- 3. (b) There was no written record of visitors to the service. The inspector was not asked to sign in or out of the service and through discussion with the registered provider, it was evident that no record is kept. This poses a risk of unauthorised access to children

- In her written response, the registered provider detailed a sign book which they had created for the use of the service and stated that it would be in use going forward

- (1) In the Inventor room, the inspector observed 18 children present. A written record of the number of children checked in to the care room on the day of inspection was reviewed and it was found that no child had been checked in. On discussion with the room leader, it was outlined to the inspector that the 18 children had been checked in on an electronic application used by the service. It was outlined that the electronic application would be used for fire drills and in the event of an emergency occurring in the service to ensure all children were evacuated safely. On discussion with other staff in the care room, it was outlined that they would use the written record to carry out fire drills or in the event of an emergency. This posed a safety risk to the children as all staff were not fully aware of which method of recording attendance was to be sued to ensure all children were evacuated in the event o an emergency

- Corrective Action The topic was reviewed with management and staff, and it was decided that the electronic application will be used to check children in and out of the service, carrying out fire drills and in the case of an emergency. Preventive Action The services fire safety policy has been updated to reflect the decision

- (1) A child had been signed into the service as having arrived at 08.30 however when reviewing the children in attendance this child was absent. This inaccurate record may cause confusion should the service need to be evacuated in an emergency. (3) (b) A record of all persons into the service other than those identified in (a) above were not maintained. This may hinder the registered provider when identifying visitors to the service retrospectively

- (1) The error in signing a child into the service was corrected on the day of inspection and the registered provider has committed to ensuring that the staff are aware of the importance of accurate singing in and out of the children. (3) (b) A visitors sign in book was created on the day of inspection. Staff have been informed of the need to use this record when visitors enter the service

Not ensuring there is an accurate record of the children in attendance at any time may hinder the safety of the children should the service need to be evacuated

- (1) The registered provider has committed to ensuring all children will be signed in on arrival and has a roll book in place for use

- (1) The attendance record for the Quaker room indicated that the children present on the day had not been checked in on arrival. It is noted by the inspector, after discussion with a staff member, the records were then completed

- The following statement was received from the registered provider: Corrective and Preventive Action The service has now assigned one staff member to check children in and out of the service on arrival and departure times and have a specific policy for recording attendance
