- The registered provider did not ensure that adequate supervision was in place for all children on the day of inspection as follows. • A child who attended the Oak room was observed running from the care room, the full length of the corridor, to the main door of the service three times between 12:08 and 12:35. • At 14:24, the same child ran from the care room through the kitchen, where two pots of food were cooking on the hob, and into the office. On these occasions, the child was observed to reach the main door and the office before a staff member reached them to bring them back to the care room. This is at variance with the supervision of children policy in place which stated that children must not be allowed in the corridor unaccompanied and that appropriate supervision should take account of individual children’s needs
Provider's corrective action:
- The registered provider liaised and consulted with staff working directly with the child, the management team, the child’s parents, external agencies and the service’s Better Start specialist in relation to the child’s AIM plan and goals. A meeting and an IEP was developed with the child’s parents which was relative to his AIM goals and other recommendations. The child’s mother has decided that the child will be finishing up with us in the next two weeks but all necessary steps were taken in the meantime. Further to the above, as part of the IAN, two keypad magnetic doors were installed both in the hall and on the kitchen door. Staff training is being provided between the dates of July 6th to July 17th for all staff including training on managing challenging behaviours, risk assessments, policy updates including supervision of children and safeguarding procedures. Staff who work with children under the AIM process will have more frequent contact directly with the better start specialist assigned for our service and can reach out to them if they are having more specific concerns that original observations at the beginning of the AIM observation and support role. IEP’s will be created for every child under the AIM programme and also other children who do not qualify for the AIM scheme but show varying levels of need. The newly installed keypad systems will also prevent further concerns of children accessing the front section of the premises outside of the care rooms

- The children were not adequately supervised in the Fox room during the transition to sleep. The transition to sleep began when children finished eating their dinner at 11.23am. A staff member brought the floor beds into the room and stacked them on top of each other. The children were observed climbing and bouncing on the floor beds, running around the room and getting upset. One staff member was changing nappies and the other was getting cots ready whilst a third member of staff was left to supervise the children on her own. There was no plan in place to ensure the children who were tired and ready for bed were put to sleep. The transition took one hour to complete before all children were in bed at 12.23pm
Provider's corrective action:
- A staff meeting was held immediately following the inspection to review the supervision requirements under Regulation 11 and reinforce staff responsibilities. A revised sleep transition procedure was implemented with immediate effect. Floor beds are no longer brought into the room while children are active. Beds are now prepared in advance of the transition period. Staffing working time has been reviewed to ensure that: One staff member is assigned solely to supervise children. Nappy changing and room preparation are staggered and do not reduce active supervision levels. Children who display signs of tiredness are now prioritised and supported to rest promptly. The transition to sleep time has been reduced and structured to avoid unnecessary waiting

- (1) & (3) The service was registered to accommodate a maximum of 22 children. Twenty-four children were present on the morning of the inspection. The registered provider had not notified the Agency of the changes as required
Provider's corrective action:
- The additional children returned to the afternoon session the next day. The service had expected absences on the day due to absences the previous week. The service is in the process of applying for a change of circumstance to 24 children. The service will no longer permit children to move between the two sessions

All Stars Montessori Preschool, Rathcoffey · Regulation 27 — Supervision
- 1. Children were observed to go to sanitary area unaccompanied by a staff member, however the staff members were aware when they left the room . The sanitary area was located in an area outside the care room along a short corridor which turned right. The sanitary area was not in the staff members immediate line of vision or hearing. Three children were observed in the sanitary area washing their hands after an art activity in the care room. One child requested assistance from the inspector with handwashing and accessing paper towels from the electronic dispenser
Provider's corrective action:
- Following a discussion with staff regarding the supervision of children in the classroom, we now have a staff rota in place at all times to ensure one staff member is positioned near the door exit to monitor children leaving the classroom. The staff member will accompany children to the bathroom to assist them if required. Regular toilet breaks are arranged so that a small group of children can exit the classroom and be supervised by a staff member. These children are being assisted in the bathroom to wash and dry their hands using the child step. All s taff are aware of their absolute responsibility to monitor children exiting the classroom. We continue the practice of one teacher being present in the hall with children while they put away lunchboxes and return to the classroom with their coats and bags. All staff are alert for children leaving the classroom and will ensure they accompany them. We have a routine at lunchtime where two staff members are present with children while washing hands and using the toilet before lunch. Also, one staff member will remain in the corridor with children while they put away lunchboxes and return to the classroom
