- (1) The service was found to be operating outside of its registration status and the following details were recorded. (a) Unauthorised addition of a care room known as the Pre-school care room on the first floor. (b) Unauthorised change in the number of children attending the service from 47 to 62. While it was acknowledged that the registered provider had submitted a number of Change in Circumstance forms to Tusla from May to June 2025, they had commenced the proposed changes before returning the outstanding information requested and before a Fit for Purpose inspection could be carried out. Operating outside registration status could potentially compromise the safety and wellbeing of the children attending the service
Provider's corrective action:
- The following statements were received from the registered provider; and found to address the IAN. The registered provider submitted a written response detailing the corrective and preventive measures taken. These were accepted by the inspector. Improvement Notice IN Code:0462 06/05/2026 Nine children were present in a care room known as the Pre-school room on the first floor of the service where there was no finalised fire certificate as the conditions previously listed by the Chief Fire Officer on 03/10/2025 were not met. Status The registered provider submitted corrective and preventive actions along with evidence of same on 14/07/2026 which were accepted and addressed the statutory notice

- 1. The preschool service did not notify the agency in writing that it was operating the preschool service outside of its registered hours of operation. On the day of inspection, there was documented evidence that early year’s children were attending the early years’ service for longer than the registered 5 hours, from 09:30 to 14:30 for a part-time service. As part of the inspection process, a sample of arrival and departure times of the children and staff were reviewed (in the child/staff attendance record books). On dates from the week of 09/02/26 to the week of 25/02/26, the departure times recorded for the preschool children ranged from 12:30 to 17:20. Nine preschool children were recorded as being collected from the service between 15:30 and 17:20 on dates between the 18/02/26 and the 24/02/26. 2. Six preschool children were recorded as first attending/commencing in the service on 25/08/25. These dates were recorded in both the children’s enrollment records and the child attendance record book, which recorded the children’s date of birth and the daily attendance records, which were made available for inspection. These children commenced as attending the preschool service prior to the registration date of the new part time service which was 24/10/25
Provider's corrective action:
- The following was acknowledged at regulatory compliance meetings on the 01/05/2026 and the 08/05/2026. • The service was operating outside of the registration status prior to the inspection. The service had taken remedial action and committed to operating within their registration status going forward

- At the opening meeting the inspectors were informed by the registered provider that a new care room, known as Baby 1, located in a portacabin at the rear of the property had been in operation since January 2026. Four children were attending, aged 0 to 1 years with the intention of accommodating a maximum of six children. The Baby 1 care room was found to be in use at the time of inspection. The proposed Change in Circumstance process had not been initiated therefore authorisation to operate the care room had not been approved by the National Registration Office

- The following statement was received from the registered provider; A submission has been made to the National Registration Office for the changes in circumstances of the service

- (1) The service was operating outside its registration status on the day of inspection. An additional part time service was in operation from 11.30am to 13.30pm. The staff advised it was in operation since September 2024. This change in service provision was notified to the inspectorate’s registration department during July/August 2024 but the application was deemed incomplete as further information was required to complete the application that was submitted at that time
Provider's corrective action:
- On the 2/4/2025 and 1/5/2025 the registered provider advised of the following On the 19th of March 2025 SOORS department acknowledged contact had been made with the preschool service. On the 20th of March 2025 The registered provider responded to SOORS, and a decision was made to revert to the registered sessional service. The operation of the part-time preschool service has ceased. On the 2nd of April 2025 The Change in Circumstances Office acknowledged the registered providers decision to withdraw the application for a part time service

- (1) There was a change in the name of the registered provider and the registered provider did not notify Tusla of this change. It is acknowledged that the change of circumstance application has been made to the Early Years Registration office

- The registered provider stated the following in their reply: Corrective and Preventive Action The application to amend the name of the register provider was submitted to the Early Years registration office. To ensure this non-compliance does not reoccur, there will be a bi-annual review of all documentation

Ballinasloe Social Services Pre-School, Ballinasloe · Regulation not named in the report text
- (1)(3) On the day of inspection, the person in charge, informed the early years inspectors that an additional separate service was operating for children who are not registered to the Ballinasloe Therapeutic Project. However, it is not a separate service as the staff member was working in the registered service at this time. The staff member was providing 1-to-1 care and consultation with children and their parents, when the registered service was in operation and when the staff member was rostered to be working in the registered service. Additionally, there was no differentiation from the documentation presented of these two services. The early years inspectors were advised that a child under the age of 2 years was availing of this service and was not registered to the registered service. This pre-school is registered to operate for children between 2 to 6 years of age
Provider's corrective action:
- On the 25th of March and 11th of April 2024, the registered provider advised of the following Corrective and Preventive Action Children under 2 years of age are not admitted to the pre-school session. The additional separate service is no longer operating during the pre-school sessional service
