- (d) The service failed to notify the Agency within 3 working days of an injury to a child who required medical treatment in a hospital following an accident sustained while in the Montessori

- (d) Notification of the incident was sent to Tusla and a risk assessment was carried out and policies reviewed. Staff were made aware of the need to report these types of incidents within three days. Our policy for managing these situations has been updated
