouse will be removed and replaced. The dark areas of discoloration on the linoleum in the Montessori classroom will be removed and replaced. Fitting to be completed within the next week. A dehumidifier will be rotated periodically among the relevant rooms to ensure no damp within the building. 3. New toilet seat purchased and loose one fixed by maintenance

In the sanitary accommodation of the Toddler and Montessori rooms the following was observed: o The surface of the step used by the children in the Toddler room to use the toilet was worn and did not have a wipeable surface. o The surface of the chair in Montessori sanitary accommodation was worn and did not have a wipeable surface. 3. However, now they have more heaters, and it is warm. (e) Suitable nappy changing facilities were not provided in the service. Nappy changing mats were placed on the floor in the lobby area at the entrance to the two toilet cubicles in the sanitary accommodation located off each pre-school room when the children’s nappies were being changed. This did not afford privacy for the children or maintain their dignity when they were having their nappies changed. This non-compliance was previously identified on inspection of the service on 09/11/2022. As part of the subsequent corrective action and preventive action response submitted to the Inspectorate, the registered providers did not demonstrate that suitable nappy changing facilities were made available in the service. The regulatory requirement remains unmet

Provider's corrective action:

(e) Registered provider spoke to their handy man/maintenance man who informed that it would be not safe to put the unit up on the wall as it’s a partition wall and would not hold such weight. Manager is looking into something on wheels that they could bring to the room, but there is no place to store it. Foldable units that they have found are not suitable for such big children who attend their service. They are searching the internet for a solution. Service needs more time
