- (h) One record did not contain contact details of the child’s registered medical practitioner. (i) Two of the sampled records did not contain written parental consent for appropriate medical treatment of the child in the event of an emergency
Provider's corrective action:
- (h) The registered provider gathered the relevant information of the child’s medical practitioner and put the documentation on file. (i) Written parental consent for appropriate medical treatment was signed by parents/guardians in the event of an emergency, and these forms were put on file. A checklist was created to ensure all required children’s information is gathered on registration going forward

- 1. There was no enrolment form for 2 children attending the service
- (e + h) There was 1 enrolment form that did not have details of authorisation for collection of the child and did not have a record of immunisations on file. (h) There was 1 enrolment form that did have records of immunisations on file
Provider's corrective action:
- 1. The registered provider submitted the 2 enrolment forms required. Enrolment forms will be collected and reviewed prior to children attending the service in the future
- (e + h) The registered provider submitted the enrolment form with all the up to date details including immunisations and authorisation to collect completed on the form. These will be fully completed prior to child beginning in the service. (h) The enrolment form has been updated to include the record of immunisations

- All immunisation records that were not fully recorded were returned to parents for completion

- (e) One of the 12 records assessed did not have adequate information for the authorisation of collection of a child. (f) Three of the 12 records assessed did not have a record of any illness, disability, or allergies recorded, if any. (g) Two of the 12 records assessed did not have a record of the registered medical practitioner for each child. (h) Two of the 12 records assessed did not have a record of immunisations, if any, received by each child. Not having children’s records complete may hinder the care and safety of children in the service if this information is needed for the provision of care
Provider's corrective action:
- The registered provider has stated that all children’s enrolment files are now updated and provide the required information of (e) Authorisation to collect a child, (f) All records are updated to include records of illnesses. (g) All records have the GP name and contact details. (h) All records have been updated to include the immunisation details for each child. The registered provider has committed to reviewing all children’s files on a regular basis and make sure they are up to date

It was acknowledged the service furnished the inspector with an email demonstrating that the requirement for the said consent was issued to the parents to complete on the afternoon of the inspection
Provider's corrective action:
- Written parental consent for appropriate medical treatment of a child in the event of an emergency was drafted and circulated to the parents immediately after the inspection, all the parents have returned the permission. The software developers who provide the software used in the service have been advised to add the required permission to the digital enrolment form also

- The following statements were received from the registered provider;
- (b) All five records were updated with start dates. (g) The General Practitioner’s contact number was added to the file. (h) All five immunisation records have been added to the children’s files. (i) Missing consent has been completed for each of the five records. Staff will check each file on enrolment to ensure that all information is completed on each child’s pre-school record

- (f) There were no individual care plans with relevant information for the provision of care for three children in the Montessori Butterflies care room and for two children in the Bumble Bees care room who had individual medical conditions. In conversation with the staff in both care rooms, it was stated that individual care plans for each of the children were not available
Provider's corrective action:
- The following statement were received from the registered provider; (f) Two individual care plans were returned by parents/guardians. The three remaining parents/guardians have stated their children do not require medication. It was explained to parents/guardians that medication for a chronic illness cannot be administered without an individual care plan in place

- (1) Of the 20 records reviewed and an attempt to review a further two records was made, however the two further enrolment forms were not available for inspection. (f) Of the 20 forms assessed one enrolment form did not contain details of any illness, disability of allergy for the child. (h) Of the 20 records assessed three records did not contain any records of vaccinations received by the children. Not having relevant information available to the staff caring for these children may hinder their appropriate care in the event of an accident or incident or in the event of an illness in the service
Provider's corrective action:
- (1) The registered provider stated the records were available in the filing cabinet. (f) The form as per the information provided by the registered provider has been updated by the parents. (h) The vaccination records are on file as per the information provided by the registered provider

- & Supporting Documentation Submitted: The registered provider stated that the enrolment forms that had omitted information have been updated. A copy of an updated form was submitted to evidence that this had been actioned. Going forward, the service sated that they would check each enrolment form when returned to ensure that all the information is recorded. Summary Comment A document in the form of a completed enrolment form was submitted to the early years office. It was reviewed and found to meet the Requirements of Regulation 15

Provider's corrective action:
- The manager stated that the children’s forms had been updated with the required information. Supporting Documentation Submitted The manager submitted samples of the children’s registration forms which had been updated. In addition, it was recorded that the manager will be responsible for ensuring that all registration forms will be completed from the outset of the the child’s attendance in the service. Summary Comment Submitted documentation was reviewed and found to be satisfactory in meeting the non-compliant subsections of Regulation 15 – Records of the pre-school child
