- (1) (g) (h) (i) One childs’ enrolment record did not have the required information on the name and telephone number of the child’s registered medical practitioner, record of immunisations, if any and written parental consent for appropriate medical treatment of the child in the event of an emergency. (3) The person in charge did not have records for 13 children who attended the service. On discussion with the person in charge, they were aware these records were not available and stated they would get them as soon as possible. The service did not have accessible children’s records on the premises for all children attending the service, posed a risk to the safety and care of the children

- (1) (g) (h) (i) The person in charge obtained the missing information identified on the child’s enrolment records. Children’s enrolment records will be reviewed and counted in early September and updated if necessary and as needed throughout the year. These records will remain stored onsite. (3) The person in charge obtained all enrolment forms for the 13 children who attended the service and did not have a record onsite on day of inspection. These records will remain onsite and available to staff and for inspection going forward. The person in charge created a new policy with regards to the storage of children’s enrolment records onsite

A system has been put in place to ensure all children’s details that may be required by the service are recorded and immediately available should they be required. A process has also been put in place for how to address any gaps in information should they be missing. The registered provider is responsible for this to be completed

The registered provider verified that all current enrolment forms have full and accurate parent/guardian contact details. As a preventive measure, an enrolment checklist has been implemented to ensure full parent/guardian addresses are provided before a child commences. (f) The missing information regarding illness, disability, allergy, or special needs has been obtained and the child records have been updated accordingly. It has been verified that all current child records now contain complete and accurate health details. A mandatory enrolment checklist has been implemented, which requires full health information to be available, before each child commences. (g) The missing medical practitioner details have been obtained and the child records have been updated. The registered provider verified that all current child records now contain complete and accurate medical practitioner information. As a preventive measure, an enrolment checklist has been implemented to ensure that the medical practitioner’s name and contact details are in place, before each child begins attending. (h) The missing information regarding the children’s immunisation status has been obtained and the forms have been updated. The registered provider verified that all current child records now contain full immunisation information and has implemented a mandatory enrolment checklist, requiring immunisation details for each child, before they commence. (i) All parents/guardians were contacted to obtain written consent for emergency medical treatment and the child records were updated accordingly. Preventive Action New child record forms from Early Childhood Ireland have been purchased. These forms are clearer for parents and guardians and all areas are identified in a clear and appropriate manner. These forms are now available for the children enrolled to start in the service in September 2025

- (b) The response from the registered provider stated that the start dates have been added to the five forms and that going forward, forms will be checked at the start of every month. (f) A staff meeting was held to discuss an action plan for the child, which is now in place. Staff then met with the child’s parents to discuss the action plan, which was subsequently signed by the child’s mother. Going forward, it will be ensured that a plan is put in place for any children attending the service, who may require same. (h) The immunisation records have been received for seven of the children. The eighth child no longer attends the service. The forms will be checked monthly by a nominated staff member
