An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). 30 June 2026 ##### [Punchbowl Creche LTD T/A First Friends Creche](/creche/punchbowl-creche-ltd-t-a-first-friends-creche-gort.md), Gort · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: • See Statutory Notice section in relation to Improvement Notice: IN 2481 served. 1. One cot in Sleep room 1 was placed directly in front of the fire exit door and a second cot in Sleep room 3 was placed under a window and beside a heat source posing a safety risk to the children. 2. The foam on the three outdoor support beams of the covered area had perished, was torn and pieces were missing posing a safety risk to the children in the event of an accident or injury. Infection Control: 3. There was no system in place for sterilising soothers. The inspector noted 1 soother on the floor of Sleep room 1 and 4 additional soothers lying in cots, posing a risk of infection. It was acknowledged that once this was discussed with the person in charge a sterilising tree was observed in use in the afternoon. 4. Contrary to the cleaning schedule available in the Toddler care room which outlined that the cots were cleaned daily on the week of the 15 of June 2026 and the week beginning 22 of June 2026 two cots in Sleep room 1 were observed to have dust hanging from the corners of the cots, posing a risk of an allergic reaction to the children. 5. There was dust present on the air vent in the upstairs toilet preventing the circulation of fresh air which helps reduce the risk of infectious diseases. 6. The pedals were broken on two tall pedal bins in use in the Diggers care room, posing a risk of cross contamination from the number of children and adults observed lifting the lid with their hands. Safe Sleep: 7. There was no adult present as required in Sleep room 1 where there were 2 children asleep on stackable beds and 2 children asleep in cots, posing a safety risk to the sleeping children. Action submitted by the Registered Provider - The following statements were received from the registered provider; General Safety: 1. The cots in Sleep room 1 and Sleep room 3 have been moved to ensure that they are not in front of the fire exit doors, under the window or placed beside the radiator. Management will carry out checks to ensure that cots are suitably positioned to ensure the safety of the sleeping children. 2. The perished foam on the three outdoor support beams of the covered area have been replaced. Staff will review regularly to make sure that they are in good condition and fit for purpose ensuring the safety of the children event of an accident or injury. Staff will check daily and replace as required. Infection Control: 3. Staff have been informed of the need to ensure that soothers are sterilised after use. A sterilising system for soothers is now in place daily to prevent the risk of infection. The inspector was shown a sterilising tree by the designated person in charge, suitable for use in the microwave, on the afternoon of inspection which had been reinstated. 4. All cots were cleaned on 1 July 2026 and a new cleaning schedule is in place. A meeting was held with all staff members and cots will be sanitised and cleaned thoroughly after each use. 5. The air vent in the upstairs toilet was cleaned and has been added to the weekly cleaning schedule. 6. The two pedal operated bins have been replaced in the Digger’s care room. Staff will check to ensure these are working properly and report any issues as they arise. Safe Sleep: 7. An adult is now stationed in Sleep room 1 to ensure the safety of the sleeping children. The service policy has been updated to reflect the change in practice 18 June 2026 ##### [Treasure Island Creche: 3](/creche/treasure-island-creche-3-oranmore.md), Oranmore · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. There was a piece of raised linoleum at the exit door to the outdoor play area in the Wobbler care room which was raised off the floor and split down the middle, posing a safety hazard for children exiting from the door. Infection Control: 2. There was no hot water available in the right-hand basin of the children’s sanitary area, preventing children from washing their hands effectively. In conversation, the manager stated that the part has been ordered and they were waiting for a plumber to come and fix it. Action submitted by the Registered Provider - The following statements were received from the registered provider; General Safety: 1. The raised linoleum has been replaced ensuring that it does not cause a safety hazard to the children. Infection Control: 2. We were awaiting a replacement part from Germany which has now been fitted, there is running hot water in the tap 8 June 2026 ##### [Burren View Creche & Montessori](/creche/burren-view-creche-montessori-gort.md), Gort · Regulation 23 — Safeguarding health, safety and welfare of child - Infection Control: The following non-compliances posed a risk of cross contamination to the children: 1. An adult changing two children at 10:15 in the Wobbler/Toddler changing area did not wear an apron while changing either child, contrary to the service’s infection control policy which outlined that gloves and aprons would be used to clean bodily fluids. In conversation, the staff member was not aware of the requirement for single use aprons to be worn on all nappy changes. 2. The pedal on the nappy bin in the Wobbler/Toddler changing area was broken and the adult was observed opening and closing the lid with their hands for each nappy change. In conversation, the adult stated it had been broken “for a few weeks” and the registered provider had been informed. Action submitted by the Registered Provider - 1. The submission received from the registered provider did not meet the requirements. 2. The pedal bin in the Wobbler/Toddler changing area has been replaced 4 June 2026 ##### [Jack & Jill Montessori School](/creche/jack-jill-montessori-school-salthill.md), Salthill · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. An adult sized doll’s house was not secured or anchored posing a safety risk should it fall or tip on top of a child. 2. A cord from a speaker, hanging down from a wall into a four-way extension lead, was accessible to children in the rest/quiet area of the Senior care room posing a safety risk to children sitting in the area. Infection Control: 3. Two vinyl sofas in the Junior care room rest/quiet area had loose stitching and the foam was exposed making effective cleaning difficult. Action submitted by the Registered Provider - The following statements were received from the registered provider: General Safety: 1. The adult sized doll’s house has been secured to ensure the safety of the children in the Senior care room. Management will ensure on a daily basis that all heavy equipment is secured. 2. A cord from a speaker, hanging down from a wall into a four-way extension lead in the Senior care room rest area has been removed. Management will conduct a daily risk assessment in order to action any potential safety issues. Infection Control: 3.The two sofas in the Junior care room have been removed and soft furnishings will be reviewed on a daily basis and any issues arising will be addressed 19 May 2026 ##### [Amy's Montessori and Afterschool](/creche/amy-s-montessori-and-afterschool-renmore-road.md), Renmore Road · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: The following posed a safety risk to the children; 1. Metal spokes, sticking out of the top of radiator cover in the rest/quiet area, were accessible to the children. 2. A nail was visible and accessible on a yellow panel which had become loose on the outdoor perimeter fencing. 3. Weather beaten hula hoops with loose conjoining pieces were accessible on the floor of the wooden house in the outdoor play area. Infection Control: 4. The corners of two vinyl sofas and one square pouffe had exposed foam where the seams had become undone making them difficult to clean effectively. 5. Three outfits on a dressing up trolley were visibly unclean, posing a risk of cross infection. In conversation, the registered provider stated that they were washed every two weeks however, no cleaning record was available on the day. It was acknowledged that a cleaning record was submitted on 20/05/2026, the day after inspection. Action submitted by the Registered Provider - The following statements were received from the registered provider; General Safety: 1. The radiator cover was removed and has been replaced. Staff will check on a daily basis to ensure that it is in good working order and report any issues which will be dealt with by the registered provider. 2. The yellow panel containing the nail has been repaired in the outdoor perimeter fencing. A daily risk assessment will be carried out to identify any risk and these will be actioned by the registered provider. 3. The hula hoops in the outdoor play area were removed. Staff will ensure that a daily assessment is carried out of all the outdoor equipment to identify and remedy any faulty equipment. Infection Control: 4. The two vinyl sofas were removed from the area along with the square pouffe and replaced with new soft furnishings. 5. The dressing up outfits will be washed regularly and this will be recorded on the cleaning schedule. All staff will ensure that they are hung up after use 14 May 2026 ##### [Williamstown Community Playschool](/creche/williamstown-community-playschool-williamstown.md), Williamstown · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: Garda vetting was available for five staff members, but the date was greater than three years for one staff member. This vetting disclosure was not in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI- RN12.3 Renewal of Garda Vetting’ to renew garda vetting every three years. Fire Safety: The fire alarm system was not in working order. The service manager informed the inspector that the fire alarm system was not working. The date of the last service of the fire alarm system was July 2023. It was due to be serviced again in July 2024. There was no record of such a service having taken place available for inspection. The date for the servicing of the firefighting equipment was dated in July 2024. The fire alarm system and the firefighting equipment were not maintained in line with the requirements set out in Fire Safety in Preschool 1999 and may pose a risk to the staff and children attending the service in the event of a fire on the premises. Action by the Registered Provider The registered provider stated in the following in their reply: - General Safety: An application for renewal of garda vetting was submitted and awaiting a reply. All garda vetting will be renewed in line with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting. Fire Safety: The fire alarm panel has been replaced. The system has been serviced and tested. The fire engineer will service the fire alarm as per the manufacturer’s instructions 12 May 2026 ##### [Gaelscoil Dara](/creche/gaelscoil-dara-renmore.md), Renmore · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: Garda vetting was available for 13 staff members in total. However, two vetting disclosures were not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. Action submitted by the Registered Provider On the 16 06 2026 the registered provider advised of the following - General Safety: Garda vetting applications have been submitted and completed for the two adults. An Excel database is currently being developed containing a full list of employees, including their garda vetting reference numbers and vetting expiry dates. A review of all garda vetting will be reviewed annually and renewal applications will be initiated immediately 7 May 2026 ##### [Mothercare Creche](/creche/mothercare-creche-loughrea.md), Loughrea · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: See Statutory Notice section in relation to Immediate Action Notice IAN0481 served. 1. On day 1 of inspection, in the main care room, children aged between 1 and 2 years of age were observed crawling under 7 highchairs posing a safety risk if the highchairs tipped or fell onto the children. In conversation, staff working with this age group stated that the highchairs were present in the area throughout the day. 2. On day 2 electrical flexes, from a lava lamp and fairy lights, in the rest/quiet area of the Montessori care room were accessible to the children, posing a safety risk. 3. On day 2, equipment in the outdoor play area including the ring of a basketball stand, roof of a high seat back car, back of a seat of a tricycle had cracked and missing plastic pieces, posing a safety risk to the children. Fire Safety: The following posed a safety risk to the adults and children. 4. Illuminated fire exit signage was not installed on the first floor posing a safety risk to the children and adults in the event of an emergency. 5. There were no fire doors fitted to the entrances of three rooms in operation on the first floor, posing a risk to the children and adults in the event of an emergency. Safe Sleep: 6. On day 1 there were no records of 10-minute physical sleep checks available for the children in the 2-3 age group sleeping/resting in the main care room, posing a safety risk to the children. In conversation, staff present in the care room stated that they thought having an adult present in the care room negated the requirement for physical sleep checks to be carried out. 7. At variance to best practice two children under 2 years of age, observed asleep in the main care room, were placed on stackable beds, posing a safety risk to the children. In conversation, staff in the care room were unaware that stackable beds are not considered suitable sleep equipment for children under 2 years of age. 8. On day 2, five mats in use for children aged over two years sleeping/resting in the main care room were fitted with covers which were doubled over and were too large for the mats, posing a risk of entanglement to the children sleeping and resting. 9. Mats and stackable beds in use in the main care room were not placed at least 50 centimetres apart posing a safety risk to the children. Infection Control: 10. Three mattresses in the designated sleep room were not fitted with waterproof covers posing a risk that they would not remain dry and clean while the children slept. 11. Not all perishable food belonging to children in the Montessori care room was stored in the fridge to prevent the food from spoiling. During morning snack time at 10:45 on day 2 the inspector observed staff distribute snacks from the children’s bags which included meat filled sandwiches. 12. Two pedal operated bins in the main care room were broken and the bin in the Pre-school sanitary area was not pedal operated, posing a risk of cross infection from the number of children lifting the lids with their hands. Action submitted by the Registered Provider - General Safety: 1. The 7 highchairs in the main care room have been removed and are now stored out of reach of the children. The highchairs will be brought into the care room for mealtimes only and removed immediately when the children are finished their food. New age and stage appropriate chairs have been purchased for the younger children. 2. The fairy lights have been removed and the lava lamp flex has been made inaccessible to the children. 3. The basketball stand, car and tricycle have been removed from the outdoor play area. Fire Safety: 4. The ongoing electrical and civil works have been completed. Illuminated fire exit signage has now been installed throughout the building. 5. All fire doors and windows have now been fitted and a letter of confirmation of same has been attached from the engineer confirming same. Safe Sleep: 6. Physical 10-minute sleep checks are now being carried out and documented by staff on children aged over 2 years of age sleeping and resting in the main care room. 7. Children under 2 years of age now have access to cots in the designated sleep room, ensuring the safety of the children while sleeping/resting on a daily basis. Additional suitable sleep equipment has been placed on order. 8. All sleep equipment is now supplied with the correct bed linen fitted to the size of the mat, bed or cot in use. Staff will ensure that each piece of sleep equipment is supplied with appropriately sized sheets and bedding. 9. Staff have been informed of the need to ensure that all sleep equipment is placed at least 50 centimetres apart in order to prevent the spread of infection. Infection Control: 10. Three new waterproof mattresses have been purchased along with suitable sleep equipment for children under 2 years of age. 11. Two new large fridges have been purchased to ensure that perishable food is stored correctly. 12. Four new foot pedal bins have been installed to prevent the spread of infection and cross contamination 28 April 2026 ##### [Mary’s Preschool](/creche/mary-s-preschool-ballinasloe.md), Ballinasloe · Regulation 23 — Safeguarding health, safety and welfare of child - Fire Safety: 1. A referral was made to the fire officer with reference to two doors in use as fire doors; the entrance door to the care room door had no lit signage and no thumb turn lock or push bar mechanism. The second door, identified as the main fire exit door in use by the service, was secured with a key lock and a night latch both posing a safety risk to the children and adults if the children and adults had to evacuate the service quickly in the event of an emergency. General Safety: The following non-compliances posed a general safety risk to the children; 2. Two gates in the outdoor play areas were accessible by unauthorised adults. 3. A mark making board in the outdoor play area had a broken frame with splinters accessible by the children. 4. The lock on the adult toilet door was broken and the area was accessible by the children. 5. Cleaning agents were accessible in an unlocked cupboard under the sink. Infection Control: 6. Contrary to the service’s nappy changing policy an unclean changing mat was observed in the children’s sanitary area. In conversation, a staff member stated that children were changed on the mat on the floor, posing a risk of infection. 7. Contrary to the service’s nappy changing policy no disposable aprons were available in the event a child required nappy changing, which may pose a risk of cross infection. 8. There were three full wheelie bins positioned in the rear outdoor play area which were accessible by the children, posing a potential risk of cross contamination. Action submitted by the Registered Provider - Fire Safety: 1. The registered provider stated that they are awaiting a response from the fire officer. General Safety: 2. Secure devices were fitted to both the large metal gate at the front of the service and the gate at the rear of the service on 29/04/2026 to ensure the safety of the children. A risk assessment will be carried out by staff prior to children playing in the outdoor areas to ensure that no unauthorised adults can enter the area while the children are on the premises. 3. The damaged mark making board has been removed from the area. 4. A secure device has been fitted to the adult toilet to ensure that the children cannot access the area. Staff will check to ensure that the door remains secured while the children are present in the service. 5. Cleaning agents have been removed and stored in a secure cupboard out of reach of the children. Infection Control: 6. A new changing unit and nappy changing mat are now in use. 7. Single use disposable aprons are now in place in the sanitary area for use by the adults, if required. Staff will ensure that a new apron is put on prior to each child being changed in order to prevent cross contamination. 8. The wheelie bins were moved into the shed which is secured and inaccessible to the children. They have been added to the daily risk assessment which will be checked by staff prior to the children being allowed in the area 24 April 2026 ##### [Cairde Early Learning Centre](/creche/cairde-early-learning-centre-loughrea.md), Loughrea · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. Five of 18 adults did not have garda vetting disclosures dated within the previous 3 years in adherence to the Early Years Inspectorate Regulatory Notice “EYI-RN12.3 Renewal of Garda Vetting” posing a potential safety risk to the children. It was acknowledged that a garda vetting disclosure for one of five of the adults was received on 25/03/2026. 2. Hot water, in the wash hand basins in the children’s sanitary area, on the ground floor on day 1, were recorded over 43°C posing a safety risk to the children. It was acknowledged that a plumber had been in attendance by day 2 and these were rectified with the temperatures recorded under 39.7°C. 3. On day 1 a flex from the air conditioning unit in sleep room 2 was accessible to the children. It was acknowledged that when informed the area manager secured the flex out of reach of the children. Infection Control: 4. On day 1 the pedal on the nappy changing bin in the Junior Toddler sanitary area was broken, posing a risk of cross infection from the number of hands in direct contact with the lid. It was acknowledged that this was replaced on the day 2 of inspection. Action submitted by the Registered Provider - The following statements were received from the registered provider; General Safety: 1. Garda vetting renewals are now on file for the 5 adults whose garda vetting had expired. In future, all garda vetting will be regularly checked to ensure compliance. Renewals will be carried out three months prior to expiry. 2. A plumber was called and the hot water in the wash hand basins in the children’s sanitary area on the ground floor was rectified. All hot water in the wash hand basins have been thermostatically controlled to ensure the correct temperature range and these will be checked regularly. 3. When informed the flex from the air conditioning unit in sleep room 2 was secured, out of reach of the children. The manager stated that staff will ensure that no flexes or cords are accessible to the children. Infection Control: 4. The nappy changing bin in the Junior Toddler sanitary area was replaced before the service opened on day 2 of inspection. A spare nappy changing bin will be kept in the service to ensure that there is no risk of cross infection in the event that there is a broken bin which requires replacing 21 April 2026 ##### [Fairyhouse Creche & Daycare Centre](/creche/fairyhouse-creche-daycare-centre-clarinbridge.md), Clarinbridge · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. Three of 10 adults- 2 existing staff whose garda vetting required renewal and 1 new staff member, did not have garda vetting disclosures dated within the previous 3 years in adherence to the Early Years Inspectorate Regulatory Notice “EYI-RN12.3 Renewal of Garda Vetting” posing a potential safety risk to the children. It was acknowledged that copies of applications submitted for garda vetting for the 3 adults was received on 29/04/2026. 2. Children in the Junior Playschool and Nursery sanitary areas had access to plastic gloves, nappy sacks and plastic aprons positioned beside the nappy changing mats, posing a safety risk to the children. It was acknowledged that when informed the staff member from the Nursery care room repositioned the items out of reach of the children. 3. Three children were observed crawling under the highchairs which were positioned in the Nursery care room, posing a risk of them tipping or falling onto the children. In conversation, an adult in the room stated that the highchairs were stored in the room at all times. Infection Control: 4. The corners of the vinyl sofas and corner units, in both the Senior Nursery and Junior Playschool care rooms, had perished preventing effective cleaning. 5. A pedal on a nappy changing bin in the Junior Playschool sanitary area was not working, posing a risk of cross infection from the number of adults using their hands to insert the nappies into the bin. Action submitted by the Registered Provider - General Safety: 1. Garda vetting submissions have been made in respect of the three adults for whom they were required. A checklist table has been created to ensure that all garda vetting renewals and course expiry dates are renewed prior to expiry. Management will review the checklist table on a monthly basis and to ensure any required documentation is in date and will follow up with staff to submit prior to expiry to ensure compliance. 2. Storage shelves have been added in the Nursery and Junior Playschool nappy changing areas to prevent children from accessing plastic gloves, nappy sacks, and plastic aprons. 3. The highchairs in the Nursery care room have been removed while the children are playing. They are put in place for mealtimes only and removed when the children have finished eating. Infection Control: 4. New soft furnishings have been ordered for the Senior Nursery and Junior Playschool care rooms. 5. A new pedal operated nappy bin has been ordered for Junior Playschool sanitary area. Staff have reminded to complete risk assessment of these areas daily and inform management of any broken equipment or areas of concern 17 April 2026 ##### [Scoil Iosa Pre School & After School](/creche/scoil-iosa-pre-school-after-school-oranmore.md), Oranmore · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: The main entrance gate was not adequately secured to prevent unauthorised adults from entering or children leaving the premises unsupervised while the children were observed playing in the outdoor play area. It was acknowledged that when informed the deputy temporarily secured the gate until a more permanent solution could be organised. Action submitted by the Registered Provider - General Safety: 1. The gate has been secured to prevent unauthorised adults entering or children leaving the premises unsupervised while playing in the outdoor play area. This may pose as a safety risk to a preschool child if administered. Action submitted by the Registered Provider The registered provider stated in their response: - General Safety: The management team has reissued the medication administration policy. Staff have been given a refresher demonstration on the procedure of administering medication. A paper copy of the administration of medication consent form has been reinstated to ensure that expiries and medication requirements are cross checked before administration 12 March 2026 ##### [Tirellan Montessori](/creche/tirellan-montessori-galway.md), Galway · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. At snack time approximately 11.00 one preschool child had whole grapes in their lunch box. The staff member observed the grapes but did not act at that time. This posed a potential risk of choking. The early years inspector brought it to the attention of the registered provider and staff member. The grapes were removed and cut into pieces. Infection Control: 2. During the morning snack at approximately 11.00 the children’s perishable items were removed from their bags in the playroom and were not stored in the service fridge as per best practice guidelines. This posed a risk of food spoiling and food contamination. Action submitted by the Registered Provider On the 13/3/2026 and the 17/4/2026 the registered provider advised of the following - General Safety: The topic was discussed at a staff meeting. All snacks/lunches will be checked daily for any choking hazards. The staff will cut the foods accordingly if necessary. Infection Control: The topic was discussed at a staff meeting. All foods to be stored in the service fridge on arrival to the preschool each day. A staff member has been assigned the duty of placing the lunch boxes/bags in the fridge each day 4 March 2026 ##### [Ballinasloe Community Childcare Group](/creche/ballinasloe-community-childcare-group-ballinasloe.md), Ballinasloe · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. The opening in the 5 windows in the senior playroom, ranged from 16.5 cm to 22.9 cm. The openings in the 4 windows in the junior playroom ranged from 16.5 cm to 19 cm. The playrooms were located on the first floor of the premises. There wasn’t a safety restrictor installed on these windows which would prevent a child from falling from the window. The safety restrictors should limit the initial movement of an opening section to not more than 10 cm. 2. There was a trailing lead that was accessible from a cot in the toddler room which may pose a risk of strangulation to a child using this cot. The inspector asked for this trailing lead to be removed. Infection Control: 3. There were two mattresses in the toddler sleep room that were ripped on the corners with exposed foam, which prevented the mattresses from being effectively cleaned between uses, thus posing a risk of cross infection. 4. The soft seating in the upstairs Junior Preschool room was ripped with exposed foam which made it difficult to effectively clean the seats, posing a risk of cross infection. 5. The children’s hands in the baby room were not consistently cleaned before eating. This posed a risk of cross infection. 6. The drop downside rail on a cot in the toddler sleep room was broken. The rail would not stay in a fixed upright position. This posed a safety risk to a child. The person in charge advised that the cot was not going to be used until the side rail was repaired. Action submitted by the Registered Provider The registered provider stated the following in their reply: - General Safety: 1. The opening of the window was restricted to less than 10 centimeters. The registered provider submitted a risk assessment and an interim plan to manage the immediate risks to the preschool children and had contacted a competent person to regulate the water temperatures. 2. At approximately 10.45 in the Cabin Junior Montessori room 2 it was observed that a child was sitting at the table for snack time with whole grapes in their lunch box. This posed a potential risk of choking for a preschool child. The staff and person in charge were informed immediately. The grapes were removed and cut accordingly. Infection Control: 1. This posed an immediate safety risk to the 4 children under 12 months of age in situ at the time of inspection. 2. The two doors identified as fire exits in the unregistered portacabin were operated with key locks posing a safety risk should the keys not be available to unlock the door to facilitate the safe evacuation of children and adults in the event of an emergency. General Safety: 3. The floor covering in the two pre-school care rooms was damaged posing as a potential tripping hazard for a preschool child. It was acknowledged that remedial work was carried out on the day to reduce the risk. 4. Both the Senior and Junior care rooms children had access to cleaning detergents which presented as a risk to a preschool child. When the inspectors pointed this risk out to the registered provider they were removed and put out of reach of children. 5. Two exposed screws were located at the entrance to the Baby care room play area where the gate was missing posing a safety risk to children. It was acknowledged that when the inspectors informed the registered provider this was rectified. 6. A panel in the rear play area was missing and three rusty nails were exposed and may present as a safety risk to children. 7. Two electrical flexes in the Baby 2 care room were not secured which may pose a risk to a child. 8. An internal floor mat located at the exit door from the main building to the Baby 1 care room had curled up corners that may pose as a tripping hazard. The following non compliances may pose as a risk of cross infection to a pre-school child: Infection Control 9. The nappy changing bin in use in the Baby care room 2 sanitary area was not pedal operated. 10. Mops/buckets and brushes were accessible to children in the Pre-school care rooms upstairs. 11. A broken bin was observed in the upstairs children’s sanitary area. It is acknowledged that this was replaced during the inspection. Safe Sleep: 12. Ten-minute physical sleep checks were not recorded in line with the service policy in the Baby 2 care room. There were two children asleep in the designated sleep room at 14:10 when the inspectors reviewed the sleep checks. The first child sleep record indicated that the last physical sleep check had been carried out at 13:40 and the second child’s record indicated that they were last checked at 13:45. The constant beeping may result in children and staff ignoring the signals in the event of a real emergency. Various items were inappropriately stored in the lobby area adjacent to the sanitary area and Naionra playroom which were accessible to the preschool children and posed a risk to safety, harm or obstruction. − A nappy changing mat, a whiteboard, folded brown cardboard were stored behind the filing cabinet, plastic bags and blue water dispensing bottles were stored on top of the filing cabinet. − A large black water basin, with grey lid and two school bags, a large open black storage unit with a bag of blocks. − Two balance boards and a grey colour quilted patterned piece of material were stored in the corner under the window opposite the sanitary area. 2. In the designated outdoor play area to the rear of the community building adjacent to the school playground a child was observed playing with a briar full of thorns. The early years inspector and staff member intervened and removed the briar. This posed a risk of safety and harm to the preschool children. 3. The designated outdoor play area was not safely secure so that children could not leave the area unsupervised and unauthorised persons could not enter the area. While it is acknowledged the children were supervised while playing in this area, the inspector could open the green sliding gate latch and enter the play area without any assistance. This posed an access risk and the potential risk of egress of a child. 4. There was a daily risk assessment checklist for the indoor and outdoor play areas, for checking that toys and equipment were not broken, that they were in a clean condition and maintained in a good state of repair, that the area was clean and free of hazards and risk to the children. Three-week records were reviewed by the inspector with generic ticks after each item and no entry documented for the 28/1/2026. This posed a potential risk of safety and harm to the preschool children as the records are not maintained accurately and no follow through was observed or any comments if a risk was identified. Safe Sleep: 5. While it was acknowledged that the service had six stackable beds, there was no evidence of individual bed linen for any children who were tired and wished to have a rest or sleep during the part time service hours. At approx. 10.45 one child was observed playing with toy cars on this countertop and one child was hiding under it. Following discussion with the person in charge she stated it belonged to the school and was waiting for it to be removed. This posed a serious risk of falling on to and causing harm to the preschool children. 2. An old broken green wooden school bench was stored in the all-weather sheltered area. This bench had no wooden back support and two metal fixtures were sticking up. The children were observed to play in this area which posed a serious risk of danger and harm. 3. An old blue wooden school desk with flaking rusted metal legs was stored in the sheltered play area where the children were observed playing hide and seek this posed a potential risk of danger to the children. 4. A tall clothes rack with pegs was in the sheltered outdoor play area. The children were observed playing in this area. The clothes rack posed a potential risk of danger if it fell on the children. 5. A large adult size table tennis table with pointed corners was situated in the sheltered outdoor play area, the children were observed to play hide and seek in this area which posed a risk of harm as the height of the table was at the Childrens level posing a risk of injury and harm. Contrary to the service’s sleep and rest policy 10-minute physical sleep checks were not documented in a timely manner to ensure the safety of the sleeping children. The following examples were observed in the Baby sleep room and in the Playschool room during the sleep period after lunch: • At 11:50 in the Baby sleep room four were placed down to sleep in cots. At 12:28 sleep records were requested and no documented records were available either on paper or on the application on the tablet. In discussion, the staff member verbally gave the sleep and awake times of the five children up to 12:35, however these were not documented at this time. • At 12:37 to 12.44 the Inspector was present in the Baby sleep room. No staff member entered the sleep room during this time. When requested, 10-minute physical sleep checks on a paper template for the five sleeping children from 12:05 onwards, including the 12:40 sleep check had since been documented on a paper template and were made available. It was noted that a sleep check was documented as having taken place at 12:40 while the Inspector was in the sleep room, but no staff member was present in the sleep room. The staff member stated that this check had been carried out using the camera in the care room and not in person contrary to the service’s sleep and rest policy which states “sleeping babies/children checked every 10 minutes by assigned staff. The sleep information will be recorded on a sleep chart including position, colour/pallor and breathing pattern.” • Documented evidence that sleep checks were carried out consistently on each child was not evident in the Playschool room. At 13:20 there were 12 children aged from 1 year 11 months to 2 years 9 months, on blue stackable beds, being monitored by 2 staff members. When records were sampled for 3 children, there were no documented sleep records available in either paper format or on the application on the tablet for 2 of the 3 children in the care room at this time. Not monitoring young children as per guidance while sleeping may pose a risk to their safety 2. Appropriate sleep equipment was not used for children in both the Wobbler and the Playschool rooms. Thirteen children aged between 16 and 23 months were observed sleeping on blue stackable beds during the sleep period. This was at variance with the “Safe Sleep Guidance for Children under 24 months” General Safety: 3. Children were observed to have access to a toilet cubicle which was being used as a storage area in the Wobbler nappy changing area. This cubicle could not be made inaccessible as it did not have a door. Several mops and a hoover were stored in this area. Access to this area could pose a risk to the safety of the children. This non-compliance was observed on the previous two inspections carried out on 7 June 2024 and 20 September 2024 and was not addressed by the registered provider. A meeting was held with staff on Monday 24 November at 09:00 followed by a staff meeting at 18.00 with all staff. We will ensure in the future that cots will have a wipeable surface or waterproof mattress protector in the future 14 November 2025 ##### [Sea Garden Childcare](/creche/sea-garden-childcare-leenane.md), Leenane · Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: The designated sleep room needed de-cluttering and safety measures to reduce the potential risk of harm and safety to the preschool children within. 1. At approx. 10.30 am one stackable bed was standing up against a cot in the designated sleep room. This posed a risk of harm to the preschool children. 2. A box of unopened new blinds and two folded highchairs were stored between 2 cots posing a risk of harm to the preschool children. 3. Staff personal belongings were accessible to the preschool children. A coat was stored on the wicker chair in the designated sleep room between 2 cots. The staff bags and coats were stored on the inside door of the main playroom. This posed a risk of harm to the preschool children. 4. An electric heater with trailing lead was stored behind the door in front of the desk in the sleep room posing a trip hazard. 5. The cosy area was located beside a wall mounted low level radiator with the turn dial directly located beside the area where the children under 12 months were observed sitting on mattresses with cushions to support them. This posed a risk of harm to the preschool children as there was no protective covering on the protruding turn dial. 6. An electrical printing device with trailing leads was located on the top shelf of the arts and crafts shelving unit beside the kitchen door, this posed a potential risk of harm to a child. 7. A lamp with trailing lead was resting on a cardboard box behind the door in the sleep room posing a trip hazard. 8. A pink soother with beads attached spelling the child’s name was noted on the desk in the sleep room posing a risk of safety and harm to the preschool child. Infection Control: 9. The children’s perishable food items were not stored in the refrigerator in the kitchen. The early years inspector observed 2 yogurt pouches in lunch boxes on a trolley inside the playroom at 10.00 and remained on the trolley until approximately 14.30 when the children were having snack time. This posed a risk of the food spoiling. It was at variance to the service policy and the guidelines on the yogurt pouch stating they should be refrigerated between 0 and 4°C. 10. On the day of inspection, items were stored inappropriately in the sleep room posing a risk of cross contamination. Equipment was not consistently stored safely and in an organised hygienic way. For example, the inspector noted the following. 11. In the children’s sanitary area at 11.00 there was a bin that was overflowing and the lid would not close. 12. Three stackable beds that were not in use were made up with sheets and blankets. However, the sheets were touching the floor posing a risk of cross infection. 13. A box with wheelie bin liners were stored under a cot. 14. Brown paper was stored under a cot. 15. Loose sheets were stored on an open shelf unit in the corner on the floor of the sleep room. It was difficult to identify if the items were used or unused. There was no evidence of individually labelled containers or storage boxes with each child s bedding items. There was no evidence of how the bed clothes/cot sheets were cleaned or laundered. The staff advised they send them home with parents/ guardians. 16. Additionally, on open shelving in the designated sleep room 3 electronic printing devices, folders, writing materials and an electric fan were inappropriately stored posing a potential risk of harm. 17. An adult’s black bedroom slippers, a child’s car seat and an orange and yellow poof seat were stored inappropriately posing a risk of cross infection. 18. The nappy changing mat was frayed and torn. This posed a risk of cross infection to the pre-school child as the mat cannot be cleaned sufficiently and needed replacement. 19. There was evidence of a build-up of dust under the cot mattresses that were on the floor in the main playroom posing a risk of cross infection. 20. A used apron and gloves were observed on top of the desk in the designated sleep room, this posed a risk of cross infection as the items were not disposed of safely. The staff were informed but the items remained on the desk throughout the inspection. 21. A pink soother was noticed on the desk in the sleep room with no storage box. This posed a risk of infection as it was not stored safely. Additionally, a blue uncovered soother was found on the wicker chair under clothes in the designated sleep room. The 2 shelf unit in the designated sleep room contained 3 loose and uncovered soothers this practice posed a risk of cross infection to the preschool children. No safe storage or sterilisation practices were observed during the inspection. Safe Sleep: 22. The designated sleep room temperature was recorded after 2 children got up. They were unsettled and did not sleep. The room temperature was recorded at 23.1° C at approximately 10.49. It was observed that an electric heater was turned on beside the desk. It was recorded at 60.4°C and the wall mounted low level radiator was recorded at 35.4°C. No child was sleeping at this time. The staff member was informed immediately. The radiator was turned off. The service sleep policy states the room temperature should be between 16-20°C. This posed a potential risk of harm to the preschool children. 23. No room temperature checking device was available, however a staff member continued to write 18°C on the sleep check records that were stuck to the door of the designated sleep room. When questioned she stated this was the temperature the room was supposed to be at. This practice posed a potential risk of harm to the preschool children. 24. The 3 mattress covers were not waterproof thereby the staff were unable to wipe them clean between use. The staff advised that 1 cot was used by 2 children at different times. This posed a risk of cross infection as the staff cannot clean the mattress cover in between use. 25. Two fleece blankets were observed in the designated sleep room. In conversation both staff were unaware that children under 1 year old required cellular blankets while sleeping. This was at variance with the safe sleep service policy and posed a potential risk of harm to the preschool children. Fire Safety: 26. Records of monthly fire drills were not available. It was acknowledged that a fire drill was completed on the 25/07/2025. This posed a safety risk to children and staff in the event of an emergency or fire. Action submitted by the Registered Provider On the 23/12/2025 the registered provider advised of the following - General Safety: The designated sleep room was reorganized immediately to reduce potential risks and ensure a safe environment for children. 1. Stackable beds are now placed flat on the floor and safely stacked one on top of another when not in use. 2. The box of blinds was removed following the installation of blinds on the windows. 3. & 4. The electric heater was removed from the childcare service and the staff members personal belongings will be left in their cars or in the locker unit that has been ordered. Management will monitor the organization of the sleep room to ensure ongoing compliance with safety requirements. 5. Temperature control dials were removed from the radiators in the playroom to prevent access by the children. 6. The cable from the electrical printing device was secured correctly to eliminate any risk to the children. 7. The box and lamp were replaced safely. A new storage unit for the children’s belongings has been ordered. Daily safety checks of the sleep room will be carried out to ensure the area remains free from hazards. 8. Staff were informed of the importance of storing soothers in each child’s individual designated container before and after use. Infection Control: 9. Perishable food items such as yoghurt and cheese are now stored in the service refrigerator upon the child’s arrival and provided at the first snack time. 10. The designated sleep room was re organized immediately following inspection to reduce the risks of cross infection. 11. Staff were instructed to remove waste bags that were full immediately and replace them with new bag. 12. Bed sheets are removed daily and stored in a designated storage unit. 13. &14. Bin liners and brown paper were removed from under cots and stored in a designated area. 15. A dedicated storage unit with closed shelving was purchased for children’s bedding, soothers and personal items. 16. & 17. Printers, fan, adult footwear and a child’s car seat were removed from the designated sleep room. 18. The frayed nappy changing mat was replaced with a new one. 19. Cots will be deep cleaned weekly in the service in addition to daily cleaning routines. 20. Used gloves and aprons will be disposed of immediately in the designated waste bin. 21. Staff were informed of the importance of storing soothers in each child’s designated container before after use. Safe Sleep: 22. Room temperature is now checked regularly to ensure it remains within the required range. 23. A new temperature monitoring device was ordered and arrived the day after the inspection. 24. Three waterproof cot mattress covers were purchased to allow effective cleaning between uses. 25. The childrens parents will provide appropriate cellular blankets for the children under 12 months of age and additional blankets were purchased by the service for the designated sleep room. Fire Safety: 26. Fire drill records were identified and relocated to the correct folders to ensure easy access for all staff members. The surface of a circular table in the ECCE 2 outdoor play area was weather beaten and the top had various cracks. In addition, there was no cover protecting the exposed wood on the rim which had areas of exposed wood posing a safety risk should a child come into contact with the splintered wood. 4. An electrical flex from a hanging light in the ECCE 2 care room was accessible to passing children posing a safety risk, when informed a staff member removed the flex straight away - The following statements were received from the registered provider: Corrective and Preventive Action 1. The rug at the exit door to the outdoor play area in the ECCE 2 care room has been replaced and no longer poses a tripping hazard. 2. The glazing company has been out to measure the replacement glass which will be replaced when manufactured. 3. The circular table in the ECCE 2 outdoor play area has been removed eliminating the risk of a child coming into contact with the splintered wood. 4. The electrical flex from a hanging light in the ECCE 2 care room was removed by a staff member when informed on the inspection. The manager explained that the application used by the service automatically stated a temperature and the staff member is required to record the actual temperature when they carry the 10-minute physical checks. There was no thermometer in the sleep room to accurately record the temperature of the sleep room and this posed a risk to the sleeping children . The staff and management are now trained in how to use them and have been trained in how to accurately record the correct temperature. The staff doing the sleep checks on the day of inspection were unable to demonstrate that the temperature of the sleep room was accurately recorded and maintained at a temperature between 16 °C and 20°C. At 12.03 pm on the day of day of inspection, when the staff member recorded the 10-minute sleep check, she was observed taking the temperature of the room by plugging in the electronic thermometer and instantly recording the temperature without first letting the reading stabilise. The thermometer sensor needed time to adjust to the ambient temperature of the room, before an accurate temperature was recorded. This posed a risk for the children sleeping in this cot room. Infection Control: 2. At 13:45 pm on the day of inspection, the nappy disposal bin in the nappy changing area was overflowing with soiled nappies which resulted in a mal odour in this sanitary area and posed a risk of cross infection. An Immediate Action Notice was issued onsite on 19/09/2025 in respect of the following which represented a safety risk to the children: • Children were observed walking around the food table at 11:05 carrying hot bowls of porridge in which the temperature was recorded at 64°C, posing a scalding risk to the children. • Two pots of porridge on the table were accessible to the children in which the temperature of the pots were recorded at 50.3 °C and 45.6°C respectively, posing a scalding risk to the children. • A lit wax candle was observed on the table as a decoration during mealtimes, posing a safety risk. 2. Splintered wood and torn felt were accessible to the children on the roof of the outdoor wooden playhouse, posing a safety risk to the children. Safe Sleep: 3. An Immediate Action Notice was issued onsite on 19/09/2025 in respect of the following which represented a safety risk to the children: • Sleep facilities were observed to be inadequate. A child who was attempting to rest/sleep in the care room on a waterproof mattress within an unstable, wooden 3-sided frame known in the service as a wooden gate, which was held together with wool and black webbing. The spaces between the wooden horizontal bars were measured between 20.5 and 23 centimetres and a cotton sheet was placed above the structure posing a safety risk should it fall on the child. • A second child was observed resting intermittently on a cover which was placed on the floor of the care room, posing a safety risk should a child fall over them while moving around the care room. • There was no evidence of documented physical sleep checks carried out every 10 minutes which was contrary to the service’s safe sleep policy, posing a safety risk to the children. In conversation, an adult stated that none were available or in use. • Ambient lighting was not provided in the care room while children were resting/sleeping. The lighting in the care room was not reduced and there were no blinds available to limit the amount of light for children wishing to rest/sleep. • While 2 children were attempting to rest/sleep the care room was noisy which was not conducive for restful sleep. The inspector observed children interacting regularly with the 2 resting children between 12:15 and 12:45. Administration of Medication: 4. There was no temperature reducing medication available in the service, posing a safety risk to the children in the event of an emergency. Infection Control: 5. Children were observed in the outdoor area washing their hands in a communal bowl which contained used cold water, posing a risk of cross infection. This was documented on the last inspection dated 01/12/ 2023. 6. Perishable food was not stored in a fridge. Children signed in at 09:00 were observed leaving their snacks, including perishable food, on a table in the care room. The inspector observed that the children did not collect these snacks until 13:00, posing a risk of the perishable food spoiling. Action submitted by the Registered Provider - The following statements were received from the registered provider: General Safety: 1. All of the safety issues identified on the Immediate Action Notice (IAN) have been addressed to ensure the safety of the children attending the service; • No hot liquid food will be served by children to minimise risk of spillage. The adults will serve the porridge once it has cooled down. • The porridge pots are now covered with heat resistant covers and placed out of reach of the children. • The lit wax candle, in use at mealtimes and only lit when all of the children are seated, is now fully encased in a lantern and placed out of reach of the children. • Staff received training on the new benefits risk assessment and a risk assessment and analysis to ensure the safety of the children. The children have received information on fire safety. 2. The roof of the outdoor playhouse has been replaced. The materials used will ensure the longevity of the playhouse. Safe Sleep: 3. All of the safety issues identified on the Immediate Action Notice (IAN) have been addressed to ensure the safety of the children attending the service in relation to safe sleep. The wooden gates have been removed from the service, the room has been reorganised and a designated sleep area has been arranged in a quiet area with 2 waterproof sleep mats available. A 10-minute physical sleep record is now available for recording the position, colour, breathing of each child and the room temperature. Administration of Medication: 4. Temperature reducing medication has been purchased and is available in the event of an emergency in the service. It is stored in a locked cabinet out of reach of the children. The details of the temperature reducing medication including the expiry date have been added to the first aid checklist and will be checked regularly. Infection Control: 5. The communal bowl at the outside water station has been removed to prevent the risk of cross infection. 6. Perishable food is removed from the children’s lunch boxes and stored in the fridge 18 September 2025 ##### [Roscam Childcare Centre](/creche/roscam-childcare-centre-roscam.md), Roscam · Regulation 23 — Safeguarding health, safety and welfare of child - Infection Control: 1. The nappy disposal bin in use in the nappy changing area located between the Cubs Playroom and the Montessori room was not adequate or suitable. It was not sealable, airtight or foot pedal controlled. The bin present in this sanitary area was a flip top bin. 2. There was no hand drying paper available in 3 of the children’s sanitary areas. The paper holders were observed to be empty. The staff and children were observed to be using a large tissue roll, which was not suitable to effectively dry hands after hand washing. Safe Sleep: 3. At 13.20 on the day of inspection, the temperature of the sleep room where the Toddlers slept was recorded by the inspection team to be 20.7°C and at 14.01 pm it was 21°C. The temperatures recorded by the staff members was not accurate. The sleep checks recorded on the electronic devices stated that the temperature was between 16 and 20°C. This is in variance with what was stated in the safe sleep policy, where it clearly stated that the temperature of the sleep room for children attending under 2 years of age will be maintained at a temperature of between 16 and 20 °C. Action submitted by the Registered Provider The registered provider stated the following in their reply: - Infection Control: 1.The nappy bin located between the Montessori and Cubs rooms has been replaced with a new model that is sealable, airtight and fitted with a foot pedal, ensuring compliance with hygiene and safety standards. 2. Blue roll dispensers have been installed in all 3 bathrooms/nappy changing rooms used by the children. Safe Sleep: 3.A wall-mounted thermometer has been installed in the sleep room (battery operated) and a new policy has been implemented to ensure that the sleep room is kept at the correct temperature (16 to 20°C). Management and will ensure that the room is well ventilated and aired before and during sleep time. Contrary to the service’s infection control policy which stated that “all equipment should be nearby” a staff member stated that aprons are brought from the Toddler room in the event a child requires changing. 5. Due to a poorly positioned safety catch cleaning agents under the counter in the Playschool were accessible to the children, posing a safety risk. 6. The inspector observed that there was a shape sorter with a wide crack accessible to children in the Wobbler care room, posing a pinching risk should a child catch their finger on it. 7. Talcum powder, nappy sacks and individual children’s creams were observed to be within reach of children during nappy changing in the Wobbler changing area, posing a safety risk. 8. There was a crack on the top of the blue slide in the outside area, posing a risk to the children from the sharp edges. 9. There was no handrail offering support and safety to children climbing the gradient to the outdoor play areas at the rear of the service, posing a safety risk. 10. Hazards were observed on the outdoor walls of the building within the courtyard which were accessible to the children. There was a white cable hanging in the walkway from the Playroom to the outdoor area, a piece of steel protruding from the wall alongside the cable and a cable coming from the window outside the yellow entrance door which was dangling down from the adjacent drain pipe, posing a safety risk to children moving around in the outdoor play areas. Infection Control: 11. While there was an up-to-date cleaning checklist furnished to the inspector for the toys in the Wobbler care room the inspector observed a soiled wire bead maze which staff stated had been brought in from the outdoor area in error, posing a risk of infection. 12. A staff member in the Wobbler care room was observed, on two occasions, lifting the lid of the nappy changing bin with their hands to dispose of used nappies, posing a risk of cross infection. In conversation, the staff member stated the lid did not always lift when operated by the pedal. 13. There was no steriliser in use in the Wobbler or Toddler care rooms for soothers observed in use throughout the day. While staff stated a sterilisation system was available and the infection control policy stated “soothers will be washed with hot water and placed in a microwavable steriliser” none was observed in use for soothers or mouthed toys. 14. Contrary to the service’s infection control policy which stated that all equipment should be at hand there were no aprons available in the nappy changing area of the Playschool sanitary area. In conversation, staff stated that if aprons are required they are sourced from the Toddler care room. 15. The inspector observed a swing bin in use in the ECCE lobby area which contained a used nappy, posing a potential risk of infection to the children. Safe Sleep: 16. Documented 10-minute physical sleep checks in the Toddler room reviewed by the inspector were not accurately recorded for each child. The number of recordings for room temperatures, breathing patterns and skin colours did not reflect the number of times the sleep checks were being carried out for individual children. Examples included 3-time checks for an individual child on the day of inspection carried out at 12:30, 12:40 and 12:50 however there were 4 temperature recordings, 4 breathing patterns and 4 skin colour changes recorded. On the previous day there were 12-time checks recorded for an individual child with 10 room temperatures, 11 breathing patterns and 12 skin colours recorded. Not keeping accurate sleeping records could potentially compromise the safety of the sleeping children. 17. The inspector observed a cot placed under a window in the Wobbler designated sleep room, posing a safety risk to the children. 18. Stackable beds in use in the Toddler care room after lunch were not placed 50 centimetres apart, posing a potential safety risk to children sleeping and resting. Action submitted by the Registered Provider - General Safety: 1. Renewed Garda vetting has not been reapplied for the adult as they are not currently working in the service. Management will ensure in future that all garda vetting is renewed in a timely manner. 2. The bait box has been removed to ensure the safety of the children in the Playschool care room. The registered provider will ensure that there are no bait boxes accessible to the children in the service. 3. The adult sized dolls houses in the ECCE and Playschool care rooms have been anchored to prevent them falling or tipping over. All heavy equipment or free-standing furniture will be secured or removed from the care rooms. 4. A new apron dispenser with aprons has been installed along with a pedal operated, lined, sealed and lidded nappy bin in the Playschool sanitary area. 5. A new shelf has been purchased and placed on the wall of the Playschool care room, out of reach of the children. In future all cleaning agents will be stored on a high shelf, inaccessible by the children. 6. The shape sorter with a wide crack accessible to children in the Wobbler care room was removed on the day of inspection. All toys will be risk assessed and removed if broken. 7. Talcum powder, nappy sacks and individual children’s creams in the Toddler changing area have been removed and placed in drawers, out of the reach of children. 8. The blue slide has been removed from the outdoor play area and a replacement has been ordered. A daily risk assessment will be carried on the outdoor play area in the future. 9. A new handrail has been installed offering support and safety to children climbing the gradient to the outdoor play areas at the rear of the service. 10. The hazards on the outdoor walls of the building within the courtyard have all been removed. The outdoor area will risk assessed on a daily basis to ensure the safety of the children playing in the area. Infection Control: 11. The wire bead maze was removed on the day of inspection. Daily risk assessments will be carried out to ensure that all of the toys and equipment are clean in the Toddler care room. 12. A new nappy changing bin which is pedal operated, lined, lidded and sealed is in place in the Toddler nappy changing area to prevent the spread of cross infection. 13. Microwave sterilisers are now available for the Wobbler and Toddler care rooms to prevent cross infection. Staff will ensure that soothers are sterilised throughout the day and as required. 14. A new apron dispenser and a supply of single use aprons have been installed in the nappy changing area of the Playschool sanitary area. Management will ensure that there are an adequate supply of aprons and gloves in each nappy changing area. 15. A new pedal operated, lidded, lined and sealed nappy changing bin is now in use in the ECCE sanitary area, helping to reduce the risk of cross infection. Safe Sleep: 16. New sleep record books are now in use for each child. The registered provider will ensure that all required elements are completed for each of the 10-minute physical sleep checks recorded for each child to ensure the safety of the children while sleeping. 17. The cot has been removed from under the window. Management will ensure that no cots are placed under a window. 18. Staff have removed large items of toys to ensure that there is sufficient room for the stackable beds. The inspector entered the sleep room at 13:25 and examined the sleep records of 3 of the 8 sleeping children. One of the children’s sleep checks had last been recorded at 12:55, 30 minutes previously, while the 2 other children’s records indicated their sleep checks were recorded at 13:00, 25 minutes previously. The inspector observed that there was no pedal operated, sealed, lined and lidded nappy changing bin available. A staff member was observed bringing a used nappy in a sack through both interconnecting rooms to dispose of the nappy in an outdoor bin. In discussion, the staff member stated that this was normal practice, posing a risk of cross infection. 7. In contrast to the service’s nappy changing procedure a staff member was observed carrying out two nappy changes at 09.27 and 11.20 without the use of disposable aprons, posing a risk of cross infection. Administration of Medication: 8. There was no temperature reducing medication available in the service, posing a safety risk to a child in the event of an emergency. The main entrance gate to the service was accessible as the hook and eye device was placed on the wrong side of the fencing. This posed a safety risk to the children as unauthorised adults could access the catch and enter the outdoor play area. In conversation, the adults in the service were unaware of the issue. 2. The main entrance door could not be appropriately secured from the inside as children had access to the push bar which was located within reach. There was no means available of securing the door, from the inside, should an unsupervised child push the bar which led to the outdoor play area. 3. Electrical flexes from the microwave, extension cable in the children’s toilets and kettle were accessible to the children, posing a safety risk. 4. Children entering the care room from the sanitary area, in a large group, were observed climbing over school bags in the care room which posed a safety risk from the children tripping or falling over them. 5. A large garden rake, wooden pole and adult brush were accessible to the children in outdoor play area, posing a safety risk. 6. Webbing in place, under the bark chippings, in the outdoor play area was noted to be torn posing a potential tripping hazard in the outdoor play area. 7. Electrical equipment including a mini fridge and microwave was located on the floor of the rest area, accessible to the children, posing a safety risk. 8. The edges of a black mat, at the entrance door of the service, were curled on all sides posing a safety risk should a child trip or fall. Infection Control: 9. Although there were four wash hand basins and three toilets allocated for sole use by the early years children, these were shared with a separate group of ten children from the primary school who were temporarily using the facilities thus posing a risk of cross infection. 10. Two children’s toilet seat pads and one potty were observed left lying on the floor of the toilet cubicles, posing a potential risk of infection. 11. There was no pedal operated, sealed, lined and lidded bin in place for used nappies in the children’s sanitary area. Staff stated that the used nappies are sealed in a plastic bag and placed in a bin for used paper towels. 12. There were no disposable aprons available for nappy changing. Staff were observed putting disposable gloves on in the sanitary area and when the inspector asked where the aprons were a staff member explained that they did not use aprons for nappy changing, posing a potential risk of cross infection. 13. Perishable food was not stored in the service’s fridge contrary to the healthy eating policy. Children were observed removing perishable food from their lunch bags, posing a risk of the food spoiling. 14. At variance with the infection control policy which stated “a wash hand basin is provided in each room” there was no wash hand basin available in the care room to wash adult hands or the used children’s plates after morning snack at 10:45. In conversation, staff stated that they wash the plates in the wash hand basins of the children’s sanitary area, posing a risk of cross infection. 15. Children putting rubbish into the overfilled pedal bin in the care room were observed coming into contact with used tissues, posing a risk of cross infection. In conversation, an adult in the care room stated that the bin was emptied every two to three days. Administration of Medication: 16. There was no temperature reducing medication available in the event of an emergency, posing a safety risk to the children. Fire Safety: 17. There was no fire assembly sign visible in the outdoor area to display the predesignated assembly point which would lead the staff and children to safety in the event of an emergency. Action submitted by the Registered Provider - General Safety: 1. A provisional date of the Easter holidays 2025 has been proposed by the school caretaker to increase the height of the fencing and to reposition the hook and eye device. These actions will prevent unauthorised adults from entering the service and prevent children from leaving the service unsupervised. 2. A safety device has been fitted to the main entrance door to alert staff in the event the bar on the door is pushed open by a child. 3. The cables have been removed from the area to prevent an accident from occurring. 4. Staff ensure the children’s bags are kept on the allocated shelving. The area is checked regularly to make certain that the walkway is free of obstruction. 5. The garden equipment has been removed and stored securely. 6. The torn webbing has been replaced. 7. The mini-fridge and the microwave have been removed from the area. 8. The mat has been replaced. The new mat is colourful and decorated with numbers and letters. Infection Control: 9. There are now designated sanitary facilities for each group reducing the risk of cross infection. 10. The potty and seat pads are sanitised and stored on nearby shelving when not in use, away from the floor area. 11. A new sealed, pedal operated and lined nappy bin has been purchased and placed in the sanitary area. Staff will ensure the bin is emptied daily. 12. Single use disposable aprons are available in the nappy changing area. 13. A new fridge is now available in the care room for storing perishable food to prevent it from spoiling. 14. The plates will be washed in the school kitchen at a time designated by the school principal. 15. Staff ensure the bins are emptied daily, or as required, to prevent the risk of cross infection. On the day of the incident children in the baby room had access to a bottle warmer filled with hot water which posed a safety risk. This piece of equipment caused a serious incident and it acknowledged the service notified the TUSLA Inspectorate of same. 2. A window in a care room was easily accessible to children by chairs that were stacked and could pose a risk to children climbing up and exiting or falling. On 20/12/2024, photographic evidence was submitted to the inspectorate of a latch that had been installed on the window. 3. In the baby room there was an iPad and a charging cable within reach of the children within the room. This posed a safety risk to the children within the room. The iPad and charging cable were removed from the care room during the inspection following feedback from the inspectors. 4. Powdered infant formula for one child in the baby room, was not prepared in accordance with the Food Safety Authority of Ireland (FSAI) best practice guidance, which advises that powdered infant formula should be made up at home. Staff advised that they made up bottles on site, as per parents’ preference. Safe Sleep: 1. A child aged less than two years of age was sleeping on a stackable floor bed in the toddler room. As per the Guidance for the Early Learning and Care sector on sleep provision for children under 24 months document stackable beds for children under 24 months are not permitted for children under 24 months as they are not sufficiently comfortable/conducive to sleep. Action submitted by the Registered Provider - General Safety 1.The bottle maker has been removed from the service. 2.A safety latch was put on the window the next day - picture provided to the inspector. 3. Ipads only to be charged at night time in the office. There has been a high shelf put in the care room to place the Ipad out of the children's reach. 4. The air vents in the nappy changing area and the room that was used as a rest area were noted to have a buildup of dust and particles and posed a risk of infection. The water temperature was recorded at 17.1 ° C, this was a previous non-compliance on the 02/06/2022. A plumber was sourced and repairs were carried out. An immediate action notice was issued on the 1st of March 2024, as the drive through main entrance double gates, off the main road and the walk-through gate into the social services premises / car park remained open throughout the inspection and while the children went to and from the outdoor play area. Infection Control: 2. On the day of inspection, the children did not wash their hands after outdoor play and prior to their going home, posing a cross infection risk. This was previously noted on the inspection dated 26th October 2022. 3. There was no designated nappy-changing unit, and designated wash-hand basin to enable a child to have their nappy changed. A nappy-changing unit and mat must be smooth and easy to clean and available for the children attending the service, wearing nappies. 4. The blue rolls and disposable aprons were loosely stored on the windowsills or on top of the cisterns in the children’s sanitary areas, while dispensers were required to prevent cross contamination. 5. There was evidence of dust and debris on the mechanical ventilation covers on the walls of the children’s sanitary areas, that required cleaning. The mechanical ventilation covers need to be included in the service’s cleaning checklist / schedule. 6. The service cleaning schedule showed only the initials of the staff member, or ticks and no staff signature. There was no evidence of the cleaning procedure process in line with public health guidance. Action submitted by the Registered Provider On 4th of March, 11th of April, and the 22nd of May 2024 the registered provider advised of the following; - General Safety: 1. Evidence that a system was in place to ensure staff close and secure the outdoor gates when children were accessing the outdoor play area was submitted. The double gates will be closed each time the children are coming to or going from the outdoor play area to the pre-school. This practice will be implemented at all times when the children are moving between the outdoor area and pre-school. Infection Control: 2. The children will wash their hands after outdoor play before they go home each day. The staff have been advised to ensure children wash their hands, after outdoor play. 3. A nappy-changing record will be kept of each child when their nappy has been changed. A nappy changing unit has been purchased. 4. Dispensers for the storage of disposable aprons, gloves and hand towels have been installed. 5. The mechanical ventilation units will be included in the cleaning schedule. 6. The cleaning schedule was upgraded to show staff names and details of the areas cleaned. Powdered infant formula for the young children in the Nursery room was prepared on the premises by the staff and not prepared in accordance with the Food Safety Authority best practice guidance. 2. There was a number of areas in the creche that had areas of peeling paint or sections of interior window ledges that were void of paint, which made it difficult to effective clean these areas. 3. The sleep room was used as a storage area for buggies , which may pose a risk of cross infection to the sleeping children. Action submitted by the Registered Provider - Infection Control: 1. Parents were informed that bottles must be made prior to creche in accordance with the Food Safety Authority best practice guidance, all parents were understanding of this and it was implemented immediately. 2. Areas of the creche that required paint have been repainted and these areas will be monitored by management to ensure that this does not occur again. 3. Buggies have been removed from the sleep and are stored in an appropriate manner.
