- The registered provider did not submit a change in circumstance to inform TUSLA that the service has been operating as a single-handed service since September 2025

- The registered provider has submitted a change in circumstances form on the Tusla portal and the change in circumstance form indicates a change to a single-handed service. The registered provider will note any changes to my service and contact local childcare committee for advice on future changes

- (8)(1) The service was found to be operating outside of its registration status and planning. The following details were recorded: The service was registered to accommodate 38 children. A sample of records from 25 May 2025 to 27 June 2025 indicated that there were 42 children present in the service. This was also documented as a non- compliance on the last inspection report on 14 October 2022 in which it was noted that a change of circumstances form had been submitted to the registration office on 27/10/2022, however this had never been finalised. Operating outside registration status could potentially compromise the safety and wellbeing of the children attending the service

- The registered provider will only allow a maximum of 38 children within the service. The registered provider will ensure by monitoring the number of children that the number of children in attendance will not exceed the registered amount

- The registered provider did not submit the required notification of proposed change in circumstances to notify Tusla in advance of changes being made to the person in charge

- (1)Notification form submitted to Tusla outlining the temporary change in Person in Charge during staff leave. The Registered Provider is responsible for ensuring all future notifications are submitted within required timeframes. This will be reviewed quarterly

- : Following submission of Corrective and Preventive Action (CAPA) 1 and CAPA 2 documents the person in charge submitted a copy of a change of circumstances application dated the 1st of July 2024 and a letter dated the 3rd of June 2025 detailing the required changes

- (4) One staff member did not have documentary evidence or letter of equivalency to show their qualification met the required minimum Level 5 Childcare qualification

- The staff member had provided a letter from her university stating she was completing her Early Years degree and had completed year 1. Following the inspection the student forwarded the documents required to the DCEDIY for clarification. We requested a more detailed letter from the college to forward to DCEDIY clarifying that the student had completed year one and was nearing completion of year two of a degree that is listed on the list of recognised qualifications. In future we will ask the students to complete the relevant application and forward it to the DCEDIY for confirmation before employing as a staff member

- Police vetting was not available in respect of two staff members who had lived outside the state for a period of longer than six consecutive months

- Police Vetting has been sought by the staff members from the relevant countries. The childcare management team assisted staff members with the process where needed. One police vetting has been processed and translated. Risk assessment carried out by the childcare manager on the situation. Recruitment policy has been reviewed and amended. Garda Vetting policy has been reviewed and amended. A section on Police vetting has been amended for clarity in both the recruitment and garda vetting policy and relevant procedures. Staff file checklist has been amended to include checks for Police vetting. Two members of management will now check that all required documentation is on file for new employees prior to their start date. Both will sign off that checks are complete
