- d) The building was observed to not be well maintained and required repair. The premises was observed to be not adequately cleaned, maintained, or repaired, as evidenced by the following: • The outdoor area adjacent to the baby room was deemed unusable due to extensive overgrowth accumulated during the winter. Furthermore, the area was cluttered with hazards, including broken toys and bags of garden rubbish. • A tile in the eaves of the roof appeared to have been displaced and was in need of repair. • Several cracked windows were identified in the care rooms on the ground floor, as well as one on the upper floor. Additionally, a window handle was missing. • An additional toilet seat was required in the children's toilets on the ground floor. • Multiple defects were observed in the functionality of sinks and taps across several facilities. These included slow drainage, active leaks, and a seized tap. Consequently, surrounding floors and walls were found to be wet
Provider's corrective action:
- The area directly outside the baby room has been cleared and all broken toys have been removed and disposed of. The misplaced roof tile above the playground has been repaired and secured. All broken and cracked windows within the crèche have been repaired and replaced. A third baby toilet seat has been purchased and installed. This will remain available in the toilet area to support the children's needs. All sinks and taps throughout the crèche have been cleaned using a limescale remover. All taps and sinks are now clear, fully functional, and draining properly. These items added to daily and weekly check list and risk assessment forms

- In the Montessori room the surface of the presses in the kitchenette area were peeling which did not provide a smooth surface for cleaning
Provider's corrective action:
- The covering on the presses has been removed and the presses painted to provide a smooth surface. A maintenance record book is now available to log any furniture and equipment that requires repair. The registered provider will ensure that maintenance checks are conducted on a regular basis

- The wallpaper was torn over the nappy changing area off the Wobbler room. In the sanitary closet the wallpaper was also torn over the sink. Smooth surfaces must be available in areas that require regular cleaning. Action submitted by the Registered Provider
Provider's corrective action:
- The area over the nappy changing mat and over the sink in the sanitary closet off the Wobbler room has been tiled. The registered provider will ensure that all sanitary areas are maintained and tiled where necessary with smooth surfaces that require regular cleaning

- (e) The service did not have sufficient sanitary accommodation for staff members. On the day of inspection there were 15 staff members attending and there was one toilet available for staff. One toilet and one handwash basin is required for every eight adults
Provider's corrective action:
- (e) The registered provider submitted evidence that the sanitary facilities, with partitions, had been installed. As a preventive action they stated that they will ensure that adequate sanitary facilities are maintained in line with health and safety regulatory requirements and the service maintenance plan will include regular review of sanitary accommodation to ensure facilities remain adequate and fully operational. Any future changes in staffing numbers or service capacity will include a review of sanitary provision to ensure compliance with the required ratio

- (c) The mechanical ventilation in the shared nappy area between the Wobbler 1 and Wobbler 2 rooms was observed not in working order and could not ensure that stale air was removed. (d) The wall next to the nappy changing mat in this area was showing signs of wear and tear, as there were sections observed where the paint had chipped off the wall
Provider's corrective action:
- (c) The registered provider has installed new mechanical ventilation units to ensure adequate airflow and compliance with hygiene and infection control requirements. The unit has been installed and is fully operational. As a preventive action the service has an established monthly inspection process which includes the ventilation unit. The fan and vents will be checked regularly and cleaned (including removal of dust build-up by vacuuming) to ensure effective operation. Ventilation will continue to be included as part of monthly inspection checks, and any faults or reduced performance will be addressed promptly by a qualified electrician. (d) Tiles have been installed on the wall behind the nappy changing mat to provide a smooth, and easily cleanable surface that supports infection prevention and hygiene standards. As a preventive action environmental checks will be included in routine cleaning and health and safety inspections to ensure all surfaces remain intact, sealed, and suitable for cleaning. Any damage or deterioration will be repaired to maintain compliance

- (d) The service was not maintained in a proper state of repair as demonstrated by the following: • The walls of the sanitary area directly located off the Blubell room had been repaired leaving multiple small areas of rough plaster, these areas were unable to be cleaned effectively. • The artificial grass in the outdoor play area to the front of the service was frayed and worn at two of the joining’s resulting in a potential trip hazard
Provider's corrective action:
- Corrective Action (d) • The walls have been sanded and repaired in the bathroom. They have also been fitted with new tiles and wallpaper. • A whole new astro turf surface has been fitted to the area in question which has been recommended for schools and creches. Preventive Action (d) Registered provider has discussed with all their branch managers, as part of their daily indoor and outdoor assessments to include the investigation of all surfaces, walls and in general all structural fittings, and to report anything they may deem to be worthy of further investigation by senior management and/or specific trade professionals

- (d) The window in the sanitary facility was cracked in several places and was re-enforced with brown tape. This was found to be non -compliant at the last inspection on 14/6/2022. The Corrective and Preventive Action (CAPA) response submitted by the registered provider afte r the last inspection stated that “The window in the sanitary facility has been replaced”. Photographic evidence was submitted after the inspection indicating that the window had been repaired however the recent inspection had found that the window was still cracked

- The window in the sanitary facility has been repaired. The registered provider will ensure that any areas accessible to the children is safe and maintained

- 1&2 All defective seating was removed from the Shining Stars and the Rising Stars rooms. Cushions were purchases instead. The manager will ensure that all equipment will be included on the daily risk assessment to ensure infection control measure are maintained. Any new equipment purchased will either have a wipeable surface or covering that can be washed in a machine

- (e)Toilet roll holders are now available in the children’s toilets at the end of the corridor. The registered providers will ensure that the holders remain in place and that the toilet roll is accessible to the children on a daily basis

- (d)In the Butterfly room the cloth dolls were visibly dirty, the couch at the rest/relaxation area was heavily stained. The children chairs were visibly stained. In the Caterpillar room the cloth dolls were visibly dirty, the toy kitchen was heavily stained. Some of the containers for storage of toys in both rooms were visibly dirty. (e) One of the toilets in the sanitary accommodation in the hallway had no seat which did not provide comfort and stability to a child when using the toilet
Provider's corrective action:
- Corrective & Preventive Action (d)The service had a deep clean with all effected areas and items cleaned or replaced. A system is now in place for regular cleaning of furniture and play equipment which have now been included on the cleaning schedule. (e) A toilet seat has been ordered for the toilet. A regular safety checks will be conducted to ensure all the equipment in the sanitary area is available

- (d) The plastic rim of the porthole window on the large outdoor activity centre was cracked and broken with small pieces missing. This posed a risk of children’s fingers becoming trapped in the gaps or an injury being caused by the sharp edges. (e) In advance of the inspection, the sole wash hand basin had been removed from the sanitary accommodation adjoining the ECCE room, preventing children from washing their hands in this area before returning to the care room. The registered provider and staff members informed the inspectors that the wall-mounted wash hand basin had recently become loose and unstable and had been removed as a safety measure following a risk assessment. It is acknowledged that the children were supported to wash their hands at the low-level sink in the ECCE room on their return to the care room but the adjoining sanitary area was not equipped with suitable and sufficient hand washing facilities
Provider's corrective action:
- (d) The edges of the porthole have been safeguarded with artificial grass to protect children from the sharp edges. Preventative Action: The service provider will conduct routine inspections in the garden area to identify any potential risks that may endanger children. (e) The hand basin in the ECCE restroom room was removed the day before the inspection after a risk assessment was done. The use of the sink within the ECCE room for handwashing was a temporary solution until the original sink in the restroom was replaced on the evening of the inspection on 20/06/2024. Preventative Action: In the interest of future safety measures, the service provider will close the ECCE preschool room until the sanitary facilities are repaired or replaced
