Provider's corrective action:
  • Corrective Action (3)(b) Following the inspection findings, attendance records were immediately corrected to accurately reflect the presence of all children on the day of inspection. The child incorrectly recorded as absent in the Pre-school room was updated as present , and the missing sign-in entries in the Toddler room were completed to ensure the attendance record accurately matched the number of children present in the room at that time. Preventive Action (3)(b) The service has reinforced the requirement for contemporaneous and accurate recording of children's attendance at the point of arrival and throughout the day . Staff have been reminded that attendance records must reflect real -time occupancy in each room to ensure accurate headcounts for safety , supervision, and emergency evacuation purposes . A strengthened sign -in procedure has been implemented , requiring staff to immediately record children upon entry to the room and to verify attendance records against actual numbers at key points during the day . Room leaders will carry out regular che cks of attendance records to ensure accuracy and compliance. Management will also monitor attendance documentation as part of ongoing supervision and compliance audits. Summary Comment The inspector has reviewed the actions and evidence submitted. The non-compliance under Regulation 24 has been addressed

  • Corrective Action (1) All staff have been advised that even children who are family and only with us for the day must be acknowledged on the attendance sheets for dropping off and collecting and roll sheet. Preventive Action (1) All staff now are aware that every child even if they are only with us for a day must be on the register whether they are family or not

  • Corrective Action (1) The manager will take roll call at the door while a second staff member welcomes the children into the school. The roll call book will be rechecked when all children have settled by a second staff member. Roll call error was human error on that day and completely overlooked. Rechecking of roll completion will rectify this. Preventive Action (1) Roll book is now stored at a box close to school door entrance

  • A record of the inspector’s visit was added to the visitor log immediately after it was mentioned. The visitor book has been relocated to the main entrance alongside the staff sign-in and sign-out book. Staff were reminded of this procedure. The person in charge will carry out regular checks of the visitor log to ensure it is consistently maintained

  • The registered providers submitted the following response: Corrective and Preventive Action The service has purchased a visitor book which will make it easier to obtain and record previous and present visitors. No person is allowed no gain entry to the service without signing in on arrival. The purchase of the book will make it easier to record and keep evidence of visitors to the service. A sign is in place to show visitors this and to remind staff to be aware that this procedure must be followed

  • Corrective Action (1) There was a d iscussion at the staff meeting regarding all staff members being responsible for marking attendance as soon as a child arrives/departs. The responsibility is on the person answering the door or accompanying the child to the door at home time. A tablet or device will be left on the shelf beside the front door to assist in this process. The manager will monitor this situation to ensure all staff are confident in using the online digital application. Preventive Action (1) The manager will monitor the recording of attendance particularly at busier times (morning & evening).   • Corrective Action (1) Staff have been addressed about the importance and had to recap on the sign-in and out policy to follow each day. Management have been checking role, books daily to correspond with room number. Preventive Action (1) Management has a checklist which they will use to run routine spot checks around each room. Checking numbers, ratios, sign/ in and out etc. will be on that list. They have also signed up to introduce using an app and digitally record children’s attendance etc. Summary Comment The evidence submitted by the registered provider in relation to regulation 24 - checking in and out and record of attendance has been reviewed and accepted

  • Corrective Action (1) The registered provider reported that there was a staff meeting to discuss the importance of the children’s attendance book and children being signed in as soon as possible and it was realised that the person on the door has too much responsibility answering the door and completing the role book. Preventive Action (1) The registered provider reported that the floater (is the person in the middle door that welcome the children) will now mark the role book as the children enter the preschool and the person on the door will not have that responsibility. Children will be marked on the attendance board (white board) for double safety. The staff will make sure that the number of children on the attendance board matches the children’s attendance book, with a regular head count carried out. Summary Comment The inspector has reviewed the actions and evidence submitted. The non-compliance under Regulation 24 has been addressed

  • Corrective Action (1) Staff in the Ducklings room were spoken to regarding their responsibility and importance of recording every child onto their roll sheet as soon as they enter the care room. This was also addressed with all staff at our recent staff meeting on the 27th of August 2024. Staff have subsequently received refresher training on their Recording of Attendance policy. Preventive Action (1) Room leaders have been charged with ensuring children’s attendance is recorded effectively. The designated person in charge and the deputy will also closely monitor attendance records in all care rooms to ensure children are being recorded in and out in an efficient manner. Summary Comment The evidence submitted by the registered provider in relation to regulation 24 - Checking in and out and record of attendance has been reviewed and accepted
