- The service did not maintain a written record of the following required information. (1)(g) The name and telephone number of the child’s registered medical practitioner was not present on any of the 10 sampled forms. (h) Record of immunisations, if any, was not present for seven of the sampled forms
Provider's corrective action:
- (1)(g) The name and contact number of each child’s medical practitioner has been added to the enrolment forms of all children currently in the service. (h) Immunisation records have been received from parents of all children vaccinated and are now on file with their registration

Provider's corrective action:
- New updated parental consent for appropriate medical treatment of the child in the event of an emergency was forwarded to parents for completion and returned. Record of immunisations were also forwarded by parents. The registration form was updated to include all the required information. Summary Comment All evidence submitted meets the requirements of the Child Care Act 1991 (Early Years Services) Regulations 2016

- (1)(i) The registration forms did not include signed parental consent for medical treatment in the event of an emergency. It is acknowledged that following the inspection the registered provider emailed the inspector with a copy of the amended form which included the missing information
Provider's corrective action:
- A Consent for Medical Treatment form was created following the inspection and all forms signed by parents were attached to each child’s current registration form. All forms were completed by 10 November 2025. An updated registration form will be provided to future children attending our service, which will include the required Parental Consent for Medical Treatment form

- A sample of 10 completed registration forms were reviewed as part of the inspection process. The service did not maintain a written record of the following required information on two children’s registration forms: (h) Record of immunisations. It is acknowledged that the remaining eight registration forms contained the required information as detailed under parts (a) to (i) of this regulation
Provider's corrective action:
- The uncompleted registration forms have since been completed and relevant documentation provided. These records must be returned to the service prior to a child commencing in the service as stated in the admission policy. All forms will be checked by service manager on return

It is acknowledged that following the inspection the service updated the registration forms to include this information and sent a copy of the new form to the early years inspector on 19 September 2025

- (1) The service has updated the registration forms to include this information and sent a copy of the new form to the early years inspector on 19 September 2025. The service stated that any further changes to the registration form for Clareen Country Creche will be reviewed carefully. - (i) One of the 12 records did not have a record of written parental consent for the service to provide emergency medical treatment if required. Not having children’s records complete may hinder the care and safety of the children in the service
Provider's corrective action:
- The following statement was made by the registered provider: Corrective and Preventive Action The service reviewed all application forms to ensure that the parent had completed the forms correctly. All incorrect completed forms were returned to the parent to complete. The current form has been altered to make it easier for the parents to complete. All forms returned by the parent on enrolment will be checked by Management to ensure correct completion

- Parents of children who had not yet submitted a copy of vaccinations were contacted by each room leader reminding them that we must receive a copy by Friday 4th April 2025 in order for compliance, a copy was obtained and placed on their file. Parents are to be reminded when starting their child with us that they must present a copy of vaccinations with their registration form
