- (b) There was no record of a start date on any of the records reviewed. (c) There was no space available for a record of a finish date on any of the records reviewed. (i) The records reviewed did not contain written parental consent for appropriate medical treatment of the child in the event of an emergency. This was found non-compliant on the last inspection dated 1st February 2023, the corrective and preventive actions submitted by the registered provider did not prevent the non-compliance from re-occurring
Provider's corrective action:
- (b), (c) and (i) Record forms have been purchased from Early Childhood Ireland and all files will now have this new record form going forward. The new up to date child record forms will be used

- (c) Four records reviewed did not have somewhere to record the date that a child ceased attending the service. (i) One record reviewed did not have written parental consent for appropriate medical treatment of a child in the event of a medical emergency
Provider's corrective action:
- (c) The four affected child record forms were updated using the current child record form and the required end dates were added. Staff have been reminded to use only the current child record form and regular checks will be carried out to ensure the correct version is issued. Any last versions have been removed. (i) The missing parental consent was obtained on the day of the inspection and added to the child’s record form. All child record forms were reviewed, and a check will be carried out to ensure parental consent is obtained and recorded for all new registration

- While the medical care plan was in place for one child outlining the steps to be followed should the child become unwell, it was observed that the emergency medication on site was out of date since December 2025 and the following information was not documented • the child’s name was not recorded on the medication • there was no prescription available for the emergency medication • the parent/guardian had not signed the care plan It is acknowledged the area manager did contact the child’s parent to request replacement medication during the inspection
Provider's corrective action:
- A new care plan was provided and in place ensuring to have outlined all the relevant details. The care plan is now displayed for educators and management to see with a clear review date

Provider's corrective action:
- We have changed our pre-admission policy to state that children cannot start until all relevant information required is provided and the registration form is complete. This will be signed off by the relevant manager or deputy manager prior to commencing in the creche

- (h) Immunisation record being sought from doctor surgery. We will ensure all documents are collected before the child’s start date in the setting, with evidence of immunisation on record or declaration of non-immunisation signed off by parents

- There was no record of the child’s medical practitioner on any of the documents reviewed as is required. (i) One record did not have written parental consent for the appropriate medical treatment of a child in the event of an emergency
Provider's corrective action:
- (g)(i) The child registration forms are now updated to include the child’s medical practitioner and parental permissions for new children attending the service. For previous children in the service parents have filled in their child’s medical practitioner and permissions details which we have attached to the child’s registration form. All child record forms have been reviewed to ensure all required fields are now fully completed in accordance with regulatory requirements. All child registration forms will be checked regularly by management. To ensure this does not reoccur, all forms will be reviewed with parents to verify that all child record forms are fully completed prior to commencement in the service

- Eight of the ten records reviewed had not recorded the date the children commenced in the service. (h) Three of the ten records reviewed had no record of the children’s immunisation status
Provider's corrective action:
- Corrective Action (1)(b) All books have been updated with start and finish date. (h) From the 3 records of children only 1 was required to be completed. Preventive Action To ensure this does not happen again one staff member will look after the registration forms to ensure all information is filled in and if not filled, they can make a note as to why

- (f) Health care plans for two children attending the service did not contain sufficient information to ensure the safe management of the children in the event they became unwell or required emergency administration of medication. One health care plan had not been updated to reflect changes in medication dosage
Provider's corrective action:
- (f) The parents of the children involved were given a new care plan to complete, and it was returned the following morning 09/10/2024. There is a checklist placed on the care plan boxes and this will be reviewed every quarter. Included in this is to check if the care plan is needed to be revised. Management will meet with the parents completing health care plans and make sure all documentation is filled out correctly and has sufficient information

Provider's corrective action:
- (1)(a)-(i) Child registration forms were sent to each parent to complete and return to the service with the required information detailed within them. There is now an enrolment folder in the service with blank enrolment forms within for any new starts

- (b) The date on which the child first attended the service was not recorded in one of the records assessed. (e) Details of the authorisation for the collection of a child were not recorded in one of the records assessed. (f) There were no written care plans for two children attending the service. A written immediate action notice under Regulation 23 in respect of Regulation 15(1)(f) was issued to the registered provider on 24 September 2024 by the Early Years Inspector. (g) The name and telephone number of the child’s registered medical practitioner was not recorded in two of the records assessed. (h) The record of immunisations received by the child was not available in four of the records assessed
Provider's corrective action:
- 6 out of the 10 records assessed now have the required information. Furthermore, all Child Record forms have been reviewed and have the required information in accordance with Reg. 15. (b) Date child first attended the service is now entered on the assessed child’s record form. (e) Details are now recorded on the assessed record (Authorisation for the Collection of the Child) (f) Two Care Plans completed, submitted and accepted. (g) Name and phone number of G.P is now recorded in two records assessed. (h) Immunisations now recorded on the four records assessed A copy of Reg 15 is attached to the front of the Child Record Folder to ensure that in future all Child Record forms have the required information in accordance with Reg 15. Checklist at the back of each child record form will be ticked and signed off by the registered provider to confirm all required information is included as per Reg 15

Happy Years Childcare, Celbridge · Regulation 15 — Record of pre-school child
- 1. There were no individual medical care plans available for two children attending the service who required prescription only medication to be administered in an emergency situation and on ‘as required’ basis for a pre-existing medical condition. A written immediate action notice under Regulation 15(1)(f) – Record of pre -school child was issued to the registered provider by the Early Years Inspector on 9 January 2024. A written response was received by the Early Years Inspector on 9 January 2024 from the registered provider in relation to the immediate action notice. The response was accepted by the Early Years Inspectorate
