Bright Stars Clonea, Dungarvan · Regulation 15 — Record of pre-school child
- A sample of 12 children’s enrolment forms completed by the parents was reviewed as part of the inspection process. These were found not to contain all the information required under parts (a) – (i) of this regulation which may compromise the capacity of staff to safely care for children. For example, the missing details including: • The contact telephone number for the parents or a relative or friend of the child can be contacted during the hours of operation of the service was absent on 11 forms. • Authorisation for the collection of the child was absent on all forms. • details of any illness, disability, allergy or special need of the child, together with all the information relevant to the provision of special care or attention not completed on 12 forms. • The name and telephone number of the child’s registered medical practitioner was not included on 12 forms. • Record of immunisations was not available for 12 children. • Written parental consent for appropriate medical treatment of the child in the event of an emergency was not completed on 12 forms
Provider's corrective action:
- The registered provider stated that they have worked with the staff team and developed a new enrolment form which includes the requirements for the information required under regulation 15. These forms have been given to parent to complete with the help of an onsite translator. Going forward, the registered provider will discuss the services requirement for information with the centre management, and they will ensure all forms are fully completed prior to a child joining the service

- (1) (h) Records of immunisations were not completed on any of the 12 children’s record forms
Provider's corrective action:
- The registered provider stated that they had contacted all the parents and received the immunisation records and placed them in each child’s file. They have amended the introduction handbook to include children’s immunisations records are required and must be kept up to date

- (1) Details of the children’s medical practitioner and a record of children’s immunisations were not included on the 12 forms reviewed
Provider's corrective action:
- The registered provider stated that the children registration form has been updated to include the details of the child’s doctor and their immunisation record. They stated a registration checklist has been devised to ensure the relevant details are completed on the child’s form before they commence in the service

- (1) (h) Of the 12 records reviewed the immunisations details for children were not available on three forms
Provider's corrective action:
- The registered provider stated that immunisation details have been received for all children and going forward no child will commence in the service without a copy of their vaccination records or a signed letter from the parent confirming that they do not wish to vaccinate their child. The registration forms have been edited to reflect this change going forward. They stated that at a staff meeting held on 19th January 2026 and staff were made aware to double check all children’s files and to ensure that no file is accepted without it being completed in total. Children’s files on receipt will also be double checked by the Co-Ordinator and Compliance Officer

- (1) On a review of six child registration forms, information required was not available. For example: • Three forms did not include details regarding authorisation for the collection the children. • Four forms did not include details of any illness, disability, allergy or special need of the child, or information relevant to the provision of special care or attention. • Five forms did not include contact details for the child’s doctor. • Three forms did not include a record of the child’s immunisations
Provider's corrective action:
- The registered provider stated that all early years forms are now complete with correct details. Going forward they will ensure all forms are complete before commencement in the future and all forms will be completed before commencement and will be double checked by staff

- (1)(i) Of the 12 child record forms reviewed, 2 did not have written parental consent for appropriate medical treatment of the child in the event of an emergency

- The registered provider stated that the required parental consent was recorded and going forward a staff member has been assigned the responsibility to ensure all forms are signed

- (1)(h) Of the 10 enrolment forms sampled, 9 did not include a record of the immunisations, if any, received by the child
Provider's corrective action:
- The registered provider stated that all child record forms have been updated to include a specific section titled ‘Immunisation status’. This section includes clear information explaining the requirement for immunisation details. All staff have been informed of the update and the necessity to have this information completed and checked at the point of registration

- A sample of 12 children’s enrolment forms completed by the parents was reviewed as part of the inspection process and was found not to contain all the information required under parts (a) – (i) of this regulation. For example, the missing details including: • the date on which the children first attended the service, • completed immunisation records for two children
Provider's corrective action:
- The registered provider has stated that all forms have been updated and parents were requested, via text message, to submit up to date information regarding their child’s vaccine records. Going forward spot checks will be carried out by management to ensure all forms are up to date with the required information

- The inspector reviewed a sample of 12 children’s registration forms; of these, 6 did not include the required information. For example, three forms did not have the date the child started in the service, two did not have the names of persons authorised to collect the child, and three did not have the child's record of immunisations
Provider's corrective action:
- The registered provider stated that every child’s registration form was reviewed and parents were requested to complete them fully. Going forward management will ensure the parent complete all the required information on registration of their child

- The inspector reviewed a sample of 12 children’s record forms; of these, 5 did not include the required information. For example, one form did not include the child's full name, and five did not have the child’s start date
Provider's corrective action:
- The registered provider stated that all forms have been updated and staff have been made aware of the requirements that all forms must completed correctly. The room leader will take responsibility of checking forms and the registered provider will review this

- The registered provider stated that they have amended the children’s registration forms to include the child’s general practitioner (GP) and the contact details. They stated that the original registration forms have been removed, and the new form, including the GP information, has been replaced, both on file on the computer for future printing and in paper form in the office to have on hand

- On review of a sample of children's registration forms, it was noted that these forms were not fully completed. Children’s immunisation details, name and contact details for a child’s doctor had not been recorded on these forms
Provider's corrective action:
- The registered provider has stated that each room leader has been requested to follow up the required details on each child’s registration forms. All enrolment forms will be checked by management prior to the child starting in the service

- A sample of seven children's registration forms were reviewed. Of these forms, four were not fully completed. The emergency contact details, records of children's immunisations and a child’s doctor's contact details had not been recorded on these forms
Provider's corrective action:
- The registered provider stated that the management team are responsible for ensuring the children’s registration forms are complete. These forms have been amended to include a section for parents to state they have chosen not to have their child vaccinated. The registered provider stated that the forms will be reviewed and signed by a member of the management team prior to the commencement of the child in the service. A secondary check will be made on the day the child starts in the service
