(a)&(b) One validated reference was not available for one adult attached to the service. (d) Police vetting was not available for two adults attached to the service. This poses a risk to the safety of children. (3) The registered provider did not ensure to carry out all required validations and checks prior to any person being appointed, assigned or allowed access to or contact with a child attending the pre-school service. This poses a risk to the safety of the care of children

Provider's corrective action:

(2) (a) & (b) One validated reference was obtained for the one staff member. A second staff member will check all the staff files to ensure compliance prior to employment commencing. (d) The two adults attached to the service were requested to provide their Police vetting as soon as possible One Police vetting was provided by 4/12/’25 and confirmation for the Police vetting request for the second adult was provided on 6/12/’25. A table for all adults attached to the service has been implemented with Police vetting as a separate column to garda vetting. A second member of management will also check the staff files prior to employment commencing. (3) The table implemented table for all the adults attached to the service has been added to the hiring process for all adults, not just staff members. This is to be completed prior to employment beginning

General Safety: 1. There was a loose cable attached to the fridge accessible to the children posing a risk of injury to children. Action submitted by the Registered Provider

General Safety: 1. Cable clips were bought and the wire secured to the wall. All wires secured has been added to the weekly maintenance checklist

General Safety: 1. On arrival to the service the door into the building and the door of the playroom were not secured to prevent unauthorised persons into the room and from children exiting unsupervised. 2. There was 1 fire extinguisher free standing and accessible to the children near the adult desk posing a risk of injury to children. Infection Control: 3. The carpeted floor covering and the large carpet mat in the playroom were stained and unclean posing a risk of cross infection. Action submitted by the Registered Provider The designated person in charge stated that: -

Provider's corrective action:

General Safety: 1. Signage has been placed on the said door to remind all staff to pull the lock across after each opening. Staff will continue to be reminded to secure the door. 2. The fire extinguisher has been attached to the wall and should it happen again it will be re-attached immediately. Infection Control: 3. The floor covering is on the priority list of works to be completed in the building within a timeframe of approximately 6 months. The carpeted floor covering is hoovered daily, it is steam cleaned monthly and a spot cleaner is used as when necessary. The large mat was removed from the room and a new mat was ordered

(d) 1. There was gaps in between some of the outdoor soft matting tiles in a section of the outdoor area that were installed since the last inspection

Provider's corrective action:

The designated person in charge stated that: - Corrective and Preventive Action 1. The matting was pushed back together. It will be maintained and will be pushed back together
