Provider's corrective action:
; The registered provider has stated in the CAPA response dated the 19/12/2025 that: 1. For the corroded the tap in the changing room attached to the Wobbler room, a new tap has been installed. 2. The worktop around the sink in Preschool room 2 has been replaced. 3. A new floor mat has been bought and located at the sink area in Preschool room 2. The monthly risk assessment will include the above areas for review and actions as required

The grassy outdoors area off the Wobblers playroom, and the areas to the front of the service used by the older preschool children remain poorly developed in terms of enough natural and open-ended materials/ equipment. The manager stated that there were plans to develop these areas and to increase the variety and quantity of open-ended materials and equipment in these areas. However, no time schedules were available as to when the work will begin and how long it will take to complete this work. Children must have the opportunities to be outside as often as possible, in all weather conditions, except where a risk assessment does not allow. The equipment and materials in the outdoor play areas must support children’s play, movement and exploration, and provide opportunities exclusive to the outdoors
Provider's corrective action:
: The manager stated in the corrective and preventive form that the following measures were taken: The long-term plan for the front play area is the removal of the gravel mound and creation of a much more open and free flowing space. This project will require input from architects and other contractors and resource availability for this is currently being examined. In the interim, the service have begun work on removal of some fences to improve flow and visibility of the outdoor space. In this space more natural and open-ended materials will be supplied e.g., tyres, planks of wood, crates, mud kitchens, pots, and pans. These items will allow the children to explore and make items that are of interest to them. This area will be complete by May 10th. The area off the Wobblers room is currently having a sensory footpath developed, leading to a construction play area. This will help the sensory needs of some children and help them to remain calm and relaxed. Sand and water play and art easels can also be used in this area as needed. Teepees have been constructed from natural timber and are used by children for seating and relaxation. A music wall is being developed, comprised of hanging pots and pans that children can use to explore sound. This area will be complete by April 29th. Once this area is completed the children will have an open plan area to explore. Opening this space will allow children of different ages to mix. Each room has a chain and hook mechanism where the side doors can be left open allowing children access to go freely between inside and out. Parents provide wet gear to be kept in the
Regulation 23 — Safeguarding health, safety and welfare of child
General Safety: 1. There were no steps to facilitate the children to climb safely onto and off the nappy changing unit located between the Baby and Wobblers playroom. The staff were observed to lift each child on and off the nappy changing table and this posed a potential risk to the health and safety of the staff members and the children. Infection Control: 2. Hygienic nappy changing practices were not observed to help prevent cross infection between children and children and between children and staff and this was at variance with the nappy changing policy for the service. This was highlighted in the previous report dated 18/07/2023. 3. Aprons and gloves were not removed by one member of staff member following each nappy changing. This staff was observed returning to the playroom with the child and still wearing the apron and gloves worn while nappy changing. This increased the risk of cross infection. Childrens hands were not observed to be washed after they had their nappies changed. This increased the risk of infection. Action submitted by the Registered Provider

General Safety: The manager stated in the corrective and preventive form that the following measures were taken: 1.Steps have been purchased to allow children to climb safely onto the changing unit. This will reduce the potential risk to staff and children. Management met with staff and explained that going forward steps are to be used for the general safety of staff and children and that the lead educators will ensure that this happens. Infection Control: The manager stated in the corrective and preventive form that the following measures were taken: 2. and 3.The procedure for nappy changing and handwashing is displayed in each changing unit. All staff have been retrained in the new procedure to reduce this risk of cross infection. All staff are aware that gloves and aprons must be removed before entering the playroom and both child and staff must wash their hands. A staff meeting was held with lead educators where they were informed that it is there responsibility to ensure that all staff follow these procedures and the importance of handwashing and that cross infection can be reduced by regular handwashing for staff and children. All signs have been put in prominent places. Lead educators will ensure handwashing occurs before mealtimes, after using the bathroom and having their nappy changed. Staff will ensure their hands are washed before returning to the playroom
