- (d)In the outdoor area work remains outstanding since the last inspections dated the 16th of September 2025 and the 5th of March 2026 and actions as stated by the registered provider to address them have not taken place. • The section of the roof where the leak occurred has been replaced however the additional work on the internal ceiling and walls remains outstanding. The ceiling and walls in the Toddler playroom were stained and damaged due to a leak in the roof. • In the outdoor area the maintenance work and the painting of the fence and wooden structures remains outstanding
Provider's corrective action:
- : The registered provider has stated in the Corrective & Preventive Action form (CAPA) the following: Outdoor area work has commenced and will be completed before July 31 st. The ceiling and walls in the Toddler playroom stained and damaged due to leak in roof: - The service has obtained a commitment from the Maintenance Department in the University that this work will be carried out and completed during the closure period first two of August. The service will endeavour to continue with regular maintenance and upkeep

- The following areas were not maintained and posed a risk to the children Toilets: 1. Mould and black staining was evident at the juncture between the wall and ceiling in one of the childrens toilets. Mould growth compromises indoor air quality and can trigger respiratory issues for children. 2. The paint on the walls in both toilets was stained under the hand towel dispenser and the yellow paint on the walls outside both toilets was stained. This posed a risk of cross-contamination and compromises infection control protocols by making cleaning of the areas difficult. 3. There was an odour emanating from the toilet used by the preschool children which demonstrated that the toilet had not been thoroughly cleaned and disinfected. Sanitary areas are high-risk environments for the spread of pathogens such as bacteria and viruses. The windows were also closed which resulted in limited fresh air circulating in this area. 4. There was a hole in the plaster of the wall behind the door in the second toilet used by the preschool children which posed a risk to the children. 5. The step up used by the children to access the wash hand basin was observed to be stained. Areas not maintained: 6. Two floor mats at the entrance to the service and outside the toilets were stained and dusty. It is acknowledged that one of the floor mats was removed on the day to be cleaned. 7. At the entrance door there was an accumulation of dust gathered on the steel surround of the door. 8. There was an accumulation of dust and rubbish behind the adult couch in the room. 9. It is acknowledged cleaning checklists had been signed for the cleaning of the toilets however on the day of the inspection this did not appear to been actioned
Provider's corrective action:
- 1. The school has been notified of some of the cleaning issues that came up. The caretaker has washed and cleaned the bathroom walls. All mould present has been removed. 2. The painting in both the bathrooms and outer yellow walls has been completed by the school caretaker 3. The odour that was present in the bathrooms has been monitored and the toilet has been disinfected regularly. 4. The chipped plaster that was present has since been filled in and the spring in the floor that caused this chip has been fixed and is working perfectly. 5. The children’s step up step has been cleaned thoroughly. 6-9. The accumulated dust by the front door has been cleaned and removed and checked more regularly. More thorough cleaning observations will be done to prevent anything cleaning issues in the future

- (d)The following areas in the outdoor areas were found to be poorly maintained and repaired with no cleaning/ maintenance records available to demonstrate that these areas were cleaned and maintained routinely on a weekly/ monthly basis. • The all-weather ground surface areas were unclean with leaves and mud from the ground surface and trees of the larger outdoor area. • The larger outdoor area had a section with dry earth where pieces of rocks were visible. This section had no additional covering to maintain the area for the children to play that’s unique to the outdoors. • There was a large stagnant pool of water with leaves underneath the swings making it difficult for the children to use. • The latch on the shed door was broken and the door was in need of maintenance. • Two manholes were protruding from the grassy area. • The children’s play equipment and materials was dirty with mud and bird excrement. • Broken toys were not removed and were left in open and damaged bins which were full of leaves and stagnant water. The above issues were identified as posing a potential risk to the preschool children while playing in the outdoor areas and to their physical wellbeing and development
Provider's corrective action:
- : The registered provider has stated in the corrective and preventive form (CAPA) that the following actions have taken place: The all-weather area was powerhouse and cleaned of leaves and mud. Large rocks were removed from outdoor play area and bark ordered from landscaper evergreen landscapes. The leaves and mud was removed from swing area and soil with sand was used to fill in the large holes. The latch on shed door replaced. The manholes have now covered. The outdoor play area cleaned weekly and chairs and tables daily. A skip was ordered and all broken toys have now been disposed of and new toys have been purchased. Regular power hosing and check of broken toys and weekly cleaning sheet for outdoor have been put in place

- (b) The storage of cleaning materials within the sanitary facilities was observed during the inspection. This practice presents a significant risk of cross-contamination and compromises infection control protocols. Sanitary areas are high-risk environments for the spread of pathogens, and storing cleaning equipment in these spaces can inadvertently facilitate the transfer of bacteria and viruses to other parts of the premises. All cleaning equipment and agents were not stored in a dedicated, secure location away from children and hygiene-sensitive areas to ensure safety. It is essential that sanitary accommodations are used exclusively for their intended purpose and are not utilised for general storage (c) Evidence of mould was observed on the painted surfaces within the disability access bathroom. Mould growth compromises indoor air quality and can trigger respiratory issues for children. (d) The timber flooring in both preschool rooms was found to be in a state of disrepair, characterised by excessive wear and visible gaps between floorboards and at the wall junctions. This poses a risk to health and hygiene. Compromised flooring surfaces create gaps that trap dust, dirt, and organic matter, facilitating the growth of bacteria and making effective cleaning impossible. These structural defects present potential trip hazards and could lead to splinter injuries for children playing on the floor. It is acknowledged that remedial work to replace the flooring commenced on the afternoon of the inspection
Provider's corrective action:
- (b) All cleaning equipment and materials were removed from the sanitary accommodation areas, and a new designated storage facility was created in an alternative location. These materials were safely stored out of reach of children and away from hygiene sensitive areas. (c) Wall areas where there was evidence of mould were treated and all areas were repainted. Regular checks throughout the premises were scheduled to ensure all areas were checked for evidence of mould or dampness. (d) New floor covering was applied to both Preschool rooms in the premises

- (b) Visitor access to the premises was not managed appropriately. On the day of the inspection the entrance gate to the area accommodating the Naionra facility and the front door of the preschool building were left open. The magnetic lock on the gate failed to engage. This allowed unrestricted access for unauthorised adults, creating a safety risk for the children
Provider's corrective action:
- (b) Following the inspection of the service, the magnetic lock on the entrance gate was adjusted to ensure it closed immediately after each use. Staff performed daily checks of the gate and the front door as people entered and exited the building

Maintaining high-quality, washable paintwork throughout all rooms and sanitary facilities is essential for effective cleaning and to ensure maximum protection against the spread of infection
Provider's corrective action:
- On April 7th, 2026, the Sunshine Childcare Service Manager inspected the affected areas. Maintenance followed on April 13th, including cleaning, sanding, and repainting the walls and nappy-changing steps with washable, scrubbable paint. To maintain compliance, the service updated its maintenance schedule to include quarterly inspections of all paintwork in all rooms and sanitary accommodations. Any identified wear will be logged and repaired within four weeks, or sooner if urgent. Moving forward, all maintenance will exclusively use high-durability, hygiene- compliant washable paint

- 1. The outdoor play area at the rear of the premises was not maintained, clean, safe and suitable for children to use on the day of the inspection. This non-compliance was highlighted in the last inspection report dated 09/05/2024 and posed a risk to the health and safety of the children. • Toys, the mud kitchen, ride on bikes and buckets were full of stagnant water and leaves. This posed an infection control hazard. • A box containing earth for sensory play contained stagnant water. • The all-weather surface was covered in moss in different sections and posed a slipping hazard to the children and staff. • A large slide was knocked over. • The gate to the oil tank and coal bunker was open and accessible to the children
Provider's corrective action:
- The outdoor play area was thoroughly cleaned and all hazards were removed to ensure it is safe, hygienic and suitable for childrens use. All equipment, including toys, ride-on bikes, buckets, and the sensory play box, were emptied, cleaned, and disinfected, with contaminated materials removed and replaced as required. The all-weather surface was treated to remove moss and eliminate slip risks. The slide was repositioned and secured to ensure stability. The gate to the oil tank and coal bunker was secured to prevent access. A full risk assessment of the outdoor area and equipment was completed

- (d) 1. Sections of the outdoor play area to the rear of the facility was waterlogged. This was detailed as a non-compliance on the inspection reports dated 9 September 2021, 22 November 2023 and 28 June 2024 and responses given by the registered provider in previous corrective and preventive actions has not prevented this reoccurrence. This posed a risk to the safety of the children. (e) 2. The water pressure and flow of water at the wash hand basin in the nappy changing area upstairs was inadequate to facilitate hygienic hand washing
Provider's corrective action:
- 1.To resolve the water-logging issue to the rear of the facility would take a substantial financial investment which the business is not in the position to undertake at the moment given the current financial climate. If the weather is particularly bad and it is not safe for the children to access this area, children will get outside time at the front play area or alternatively the staff will bring the children on trips to the local park. The play area at the rear of the facility will only be used when it is safe to do so. 2. The taps in the upstairs nappy changing area were assessed by a plumber and the water pressure is now at an adequate pressure to ensure hygienic hand washing can take place. Management will monitor water pressure regularly in the service

- (d) 1.There was a tap in the nappy changing area that appeared corroded. 2.The worktop in Preschool room 2 appeared torn and worn in particular around the sink area. 3. The floor surface at the sink area in Preschool room 2 was in a poor condition. This posed a potential risk to the wellbeing of the staff and children using these areas

- ; The registered provider has stated in the CAPA response dated the 19/12/2025 that: 1. For the corroded the tap in the changing room attached to the Wobbler room, a new tap has been installed. 2. The worktop around the sink in Preschool room 2 has been replaced. 3. A new floor mat has been bought and located at the sink area in Preschool room 2. The monthly risk assessment will include the above areas for review and actions as required

- : The registered provider has stated that: The skylights has been sealed from the outside. The walls below the skylight in the staff bathroom were cleaned and treated for mould and the walls will be painted by the end of the week. The surrounding ceiling panels that were stained have been replaced. Regular checks of the skylights for the year to pre-empt issues arising in the future and to act as reminders to address any issues immediately have been scheduled

- (d)There were areas on the premises that were not adequately cleaned, maintained and repaired. • The outdoors ground matting was unclean with dirt under and around the climbing frame and other permanent play fixtures. • The timber play fixtures appeared unkempt and in need of maintenance. • The all-weather ground surface matting was noted to have wide cracks starting from the playroom exit door down to the end of the outdoor play area. • There were areas on the wall of the bathroom lobby where it was not painted and maintained. - : The person in charge stated in the CAPA response form that: The service has contacted a local group to work on the maintenance and cleaning of the outdoor areas including the play and work equipment and materials. This work will be in progress over the coming months to get the identified areas to a better standard or replaced

- (d) The service was not maintained as evidenced by the following: 1.There was an old rustic waste bin in the outdoor play area that was accessible to the preschool children. This posed a safety risk to the children and staff. 2. There was an area to the rear of the premises that was unkempt and had accumulated a large collection of waste such as cardboard paper, broken play and work equipment. This area was behind a fence, but it posed the risk of attracting rodents onto the premises. 3. A large set of goal post at the end of the garden appeared to be broken and this posed a safety risk to the children

- The registered provider has stated that: 1. The old rustic waste bin in the outdoor play area was removed from the premises. 2. All the waste materials were removed. 3. Pins were purchased to secure the goal post into the ground to ensure the safety of the children while playing in the outdoor area. The registered provider will ensure that maintenance work will be carried out regularly by the service and this includes the committee involved in the care of the community building

A broken basketball net and poll was lying on the ground. These hazards in this area of the service could cause potential tripped risks for the children playing in this area of the service

- (d)The registered provider has arranged for a maintenance company to carry out cleaning works in the large outdoor play area. An ongoing arrangement for regular maintenance of all outdoor areas is planned. All broken equipment in the outdoor area has been removed or repaired

- (b) 1. Secure access to the service must be provided. On the day of the inspection the front door of the service remained opened for long periods of the day with a bin against the door holding it open. This was to facilitate staff and children accessing and leaving the outdoor play area. The gate entrance at the road had a handle located above the height of a child but was unlocked. To ensure the safety of all children on the premises, a system must be in place at the entrance to the premises to ensure any person entering the creche is always checked in by a staff member. (d) 2. The adult couches in each room were worn and some had areas where the covers were torn. To ensure a high standard of cleanliness in the service, all furnishings need to be a proper state of repair and easily cleanable. 3. Throughout the premises the paintwork has signs of wear and tear. Pain was chipping on many of the walls throughout the building. 4.The natural grassed area in the outdoor play space was not maintained and available for the children to use. The grass was high and needed to be cut. The pots used for planting were full of dirty, stagnant water
Provider's corrective action:
- 1.A keypad security system was fitted at the front door of the service on both the inside and the outside. 2. New couches were purchased for each room in the service. 3. A plan for the repainting of all rooms in the service was submitted with dates arranged for the refurbishments. 4. The grassed area outdoors was cut, the area cleaned, and all pots replanted

- Following the inspection a meeting with the landlords was held to discuss maintenance issues. They have informed the registered provider that they will be painting the premises over the Christmas preschool holidays as the work can only be carried out while the school is closed

- (d) 1. The floor covering adjacent to the kitchen area and in the centre of the room was worn and torn and posed a tripping hazard for both the children and the staff. Flooring in the sanitary accommodation was not flush with the wall and flooring between the lobby and the sanitary accommodation was worn. Routine thorough cleaning of the floors could take place when the floor surface was not smooth and durable. 2. Paintwork on the ceiling near the florescent light and the wall where a fridge was located had not been repainted therefore the areas were not smooth to facilitate cleaning. 3. Sections of the wooden countertop were worn which prevented thorough cleaning of the area

- 1. Quotes to replace the flooring in the sanitary area and the kitchen area have been sought. 2. A painter to paint the ceiling and walls in kitchen area and to repaint the bathroom has been sought. 3. The counter and kitchen will be replaced within the next 4-6 weeks

- (d). 1.The outdoor covering in play area was damaged and large sheets of this damaged hard plastic were positioned against a wall in the outdoor area and were accessible to the children which posed a safety risk. The registered provider stated that she was in consultation with a company to have these replaced and new covering to be erected. 2. Doors, skirting boards and walls in the upstairs playrooms and corridor were in need of maintenance. The registered provider stated that this work is to be completed over the summer

- :The registered provider has stated in the response that: (d) 1.The hard plastic sheets in the outdoor area have been removed from the service. The service has received two quotes for the construction of an new overhead shelter in the outdoor area. 2. This work is scheduled for the summer
