- (1) There were no accurate up to date records/staff roster since the 26/01/2026 to identify if the registered provider had an adequate number of adults working directly with the children attending the pre-school service. (8)(a) The registered provider was unable to provide evidence that there were at least two adults on the premises during the operational hours of the service
Provider's corrective action:
- The following was acknowledged at regulatory compliance meetings on the 01/05/2026 and the 08/05/2026. • The staff received a text message at the end of each week with their working hours for the following week, but a staff roster had been implemented since the inspection

Child's Play Creche, Galway · Regulation 11 — Staffing levels
- (1) During the period of inspection when 3 staff members were taking their lunch break and children in the playschool room were asleep, there were inadequate numbers of adults working and supervising the pre- school children in the upstairs playrooms and may pose a risk to the children. At times, between 12:00 and 13:00 on the day of inspection, 3 staff members were observed to take their breaks and during this time 2 staff members were observed caring for 21 children attending the Montessori room, which included 5 children from the playschool room. Three children (aged 2 to 3) were asleep in a nearby playroom. The staff in the Montessori room also carried out the sleep checks on the children sleeping in the playschool room. There were 9 children aged between 2 and 3 years of age and there were 12 children aged between 3 and 6 years of age. The required number of adults to care for the number of children was 3 staff members. See table below: Number of children present. Age range of children. Required Adult/child Ratio. Provider's corrective action:
- The registered provider stated the following in their reply: Corrective and Preventive Action 1. We have reviewed the breaks and lunches and have staggered the times to ensure that we adhere to adult to child ratios as outlined in the regulations. We are in the process of recruiting an extra staff member to cover break times and staff leave. Summary Comment The Inspector reviewed the actions submitted and the noncompliance identified under regulation 9 have been adequately addressed

Millbrook Creche, Tuam · Regulation 11 — Staffing levels
- (8)(a) The service manager did not demonstrate that a minimum of 2 adults were on the premises at all times when pre-school children were present. On the morning of the inspection, the attendance records stated there were 8 children signed in between 07:50 and 07:58. The staff records indicate that the first staff member arrived on the premises at 07.36 and the second staff member did not sign in electronically till 07:58. The manager stated that the staff member arrived on the premises at 07:45 but the roster indicated this staff member was not rostered to start work till 08:00 on the day of inspection. This posed a safety risk to the children as there was not an adequate number of staff available to the children
Provider's corrective action:
- The registered provider stated the following in their reply: Corrective and Preventive Action There are 2 staff members on the premises each morning before children arrive on the premises. Staff are aware of the importance of signing in each day, when they arrive on the premises

Teddy Bears Lodge, Turloughmore · Regulation 11 — Staffing levels
- (a) Contrary to the requirements, the registered provider did not ensure that there were 2 adults on the premises while the early years children were present in the service. When the inspector arrived on the premises at 08:05 there was 1 adult on the premises with 4 children. The staff roster for the weeks 9 – 13 June and 16 – 20 June indicated that there was only 1 staff member rostered on at the opening time of 08:00 and closing time of 18:00 where 2 were required

- (a) Management will ensure that there are 2 adults on the premises while the early years children are attending the service

- (8)(a) No records were available to assess whether there were two staff working on the premises while the pre- school was operating

- On the 1/5/2025 the registered provider advised of the following. Corrective and Preventive Action A sign-in-and-out sheet is available for all staff in the school; it was available in the staff room on a daily basis and the staff in the preschool sign the staff attendance sheet in the preschool service

- (1) During the period of 9.00 am to 10.15 am the registered provider did not ensure that an adequate number of adults were working directly with the children. There were 2 adults working in the service with 24 pre- school children. (3) The registered provider did not ensure that the minimum ratio of adults to children was maintained, there were 2 adults working in the service with 24 pre-school children between 9.00 am to 10.15 am. The adult child ratio is 1 adult for every 11 preschool children on a sessional basis
Provider's corrective action:
- The registered provider stated in their response: Corrective and Preventive Action (1) and (3) We have now three adults always working directly with children and a relief. We have also (2) advertised for another relief adult

Pixie Garden Creche, Gort · Regulation 11 — Staffing levels
- The inspection team were unable to determine if the adult child ratio was maintained at all times during the operation of the service as a staff roster was not maintained on a daily basis. There was a roster was available for the week of 3rd June 2024. However, an incomplete roster was presented for the week of the 10th of June 2024 and no roster was available for the weeks commencing 17th June and 24th June. Therefore, it cannot be determined if the adult child ratio was adhered to for these weeks
