Naionra Montessori, Swinford · Regulation 8 — Notification of change in circumstances
- (1) and (3) Two children were found to attend a 3-hour morning session and return to the service to attend for a further 3 hours in the afternoon totalling 6 hours of daily attendance in the service. This was outside the service’s registration for sessional service type
Provider's corrective action:
- The registered providers advised that: (1) and (3) Corrective and Preventive Action Once the non‑compliance was identified, the service took immediate steps to correct the issue. The parents of the children were informed, and their attendance patterns were reviewed. The service applied for Part‑Time hours that have since been approved. Both children are now registered under Part‑Time services and attend only until 1:30 p.m. Staff and management were reminded of the attendance limits associated with each service type, and a daily attendance check is now carried out by the room leader and reviewed by management. The registration book has been updated to clearly record each child’s approved hours. The sign‑in/sign‑out system has been updated to ensure no child attends beyond their permitted hours

- (1) & (3) A review of records found that on numerous dates there were eighteen or nineteen children present attending sessional services at any one time. This was confirmed through discussion with the staff. The service was registered for a maximum of seventeen children. The service had also been found non- compliant in relation to this on the most recent inspection of the 25 January 2022
Provider's corrective action:
- The registered provider advised in writing that: Corrective and Preventive Action (1) & (3) Two children have been moved from the morning group to the afternoon group bringing the number of children attending the morning session back to 17. The service has been in contact with registration office and have provided copies of the attendance role book. The service continues to look into changing circumstances so that the service can cater for more children. The service is to pursue a change of circumstances but in the meantime will ensure that the maximum number of 17 children is adhered to

- (1) & (3) (1) The service was registered to accommodate sessional children aged from two to six years of age however on the inspector’s arrival at 9:36am there was a child aged under two years present in the service until 10:04am. (3) The service was registered to operate a morning session from 09:15-12:15 hours but was operating from 9:00-12:00 hours without notification received by the Agency of the change
Provider's corrective action:
- The registered provider advised in a written response that: Corrective and Preventive Action (1) The registered provider stated that there will not be a reoccurrence. (3) A change in circumstances form was submitted to Tusla registration department on the 20 March 2024 requesting a change in the time of the morning session to 9am to 12 noon instead of 9:15am to 12 15midday. The change has been approved

- : A significant risk in relation to the safety of the children has been identified in relation to: 1. The number of children attending the service had been increased without the registered provider notifying TUSLA and receiving approval to proceed with the proposed change. The service was registered for a maximum of 16 children, however there were 17 children in attendance for the duration of the session on the day of inspection. The names of 17 children were observed to be documented in the roll book with one child in attendance on the day was not recorded on it
