- (1) The registered provider did not notify the agency in writing of a 2nd session prior to its commencement. The 2nd session was observed in operation from 12:00 - 15:00 on the day of the inspection with 8 preschool children in attendance
Provider's corrective action:
- s A change in circumstances form has been submitted to the registration office. In future the registered provider will ensure any changes are requested within the required timeframe

- (1) The registered provider failed to notify the Early Years Inspectorate of a change in circumstances in relation to the following as per the schedule 4 Form for Notification of Change in Circumstances: • The name of the designated person in charge of this service did not correlate to the name detailed on the National Register
Provider's corrective action:
- Corrective Action (1) A change of circumstances has been made on 29th May and the new Designated Person in Charge of Collon facility has been appointed. Preventive Action Ensure that paperwork reflects any changes in circumstances and that all changes are applied for before any changes take place. Changes in person in charge will not take place until paperwork is in order

- (1)(3) The registered provider failed to notify the Early Years Inspectorate of a change in circumstances as required as evidenced in the following: The operational hours of the service are at variance with the registered hours of operation. The service is currently operating from 08:30hrs to 17:30 hrs and is registered to operate from 09:00hrs to 17:30hrs
Provider's corrective action:
- Inspection findings are documented in the inspection report which is first issued in draft format to the service with an opportunity to respond to any findings. Where statutory requirements are identified as not being met, the registered provider must demonstrate how they have rectified the non-compliance and will prevent any non - compliance from re occurring. The Corrective Action and Preventive Action plan (CAPA) will be used to inform decisions about compliance with regulatory requirements. Where the registered provider fails to meet the statutory requirements an escalation process may be commenced. The inspectorate reserves the right to edit responses received for reasons including clarity, completeness and compliance with administrative and legal processes. The contents of the report are compiled by the inspectorate body.
