- Immunisation records were not available in respect of seven out of the twelve child enrolment records reviewed
Provider's corrective action:
- All parents were contacted and immunisation records were obtained and updated. Added to enrolment that no child will start until immunisation record is on file. The person in charge will be responsible for assuring this. The registered provider will check at the start of term

Little Acorns, Tullamore · Regulation 15 — Record of pre-school child
- (e) Two of the twelve records did not have information for the authorisation for the collection of the child. (f) The registered provider did not ensure that a health care plan was available in writing in respect of two pre-school children who required the provision of special care and attention. (g) One record did not have information regarding the child’s medical practitioner
Provider's corrective action:
- Children’s registration forms were reviewed and updated with any outstanding information. Any child requiring a health care plan now has one on file. Care plans will be asked to be provided by parents before children start preschool so that staff are aware of children’s needs and can provide this promptly. All children’s registration forms will be double checked to ensure that details are correct

Mie World, Ballycumber · Regulation 15 — Record of pre-school child
- In a sample review of twelve child enrolment forms the immunisation records were not available in respect of three children
- Written parental consent for appropriate medical treatment of the child in the event of an emergency was not available in respect of one child. It is acknowledged that the form was signed by the parent and was submitted to the Early Years Inspectorate on the 22.10.25
Provider's corrective action:
- All enrolment forms were checked, the three missing were sought from parents and submitted to TUSLA
- The child’s form was signed by the parent of the child in question which was acquired by management and submitted to TUSLA. Checks per term by compliance coordinator have been added to her list of duties, check of new children have been added to mangers role and compliance coordinators’ role

Stepping Stones Early Learning Centre, Ballinahown · Regulation 15 — Record of pre-school child
- Child enrolment records reviewed did not have details of immunisations in respect of nine children attending the service
Provider's corrective action:
- Immunisation records have been updated on child enrolment forms. All children’s files have been audited and updated. Service provider will ensure this information is provided with each future enrolment. Summary Comment The non compliance has been addressed. The corrective action stated will be assessed on the next TUSLA inspection

- The sample of children’s record forms reviewed by the inspector did not include signed written parental consent for medical treatment to a child in the event of an emergency
Provider's corrective action:
- The registered provider advised in the response to the inspection report that a consent form signed by parents and guardians for administration of medication to children in the event of an emergency is now in use in the service. As a preventive measure, the registered provider advised that the signed consent form for administration of medication in the event of an emergency will be included with the enrolment forms for children in the service

Provider's corrective action:
- Corrective Action (1) (e) (h) The registered provider stated in their response that children’s registration forms now include details of authorisation to collect their child and that immunisation records have been completed. The registered provider will ensure all children’s records are complete prior to the children commencing pre- school

- The registered provider stated in the response that the name and telephone number of the child’s registered medical practitioner is now recorded and a new revised standardised booking form has been developed (form included) (h) The registered provider stated in the response that a record of immunisations is now recorded and a new revised standardised booking form has been developed (form included) (i) The registered provider stated in the response that consent for the administration of an anti-febrile agent in an emergency is now recorded and a new revised standardised booking form has been developed (form included) Summary Comment In respect of the corrective actions taken documentary evidence was submitted to the office of the Early Years Inspectorate, reviewed by the Early Years Inspector and deemed to meet the regulatory requirement. The practices as stated will be reviewed on the next inspection
