- On the day of inspection, the inspector observed the following which posed a risk of safety and infection to the children: 1. One window in the sanitary area for the Junior Preschool room was observed to have cobwebs attached. It is acknowledged that cleaning records were available however evidence that effective cleaning had taken place was not available. 2. A plug in the hallway downstairs was broken, and electrical cables were accessible behind the plug. This was identified on last inspection on 13 October 2025. The corrective actions submitted following those inspections failed to prevent recurrence of this non-compliance. 3. A radiator in the hallway in the Junior Preschool room was not fixed to the wall; it was observed to be hanging off the wall and loose. This practice was found to be non-compliant on the previous inspection on 13 October 2025. - 1. All windows were cleaned to ensure cobwebs were removed. This will be monitored going forward. 2. The plug downstairs was repaired following the inspection. The manager will ensure to conduct regular checks of the premises going forward. 3. The radiator was fixed to the wall. - (c) At 11:40 in Little Oaks 2 room, a room temperature of 16.3° was recorded. Care room temperatures should be between 18° and 22°C. (d) The following areas were observed to require cleaning, maintenance or repair: • A unit for storing art supplies in Little Oaks 2 was observed to have marks on the side. • Mats in Little Oaks 1 were observed to be soiled. • The kitchen units in Little Oaks 1 room were observed to be unclean. • A fabric pizza toy in Little Oaks 1 room was observed to be soiled. • The nappy changing unit was observed to have debris in the corners. This was found as a previous non- compliance on inspections carried out on 10 October 2022 and 21 January 2025. • There was a hole in the corner of the floor in the corridor near Little Oaks 2 room. • There was evidence of peeling paint on the ceiling in the corridor area. • There was visible evidence of mould like marks on the ceilings and walls in both care rooms. • The walls in Little Oaks 2 care room were visibly dirty. • A metal tin located in the kitchen area was observed to be rusty and sticky in Little Oaks 1 room which did not allow for effective cleaning

- The following response was received from the registered provider: Corrective and Preventive Action (c) Additional heaters have been purchased and the room temperature will be monitored. The heaters and oil heating will be turned on early in the morning. (d) The items and areas listed have been cleaned and painted. Unclean toys, a mat and the metal tin have been removed. The hole has been repaired. Daily cleaning will be carried out and risk assessments to ensure damaged items are removed

- (d) The following were observed as needing repair on day of inspection. 1. Approximately five ceiling tiles in the Toddler room were cracked and broken. Two ceiling tiles in the children’s sanitary adjacent to the Toddler room were stained. One ceiling tile in the Toddler nappy changing room was observed as cracked and broken. These posed a safety risk to children and staff using these rooms. 2. One of the toilets in the Montessori 3 sanitary facility was not maintained in proper working order and required repair. The toilet cubicle door was secured from the interior and an out of order sign was displayed. During subsequent discussion, a staff member advised that the flush mechanism on the toilet was not working, and this was later confirmed when tested by the person in charge
Provider's corrective action:
- 1. The person in charge ordered new ceiling tiles to replace the cracked and broken ceiling tiles. The ceiling tiles will be checked regularly going forward to avoid cracks and staining. 2. The person in charge organised for the plumber to come to the service to repair the flush mechanism on one of the toilets. The person in charge stated that all plumbing works will be reviewed regularly

- (c) 1. There was no means of heating the child sanitary areas. The temperature was recorded at 16.0 °C. In order to facilitate the care needs of the children the temperature of the room should be maintained between 18°C and 22°C. This was a non-compliance in the last inspection report from the 26 April 2023 where the registered provider had stated that the radiator in the toilet area had been switched on. This was not observed on inspection. (d) 2. The toys in the outdoor area required cleaning. It is acknowledged that cleaning records were available and signed. Evidence that effective cleaning of the outdoor toys had taken place in the service was not available. The lack of cleaning posed a risk of infection to both staff and children. 3. No hot water was available from the sink in Room B as the hot water tap was not functioning. The children in this room were observed to wash their hands in cold water
Provider's corrective action:
- The following response was received from the registered provider: Corrective and Preventive Action 1. The water and heating had been turned off the night prior to inspection dure to new taps being installed. They have now been turned on and the heating has returned to normal. 2. The outdoors toys have been power washed and a new cleaning schedule is in place. The service plan to adhere to the cleaning schedule. 3. A new tap is ordered and the service is awaiting the plumber. The service will monitor all water temperatures and flow in the future

- (d) 1. In the Pre-school 2 sanitary facility, dirt and debris were visible on the floor beside the skirting board, in one of the toilet cubicles. There was also a black speckled residue on the skirting board and on the ceiling leading up to the skylight window. Non-compliance in relation to residue was also found in the same sanitary facility, at the previous inspection. 2. A malodour was evident in the sanitary facility adjoining Pre-school 2, despite two windows being open. 3. Evidence that effective cleaning had taken place in the Baby room nappy changing area, was not available. It was acknowledged that cleaning records were available and signed, however, there was a build-up of debris along the edges of the nappy changing unit and the extractor fan had a build-up of dust
Provider's corrective action:
- (d) 1. The CAPA response stated that this skirting board has been cleaned and repainted. The sky light has also been painted. Going forward, staff will ensure that the sky light is not opened during bad weather, in order to help prevent the area from getting wet and causing black spots. 2. The malodour was no longer apparent when the toilets were flushed. Going forward, all toilets will be flushed before children enter the room. 3. The changing area has been cleaned and the vent has been taken down and cleaned. The registered provider spoke to the staff about making sure that all of the changing area is cleaned. The registered provider also directed staff to check the vents regularly, in order to ensure that they are clean. Going forward, the changing mat will be removed and the changing table will be cleaned and the mat will be cleaned in between nappy changes. The vents will be checked regularly, to ensure that they are clean

It was acknowledged that a window was open at this time, however, the temperature was five degrees below the required minimum temperature

- An electrician has installed a third radiator in the care room, which is larger than the other radiators. Staff will ensure that the window in the sanitary facility is kept closed during colder days. Going forward, staff will take daily air temperatures and if required, another radiator will be installed

- (d) Two of the ceiling tiles in the Busy Bees room required repair. One of the ceiling tiles was covered with black tape and the second ceiling tile was partially covered with tape. The inspector was advised that replacement tiles had been acquired but were damaged before installation was possible. (e) One of the toilets in the Talented Turtles sanitary facility was not maintained in proper working order and required repair. The toilet cubicle door was secured and could not be accessed. During subsequent discussion, a staff member advised that the flush mechanism on the toilet was not working. This issue was found in relation to a different toilet at the previous inspection on 19 January 2023

- (d) The ceiling tiles have been replaced. Any broken ceiling tiles will be replaced by the landlord. (e) The toilet has been repaired. As the building is old, repairs are sometimes required. A plumber will be contacted to fix any issue that may arise

- (d) 1. A tap at one of the wash hand basins in the children’s sanitary facility was not maintained in proper working order and required repair. There was no running water available. 2. High water pressure was noted at one of the wash hand basins in the nappy changing facility. When the tap was turned on to test the water temperature, the force of the water caused it to spill out over the wash hand basin and onto the floor. The use of this tap therefore posed as a slip hazard

- (d) 1. The written reply stated that the tap in the children’s toilet facility has been fixed and now provides running water. In order to ensure that this does not happen again, management will ensure that any broken equipment will be attended to immediately. 2. The broken tap at the staff wash hand basin has been fixed. The water pressure has been reduced and it is flowing as normal. To prevent this from reoccurring, all equipment will be fixed immediately by the service’s plumber

A foot pedal operated bin was observed to be not sealed, and a foul smell was evident in the nappy changing area in the Baby and Wobbler care room. 3. - Corrective Action 1. The mechanical ventilation system vent was cleaned. 2. The nappy bin has been replaced. 3. Small flies have been removed from the area and there is no evidence of them in the room now. Preventive Action 1. Ensure that staff clean the vent regularly as part of the cleaning schedule. 2. Ensure the nappy bin is sealed to prevent foul smells. 3. Prevent small flies from entering. The door to the nappy changing room will be closed

- (e) 1. While it was acknowledged that, on the day of the inspection, a sufficient number of toilets and wash hand basins were available to accommodate the 39 children present in the adjoining premises, this was not consistently ensured. A review of past attendance records demonstrated that: • 49 children attended this building on 11 March 2025. • 46 children were present on the 12 March 2025. • 47 children were in attendance on 13 March 2025. • 45 children attended on 14 March 2025. As 1 toilet and 1 wash hand basin are required for every 11 toilet trained children, the 4 toilets and 4 wash hand basins allocated for the children’s use were therefore insufficient. 2. There was inadequate drainage at the wash hand basin in the staff toilet facility, located in between the Wobbler 1 and Wobbler 2 care rooms. Water was observed to pool in the basin when the tap was turned on. In addition, high water pressure was noted at the wash hand basin in the Playschool 1 care room, when the handle of the mixer tap was turned to allow the flow of warm water. Non-compliance in relation to water pooling and high water pressure at wash hand basins, was also found at the previous inspection on 8 April 2024. The CAPA response at the time outlined that a plumber had attended the service and made the necessary repairs. However, this did not prevent the issues from reoccurring
Provider's corrective action:
- (e) 1. As per the corrective action stated under Regulation 23, in the adjoining premises, there are now five toilets and five wash basins solely allocated for the children’s use, as the adult toilet has been redesignated as the fifth children’s toilet. A new staff toilet facility has been fitted in the adjoining main building, to replace the toilet that is now designated for the children’s use. Going forward, a designated sleep room, which is no longer in use, will be converted to a second staff room/kitchen area. This will allow the service to convert the existing staff room/kitchen in the single storey premises, into additional sanitary facilities. This work is scheduled to take place during the October mid-term break. 2. The stopper has been removed from the wash basin in the staff toilet facility, located between Wobbler 1 and Wobbler 2. It was identified that the stopper was not releasing fully, which was restricting drainage and causing the water to pool. With the stopper now removed, the sink is draining adequately and functioning correctly. The plumber initially attempted to resolve the high pressure issue by reducing the water pressure locally at the affected sink. However, this did not fully resolve the problem. As a result, it was necessary to reduce the water pressure at the main source before the water enters the building. The plumber has advised that this adjustment may negatively impact water pressure in other areas of the building and could potentially lead to further issues. The service will continue to monitor the situation and will address any issues that arise

- (d) The soft matting outside was splitting. It had some repairs carried out since the last inspection however the spaces between each square remained a hazard. This reduced the opportunity for children to wash their hands comfortably in warm water

- 1. The matting on the playground has been repaired with soft matting granules and sealant. The registered provider has committed to ensuring the matting on the playground will be monitored and maintained as required. 2. The registered provider has stated all taps have been repaired by himself and stated that they will be monitored by the staff daily to ensure they are working and have hot water. The registered provider has stated that they have not been added to the checklist as they are to be monitored by the staff in each care room

- (e) There was an insufficient number of wash hand basins available for the 29 children in attendance, at the time of the inspection. It was acknowledged that two toilets and two wash hand basins were accessible adjoining the care rooms and that a nappy changing facility was provided within the staff toilet facility, which had three toilets in total. However, only 1 wash hand basin was available in this staff toilet facility, which was allocated to the staff members and to the 10 children who required nappy changing. One wash hand basin is required for every 11 children and one wash hand basin is required for every 8 adults

- The accessible toilet has been designated for use by staff members and staff have been informed of same. The toilet facility formerly referred to as the staff toilet is now solely for the children’s use

- (c) 1. There was a thick accumulation of dust visible on the mechanical vent, in the staff sanitary facility. While sound from the vent was audible, a musty malodour was apparent and there was evidence of mould on the ceiling. This posed a risk of inadequate ventilation. (d) 2. In the children’s sanitary facility, there were plaster chippings on the floor of one of the toilet cubicles. The corners of both toilet cubicles and the skirting boards were dirty and cobwebs were also visible in the area. The risk of cross infection due to lack of cleanliness in the children’s sanitary facility, was also found at the previous inspection on 17 May 2023. The CAPA response at the time outlined that all areas of the sanitary facility would be cleaned daily. However, this remained an issue
Provider's corrective action:
- (c) 1. The CAPA response stated that the staff sanitary facility has been cleaned thoroughly and that the vent was opened and cleaned. In July 2025, the staff sanitary facility will be painted and the ventilation system will be thoroughly cleaned again. The hall committee will ensure that this is working correctly. In addition, the registered provider has arranged to meet with the committee to make plans for toilet facility renovations. (d) 2. The children’s sanitary facilities have been deep cleaned. Staff have advised that due to the window being open for ventilation, dust comes in from the outdoor play area, as the mud kitchen is located outside. The sanitary facilities will be deep cleaned every day, specifically where the dust enters. A notice has been placed on the wall in the sanitary accommodation to remind staff of this. The reply also stated that new skirting boards will be fitted in the children’s sanitary facilities, during the summer months, in order to prevent the wall from becoming damaged in the future

- (c) The ceiling light in the staff sanitary facility was not functioning. It was noted that the globe lampshade had been removed from the ceiling and was stored on top of the toilet. The absence of functioning lighting in this area was also found at the previous inspection on 8 September 2022. The CAPA response that followed stated that the light had been repaired and would be checked daily. However, this remained an issue. (d) 1. In the staff sanitary facility, there were two wall tiles missing from the wall behind the wash hand basin. This issue was found at the previous inspection on 22 September 2022. In the subsequent CAPA reply, the registered provider outlined that the tiles would be checked daily for damage, however this failed to prevent the recurrence of the non-compliance. 2. In the outdoor play area, one of the concrete slabs was broken and posed as a potential trip hazard
Provider's corrective action:
- (c) The electrician has called to the service and has fixed the light. The electrician was also asked to check this issue thoroughly, to prevent it from reoccurring. The registered provider will do a daily check on all lights, to prevent this from happening in the future. (d) 1. The wall tiles have been fixed to the wall securely. Checks will be conducted to ensure that the wall tiles are secure. 2. The gap in the concrete has been covered safely. The registered provider will ensure that checks for gaps in concrete are carried out regularly

- (d) (1) Mechanical air vents in the sanitary areas of Pods 3 and 5 were dusty, hindering the extraction of moist and stale air. (2) Two accessible cupboards, under the children’s sinks in Pod 4, were observed to be dirty and one had used tissues lying on the bottom of the shelf. In discussion, staff stated that the outside doors and handles are sanitised but the inside cupboards are not cleaned on a daily basis

- (d) (1) The air vents in Pods 3 and 5 were cleaned. Air vents have been added to the cleaning schedules. (2) The cupboards under the sinks have been cleaned and these will be checked by management regularly

- (e) There was an insufficient number of staff toilets and wash hand basins available. The service’s staff toilet facility consisted of one toilet and one wash hand basin. However, 1 toilet and 1 wash hand basin are required for every 8 adults and there were 14 adults working at the setting on the morning of the inspection. This included the registered provider and three students
Provider's corrective action:
- The response outlined that the service will no longer take work experience students and this will reduce the number of adults on the premises. From next September, the service will consider the number of children attending who require AIM support, as this is also having an impact on the number of staff required. Going forward, it will be ensured that no more than eight staff are on the premises at one time

- (e) (1) While there were adequate sanitary facilities for both adults and children, the downstairs sanitary area, where there were two toilets facing each other, were observed not have any doors separating the areas. This could potentially compromise the privacy of children; should two children require to use the facilities at the same time. In conversation, the registered provider stated that they are currently seeking a tradesperson in order to resolve the issue

- The following statement was made by the registered provider; (e) (1) Custom made doors are being made due to the size of the area , when ready these will be fitted by the carpenter. Adults will supervise the toilets to ensure that only one child at a time is in the area until the doors have been installed

- (d) 1. One of the timber fences in the outdoor play area that divides the two outdoor play spaces for the Creche and Playschool rooms was in poor repair and needed to be replaced. The service was aware of the damaged fence prior to inspection and plans replace the damaged fence. 2. The two wall surfaces around the nappy changing mat area located beside the Creche were damaged and not provided with a smooth durable easily cleanable surface
Provider's corrective action:
- (d) 1. The fence in poor repair has been taken down and the service is awaiting a new fence to be installed. This fence was separating the two designated play areas for the creche and playschool room. This fence was not part of the boundary securing the outdoor play area. Daily check of the outdoors including fences has been added to the service daily hazards check list. The fence will be replaced in due course and will be reviewed on the next inspection. 2. An easily cleanable surface has been placed on the wall behind the nappy changing unit and will be checked on a regular basis to ensure it is in a proper state of repair

- (d) In the Toddler room, there was evidence of mould growth adjacent to both windows. (e) Two of the taps within the sanitary facilities on the first floor were not maintained in proper working order, as follows: − A tap at one of the wash hand basins in the sanitary facility adjacent to Junior Preschool 3, was covered with a disposable glove and was not working. The supervisor later advised that this tap had been leaking. − A mixer tap in the nappy changing facility adjacent to the Toddler room, was wrapped in cello tape and was unavailable for use

- (d) The response stated that the mould growth adjacent to both windows in the Toddler room has been removed. The area has been painted and will be closely monitored to prevent reoccurrence. (e) The taps within both sanitary facilities have been replaced. Going forward, the service’s safety officer will ensure that all taps are working correctly and will replace any broken taps immediately

- (c) In the written response the registered provider has stated that a new heating appliance was installed in the children’s toilets and nappy changing facility located beside Preschool Room 2. (d) 1. In the written response the registered provider has stated that the red couch in poor repair was removed and replaced with new soft seating purchased and now located by the book area in the Montessori room 2. When the Inspector reviewed the two refuse bins on the second day of the inspection, the bins in poor repair were removed and replaced with newly purchased lidded pedal operated bins located in the Montessori playroom and children’s toilets therefore the non-compliance was corrected at time of inspection. 3. When the Inspector reviewed the toilet seat on the child size toilet located beside the Montessori Room on the second day of the inspection, it was fixed in position on the toilet seat and no longer loose therefore it was corrected at time of inspection

- (d) 1. The paintwork on the walls in the Senior Discoverer’s room, the Explorer’s room and the Junior Active care rooms was not kept in good repair. The paintwork, particularly at the lower wall levels, was observed to be flaky, chipped and peeling and was not maintained in good condition which was required when caring for children. 2.There was a hole measuring 1 inch in diameter on an internal wall directly beside the nappy changing area attached to the Explorer’s room that was potentially accessible to any child having their nappy changed. The wall area was in disrepair and was not maintained in good condition. It was acknowledged that the person in charge informed the inspector that the above 2 issues were on a list of repairs/works to be carried out by a maintenance person
Provider's corrective action:
- 1. The paintwork in the Senior Discoverer’s room, the Explorer’s room and the Junior Active care rooms has been assessed by 2 painters who are currently in the process of preparing paintwork quotations for the registered provider. The staff attached to the premises will continue to submit maintenance request forms to the services’ operations team when repairs need to be addressed. 2. The hole on an internal wall directly beside the nappy changing area attached to the Explorer’s room has been repaired. Maintenance request forms will be submitted to the services’ operations team when repairs need to be addressed

Rosscarbery Montessori School, Rosscarbery · Regulation 29 — Premises
- (e) There were no adult sanitary facilities available for use in the service. • At the previous inspection, on 21 June 2022, it was found that both an adult and child toilet, as well as the nappy changing table, were in the same room with no cubicle to separate the adult toilet. In addition, there was no wash hand basin for adult use. • A regulatory compliance meeting was held on 09 November 2022 and the registered provider stated that building works would be carried out during the Christmas 2022 break. • Further to this, the registered provider contacted Tusla to state the required building works would be carried out in the summer of 2023 to rectify this issue. • At this inspection it was found that building works had not happened. An adult wash hand basin had not been installed and the toilet had been rendered out of use with black and yellow tape applied over the lid. • Separate, adequate, hygienic and soundly constructed toilet and sink facilities are required to be provided for adults employed in the service
Provider's corrective action:
- & Supporting Documentation Submitted The registered provider stated that a builder has agreed to work on the disability bathroom to return it to a cubicle and to install a wash hand basin. A headed invoice from a building contractor was submitted to evidence that the remedial works are to be completed within the next few weeks. Summary Comment A document in the form of an invoice was submitted to the early years office; it was reviewed and found to adequately evidence that the non-compliance is to be addressed. This will be reviewed at the next inspection
