The Nest School, Greystones · Regulation 15 — Record of pre-school child
- (d) Of the sample of 10 children’s records reviewed, 3 records did not detail the address of the parent/guardian. (h) Of the sample of 10 children’s records reviewed, while it is acknowledged that each of the parents/guardians confirmed that children had been immunised, there was no evidence of a record of these immunisations received by the children
Provider's corrective action:
- To correct the non-compliance here, the Manager has asked the teachers to check Childpaths profiles and flag any gaps in the information. This is currently being completed and updated. A group text was sent to each class by management requesting updated pre-school child details are on Childpaths to include a copy of each child immunisations for the child’s individual file. To prevent a re-occurrence of this non-compliance, an updated induction letter has been sent to all families starting in August to show what is required for compliance purposes. The software platform utilised has now added a feature where the address is submitted by management at the point of creating the parents profile. An updated check list for each class will be issued prior to new enrolments and new class members to ensure all these details are up to date and on the system before the child starts. Management to sign off on checklist in September that completed for coming year. This will be monitored by management on a continuous basis as new enrolment come in over the school year

Provider's corrective action:
- All parents have filled out their child’s immunisation record and included a copy of their records from their doctors. We have updated our application form to include the child’s immunisation records

- (1)(b) & (h) All enrolment forms have been updated with a start date for each child photo attached All enrolment forms now include a section requesting the child’s immunisation passport or a letter from their doctor with details of vaccinations received before the child can commence preschool. This has also been confirmed with the local medical centre. All enrolment forms will be checked before the child starts. Enrolment forms are more up to date with a section requesting confirmation of vaccinations

Provider's corrective action:
- (1)(b)(c) &(h) We have gotten parents to fill out Pre-printed Record forms with start and end dates for their children. We have also gotten copies of their vaccines for our files. Going forward we will use Pre-printed record forms for each child when they start. We will ask parents for a copy of their children’s vaccines too and attach them to the form

- The date on which the child first attended the service was completed on the 11 registrations forms that did not contain this information. Application forms will be closely monitored to ensure they also contain the correct information when the children start the service

Provider's corrective action:
- All child record forms for children will be double checked before being put into the file to ensure that all dates are completed. When child record forms are returned to the service they will be checked and dated correctly
