- (a) The service’s complaint policy did not provide an accurate procedure to be followed by a person for the purposes of making a complaint in relation to the service. The person in charge confirmed that both persons named on the service’s complaint policy are no longer in position

- We have reviewed and rectified the complaints policy to correct persons named on said policy. The complaints policy along with all policies will be reviewed quarterly by administration staff and signed off by management

- (1) The complaints policy was available however it did not document how the complainant will be kept informed and the process for the storage of complaint records and the timeframe for retention of these records

- The complaints policy now documents that a complainant should contact the manager to discuss the complaint. If they feel the matter is unresolved, they should put it in writing and will receive a written acknowledgment within a week. The complainant will be kept informed throughout the process in writing by the manager. All written records will be kept in a locked filing cabinet and a copy of all relevant documents will be given to the complainant. These records will be kept for a period of two years from the date the complaint was dealt with. The complaints policy will be regularly checked and updated if needed

- (2) (a) (b) There were no records of complaints available for review on the day of inspection. Through discussion with staff, it was evident that informal complaints had been made to the service by parents. The services policy on complaints is clear in the manner in which informal complaints are dealt with and that these will be recorded. However, this policy was not followed. This poses a potential risk of leaving the safety and wellbeing of children unaddressed

- In her written response, the registered provider detailed how the complaints procedure had been updated, and any informal complaints had been filed retrospectively

- (2) (a) (b) The service did not demonstrate compliance with their complaints policy as follows: • The service policy stated that ‘All complaints both formal and informal will be recorded in detail……Records will be stored in a complaints file and retained for two years’. There was no complaints file available in the service on the day. Minutes of a meeting with a parent in December 2024 referenced two complaints. There was no record of these complaints or the investigation into these complaints. • A complaint received by the service verbally and submitted to the inspectorate was not recorded or investigated at the time of the inspection. (3) (a) (b) There was no written record of any complaint in the service for the previous two years available to the inspectors on the day. This is contrary to records of meetings reviewed on the day where concerns had been raised to the person in charge

- In a written response the registered provide stated: (2) (a) (b) • The complaints folder was located following the inspection. The registered provider informed staff that any discussion based on issues arising and brought to her attention, even though spoken about will be documented and addressed. These will be stored in the complaints file. The correspondence from the incident was added to the folder where the investigation was still on-going • The complaint made verbally by parent was typed up post inspection. We will ensure that the complaints folder is easily located within the office and management know where to locate it. Any concerns/complaints from staff or parents will be documented and stored within the folder accordingly.
