Children were not assisted to have their hands washed under warm running water following nappy changes observed, after coming in from outdoor play and before meals. Action submitted by the Registered Provider - Infection Control: The staff member was spoken to and the handwashing policy was provided again to read. Management will monitor the preschool room, and spot checks will be done on a regular basis. All staff got a copy of the hand- washing policy just to keep fresh in mind. Constant reminding and monitoring of this with all staff 3 June 2026 ##### [Timahoe Tots LTD](/creche/timahoe-tots-ltd-timahoe.md), Timahoe · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety 1. It was observed that the entrance route door to the service at drop off time was not the exit route used at collection time. At the exit route door there was no physical barrier in place to prevent children from accessing the public road located to the front of the premises at collection time. This presented a potential risk to children’s safety due to the presence of moving vehicles on the road. It is acknowledged that the designated person in charge took an immediate corrective action and changed the collection point location for parents to the original entrance route door and followed this up with signage and a notice to parents to inform them of the new practice. 2. Refrigerated storage facilities were not provided for the safe storage of children’s morning and afternoon perishable snacks which were provided by parents and stored in the children’s school bags throughout the day. Administration of Medication: 1. A temperature reducing medication was not available in the premises in the event that a child with a temperature required it. It is acknowledged that the designated person in charge took an immediate corrective action and temperature reducing medication was available in the premises before the inspection concluded. Action submitted by the Registered Provider - General Safety: 1. The registered provider stated in the response that the collection point was immediately changed to the side entrance, and parents were informed by our social media group immediately of the collection change. Our collection policy was updated and submitted to show this change. The entrance door and exit door (the side door) will be always monitored. The service will use the internal gate as an extra barrier for children before they reach the door. We will not use the front door for entering or exiting the building. (Procedure included) 2. The registered provider stated in the response that a refrigerator has been purchased and placed in the ladybird room, so children in the ladybird room and turtle room can place their lunches in this refrigerator when they arrive. A second fridge has been placed in the butterfly room, for use by the butterfl ies and caterpillar room. All children’s lunches are now placed into the refrigerators when they arrive in the morning. (Photographs included) Administration of Medication: The registered provider stated in the response that temperature reducing medication was purchased immediately and is now stored on the premises in a press in the kitchen. Temperature reducing medication will be checked weekly by the person in charge to make sure there is a supply and they are in date. Handwashing practices were inadequate to control the spread of infection as children’s hands were not routinely washed. Children were not brought to wash their hands before eating meals and snacks. Outing: 2. Written Risk Assessments were not recorded prior to outings. This was at variance to the outings policy which stated that a risk assessment will be carried out before an outing takes place. Action submitted by the Registered Provider - Infection Control: Hand washing is now part of every transition in the preschool. When the children come to school in the morning, they wash their hands. They also wash their hands before lunch and after playing outside. The fire evacuation door in pre-school room two opened directly onto the rear of the premises into the grounds of the community pitch facility. The door was not alarmed therefore no notice could be given in the event of a child opening the door, exiting pre-school room two and gaining access to an unsecured area. 2. It was observed that children were dropped and collected from the fire evacuation door in pre-school room two. The door was a solid structure and there was no means to verify the identity of any individual knocking on the door prior to opening the door and potentially providing access to the service and children within. 3. An unsafe practice was observed when the children accompanied by staff from pre-school room two were observed to leave the main outdoor play area through a gate which opening onto an unsecured space at the rear of the premises to return to their pre-school room through the fire evacuation door into pre- school room two. A risk assessment was not in place detailing the control measures to minimise the potential risk of a child accessing an unauthorised area within the community grounds. 4. An unsafe practice was observed when the children accompanied by staff from the crèche were observed to leave the main outdoor play area through a gate which opening onto an unsecured space at the rear of the premises to return to the crèche room by walking along the side of the building to return to the premises by the main entrance door to the service. A risk assessment was not in place detailing the control measures to minimise the potential risk of a child accessing an unauthorised area within the community grounds. 5. There was an absence of documented training and demonstrated competency in medical tasks undertaken by staff members including the provision of nutritional support for a child with a defined medical condition. 6. A broken cupboard was noted in an area adjacent to the crèche room. The door was no longer attached leaving exposed sharp edges and a potential injury risk. 7. A lamp in the crèche room was not secure and posed a potential injury risk if pulled by a child. Infection Control: 1. The nappy changing procedure displayed was inadequate for infection control purposes: • There was no reference to handwashing after nappy changing for the adult or the child. • There was no reference to the removal of the gloves and apron prior to the redress of the child. 2. The nappy changing practice observed was inadequate for infection control purposes as the staff did not remove the gloves or apron prior to redressing the child. Action submitted by the Registered Provider - General Safety: The person in charge stated in the response that: 1. The fire alarm company for the service inspected the door on 20/05/26 and have ordered the alarm needed. It will be fitted by 01/07/2026. Staff will always supervise the door area to ensure the push bar is not used by children. (Confirmation email and work docket submitted). 2. The use of the fire door to drop and collect children has been stopped. Children will be dropped and collect through the main door or back door which has glass panels to verify the identity of any individual. Parents have been made aware of the change and have agree to use alternative doors for safety. Staff have acknowledged and implemented the change. (Digital platform message to parents and notice to staff included). 3. A risk assessment has been put in place to cover the space between the garden gate and the back door. It was also explained to staff about using the entry door within our premises to enter and exit the garden where possible. It was explained to each staff member to use the door located in preschool 1 to enter and exit the garden from the building. (Risk assessment and notice to staff included). 4. A risk assessment has been put in place for using the footpath outside the building. It was also explained to staff about using the entry door within our premises to enter and exit the garden when in the garden. It was explained to each staff member to use the door located in preschool 1 to enter and exit the garden from the building. (Risk assessment and notice to staff included). 5. I have followed up with the nurse that completed the training with the relevant staff member regarding documenting the training provided. I have also had the staff member involved complete another training. (Certificate and training policy included). 6. The door on the cupboard has been repaired to remove any risk to children or staff. Staff have been made aware of the importance of flagging broken materials and taking them from the floor until it can be repaired or disposed of. (Staff meeting notes and photograph included). 7. The lamp has been removed from the creche room to eliminate the risk of potential harm. (Photograph included). Infection Control: The person in charge stated in the response that: 1. The nappy changing procedure has been updated to reflect the removal of gloves and apron before redressing and handwashing after nappy changing has been completed. The updated procedure has been displayed and all staff have been made aware of the change. (Procedure and photograph included). 2. Summary Comment In respect of corrective actions taken to date photographic and documentary evidence was submitted to the office of the Early Years Inspectorate reviewed by the Early Years Inspector and deemed to meet regulatory compliance. The proposed actions in respect of door alarm installation will meet the regulatory requirement upon completion in respect of general safety - 1 but remains outstanding until the evidence of works completed is submitted by July 10th 2026 24 March 2026 ##### [Silly Billies Playschool](/creche/silly-billies-playschool-portarlington.md), Portarlington · Regulation 23 — Safeguarding health, safety and welfare of child - Outing: Written Risk Assessments were not recorded prior to outings. This was at variance to the services Policy on Outings which stated, ‘Risk Assessments will be recorded and ready for inspection’. Action submitted by the Registered Provider - Outing: The registered provider has created a Risk Assessment Form to fill in prior to outings/school tours with the children. These will be kept on the premises for inspection. All staff have been advised of the new process 19 March 2026 ##### [Darling Buds Montessori & Daycare](/creche/darling-buds-montessori-daycare-portlaoise-2.md), Portlaoise · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. The following water temperatures presented a potential risk of a scald injury to children • the downstairs children’s sanitary accommodation ranged from 55.4°Celsius - 55.8°- Celsius • the ECCE 1 children’s sanitary accommodation was 55.4°C. An immediate action notice was issued to the registered provider on site in respect of Regulation 23 Safeguarding Health, Safety and Welfare of Child, General Safety. This was a noncompliance during inspection on 17/02/2025. The following hazards were noted in the outdoor play area: • Four loose toilet bowls were stored in the outdoor play area where children from the Toddler room were playing. Lids from the cisterns were lying on the ground. Children were observed to run up to the toilets. These presented a risk of injury if a child were to pick up a cistern lid and drop one. The toilets had been removed from the upstairs sanitary accommodation and presented a risk of cross contamination or posed a risk of infection. • A sand/water table was noted to have stagnant brown water presenting a risk of infection if a child were to put their hands or toys into the stagnant water. • A plastic table stored upside down and a green container had stagnant rainwater presenting a risk of infection. • A child approached the inspector and handed her a discarded animal bone which they had been observed putting to their mouth whilst in the outdoor area. The inspector alerted staff to the risk and asked them to dispose of the bone immediately. This posed a risk of choking or injury to the child. 2. Daily outdoor risk assessments were not being completed to identify and manage the risks of any hazards in the service. An ‘Outdoor Risk Assessment’ was noted on the wall leading to the outdoor play area titled ‘garden’. The checklist had been ticked and signed by staff indicating that outdoor risk assessments had been completed on the day of inspection and the days prior. The risk assessments did not identify the above hazards. 3. Christmas lights were placed around the window in the Toddler room for decoration in reach of the children. The trailing cord was hanging down to the ground and plugged in behind a storage box. This presented a potential risk of injury to a child such as choking or tripping. Infection Control: 4. Handwashing practices in the Wobbler room were inadequate to control the spread of infection. Staff from the Wobbler room did not wash their hands after wiping children’s noses. Children were not brought to wash their hands before eating their meals and did not wash children’s hands prior or following nappy changing. 5. The following was noted in the sanitary accommodation of ECCE 1, • Two cloth towels were in the sanitary accommodation in the ECCE 1 room. One towel was on the floor and another hanging on the door handle. Staff advised that the towels were left in the area following the toilets being replaced on 17 March. A child was observed to dry their hands with one of the towels. This presented a risk of cross contamination or infection from using unclean towels. • A ‘Daily Children Toilet’ checklist was observed in the sanitary accommodation of ECCE 1. It was last recorded and updated on 13 March. • A swing lid bin in the ECCE 1 sanitary accommodation was in use for used paper towels. Foot operated pedal bins are required for the disposal of waste in sanitary accommodations. 6. Plastic drinking cups were stored stacked on top of each other on the sink in the Toddler room. The sink was used for hand washing. The sink was noted to be unclean with a used cleaning cloth and a sponge. Children were observed to put their bowls in the sink after their hot meal. This presented a risk of cross contamination from the cleaning cloth, hand washing and the drinking cups. 7. A child was observed to pick up a discarded piece of a cereal bar from the garden and eat it. This poses a risk of cross contamination. 8. In the Wobbler room, a child’s bowl of hot food was observed to fall upside down onto the floor. A staff member saw this and turned the bowl back up and was observed scooping some of the food back into the bowl and return it to the child. They did not replace it with a new bowl; this posed a risk of cross contamination. 9. The children’s soothers were not stored in individual containers, which posed a risk of cross contamination. Administration of Medication: 10. Two auto adrenaline injectors available for one child in ECCE 1 were out of date and had expired in July 2025. 11. Two children in the ECCE 1 room who required specific medication did not have a specific care plan for inhaled medication. Staff were unable to advise the inspectors of the steps to take if the medication was required. This poses a risk to the child in the event of a medical emergency. Action submitted by the Registered Provider - General Safety: 1. The water temperatures were regulated by a plumber in the children’s sanitary accommodation sinks downstairs and ECCE 1. The service will ensure all sanitary areas water temperatures are checked and correct. • The toilet bowels were removed from the outdoor play area. Any materials left on the premises by maintenance workers will be discarded appropriately and out of reach by children. A new risk assessment for maintenance workers put in place. Staff spoken to about the importance of checking all rooms once maintenance workers have completed their job. New risk assessment in place. • Sand and water tables have been cleaned and covered. Sand and water tables outdoors will be covered to prevent any risk of infection. • The Laois County Council warden has been contacted regarding issues with neighbours discarding food in the garden resulting in birds picking the food waste or bones and dropping it in the services outdoor area. Until this issue is resolved staff will check for any food or bones in the garden regularly. 2. All outdoor toys have been cleaned and will be checked daily to prevent any harm of infection to children. All outdoor equipment and toys will be checked each morning and afternoon before signing the risk assessment. The staff have been spoken to regarding the potential hazards or harm to children if risk assessments are signed off without the assessment being carried out beforehand. 3. The Christmas lights have been removed from the Toddler room. The service will ensure staff are aware of the potential hazard that these cables may cause to a child and ensure any cables are out of reach to any child. Infection Control: 4. Hand washing procedures have been placed at all hand-washing stations. The hand-washing policy has been given to all staff and spoken to about the importance of hand washing for themselves and children to prevent spread of potential infections. 5. Daily check lists for the children’s sanitary accommodation have been updated and will be checked regularly. • The swing lid bin in use in ECCE1 sanitary accommodation has been replaced with a pedal bin. • Checklist being monitored, checked and completed daily by staff. 6. Plastic drinking cups and bowls are no longer stacked on or in the sink in the toddler room. The sink is cleaned regularly, and cloths and sponges are now stored in a separate basket out of reach from children. A basket has been provided for children to place used cups and bowls in. A cleaning checklist in in place to ensure sinks are being cleaned regularly. A basket provided for cloths and sponges away from children’s reach. Staff spoken to about the importance of cleanliness. 7. Children are fully supervised when having snacks in the outdoor area. Staff spoken to about the importance of supervision when children eat in the outdoor area and the importance of cleaning up and discarding all foods that may fall on the ground. 8. All staff have been spoken to about the importance of health and hygiene and how if a dinner spills a new dinner should be offered. 9. All soothers are now placed in individual labelled boxes to prevent cross contamination and staff have been spoken to regarding the potential risk of infection if soothers are shared. Administration of Medication: Parents of children requiring specific medication and care plans were contacted immediately on the day of inspection and out of date epi pens have been replaced. Staff will check dates on all auto injectors regularly and care plans in place. Care plans are in place for children requiring one. Staff training has been completed in administration medication 13 March 2026 ##### [Durrow Community Playgroup Ltd.](/creche/durrow-community-playgroup-ltd-durrow.md), Durrow · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: The most recent Garda vetting disclosure presented in respect of one of the staff members was not dated within the previous three years in adherence with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. This non-compliance was observed on the previous inspection 29 February 2024. The preventive action following this inspection failed to prevent recurrence of this non-compliance. Action submitted by the Registered Provider - General Safety: The registered provider stated in the response that renewal of Garda vetting was immediately applied for the staff member. The garda vetting disclosure was returned, reviewed and submitted to the Early Years Inspectorate. (Garda vetting disclosure application confirmation included). A reminder date has been set for all staff members for when their Garda vetting is up for renewal. Summary Comment In respect of the corrective actions taken documentary evidence was submitted to the office of the Early Years Inspectorate and reviewed by the Early Years Inspector. The regulatory requirements remain outstanding until the Garda vetting disclosure is returned and submitted to Tusla 9 March 2026 ##### [The Maples Creche](/creche/the-maples-creche-portlaoise.md), Portlaoise · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. A toaster was stored on top of the microwave, which was on top of the fridge in the Butterfly room and Chicks room, there was a trailing cable within reach of the children which posed a risk of injury to children. 2. A number of hazards were identified in the outdoor play area which posed a risk of injury to the children as follows: • A wooden piece of equipment located in the outdoor play area off the Butterfly room, was damaged with exposed jagged edges and sharp nails sticking out; this was accessible to children playing in the outdoor area and posed a risk of injury to the children. • A damaged tree had fallen on the outside of the fence in the outdoor play area. The wire fence located to the rear of the outdoor play area could not support the tree and the fence was leaning inwards into the outdoor play area compromising the security of the perimeter fencing. • A gas heater was located in the roofed outdoor area to the side of the Butterfly room; there was a gas cylinder attached with a pipe which was accessible to the children. • A wooden climbing frame was severely water damaged was unsafe for children to use. It is acknowledged that staff used a slide and chairs to prevent children from gaining access to the equipment. 3. The surface radiator temperature in the Chicks room was recorded at 58.7oC which posed a risk of a child receiving a burn injury. Infection Control: 4. Inadequate infection control measures to prevent the spread of infection were observed on the day of inspection as follows: • There was no effective system in place to sterilise the mouthed toys in the Chicklet and Chicks room. Children with runny noses were observed to place toys in their mouth and staff did not remove these from circulation. This non-compliance was found on a previous inspection dated 27th February 2025. The corrective and preventive actions submitted by the registered provide did not prevent the re- occurrence of this non-compliance. • Staff did not wash their hands following wiping children’s noses. • Staff were observed to manually handle bins in the service and not use the foot pedals. • Waterproof mattress protectors were not provided on the mattresses of the cots in the sleep room. Safe Sleep: 5. The sleep records demonstrated that staff recorded the incorrect sleep room temperatures. During inspection, staff recorded the temperature as 18oC. The temperature stated on the thermometer in the sleep room was 20oC. Fire Safety: 6. Exit doors in all the care rooms identified with fire exit signage were obstructed with internal half doors and bolts at the top of the door which were locked during the inspection. This posed a risk to the safe evacuation of children in the event of a fire. 7. The bar on the fire exit door in the Caterpillar room was not functioning, a slide bolt on top of the door was used keep the door closed. Action submitted by the Registered Provider - General Safety: The toaster has been removed to a more suitable location. All hazards identified on the day have been removed. New fencing has been installed in the outdoor play area. A radiator cover has been fitted to the radiator. Infection Control: A sign has been printed and laminated and put on boxes for mouthed toys to ensure greater clarity for everyone. The staff will place mouthed in these boxes and then wash these at the end of the day. Staff have been retrained in hand washing. Staff have been retrained in the correct use of bins. Waterproof mattress protectors have been placed on all cot mattresses. Safe Sleep: The temperature of the sleep room will be recorded correctly. Fire Safety: The push bar on the fire exit door in the Caterpillar room has been replaced 25 February 2026 ##### [The Village Creche](/creche/the-village-creche-ballyroan.md), Ballyroan · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. All access gates were unsecured creating the possibility of unauthorised entry and access to the outdoor play areas, the main premises building and to the three standalone buildings on the grounds of the service. An immediate action notice in respect of Regulation 23 Safety was issued to the registered provider on 26 February 2026. 2. One of the fire evacuation doors in the Purple Ants room equipped with panic hardware and openable by means of a simple push of a horizontal bar and opened directly onto the rear driveway of the home of the registered provider. The door was not alarmed therefore no notice could be given in the event of a child opening the door, exiting the Purple Ants room and gaining access to the rear driveway of the home of the registered provider. 3. There was no physical barrier in place to prevent children from accessing the car park area located to the front of the premises at collection time. This presented a potential risk to children’s safety due to the presence of moving vehicles in the car park. 4. In the sanitary accommodation adjacent to the Purple Ants room the sliding doors of the cubicles were not secure and could move forward and backward which posed a potential injury risk to children who used the toilet independent of an adult. 5. A long hanging cable from the listening monitor in the sleep room adjacent to the Busy Bee room was noted trailing over two electrical sockets and within reach of a child in a cot which posed a potential risk of harm. 6. The kitchen and the contents within were accessible to children at times during the inspection as the stairgate was not closed when the staff member left the kitchen which posed a potential risk of harm. This non-compliance was noted on the last inspection on the 7 July 2025. 7. The office and the contents within were accessible to children at times during the inspection as the door was not closed which posed a potential risk of harm. This non-compliance was noted on the last inspection on the 7 July 2025. 8. The newly developed manager safety checklists did not include sufficient detail to ensure that potential hazards were recognised and eliminated including but not limited to water temperature, radiator temperature, child safety cupboard locks, waste bins, operational ventilation in the sanitary accommodation, broken toys, exit doors and routes and building access. Infection Control: 1. No handwashing was observed by both staff members prior to changing the children’s nappies which posed a potential infection control risk. This non-compliance was noted on the last inspection on the 7 July 2025. 2. The doors of the sanitary accommodation incorporating nappy changing remained open during the inspection which posed a potential infection control risk. This non-compliance was noted on the last inspection on the 7 July 2025. 3. In the sanitary accommodation incorporating the nappy changing area cleaning solutions were decanted into a bottle without content label or instructions for use attached. This non-compliance was noted on the last inspection on the 7 July 2025. 4. Waste was accessible to children in the sanitary accommodation adjacent to the Butterfly, Purple Ants and the Beetle rooms which posed a potential infection control risk. This non-compliance was noted on the last inspection on the 7 July 2025. Action submitted by the Registered Provider - General Safety: The person in charge stated in the response that: 1. On Thursday 26th February an immediate action was taken, the side gates were secured and all entry was through the front door only for all classes. The electrician has now fitted a permanent electromagnetic device with keypad to both gates, that will only be used by staff. Parents drop off and collection point is at front door. (Photographs included). 2. The fire door in Purple Ants room has been fitted with an alarm. (Photographs included). 3. The service had a health and safety assessment conducted to be sure any changes are in line with fire safety, ambulance and wheelchair access. A barrier was ordered to be made that meet this requirement and is due to be fitted which will be a permanent structure. (Sample type photograph included). 4. The sliding door has been secured and a stopper to prevent door from being derailed has been installed. (Photographs included). 5. A shelf has been fitted in the sleep room and the monitor and cable are safely secured. (Photograph included). 6. In respect of the kitchen, signs have been placed on the gate and all staff have been made aware that the gate must be always closed. (Photograph included). 7. In respect of the office administrative and management staff have been made aware that the door must be always closed. (Photograph included). 8. Indoor and outdoor monthly checklists were completed following the July 2025 inspection by management and we have now introduced individual classroom checklists also. (Photograph included). Infection Control: The person in charge stated in the response that: 1. Visual reminders for handwashing have been placed in the nappy changing areas and all staff have been re-trained in procedures. We are happy to observe staff using correct steps. (Photograph included). 2. Staff have been made aware and signs have been placed on doors. (Photograph included). 3. Our industry supplier was contacted and we have appropriate labelled bottles and instructions for staff. (Photograph included). 4. All bins in sanitary accommodation are no longer accessible to children. (Photograph included). Summary Comment In respect of the corrective actions taken documentary evidence was submitted to the office of the Early Years Inspectorate, reviewed by the Early Years Inspector and deemed to meet the regulatory requirement with the exception of General Safety – number 3 which remains outstanding until the building works are completed. Infection Control: 1. Perishable food products including snacks and meals for children availing of part time provision were not stored appropriately in a refrigerator. The main meal provision for a child and their specific supplementary food products was observed to be stored in the child’s school bag with an ice pack. This posed a potential risk of foodborne illness to the children as perishable food items were not stored in a refrigerator with a temperature below 5 degrees Celsius. 2. The hook and eye lock on the outside of the side gate was not secured for the duration of the inspection. Children were observed in the outdoor play area playing at the gate out of sight of the two staff members in the outdoor area. A slide bolt on the inside of the gate was within children’s reach. If children opened the side gate, they could exit the front of the service gate which was open throughout the inspection. Infection Control: 3. The temperatures recorded ranged from 45 – 55◦ Celsius which presented a potential risk of a scald injury to children. It is acknowledged that the designated person in charge took an immediate corrective action once the risk was identified by the early year’s inspectors. A plumber was contacted and came on site to take remedial action. The risk was eliminated prior to the conclusion of the inspection. Following the corrective action the temperature at the wash hand basin in the nappy changing area for the baby room was recorded at 34.8◦ Celsius at 14:25. Following the corrective action the temperature at the wash hand basins in the sanitary accommodation for the wobbler room was recorded at 39.3◦ Celsius and 40.7◦ Celsius at 14:29 Following the corrective action the temperature at the wash hand basins in the sanitary accommodation for the pre-school room was recorded at 37.1◦ Celsius and 36.1◦ Celsius at 14:32. The plumber fixed the issue straight away to meet the required temperature to be safe. Two children were observed to have whole grapes for their snack which posed a potential risk of choking. Upon request of the inspector a corrective action was taken, the staff member cut the grapes in half and returned them to the children. 2. Carbon monoxide alarms were not in place in the premises to give notice of any escaping gas in the environment. A gas boiler was located in the kitchen on the ground floor adjacent to the dining area and pre-school rooms. 3. A child was observed to wear hooped earrings which posed a potential risk of injury if the ear was pulled accidently by another child. Fire Safety: 1. The outdoor play area has been extended since the last inspection on the 2 October 2023. A new lock system was put in place on the perimeter gate at the fire assembly point. This consists of a sliding bolt on the inner part of the gate and a coded system is in place on the external part of the gate. Only the designated person in charge and deputy have knowledge of the code to unlock the external part of the gate. As both persons were not present in the service between the hours of 08:00 and 10:50 on the 14 July 2025 the gate could not be used during this time as an evacuation route from the building in the event of an emergency. Two children were observed to have popcorn for their lunch in the blue and red rooms presenting a potential choking hazard to a child. This is at variance with the service’s healthy eating policy that stated ‘’Lunches will be healthy containing no junk food, fizzy drinks, no nuts or popcorn.’’ 2. A hanging cable from a television unit was noted in the red room which posed a potential risk of injury to a child. Infection Control: 1. Thermostatically controlled warm water was not provided to encourage handwashing. Children were observed to wash their hands in cold water in the sanitary accommodation of the red and blue room. It is acknowledged that the upon request of the inspector the registered provider took a corrective action and put on the immersion to heat the water. 2. The child height hand towel dispenser in the sanitary accommodation was not in use. Staff assistance was required for some children to reach the paper towels after hand washing which were hanging on a roller on the back of the door in the sanitary accommodation. Staff were observed taking paper towels from the dispenser and handing them to the children. This posed a potential risk of cross contamination. 3. The corner of a nappy changing mat in the sanitary accommodation in the red room was torn and therefore could not be effectively cleaned which increases the risk of cross contamination. 4. Craft felt materials and children’s high vis jackets were stored in the sanitary accommodation in the red room which posed a risk of potential cross contamination. 5. Persishible items such as yoghurts and ham sandwiches contained within the children’s lunchboxes were not refrigerated. Outing: 1. Documented risk assessments relating to outings from the service that had previously occurred were not in place to support practice and ensure the safety of the children. The inspectors were informed that the children are brought on outings weekly to the local pitch which is located at the end of the Cul de sac where the service is located. Action submitted by the Registered Provider - General Safety: 1. The healthy eating policy and risk management policies were reviewed and updated. All staff were trained in the new policies. 2. The child’s hands were not washed after the nappy changing procedure was carried out and before returning the child to the pre-school room. In the nappy changing area, the liquid hand soap dispenser was located out of the reach for children’s use. 2. In the nappy changing area cleaning solutions were decanted into a bottle without content label or instructions for use attached. 3. The nappy disposal bin had a flap top access system which required the staff to handle the surface area which posed a potential risk of cross contamination. 4. Children’s clothes were stored in the nappy changing area which posed a potential risk of cross contamination. 5. A brush pan and brush were stored directly onto children’s art aprons in the wobbler room which was not suitable for infection control purposes. Fire Safety: 1. There were no self-illuminated fire exit signs in place to identify the appropriate evacuation routes from the pre-school rooms and sleep rooms in the event of an emergency. 2. Exit doors identified with paper fire exit signage were obstructed with toys and play equipment in the wobbler and toddler rooms. A wooden unit used to store wellington boots was located along the corridor leading to a fire exit which may imped the prompt evacuation of children in the event of a fire in the premises. Thermostatically controlled water was not available at the wash hand basins in the pre-school rooms on the ground floor and the first floor of the service and in the sanitary accommodation at taps 3 and 5 in the toilet block on the ground floor, at both taps in the sanitary accommodation adjacent to ECCE 3 on the first floor and in the nappy changing areas. The temperatures recorded ranged from 52 – 58 degrees Celsius which presented a potential risk of a scald injury to children. It was noted in the checklists completed by staff and displayed in the premises that the water temperatures were confirmed as being checked and no greater than 43 Celsius. It is acknowledged that the designated person in charge attempted to take an immediate corrective action once the risk was identified by the early year’s inspectors. The risk was not eliminated prior to the conclusion of the inspection. An immediate action notice was issued to the designated person in charge on site in respect of Regulation 23 Safeguarding Health, Safety and Welfare of Child, General Safety. 2. The most recent Garda vetting disclosure presented in respect of a staff member was not dated within the previous three years in adherence with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 3. Three of the five floor mats in ECCE room 3 were not non-slip which posed a potential slip, trip or fall hazard. Infection Control: 1. The nappy changing practice observed was inadequate for infection control purposes due to the following. • The staff members did not wash their hands prior to the nappy changing procedure. • One out of two nappy changing procedures observed the child was redressed with the same gloves used for nappy changing. 2. The nappy changing bin located in the wobbler nappy changing area was not suitable as it was not foot operated, sealed and airtight. In the sanitary accommodation for children on the ground and first floor, foot operated pedal bins were not in place for the hygienic disposal of waste. 3. A build-up of dust and physical debris was noted on the mechanical ventilation in the sanitary accommodation and nappy changing areas which required cleaning. Fire Safety: 1.
