- (1) Of the all records reviewed the requirements under (f)- (i) were not available. This included records of illnesses or allergies, whether the child had been immunised or the availability of the GP contact details. Not ensuring this information is available to the staff caring for the children may delay the appropriate care of the child
Provider's corrective action:
- (1) The registered provider has developed a software process for parents/guardians to complete and update all requirements as set out on (f) –(i) above. The prototype was reviewed and contained all requirements

- (1)(h) Five of the thirteen records sampled for preschool children did not contain the dates immunisations, if any, which were administered to children
Provider's corrective action:
- (1)(h) All forms have been returned with immunisations signed and dated. The service will ensure that all immunisations administered to a child are dated and signed

- (1)(h) Each of the 10 records for preschool children did not contain the dates immunisations were administered
Provider's corrective action:
- (h) The service is committed to promoting the health, safety and well-being of all children by maintaining accurate and up to date immunisation records. A message has been sent out to all parents/guardians to review current immunisation status and to provide updated documentation with dates. To ensure all immunisation records remain accurate and up to date, checks on all files will be carried out. Parents are requested to provide immunisation details with dates on enrolment. The records will then be reviewed three times a year and also when a new child starts in the setting. A designated staff member will be responsible for: monitoring updates and ensuring all records are maintained appropriately

- The start and end date have been hand written on all existing registration forms. Registration forms have been amended to add child start and end dates
