- (1) The registered provider had not submitted a change in circumstances to advise the agency that the hours of operation of the service were changed to from 9.00am to 1.00pm to from 9.00am to 2.00pm
Provider's corrective action:
- (1) The registered provider submitted a copy of the approved change in circumstances to the inspector on 9 December 2025. As a preventive measure the registered provider advised in the CAPA response to the inspection report that approval for any further changes in circumstances will be secured prior to making the change

- There was no evidence of current insurance cover available on the day of the inspection
Provider's corrective action:
- Insurance certificate detailing insurance cover from 28th March 2025 to 27th March 2026 obtained and submitted. On renewal next year certificate will be requested

Chestnut Daycare, Dublin 12 · Regulation 28 — Insurance
- • The registered provider failed to notify the agency that the designated person in charge of the service had changed. Staff reported that the person detailed on the national register as the designated person in charge had ceased employment in the service in 2020. • The service is currently registered to provide full day, part-time, and sessional care to preschool children. The person in charge confirmed that the only type of care currently offered by the service is full day care. • The service is currently registered to open from 7:30am to 18:00pm each weekday. The person in charge stated this information is incorrect and the service currently opens from 8:00am to 18:00pm
Provider's corrective action:
- The registered provider submitted the following response: Corrective and Preventive Action The service had notified Tusla of the change in designated person in charge. There was a staff change in Tusla which meant the form got passed around. It has now been rectified. The service still currently provides Part Time/ Sessional and Full-Time places. The service has sent in a CIC form with the time changes. The service will confirm by email that Tusla have received the CIC form and has lodged the request. The service plan to change to full time in the Autumn this year, the service will lodge a CIC as soon as they have made a full decision on this. The service will check the register to make sure all details are correct moving forward. The contents of the report are compiled by the inspectorate body. Acknowledgments The inspectors wish to acknowledge the cooperation of the person in charge, staff and children who were present on the day of the inspection

- It is acknowledged that the registered provider submitted an email to demonstrate insurance of the service was it place but no Insurance certificate was available to demonstrate specific details of the insurance policy to include the start and expiry date of the insurance cover

Apples Montessori School, Dublin 9 · Regulation 28 — Insurance
- The service was found to be operating outside of the hours of operation currently registered. The service is registered to operate a sessional service between 09:30 and 12:30pm. From documentation reviewed and discussion with the registered provider the service has been offering parents/guardians an option to avail of an extra half an hour childcare daily between 9am-9.30am or 12.30pm-13.00pm. A change of circumstance application to change the hours of operation was not submitted to the Registration department
Provider's corrective action:
- Corrective Action From Sept 2024 the service will be operating with the hours that are registered with Tusla 9.30am - 12.30pm. From June 1st, 2024, the registered provider has changed the drop off and pick up times to the times stated. Preventative Action The registered provider has stated from September 2024 the times of operation are from 9.30am to 12.30pm
