- (1)(e) The names and contact details of other adults who were authorised to collect children were not available on the 19 records of a Pre School Child (enrolment form) reviewed on the 26/02/2026. (h) Twelve forms did not provide the information by parents indicating which immunisations their children had received. (i) There was no written parental consent for medical treatment of children in the event of an emergency documented on the Record of Pre-school Child (enrolment form). It was acknowledged that a parental consent was provided for the administration of medication if necessary
Provider's corrective action:
- The following was acknowledged at regulatory compliance meetings on the 01/05/2026 and the 08/05/2026. • The registered provider acknowledged the records were incomplete on the day of the inspection. The children’s records were updated following the inspection. • The children’s records contained all the required information at the time of the regulatory compliance meetings

- (1)(i) Written records of consent for appropriate medical treatment of the child in the event of an emergency were missing in respect of the 20 pre-school records sampled posing a potential risk to the children’s well- being in the event of an emergency
Provider's corrective action:
- The following statements were received from the registered provider: (i) All 20 written records of consent for appropriate medical treatment of the children in the event of an emergency are now on file. The pre-school record has been amended to include the missing information going forward. All forms will be reviewed annually to ensure compliance

Bright Minds Preschool, Salthill · Regulation 15 — Record of pre-school child
- (1) All 27 records of children attending the service were reviewed. None of the items listed in (1) (a) – (i) above were available for one child. In respect of the remaining 26 records the following information was missing: (b) Nineteen dates on individual records noting when the children first attended the service. (c) Provision of a cease date for twelve records. (d) Five parent’s addresses and one parent’s name. (e) Three records of authorisation for collection (f) One record of the details of illness, disability, allergies or special needs. (g) Two records of the general practitioners names and telephone numbers. (h) Nine records of immunisations (i) Twenty-one records did not contain any wording regarding written parental consent for appropriate medical treatment in the event of an emergency. Failure to maintain the required information of children’s pre-school records may potentially compromise the care and safety of the children
Provider's corrective action:
- (1) (a) – (h) The missing information has been completed for the relevant children’s pre -school records and a complete record is now on file in the service for one child for whom the information was unavailable. At the beginning of term staff will review each child’s pre-school record to ensure that the information required has been submitted. (i) Wording regarding written parental consent for appropriate medical treatment in the event of an emergency has been added to seventeen of the twenty-one records. Four children have since left the service and the information for these children is no longer required

Pixie Garden Creche, Gort · Regulation 15 — Record of pre-school child
- (1) A record in writing (registration form) for each child attending the service was not maintained by the registered provider. This posed a risk to the health and safety of the children attending in the event of an emerging. • There were 28 preschool children recorded as attending the service on the children’s attendance roster during the week of the inspection and only 13 of the 28 registration records were available in the service. • Of the 17 children present on the day of inspection, records were not available for 7 of the 17 children in attendance. Of 13 records available, the following was noted: (a) The name and date of birth of the child was incorrectly recorded for one child, the date recorded stated the child was 4 months old, when the child was older. The manager was aware of this error but had not corrected the mistake. (b) The date on which the child first attended the service; was not recorded on 8 of the 13 registers reviewed. (c) The date on which the child ceased to attend the service; was only available on 1 form reviewed, this question was not an option on the other 12 form reviewed. (e) Who had authorisation for the collection of a child was not recorded in 11 of the 13 forms available. (f) Details of any illness, disability, allergy or special need of the child, together with all the information relevant to the provision of special care or attention was not recorded in 1 form. (g) The name and telephone number of the child’s registered medical practitioner was not recorded in 5 forms. (h) Record of immunisations, if any, received by the child was not recorded in 4 forms. (i) Written parental consent for appropriate medical treatment of the child in the event of an emergency was not recorded in 12 of the 13 forms
- Records were not available for each child attending the service and therefore not available: - to parents for their own child. - not available to staff who were caring for these children. - not available to the inspection team during the inspection (4) All records were not retained for the required period of 2 years from the date on which the child ceases to attends the service
