- (2) (a) & (b) One validated reference was obtained for the one staff member. A second staff member will check all the staff files to ensure compliance prior to employment commencing. (d) The two adults attached to the service were requested to provide their Police vetting as soon as possible One Police vetting was provided by 4/12/’25 and confirmation for the Police vetting request for the second adult was provided on 6/12/’25. A table for all adults attached to the service has been implemented with Police vetting as a separate column to garda vetting. A second member of management will also check the staff files prior to employment commencing. (3) The table implemented table for all the adults attached to the service has been added to the hiring process for all adults, not just staff members. This is to be completed prior to employment beginning

An Immediate Action Notice (IAN) was issued to the designated person in charge during the onsite inspection in relation to a Garda vetting Disclosure not on file for one adult working with the children in the service. A response to the IAN issued was received after the inspection on 8.8.’25, it was deemed adequate in mitigating the risk
Provider's corrective action:
- (a) & (b) The additional references were obtained and validated for both staff members. All the reference documentation is now complete and filed in the staff folders. There is a new staff checklist updated to ensure that two validated references are required before employment starts. Regular file audits are scheduled quarterly to maintain compliance. (c) The Garda vetting for the one staff member has now been completed, verified and filed in her personal records. The recruitment policy has been updated to ensure that no staff member begins work until the Garda vetting confirmation is received. The compliance checks are added to the onboarding checklist

- (2) (a) 1. The references are now on file. 2. The references that had not been validated have been validated via telephone. To prevent these non-compliances from reoccurring, management will review the staff files to ensure the appropriate information is recorded. New employees will not be permitted to begin work until the references and validations are on file. (d) The Police vetting for 2 adults was applied for and received. The 1 adult who required Police vetting from 2 countries resided in outside of Ireland, ceased employment in the service on 12.09.’25

- The person in charge ensured police vetting was applied for on the day of inspection. The person in charge reviewed their recruitment policy and will ensure to monitor recruitment files regularly

(c) Garda vetting disclosures received from the National Vetting Bureau of An Garda Síochána were not available for 1 staff working at the service. An immediate action notice was issued to the registered provider on the day of the inspection. In a reply to the notice on the 22nd of October 2024, the registered provider stated that this person would not attend the service until the vetting disclosure was returned. A staff who was on leave was resuming work at the service in the meantime to ensure the adult/child ratio was maintained. (d) Police vetting was not available for 1 staff member who had lived in a state other than Ireland, for a period of longer than 6 consecutive months. (3) The procedures specified in paragraph (2) in relation to the vetting of all staff were not carried out prior to any person being appointed, assigned or allowed access to or contact with a child attending the pre-school service

- (2)(a) The staff member who was present on the day of the inspection who did not have references on file no longer works at the facility. (2)(c) Garda vetting has been applied for the relevant staff member. While awaiting the disclosure to be returned to the registered provider, this staff member is not working at the service. (d)The staff member who required Police vetting is no longer employed at the service. (3) The vetting of all staff will be carried out prior to any person being appointed, assigned or allowed access two children in the service
