- General Safety: 1. Garda vetting disclosures had been obtained for 24 adults. However, the service did not adhere to the re- vetting timeframes for two adults as outlined in the Early Years Inspectorate Regulatory Notice, requiring services to renew Garda vetting every three years 2. The person in charges signature was missing from 5 out of 10 incident and accident forms reviewed in the Giraffe room. The person in charges signatures was also missing from 2 out of 10 incident and accident forms reviewed in the bizzy bee’s room. Infection Control: 3. The foot operated bins in the Giraffe room and sanitary area were broken and needed to be replaced; children were seen lifting lids of bins with soiled paper towel disposal. 4. Children's hands were not observed to be washed after eating or before going down for a sleep, which creates a risk of cross-contamination

- Corrective & Preventive Action General Safety: 1. Garda vetting was submitted to the inspectorate in relation to two adults. 2. All forms have now been signed by the person in charge. All staff have been made aware that it is their responsibility to get the person in charge to sign accident forms on the day they happen and before being presented to parent/s to sign. Management will check accident books regular to ensure they are being signed. Infection Control: 3. The bin in the Giraffe room has been replaced with a new foot operated bin. The bin in the EECE building has also been replaced. Management will do weekly checks of all foot operated bins in the rooms to ensure they are fit for purpose. Staff have been asked to report all broken equipment to management. Management will ensure all staff are made aware of the nappy changing policy and infection control policy. Policies have been handed out to all staff. 4. An Infection control policy and nappy changing policy was distributed to all staff members. Management will do regular checks to ensure handwashing is preformed after eating, and before going to sleep. Management will ensure all staff are made aware of the nappy changing policy and infection control policy. Policies have been circulated to all staff

- General Safety: 1. On the day an external door leading to two of the care rooms was unsecure which posed a risk of unauthorised access to the service. 2. On the day of inspection, a suction-based anti-choking/airway clearance device was observed in the main hall, positioned directly beside the primary first aid box. These devices are not permitted to be used in preschool services. It is acknowledged that once brought to the attention of the person in charge it was removed. Infection Control: 3. There were two broken foot operated bins in the Little Smarties room, one in the sanitary room and one in the nappy changing room. This could pose a risk of infection control for preschool children

- Corrective & Preventive Action General Safety: 1. The keypad on the door was reset and fixed that same evening. Weekly checks on all doors are now completed by our maintenance 2. The device was removed immediately. All First aid items are checked monthly, updates from TUSLA on “non permitted” items will also be checked during these checks to ensure future compliance Infection Control: 3. - General Safety: 1. Two floor mats in the service appeared worn, frayed and dirty posing a trip hazard to preschool children. 2. A fire door was held open with a door stop in the sanitary area in the junior room posing a risk to preschool children in the event of a fire. 3. A sharp kitchen utensil was found in a drawer in the senior preschool room posing a risk to the preschool children. Infection Control: 4. Foot-operated waste bins were not in operation throughout the service on the day of inspection. Bins observed in the preschool rooms and handwashing areas required manual handling to open, which failed to prevent the cross-contamination of hands and posed an avoidable infection control risk to the children attending the service. 5. Hand hygiene practices were inadequate in the senior preschool room. Inspectors observed three separate occasions where children did not wash their hands after using the toilet. Staff failed to prompt or assist the children

- Corrective & Preventive Action General Safety: 1.Floor mat have been removed and replaced 2. Door stopper has been removed and staff made aware door has to be closed at all times. 3. Kitchen knife has been removed and staff informed to keep out of room. We have had a staff meeting and discussed the non-compliant areas, we have put a plan in place going forward on staff to inform management of any mats etc that need replacing, door stoppers removed and no sharp knifes allowed in any rooms. Infection Control: 4. . We have replaced all bins with new foot pedal bins. 5. We have updated our hand hygiene policy as well as updating the staff on the importance of role modelling and observing hand always washing. In every hand-washing area we have put a visual aid to support children during hand washing. New policy update and staff made aware of the changes going forward

- (1) On the day of inspection, after nappy changing, the hands of the babies were observed to be not consistently washed, posing a risk of infection. Gloves and aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care activity of placing on a clean nappy, followed with redressing the child. This could lead to a risk of cross infection

- A staff meeting was held, and a review of the policies and procedures was undertaken with each staff member. Following a revise of the policies and procedures we held a demonstration in which all staff participated in a trail run of nappy changing using the corrective measures we have since put in place, which are as follows. Staff must start by ensuring they themselves have washed their hands thoroughly with soap and water, this must be done before putting on the correct PPE. Once gloves and aprons have been removed after each child they must be placed in the correct designated bin. Staff must change their gloves and apron during the care routine in order to prevent cross contamination. Each child must also be promoted to practice good hand hygiene by washing hands after the care routine is finished. To prevent the non-compliance from reoccurring in the future we will ensure to gain feedback from staff and ensure that all staff are aware of the changes that have been made. Ensure that all staff are aware of the importance of infection control and good hand hygrine. In line with the above revision that was taken with the policies and procedures we have moved PPE closer to where the staff member is undertaking the care activity. This allows the staff to change their PPE while also keeping in line with child safety

There were no paper towels for effective hand hygiene in an additional care room being used by the service. 2. There was no foot operated bin in an additional care room that the service. Safe Sleep: 3. Two children were observed to take their milk bottles to bed and lie down on stackable beds to have their bottle before going to sleep. This is against safe sleep practises and poses a choking risk to children. It is acknowledged that the service's safe sleep policy outlines “no bottles will be permitted in cots, unless permitted by parent and disclaimer is signed” However, the disclaimers reviewed did not detail that bottles of milk were permitted while children lay to sleep. Outing: 4. The service had recently taken part in an outing to a farm; however, there were no risk assessment records available. This is not in line with the service policy, which details that “risk assessments of outings/and or travel” is required. It is acknowledged that a template named “preschool farm trip risk assessment” was available, although it was not completed for the outing in line with the services policy requirements

- Corrective & Preventive Action Infection Control: 1 & 2. Additional room was put out of use until all stocks were replenished. 1 & 2 Risk assessment and checklist complied to ensure sufficient stocks available in adjacent room Safe Sleep: 3. Parents were informed that children will no longer be allowed to take milk bottles to bed. Three children now receive their bottles before going to bed Outing: 4. - General Safety: 1. The iron was removed immediately during the inspection and placed in an area inaccessible to children. The nappy changing area was checked to ensure there were no further hazards present. Infection Control: 2&3. The hand-operated bin in the nappy changing area and the swing-top bin in the adult sanitary accommodation have been replaced with foot-operated pedal bins. All waste bins used in nappy changing areas, sanitary areas and hygiene-related areas will be foot-operated pedal bins. - Infection Control: 1. Water in the sinks both inside and outside of the sanitary areas did not have warm water for effective hand hygiene throughout the service. 2. Children's mattresses were being stored in a storeroom when not in use. The mattresses and sheets were observed touching the floor, which may cause cross-contamination 3. On the day of inspection, after nappy changing, the hands of the babies were observed to be not consistently washed, posing a risk of infection. Safe Sleep: 4. On the day of inspection, a child aged 12 months was observed sleeping on her tummy. Additionally, in this position, it was difficult for staff to effectively carry out checks, as they could not observe the child’s chest rising and falling or the colour of the child’s face. 5. The mattress covers appeared too small for the mattress, causing the sides to curl upwards. This may hinder the child's ability to turn if needed. Action submitted by the Registered Provider

- Infection Control: 1. Following the closing meeting with the inspectors the service immediately contacted the contractors to begin addressing issues. The service got a plumber to the service to fix or replace the immersion systems in the rooms affected. 2. The children's mattresses are now stored on top of the cots without the sheets, with just mattress protectors on them. They are being dressed daily before sleep time. 3. Nappy Changing Policy clearly details the required handwashing procedures. To further reinforce compliance, all staff members have signed a memo acknowledging their understanding of and commitment to following this policy. 4. At the time of the inspection, the service contacted parents. When speaking with the parent of a child they expressed that they have no concerns with the child sleeping on her tummy as that is her normal sleeping pattern. The registered provider explained the safe sleep procedures mandated and informed parents that staff will be instructed to turn the child over on her back. The child turned one year old on the day of inspection. 5. Following the inspection management reviewed the mattress covers. It was discovered that the mattress covers were not too small for the mattresses. Instead, they were not put on clearly by staff members. The service has shown each staff member working in the nursery how to put on the mattress covers correctly. Staff have been reminded staff of safe sleep policies and procedures. All staff responsible for the dressing of mattresses have signed a form to say that they will put the mattress covers on correctly. All staff have re-read safe sleep policies and procedures. The registered provider has discussed the importance of safe sleep, lying positions, and how children cannot sleep on their tummies, and how important it is to put mattress covers on correctly

- General Safety: (1) A garda vetting disclosure reviewed did not have the required risk assessment in place to ensure safety of the preschool child. The submission to the National Regulation and Enforcement Panel by the Registered provider outlined measures the registered provider would take to ensure compliance in relation to vetting procedures for adults. This was not observed on the day of inspection and the registered provider failed to provide evidence of same. (2) The water temperature on a staff handwash basin accessible to preschool children in Montessori 2 measured at 51.2°c which is above the safe limits and posed a risk of scalding should a child access it. (3) Staff members in the baby toddler room were observed to use the hot water from the staff sink measuring at 51.9°c to clean children’s hands and face after dinner time, which could pose a risk to the child. (4) A storage shed located in the outdoor play area used to store toys and the service washing machine and dryer was unsecure, if accessed unsupervised this could pose a risk to a child. Infection Control: (5) The five standard cots in the baby/toddler sleep room did not have the required waterproof mattress protectors. Administration of Medication: (6) Medication was not stored in line with the service policy in a locked box in the kitchen. Action submitted by the Registered Provider

- General Safety: In a written response the registered provider stated: 1. Garda vetting disclosure risk assessment has been completed. We will review and will have risk assessment done for all garda vetting disclosure if there is any. The staff file checklist has been updated 2. We have turned off the hot water on the adult sink in the concerned room. This adult sink will be used to clean art paint pots and not for handwashing. 3. The adult sink hot water feature is turned off, the other sink in the classroom is controlled and we will use this for handwashing. 4. Shed has been secured with another latch high enough to reach of kids. All safety measurements are being reviewed in monthly audit checklist. Infection Control: 5. Waterproof mattress compliant with manufacturer guidance have now been ordered for all standard cots in the baby/toddler sleep room. Administration of Medication: 6. The medication cabinet now has a lock

- General Safety: 1. Garda vetting was available for one staff member. However, this vetting disclosure was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. Infection Control: 2. In the sanitary accommodation, swing top bins were in place for the disposal of waste. Foot operated pedal bins are required to reduce cross contamination. Administration of Medication: 3. On discussion with staff, the inspector was informed that one child in the service occasionally required medication. However, while a care plan and medication consent form were available, no records of the details of medication administered in the service were kept. Safe Sleep: 4. Between 13:10 and 13:50, four children were observed sleeping on low level stackable beds. No physical sleep checks were carried out or recorded. Action submitted by the Registered Provider

- General Safety: (1) The plastic cover on a steering wheel in the outdoor play area was cracked and broken posing a risk of injury. (2) The door to the staff room which was used to store medication, staff belongings and hazardous kitchen equipment was unsecured when checked by the inspectors. (3) A single use trampoline was observed in the outdoor area. Trampolines are prohibited for the use in the pew schools. (4) There were no restrictive opening devices on the windows in the little explorer’s room. Infection Control: (5) In the little explorer’s room, children’s handwashing was not carried out as required immediately after snack time. Fire Safety: (6) A cot was positioned in front of the emergency exit route in the sleep room off the little explorer’s sleep room and preventing safe evacuation in the event of a fire. Action submitted by the Registered Provider

- General Safety: 1. The toy Car has been removed from Little Explorer’s playground. If we install a similar car in future, it will be checked for damage regularly. 2. Staff have been advised to keep the staff room door closed. This was addressed at a staff meeting. Child locks have been placed on the cabinets and a fridge lock has been installed in case the staff door is left open again. Staff in Little Explorers will advise if one of the child locks is broken and needs replacing. Spot checks will be carried out to ensure staff door is being kept closed. 3. Trampoline has been removed. Trampolines will no longer be used in the service unless for therapeutic needs, and with a letter stating such from an authorised specialist. Staff have been informed of this in a staff meeting. 4. Restrictive window locks have been installed. Staff in Little Explorers will advise if a window lock needs replacing. Spot checks will be carried out to ensure restrictive locks are being used. Infection Control: 5. All staff were advised about necessary handwashing after meals during a staff meeting. Spot checks will be carried out around snack times. Fire Safety: 6. A cot has been removed and there is nothing in front of the fire exit. We will ensure that a maximum of 11 cots in the Explorers sleep room to ensure that there is no cots blocking the fire exit. Spot checks will be carried out to make sure cots are far enough away from the fire exit

- General Safety: 1. In respect of two staff members, the service did not demonstrate compliance with the Early Years Inspectorate Regulatory Notice requiring services to renew Garda vetting every three years. A period more than 3 years had lapsed since their Garda vetting disclosures were issued. 2. A wooden table in the outdoor area was frayed and damaged with some sharp edges exposed on the structure which posed a risk of injury to a child. Infection Control: 3. In the little lions and terrific tigers’ rooms two, foot operated bins were broken posing a risk of infection. 4. Water temperatures were recorded between 13.6°C to 14.4°C which was below the recommended temperature for effective hand hygiene in all three upstairs rooms, posing a risk of infection. 5. Staff was observed in the downstairs rooms failing to implement appropriate hand hygiene during nappy changes. Staff remained in the same gloves and aprons used for the removal of soiled nappies while transitioning to other care activities. Action submitted by the Registered Provider

- General Safety: (1) Garda vetting had been applied for but had not yet been returned. The service attached copies of both disclosures. The service created a new Garda Vetting Calendar to display to remind themselves to apply in advance of the 3-year renewal period. This will help track all 38 staff members in a timely manner. (2) Wooden table has been disposed of it was immediately removed from the playground the day following the inspection and has since been removed from the premises. Although children were not using the area that the wooden table was in, the service will ensure in future any broken items are disposed of outside of the premises to ensure no risk to any children. Infection Control: (3) Bins were ordered for replacement, and the service are awaiting delivery of bins. Bins have since been placed into each of the rooms. The service have added a check of our bins to current weekly cleaning schedule to remind all staff to inspect bins and ensure the foot pedals and bins are working appropriately. (4) Changed the settings on the timing clock to ensure the hot water setting is on for more hours throughout the day. The service has added water temperature to daily checks. Staff to record the water temperature daily, morning and evening to prevent the temperature dropping to an inappropriate level in the future or to help rectify the problem quickly. (5) Individual staff member spoken to regarding the incident, reminded of the nappy changing policy and given a copy to re-read and refresh memory. Nappy Changing policy handed out to all rooms to refresh everyone’s knowledge. Staff memo sent out to all staff along with nappy changing policy to remind all staff of the appropriate procedure when changing nappies and the importance of infection control

- Safe Sleep: 1. It was observed that children sleeping on floor beds were not supervised in accordance with regulatory requirements. The sleep environment requires continuous in‑room supervision; however, at the time of inspection, no staff member was present in the sleep room while children were sleeping. Sleep was observed by the inspector between the times of 12pm to 12.30pm. 2. It was identified that the supervising staff member did not carry out or record physical sleep checks every 10 minutes, as required. Sleep observations must be completed, documented, and recorded in accordance with the service’s sleep policy. Sleep was observed by the inspector between the times of 12pm to 12.30pm. Action submitted by the Registered Provider

- Safe Sleep: 1. Following the inspection, we reviewed our sleep supervision procedures to ensure full compliance with Regulation 23 Safe Sleep requirements. Immediate action was taken to ensure that children sleeping on floor beds who are under the age of 2 are always supervised by a staff member physically present in the sleep room rather than 10-minute checks that we thought were in line with requirements. All staff have been made aware of proper requirements to ensure safe sleep. 2. Following the inspection, we reviewed sleep monitoring practices in relation to Regulation 23 and the service’s Safe Sleep Policy. Visual sleep checks were being conducted and recorded by staff; however, physical sleep checks every 10 minutes were not consistently carried out in line with regulatory requirements. The matter was addressed immediately with all staff members. Going forward, staff will ensure that physical sleep checks are completed every 10 minutes for all sleeping children and accurately recorded on the sleep monitoring records

- Infection Control: 1. After rolling snack children were not always supported or prompted to wash their hands immediately after finishing their snack, which is a necessary step in preventing the spread of infection 2. Foot-operated bins, which are essential for touch-free waste disposal, were found to be not working in three separate rooms. 3. The bathroom adjacent to ‘dazzling lights’ had two sinks with no access to hot water for effective hand hygiene as the hot water tap was not working on the day of inspection. Action submitted by the Registered Provider

- Infection Control: 1. Staff in the rooms have been altered of this and are now keeping a closer eye on children after they are finished snack and encouraging them to wash their hands after eating. Same as above- to guide children to ensure the step of washing hands after snack 2. The mechanism at back of bin had clicked out- needed to be clicked back in again to allow the pedal to work. To include these in our daily checklist- that if the mechanism comes loose to ensure to correct and fix it back on 3. Both rooms tap have been fixed and the flow of hot water is now working. - General Safety: (1) An Immediate Action Notice (IAN) was issued to the registered provider on 11 March 2026 in relation to regulation 23 General Safety in respect of unsecured looped cable at child’s height accessible in the outdoor area. A written response was received on the 12 March 2026 to the IAN stating that the area had been secured. This was deemed adequate by the inspectors. (2) An Immediate Action Notice (IAN) was issued to the registered provider on 11 March 2026 in relation to regulation 23 General Safety in respect of two unsecured blind chords accessible to preschool children in the toddler room. A written response was received on 12 March 2026 stating that the blind chords had been secured. (3) An Immediate Action Notice (IAN) was issued to the registered provider on 11 March 2026 in relation to regulation 23 General Safety in respect of water temperatures in the children’s hand wash sinks exceeding 50°C posing a risk of scalding to the preschool child. A written response received on 12 March 2026 stating that a thermostatically controlled water system had been installed and safety issue had been resolved (4) A large pane of glass was stored in the corner of the toddler outdoor play area posed a risk of injury to the preschool children. (5) Unsecure wires accessible to the preschool children from the baby monitor were observed in the sleep over room. (6) The service used shaving foam for sensory activities this was stored in an open cupboard accessible to the pre- school children in the Aragon room. Shaving foam cannot be used in preschool services as it poses a risk to the preschool child if ingested. (7) There was a number of exposed pipes in the outdoor play area which posed a trip hazard to the preschool children. (8) A cot in the baby sleep room had sharp exposed edges or points, which may cause injury to the child. Infection Control: Inspectors observed a number of practices which posed a risk of transmission of infection within the service. (9) There was no waterproof covering on four cot mattresses, therefore the cot mattress could not be cleaned efficiently after using posing a risk of contamination. (10) One mattress in the baby sleep room was heavily soiled with a black substance, posing a risk to the preschool child. (11) Nappy changing units and mats were observed to require thorough cleaning as evident from a build-up of dust and dirt in the corners. In respect of 2 staff members, the service did not demonstrate compliance with the Early Years Inspectorate Regulatory Notice requiring services to renew Garda vetting every three years. A period more than 3 years had lapsed since their Garda vetting disclosures were issued. 2. Upon the inspector's arrival to the service, the side door into the service was observed to be unlocked, which does not ensure that no unauthorised person could enter the premises without the approval or notification to staff. It is acknowledged that the person in charge advised the inspectors that they used to have a lock in place on that door, which was currently not operating. 3. The Junior Preschool (1) and Baby/Toddler room door, accessible from their own outdoor areas were observed unsecured during the inspection. The inspectors acknowledge that due to the supervision levels of staff on the day of inspection, a child may not leave the room unsupervised or unknown to staff through the placement of wooden gates and the height of the door handle; however, external access by an unauthorised person may be possible via the outdoor area and gaining access directly into these care rooms. Safe Sleep: 4. Two children aged 16 months and 20 months were placed to sleep on a stackable bed. The stackable bed was not suitable for a child aged under 24 months, in line with Tusla’s Early Learning and Care sleep provision guidance for children under 24 months, which requires a floor bed to be fitted with a mattress of at least 6cm in thickness. 5. Sleep plans were not in place for the two children aged under 24 months who were not sleeping in cots. This is at variance with the guidelines that state a child can sleep in an approved alternative to a cot, provided it is agreed with parents, and a sleep plan is established and signed. 6. One child in the baby/toddler room was observed to sleep in a highchair from 12.48pm to 13.14pm. Highchairs are for mealtimes and are not considered safe and/or suitable as a sleeping environment for infants and children. If a child falls asleep in such an item, they should be removed and placed in a suitable sleep facility such as a cot or floor bed. There was one cot available throughout the time this child was asleep in the highchair. Infection Control: 7. Ineffective hand-washing practices were observed in the baby/toddler room on the day of inspection. Not all children were not observed to have their hands washed before snack time at 1pm. One child was observed to dry hands on a towel that was on a nearby radiator at 1.15pm after a nappy change. Another child was observed to not wash their hands after nappy change at 1.18pm. 8. The foot-pedal operated waste bin in the baby/toddler was broken. Staff and children were seen throughout the day using the bin, lifting the lid by hand. This was a potential infection control issue. - General Safety: General Safety: (1) The two staff members were absent for a long period of time: one for maternity and parental leave, the other due to a prolonged illness. Their renewal application was being processed but had not returned by the day of inspection. One staff members has been received but one staff members has still not arrived. The service has included the email stating that they have received the application for renewal. One staff is currently off sick, so the service hopes to have received her garda vetting (renewal) before their return. Moving forward the service will ensure that all staff garda vetting is renewed before the 3-year period is up. The service will also ensure that staff on maternity or on sick leave has their application renewed as well. (2) All doors have been locked. Side entrance door is locked at all times. Staff have been informed to keep doors locked at all times when children are present in the rooms (3) The doors are also locked. These doors remain locked at all times, except when they are opened to allow children, under staff supervision, to play outside. This procedure permits children to access restroom facilities and transition comfortably between indoor and outdoor environments. A staff meeting was organised to discuss Tusla report and this was highlighted to all staff. Safe Sleep: (4) Staff in the Baby/Toddler room have been made aware that no children under 24 months should be sleeping on stackable beds. This was also highlighted at staff meeting. A new safe sleeping policy was drawn up referencing guidelines. All new staff will also be informed of this and training provided. (5) Safe sleep plans have been drawn up and discussed with each parent. These are signed and stored on file as well as reviewed regularly as children get older and their sleep patterns change. This was also highlighted at staff meeting. A new safe sleeping policy was drawn up referencing guidelines. All new staff will also be informed of this and training provided. (6) This is not normal practice to let a child sleep in a highchair. Staff were made aware that no child should be let sleep in a pram or highchair. At a staff meeting, the service thoroughly discussed the Safe Sleep policy and emphasized the importance of safety, comfort, and regular monitoring of the children while they sleep to all employees. This will be checked by the manager on a daily basis. Infection Control: (7) Hand washing was discussed at the meeting, and it was conveyed to all staff the importance of the children washing their hands regularly. A poster was displayed on the wall in the playrooms and above the sinks in the toilets. Staff were also reminded about washing their own hands and adhering to good hygiene and infection control measures. The preschool rooms did activities, listened to stories and learned songs about washing their hands and good hygiene practices. All staff have been informed of the importance of hand washing. The service have put posters up on the walls and incorporated this topic into the curriculum. (8) The foot bin in the Baby/Toddler room was fixed. The hook to operate the pedal kept popping out at the back of the bin. This has now been secured with a washer and bolt. All equipment will undergo closer inspection and monitoring. Staff have received a maintenance checklist for each room. Safety checks are conducted in every room, and a messaging text group has been created with the maintenance personnel to ensure all tasks are completed promptly and efficiently

The windows on the ground floor of the service had no restrictors in place posing a risk of safety to the preschool children. 3. In the toddler sleep room, a nail had become loose from a fixture and was protruding from the wall. A cot was placed beside this wall which could potentially be accessed by the toddler. This was brought to the attention of the registered provider who ensured this cot was not in use until the matter was rectified. 4. A child was placed in a highchair which had no safety harness in place, posing a risk of safety to the preschool child. 5. Shaving foam was stored in the care room. Shaving foam is not permitted in early years services as it could be ingested by children. It is acknowledged that the registered provider stated it was in use for arts and crafts. Infection Control: 6. The nappy bin did not have the required working foot pedal, which resulted in the adults using their hands to dispose of nappies resulting in a risk of cross infection. 7. Water temperatures were recorded at 12.9°c which is below the recommended temperature for effective hand hygiene, posing a risk of infection. This non-compliance was identified on the previous inspection on the 20 April 2025 where measures taken by the registered provider failed to prevent a reoccurrence. 8. Soft furnishings in the outdoor shelters were worn and visibly dirty, posing an infection control risk. Safe Sleep: 9. Four cots in a sleep room where not the recommended 50cms apart. This could lead to infection and control risks for the preschool children. Action submitted by the Registered Provider

The pipes, situated at a low level, feature sharp metal components could pose a risk of injury. 5) During the inspection, it was observed that a radiator cover in the main hall was damaged, with several slats broken and splintered. - General Safety: 1. We have put a safety lock on the cabinet doors that store the cleaning chemicals. All staff have been informed on the safe storage of cleaning materials and the importance of ensuring the safety lock is used at all times to store the chemicals safely, out of reach of children. 2. I had a meeting with our pest control company; they removed any bait boxes that could potentially be reached by the pre-school children. Staff will check that all bait boxes are kept out of reach of children at the start of each session. 3. Coverings on tables in the outdoor area have been replaced. The manager will report any wear and tear to materials in the outdoor environment and ensure they are replaced in a timely manner. 4. In respect of 3 out of 6 staff members, the service did not demonstrate compliance with the Early Years Inspectorate Regulatory Notice requiring services to renew Garda vetting every three years, as a period in excess of 3 years had lapsed since their Garda vetting disclosures were issued. Infection Control: 2. The bin used for disposing of soiled nappies was an open bin, and not a sealed, lidded pedal-operated bin as required, which poses a risk of cross-contamination. 3. The handwashing facilities at the children’s sanitary accommodation were not in full working order. One tap was not working, and the second tap ran at a slow trickle. The flow of water was insufficient at times, which had an impact on the temperature, increasing the risk of ineffective infection control measures. 4. The children's snack items taken in from home, which were observed to contain perishable items such as yogurts and cheeses, were stored in their bags in the outdoor area on the day of inspection. This was a non- compliance upon the service's most recent inspection on 07 April 2022, where the registered provider's corrective and preventive action response was “perishable foods are now being stored in the fridge daily”. - General Safety: 1. An immediate action notice was issued on the day of inspection due to the door of the kitchen, located in the senior room being open because of a broken lock. This resulted in a risk of children accessing the area where cleaning products and a kettle were located. When brought to the attention of staff the door was immediately blocked off as an interim measure. A new lock was fitted to the kitchen door, and evidence submitted to the inspector on the 24 November 2025. 2. An immediate action was issued on the day of inspection, the hot water for handwashing in the junior room was found to be more than 60°C presenting a risk of scalding. When brought to the attention of staff, handwashing was strictly supervised by using the colder option of the mixer tap present. It is acknowledged that maintenance was subsequently completed to reduce the temperature to 42°C and confirmation of this was sent to the inspector on the 24 November 2025. 3. The door to an unused care room located off the senior room sanitary facilities was found unlocked with decorators’ paint accessible inside. This presented as a risk as there was potential for a child to gain unsupervised access to the room. It is acknowledged a divider was present and placed across the doorway as a barrier, however this did not reduce the risk fully. 4. A blind cord was observed to be unsecured in the senior room presenting as a potential hazard. When brought to the attention of staff the cord was immediately made safe. 5. In the junior room, Popcorn was observed to be present in a child’s lunch. Popcorn is classified as a high- risk choking food and is not suitable for consumption by young children. 6. On the day of inspection, no evidence of a documented risk assessment was available for a disclosure that had been recorded on an adults Garda Vetting. It is acknowledged the registered provider was aware of the disclosure; however documentary evidence to demonstrate that an evaluation took place and that the individual does not pose a risk to children is required. Infection Control: 7. It was confirmed in conversation with staff that the high-level bin present in the bathroom was used to dispose used nappies in. This practice presented a risk of cross contamination. 8. Worn paint on the back of the sink in the senior room sanitary facilities requires refreshing to ensure effective cleaning of the area can occur. Outing: 9. No risk assessments had been carried out for two recent outings to a local park and a local play centre. As a result, it was not possible for the service to identify and minimise any potential hazards during these outings. It was also not possible to demonstrate any safety procedures that were implemented to ensure safety of the children whilst located off the premises. Action submitted by the Registered Provider

- General Safety: The registered provider has stated. 1. A new lock was fitted to the kitchen door, and evidence submitted to the inspector on the 24 November 2025. 2. A new thermostat has been added to the service. A service is conducted each year by the plumber and regular temperature checks are conducted. 3. A barrier was available on the day of inspection to ensure children could not access the unused care room; however this room will now be fully locked at all times by staff members. Staff were updated during the staff meeting to ensure the door is always locked and unavailable to all children. 4. Blinds cords have been reattached. All blind cords have been checked. 5. A text message has been sent to parents to ensure popcorn does not enter the service and the healthy eating policy has been updated. 6. Risk assessment of Garda vetting has been completed. All risk assessments to be carried out where areas are flagged up from staffs garda vetting disclosures. Following receipt of the inspector’s feedback, a staff meeting was held to address the findings in full. All corrective actions identified were implemented immediately. During this meeting, staff were also updated on the newly revised and updated policies, including changes introduced to strengthen preventative measures. The purpose of the meeting was to ensure that all staff clearly understood the issues raised, the actions taken, and their individual and collective responsibility to maintain these standards at all times. Staff were reminded of the requirement to ensure doors are locked and restricted areas remain inaccessible to children at all times, that environmental hazards such as blind cords, bins, walls and surfaces are maintained to a high standard, and that all safety, hygiene and supervision requirements are consistently met. Procedures relating to water temperature checks, annual servicing, healthy eating practices, completion of risk assessments (including for outings and where Garda vetting disclosures arise), and accident and incident prevention were reviewed in detail. Staff were informed that compliance with policies and procedures is an ongoing requirement and that daily checks, documentation, and safe practice must be followed at all times. Management will continue to provide oversight through regular monitoring, review of documentation, and follow- up discussions to ensure preventative measures remain effective and that the risk of reoccurrence is minimised. Infection Control: The registered provider has stated. 7. A new bin was purchased. All bins meet regulation standards. 8. Worn paint area is disinfected on a daily basis and a painter has been arranged to touch it up for after the Christmas Holidays. All walls and surfaces are of high clean standards. Outing: The registered provider has stated. 9. Risk assessments for outings have been completed. All risk assessments are completed ahead of outings

- Infection Control: 1. The bins in use for the Senior Preschool were not pedal-operated and required staff and children to lift the lid each time they used the bin; this is a cross-contamination risk. 2. The paper towels in the Senior preschool sanitary area did not support effective infection control. The paper towels were not stored in a dispenser; therefore, the repeated handling of the paper hand towels increases the risk of cross-contamination. 3. During nappy changing, inappropriate infection control practices were observed, for example; • A staff member in the Toddler Room was observed to use the same apron whilst changing different children’s nappies. The staff member left the nappy changing room with this apron on and went into the care room to collect other children for nappy changing. The staff member did not wear gloves for the three nappy changes observed. • Following nappy changing in the Toddler Room, the children’s hands were not washed, and the children were observed returning to the care room to play. • Following a nappy change observed in the Junior Preschool Room, the child did not wash their hands in the handwash basin available in the nappy changing area for the Junior Preschool children. Whilst their hands were washed, this was in the sanitary facilities that were accessed by walking through the care room. • The nappy bin used in the Toddler room was not pedal operated as per infection control requirements. 4. Ineffective infection control practices were observed in relation to mouthed toys in the Toddler Room. The inspector observed a child playing with two different toy drills and putting them in their mouth. These toys were not removed from play by the staff members. It is acknowledged that when asked, staff were knowledgeable about the need to wash toys and disinfect these toys. Safe Sleep: 5. Safe sleep practices were not always implemented in the Toddler room in line with Tusla’s safe sleep guidelines for children under 24 months. • Three children aged under 24 months were placed to sleep on a stackable bed. The stackable bed was not suitable for a child aged under 24 months, in line with Tusla’s Early Learning and Care sleep provision guidance for children under 24 months, which requires a floor bed to be fitted with a mattress of at least 6cm in thickness. • A sleep plan was not in place for three children under 24 months who were not sleeping in cots. This is at variance to the guidelines that state a child can sleep in an approved alternative to a cot provided it is agreed with parents, and a sleep plan is established and signed. 6. One of the four mattresses available in the Baby cot room did not contain the required recognised safety standard label. Action submitted by the Registered Provider

- Infection Control: 1. Bins were replaced that afternoon, as we had some in the attic. This non-compliance was amended the very next day following the inspection. Our bins are going to be inspected monthly to make sure they are in good condition, and the lid is still lifting properly. This is added to our monthly cleaning schedule in the rooms, and if bins are broken before the month, they will be replaced. 2. We have put in the z-fold paper towels for the children to dry their hands and won't be using the blue roll for this in future. This non-compliance was amended the very next day following the inspection. These checks are also added to the cleaning schedule, and whoever is on cleaning that day is responsible for making sure the paper towel holder is stocked up. 3. We had a separate meeting with the toddler room staff regarding the nappy changing and use of aprons and gloves. Since the meeting, both registered providers have been monitoring the staff and observing them at nappy changing times, making sure aprons and gloves are being changed, and children's hands are being washed. The bin is changed in the toddler nappy area to a pedal bin. We have added an apron dispenser to our changing area, and the room leader is responsible for making sure each staff member changes their apron and gloves and that children's hands are washed before they go back into the room. Following our meeting with staff, this seems to be working effectively, and management is carrying out visits to the room at nappy changing times to make sure the staff are carrying out these procedures. A reminder poster had also been put on the wall. The water in the changing room has had thermostatic controls fixed by a plumber. 4. During the meeting, we discussed with the staff of the toddler room about the mouthed toys and being more vigilant when children put toys into their mouths. Staff in the baby and toddler rooms are reminded of mouthed toys and are making sure they are keeping an eye on toys being put into kids mouths. Each room has added a new box with a sign on it saying mouthed toys, and toys are placed into the box and washed when the staff have time to do so. Safe Sleep: 5. The new beds were ordered the next day. A parent's letter regarding the children's sleep is now in place, where we have issued it to the parents to give them the choice of where their child can sleep. We will ensure we are up to date with all sleep policies regarding safe sleep for children. We will have monthly reviews with our toddler room staff regarding children transitioning from cots to beds. Stackable beds are no longer used in the Toddler room for children aged under 2 years. Our beds for under 2’s will have a mattress of a thickness of between 6 and 15 cms and meet EU safety standards. We are going to add our sleep plan to our registration forms for all children under 2 years old. These forms will be reviewed with parents every few months in case there are any changes to their child's sleep routine. A copy of our sleep plan will be added to our parents' handbook and also to our policy book. 6. The mattress was replaced straight away as we had spare ones in the attic. Mattresses will be checked daily before use by the room leader and staff within the room. We have placed a daily check for the mattresses on the wall, and this must be ticked off each day before use. Management will also inspect beds monthly to make sure they are safe to be used

The fencing and gate located in the outdoor area, which leads into the car park area located directly off the busy road outside the premises, had a gap between where the fencing and gate ended and the ground, which poses a potential risk of a child passing through and exiting the outdoor play area. Fire Safety: 3. The most recent fire drill recorded in the service was dated 28 August 2025. Through discussion with staff, it was confirmed that no fire drills had been carried out since this date. Fire drills are required to be carried out monthly to familiarise both adults and children with the correct procedures to follow in the event of a fire

- Corrective & Preventive Action General Safety: 1. One staff member's Garda vetting had expired while she was on extended maternity leave, and she had not commenced working in the service without her vetting. The second staff member was the substitute Teacher and had made an application before its expiry and was awaiting her certificate. Since has received the same. We will ensure that all vetting due to expire is applied for a number of months in advance to ensure it is processed in time. 2. A small gap was identified and established that although the child could not exit the perimeter, they may get stuck in an attempt to do so. Therefore, the identified risk was corrected by sealing the gap completely that evening. We will ensure a thorough risk assessment is carried out of the outdoor area and consider all risks carefully. Fire Safety: 3. We understand the importance of carrying out a Fire Drill monthly; this was overlooked. Two Fire Drills have been completed since our Inspection. We will ensure that a fire drill is completed monthly in future

On the day of inspection indoor and outdoor risk assessments were not available. The absence of documented risk assessments prevented verification that any risk or hazards that present are identified and managed within a suitable time frame, limiting risk of harm to a child. 3. A door on a cabinet within one of the care rooms was very loose and at risk of falling off the unit. This could lead to harm or injury of child. Infection Control: 4. On the day of inspection, children’s lunch boxes containing perishable food items were observed not to be stored in a fridge but remained in the outdoor area. The outdoor temperature was noted on the day as 14°C. This poses risk of bacterial growth on food items which could lead to infection and illness in children. 5. Items such as paint brushes and children’s wellington boots were observed to be stored in the staff sanitary facilities. Sanitary facilities are considered high-risk zones for cross-contamination due to aerosolization during toilet flushing and should not be used for storage. Action submitted by the Registered Provider

- General Safety: In a written response the registered provider stated: 1. Garda vetting has been reapplied for and is now received and on file. Garda Vetting and Recruitment officer has been appointed to ensure this is kept up to date. 2. A daily safety and risk assessment has been drawn up and a designated staff member completes this checklist daily. The service safety statement lists all potential hazards and risks and is in the policy folder 3. The door on the cabinet has been repaired. Infection Control: 4. On the day of inspection, a wooden framed ‘Pirate Ship’ was unsecured and exposed nails were present posing as a risk of harm to a child. 3. The service policy stated that ‘a formal outdoor risk assessment will be carried out regularly and risk assessments will be kept up to date’. On the day of inspection risk assessments for the outdoor area were not available to the inspector. 4. Low hanging wires were accessible to children from a speaker. This posed as a risk of strangulation to a child. When brought to the attention of staff, the wires were secured. Fire Safety: 5. Fire doors were observed to be held open with a door stop. Fire doors are designed to impede the spread of smoke and flames for up to 30 minutes, if left open fire and smoke can travel freely, accelerating the spread. It is acknowledged the doorstops were removed when brought to the attention of the registered provider. Action submitted by the Registered Provider

- General Safety: 1. All staff vetting are now up to date. Monthly checks on staff files will be carried out on in relation to vetting to ensure they are up to date. 2. The pirate ship was removed and fixed to adhere with regulations before it was put back on the playground. 3. Outdoor risk assessments are to be filled out every time the children go outside. The assessments are very easily accessible as they are now kept in the trolley that goes outside with staff and children. 3. All risk assessments are checked over and signed by the manager at the end of the day. 4. Staff will ensure that the speaker will no longer be plugged into the socket that could cause potential harm. It has been moved to a safer location. Fire Safety: 5. Doorstops were removed on the day of inspection when brought to the attention of staff. They are no longer in use on our premises

- General Safety: 1. A blind cord was observed not to be secured in the Little Scholar’s room. The cord was out of reach of children; however, it could pose a risk should a child climb to reach it. 2. Several areas of the outdoor playground surface had contracted, creating gaps that could present as a tripping hazard. Infection Control: 3. On the day of inspection, after nappy changing, the hands of the babies and staff were observed not to be consistently washed, posing a risk of infection. 4. Mechanical ventilation units in the Little Stars sanitary area were observed to have visible dust accumulation, indicating they require cleaning to prevent potential airborne contamination and maintain appropriate infection control standards. Safe Sleep: 5. The Sleep room temperatures were recorded more than the acceptable ranges of 16 to 20°C for children under the age of 12 months. It is acknowledged when brought to the attention of management, air conditioning units were brought to the area which reduced the room temperature immediately. Fire Safety: 6. It was noted that children were at times difficult to account for during the day due to transitions between rooms, particularly following sleep times. This practice may compromise the accuracy of headcounts and cause delay posing as a potential risk in the event of a fire evacuation, or other emergency. 7. A fire door in the Junior Einsteins room was observed to be held open with a chair. An open fire door allows smoke and flames to travel freely, accelerating the spread of fire. - General Safety: 1. The blind cord, previously positioned out of children’s reach, has now been securely fastened to the wall as an additional safety precaution. Any new blinds moving forward will be fitted with the safety attachments to the wall. 2. Areas of the soft pour surfacing that had lifted have since been securely re-adhered in place. This is part of the playground risk assessment and will be monitored and recorded weekly Infection Control: 3. Measures have been reinforced through the circulation of information on effective handwashing, with emphasis on nappy-changing procedures. This will be revisited during monthly team progression sessions. Preventative action has been noted, and closer monitoring will be implemented going forward. 4. The mechanical ventilation fans were on a weekly cleaning schedule this is now increased to twice per week. The housekeeping team has been informed, and the cleaning schedule will be adjusted and increased as necessary. Safe Sleep: 5. On the day of inspection, the sleep room temperature was noted to be above the acceptable range of 16– 20°C. Two new air conditioning units were purchased and installed immediately. All staff have been reminded of the safe sleep procedures and are actively monitoring room temperatures. Continued monitoring is in place to ensure compliance with safe sleep guidelines. Fire Safety: 6. It was observed during sleep time transitions that temporary room changes were not consistently recorded on sign-in charts. To address this, additional lines have been added to all charts to allow educators to record such movements. Evidence of this revised system was submitted, and its implementation is expected to resolve the issue. 7. On the day of inspection, it was noted that an internal fire door had been temporarily propped open with a chair to allow air circulation, as the room was warm. While the intention was to cool the room, this practice is not in line with fire safety requirements. All employees have since been reminded that fire doors must always remain closed and have been instructed to refrain from using this practice going forward. - General Safety: 1. During routine safety checks in the baby room, the inspector observed that one of the cots had a broken slat on the base, posing a potential risk to any child placed in it. However, during a later observation of the sleep room, the inspector noted that a child had been placed in the same damaged cot. This was brought to the immediate attention of the staff, and the child was promptly removed. The cot was then taken out of use. 2. During the inspection, it was observed that the climbing frame located outside the baby room had rusty nails protruding from the wooden structure. While it is acknowledged that the steps to access the climbing frame had been removed from active use to prevent children from climbing on it, the area remained accessible to children’s hands, posing a potential safety hazard 3. Garda vetting was available for one staff member. However, this vetting disclosure was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. Infection Control: 4. It was observed during the inspection that three cot beds did not have waterproof mattress covers in place. The absence of waterproof covers poses an infection control risk, as it may lead to contamination and difficulty in maintaining a clean and sanitary sleep environment for young children. 5. During three separate nappy changes observed, staff did not consistently follow the service’s nappy changing procedure—children’s hands were not routinely washed, and on one occasion, a disposable apron was not used, posing a potential hygiene risk Safe Sleep: 6. It was noted in the baby sleep room that temperatures fluctuated throughout the day. It must be acknowledged that it was an exceptionally hot day, and fans were available in both sleep rooms. Temperatures were recorded in Baby Sleep Room 1, Baby Sleep Room 2, and the Toddler Room. - General Safety: 1. A new cot was purchased to replace the broken cot. At a team meeting the risk management policy was collectively reviewed. The register provider highlighted that any safety risks (e.g. the broken slat in the cot) should be reported immediately so it can be repaired or replaced. 2. A new swings and slide set has been installed. At a recent team meeting a review of the risk management policy took place. It was also noted that as an extra safety precaution, plastic fencing would be placed around the structure to avoid children being hit by the swings. 3. A new garda vetting application has been submitted. All expiry dates will be clearly marked at the beginning of each year’s diary and aim to reapply at least three months before current vetting expires. This was also highlighted at the recent team meeting. Infection Control: 4. There was an insufficient supply of waterproof mattress sheets. Six new sheets were purchased, and additional ones ordered to ensure availability during laundering. 5. Staff were reminded that children’s hands must be washed after each nappy change and that disposable aprons must be changed after every nappy change to prevent cross-contamination. A review of the nappy changing policy confirmed these procedures Safe Sleep: 6. Extra fans were placed in the rooms to help with bringing down the room temperature. Children were put in bed with minimum clothes and a light cellular blanket. The following day the outside temperatures were not as high, and room temperatures returned to normal. The Register Provider contacted the company that recently supplied and fitted the air to water heating system-Northern Refrigeration Ltd. They are coming to service the system and advise how to control the core temperature of the building from the boiler room. This will allow the temperature gauges in each room to be individually controlled to avoid the temperatures exceeding recommended levels in the future

An entrance door to the service was found to be unsecured on one occasion during the inspection. It is acknowledged that a staff immediately secured the door when it was brought to their attention. On a separate occasion, the side gate was propped open by a parent during collection time. It is again acknowledged that, when this was noticed by staff it was immediately secured. 3. The water temperature at two of the children’s wash hand basins were found to be over the maximum safe limit of 43°C, one in the baby toddler nappy change area and a second in the Sr ECCE room. 4. There was a large waste bin located in the outdoor play area. Infection Control: 5. Children’s hands were not routinely washed in the baby room after nappy changing posing a risk of cross infection. 6. There was no waterproof covering on the cot mattresses in the sleep room. Safe Sleep: 7. Staff did not maintain a record of room temperature as part of their routine sleep checks. Staff informed the inspector that they check the room temperature routinely and increase or reduce the temperature accordingly, however there is no written record of this occurring. 8. Contrary to safe sleep guidance which states ‘mattresses should not have any raised or cushioned areas’ a cushion had also been placed between the cot base and the mattress of two cots. Action submitted by the Registered Provider

- General Safety: 1. Two of the vetting’s have come back but there are still two outstanding. Management will make sure all vetting is up to date at all times. 2. Management have sent correspondence to all parents regarding the importance of keeping the gate closed at all times. A member of staff will stand by the gate in the morning at that very busy period between 8:45 and nine. A new door releaser has been fitted to ensure the gate closes after exit and a new magnet also. New signage has been ordered. Ensure exits are secure at all times and regular checks to make sure in good working order. 3. Contacted plumbers, they have put a thermostat on the two taps. Taps will be regularly checked. 4. That bin was recycling bin and had been taken from behind the gate to be removed outside that afternoon. In future leave outside gate until being put out the front for collection. Management spoke to the staff about only removing from outside the gate at the back when going to place outside the front. Infection Control: 5. Management had a meeting with the staff in this area and stressed the importance of always washing hands when they in the nappy changing room. Staff will ensure hands are washed at all times. 6. All cots now have waterproof covers and extra ones have been bought. Safe Sleep: 7. Management devised a new sleep room temperature checklist, which will be checked four times daily. A standalone thermometer was purchased for the room. The sleep room temperature checklist will be visible in the sleep room and monitored at all times

- General Safety: 1. On 29 May 2025, a written Immediate Action Notice was issued to the registered provider under Regulation 23 – Safeguarding Health, Safety and Welfare of the Child. A large open grate on the premises posed a significant risk to a child. When brought to the attention of the person in charge he responded promptly by securing the area and ensuring the grate was covered, made safe and inaccessible to children. 2. The windows in one of the care rooms were not fitted with fire safe window restrictors to prevent a child from potentially exiting or falling. 3. Children were observed seated in highchairs without being secured using the provided harnesses, posing a risk of falling. When questioned, adults appeared unfamiliar with the correct use of the harness system and advised the inspector that the straps were not fully working. 4. In the outdoor play area, a wooden pole with a protruding nail was observed, posing a potential injury risk to children. This issue was brought to the attention of the person in charge for prompt action to ensure the area was made safe. 5. It was observed that one of the outdoor bikes was missing a seat. This limited their usability and affected the range of equipment available for children's outdoor play. 6. On the day of inspection, no indoor or outdoor risk assessments were available for review. The absence of these documents meant that the inspector was unable to verify whether potential hazards within the environment had been identified, assessed, and appropriately managed prior to their arrival. Infection Control: 7. It was observed that there was insufficient space between cots in the sleep room, which poses a risk of cross- contamination and does not support best practices for infection control. Adequate spacing between sleep equipment is essential to ensure a safe and hygienic rest environment for children. 8. No daily cleaning schedules were available for review on the day of inspection. When questioned, adults informed the inspector that these schedules were not being completed. The absence of documented cleaning routines limited the ability to verify that regular and consistent hygiene practices were being carried out across the service. 9. Gloves and aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care practice of placing on a clean nappy and redressing the child. This could lead to a risk of cross infection. Administration of Medication: 10. Each child in the Rainbow Room had an individual bottle of antipyretic (fever-reducing) medication stored on- site. Upon inspection, four of these bottles were found to be expired. While it was acknowledged that the service had an in-date bottle available for emergency use, the presence of expired medication indicates a lapse in the regular monitoring and management of stored medications. Maintenance staff have been asked in future to please complete work in a timely manner as there was a passage of 5 weeks between the concern being raised in the risk assessment and the date of the inspection. 2. Upon review, several of the windows have need of repair. Works have been contracted and are estimated to be completed by 25/07/2025. 3. Highchairs without working straps have been removed. Staff have been reminded of the importance of highchair safety in a meeting following the inspection. 4. Nail was removed following the inspection. Perimeter fencing will be more closely monitored in the Indoor/Outdoor Risk Assessment not just for security but for hazards like this. This has been communicated to maintenance staff and staff completing Indoor/Outdoor Risk Assessments. 5. Outdoor toy in question was removed from use. Remaining outdoor play equipment was observed to be in good working order. 6. A copy of the Indoor/Outdoor Risk Assessments Dating from 06/01/2025 until the day of the inspection was forwarded to the inspector on 11/07/2025. Infection Control: 7. Staff have been made aware of their roles and responsibilities relating to the daily cleaning requirements of the service. Daily Cleaning Checklists are now included in the ‘End of Day’ Checklist to ensure checks and balances are maintained in filling out the Daily Cleaning Checklists. End of Day checklist will continue to be carried out to maintain compliance. 8. Staff have received a refresher meeting on following the appropriate nappy changing procedure. One of the Supervisors/Deputised Persons have been made aware they are responsible for identifying and checking nappy changing procedures are being followed. 9. Cots have been moved farther apart to support safe sleep practices. Refresher meeting was held in relation to safe sleep practices. Administration of Medication: 10. - General Safety: 1. Garda vetting was available for eight staff members. However, these vetting disclosures were not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI- RN12.3 Renewal of Garda Vetting’. 2. The main entrance door to the service was found unsecured on the inspector’s arrival to the service and three rear doors were unsecured at various times throughout the day. This posed a risk that unauthorised persons may gain entry to the service, or a child may exit the service unsupervised. An immediate action notice was issued to the registered provider on the day of inspection. 3. There was a trampoline in the sensory room off the Bright Stars room. Staff were not aware of the use of a trampoline being prescribed for an individual child. Trampolines are prohibited from being used on site by pre-school children unless prescribed as a therapeutic intervention, in which case they should only be used for that purpose. 4. There was a kettle on a worktop in the Bright Stars room posing a safety risk to children. 5. A sharp edge was visible on a radiator cover in the Twinkle Stars room. 6. The fridge in the Twinkle Star room was not secure. The lock needed to be replaced. Infection Control: 7. The nappy change mat in the Bright Stars room was in need of cleaning to prevent cross infection. 8. Children from the Twinkle Star room hands were not immediately washed after their nappy was changed. Children were observed to play on the floor of the sanitary area with toys until all children were changed. A staff member was observed not to wash their hands in between each change. A staff member was observed to open a foot pedalled bin with their hand during the nappy changing procedure. These instances could result in the risk of cross contamination. It is acknowledged that hands of staff and children were washed prior to returning to their group. 9. The door seal on the fridge in the Bright Stars room was visibly dirty and required a deep clean. 10. The foot pedal bin in the Twinkle Stars room was broken and needed to be replaced to prevent cross infection. 11. A small sofa in the Twinkle Star room required attention, the material was torn leaving the internal foam exposed. The area could not be effectively cleaned. Safe Sleep: 12. The sleep rooms in the Twinkle Stars and the Shooting Stars room did not have ambient lighting, making it difficult to carry out adequate sleep checks. The lack of ambient light did not align with supporting the child’s circadian rhythm. Action submitted by the Registered Provider

- The registered provider has stated staff are to be extra cautious when it comes to doors into the building. Staff are to be mindful of all areas in room and to risk assess the room through these eyes every day, to help reduce any new risks that may come up with the introduction of new equipment. General Safety: 1. Garda vetting has been re-sent and these new vetting’s are now coming in and being filled. 2. Our doors into main building are secured and are to always remain locked within the building during all times of the day. Also, staff have been instructed that this is to remain the case even during hot weather. 3. Trampoline was in place for one of our AIM children. A risk assessment was in place however we did not have an OT letter. Trampoline was removed. 4. Kettle was removed from room. 5. The radiator cover was repaired, and the corner was safe. 6. Fridge lock was replaced. Infection Control: 7. Nappy changing mat was replaced. 8. Staff in the younger rooms revisited the nappy changing policy and were given a physical demonstration on the correct procedure. 9. The fridge door was cleaned. 10. The pedal bin was replaced. 11. The small sofa was taken out of the room. Safe Sleep: 12. Both rooms were informed of allowing natural light into the rooms while children sleep, and not to have the rooms darkened during sleep

For safety and accountability, it is essential that children are signed in and out of each room as they move, ensuring staff have an accurate and up-to-date list of all children in their care, particularly in the event of an emergency. Infection Control: • Some of the older children were observed having their nappies changed on a changing mat placed directly on the floor, which could pose a risk of cross-contamination and compromise hygiene standards. Action submitted by the Registered Provider

- General Safety: Changes have been made to the attendance records in each room to improve visibility and clarity for staff. The updated format allows for easier tracking of transitions between rooms, making the process more understandable and efficient. Staff training on the importance of accurate attendance recording has been incorporated into regular team meetings, emphasizing its role in ensuring health and safety. Although this procedure is outlined in the staff induction file, it was not consistently followed on the day of the inspection. To address this, the manager and deputy manager will now conduct regular checks throughout the day to ensure proper use and compliance with attendance recording procedures Infection Control: The practice of changing a child on the floor was initially introduced to accommodate a larger child for whom the counter was not suitable. However, I acknowledge that I was unaware this method was also being used by some staff for other children who could have been appropriately changed on the provided counter. To address this, I have included a sketch from a carpenter outlining proposed steps to assist children in accessing the changing counter safely. Unfortunately, due to his current workload, he is unable to complete the project. I have since contacted two additional carpenters, both of whom have committed to arranging a visit to discuss the construction of the steps as soon as their schedules allow. I am aiming to have the steps completed and in use within the next four weeks. Summary Comment Following receipt of the corrective and preventive actions and the evidence submitted for this regulation, the requirement for this regulation has been met. Evidence of the action will be submitted to the Early Years Inspectorate on completion and will be reviewed on next inspection

- General Safety: 1. Garda vetting was available for 1 staff member. However, this vetting disclosure was not dated within the previous three years in adherence to with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. 2. The sand pit in the outdoor area was left uncovered when not in use, posing as a risk of infection should vermin access it. 3. A service trolley stored in the hallway outside of the kitchen door could impede or delay exiting the premises in the case of an emergency. 4. Digital clocks to record room temperatures were found not to be working correctly on the day of inspection. In the Ladybird room at 12:13 one clock was noted to return a recording of 15°C. The room temperature was recorded by the inspector as being 23.8°C. It is noted that immediate action was taken by staff, with windows being opened and sun blinds coming down, once this was brought to their attention. The temperature was subsequently recorded to be 20.8°C within the guidance parameters. There was no means to record the room temperature on the upper floor, Little Footprints room. 5. A fridge in the Little Footprints room was observed to be unlocked. Medication was found to be stored on a lower shelf, accessible to the children. Infection Control: 6. Gloves and aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care activity of placing on a clean nappy, followed with redressing the child. This could lead to a risk of cross infection. Administration of Medication: 7. On review of stored medications, - Two bottles of febrile reducing medications were found to be out of date. - Two bottles of medication were observed to be unlabelled. Fire Safety: 8. A fire extinguisher located in the hallway on the ground floor was found to be blocked with a clothes drying rack. The fire extinguisher was not visible and therefore imposed a risk of delay in the event of a fire breaking out. 9. A fire door in the upstairs room, Little Footprints, was observed to be held open with a stack of chairs. An open fire door allows smoke and flames to travel freely, accelerating the spread of fire. Action submitted by the Registered Provider

- General Safety: 1. Immediate action was taken, and the service provider contacted the garda vetting team and the completed vetting form was available in the following few days. The registered provider has stated all garda vetting forms will be routinely checked over for expiry dates. 2. A meeting took place with our groundskeeper, and we discussed putting in place a daily risk assessment checklist specifically for the sandpit. This includes the ground keeper carrying out a morning and evening check over and raking of the sand pit area, to eliminate risk of infection. The registered provider has stated that a checklist had been formatted and in display in our outdoor display area. 3. Immediate action was taken, and the trolley was removed from this area. The registered provider has stated that this area will remain clear. 4. Immediate action was taken, and an electrician visited the setting, which resulted in the original sensors being removed and new sensors fitted also adding insulating padding around sensor. The electrician sourced temperature monitors and these were placed in all care rooms. The registered provider has stated that the electrician will make routine visits to check monitors are working effectively. 5. Fridge locks were placed on fridges Medication has been placed on top shelf out of reach for children. The registered provider has stated that all fridge locks will be placed on fridges. Infection Control: 6. Immediate action was taken, and staff have reviewed nappy changing practice to include hand washing at the start before placing gloves. Gloves are then removed and binned along with nappy resulting in clean hands to place clean nappy/clothes. Hand washing will again take place at the end of this procedure for each child. The registered provider has stated that all staff have been informed of new nappy change practices. Administration of Medication: 7. Immediate action was taken, and the out-of-date medication was handed back to parents and new medication was replaced. All medication is now checked and labelled. The registered provider has stated that all staff will check medication on a monthly basis to ensure in date medication Fire Safety: 8. Immediate action was taken and the clothes drying rack wad removed from this area. The registered provider has stated that all fire extinguish areas will remain clear and accessible at all times. 9. Immediate action was taken, and staff were informed of the fire risk, chairs were removed from holding the door open and will no longer be placed there. The registered provider has stated that Staff are now aware of this fire risk and chairs will no longer be holding door open

During the time the gate was unsecure staff members placed themselves strategically to ensure children were supervised at all times while playing outdoors. 2. An area in the outdoor space was used to store broken toys and materials was cordoned off with metal barriers. This area was not secure resulting in some children playing hide and seek in the area with broken equipment, sharp edges and exposed screws which could pose a risk of injury to the preschool child. - General Safety: In a written response the registered provider stated: 1. A new lock was purchased on the morning of the inspection, but we have now purchased a smart Biometric lock. On three occasions, when entering the rooms, children were located behind the doors. As a result, the doors came into contact with the children, resulting in a bump to a head or body. 2. The radiator in the toddler room was observed to have a sharp point and was accessible to the children, posing a risk of injury. 3. Several paint tins and wooden building materials had been left accessible in the outdoor play area, posing a risk of harm. 4. A vent in the outer wall of the building was broken, posing a risk of harm and was easily accessible for vermin. 5. The covering on a large swing had torn, leaving the internal foam exposed and in need of repair. 6. Children in the baby room were seated on inappropriate height chairs for their age and stage of development. Children’s feet were not able to touch the ground and therefore could not stabilize themselves. Infection Control: 7. Swing top bins were present throughout the service, posing a risk of cross-contamination. 8. Handwashing practices were observed to be inconsistent among staff and children on the day of inspection. In the Baby room, baby wipes were used in place of warm water and soap; however, baby wipes are not an acceptable method for proper hand hygiene. 9. A roller cloth towel was observed to be in use in the downstairs sanitary area, disposable paper towels should be used. 10. Nappy changing mats in the changing room needed replacing as they were worn and could not be effectively cleaned. 11. Mattresses in the sleep room did not have a wipeable and waterproof cover and could not be effectively cleaned posing a risk of cross contamination. Safe Sleep: 12. Children under the age of 24 months were observed to sleep on fold up foam mats in the Wobbler room and not the required floor bed for children under 24 months. Children over 24 months were observed to sleep on the same mats. The sleep mats did not measure above 6cm in depth. Floor beds for children under 24 months must have a firm, flat and waterproof mattress. Mats for all aged groups must be a minimum depth of 6cm, so that they offer the same level of support and comfort that they would have when sleeping in a cot or a bed

- Corrective & Preventive Action The registered provider has stated that all non-compliances have now been rectified and will not occur in the future. General Safety: 1. The Fire Doors in both the Baby Room & Wobbler Room have now been replaced with fire doors with windows, so that staff can now view through the door to ensure that no child is behind the door when entering. 2. A new cover cap has now been put on the radiator in the toddler room. 3. Paint tins and building materials were removed immediately, works had been ongoing and hence why the materials were there. All is now removed. 4. The vent in the outer walls has now been replaced with a new one. 5. A new large swing is now in place. 6. Chairs will not be used in the Baby room for babies whose feet do not touch the floor. All babies in this room are now kept in Highchairs. Infection Control: 7. New foot pedal bins have now been placed in all rooms. 8. It has been reiterated to all staff that children must be brought to the sink to have hands washed with soap and water. Baby wipes must not be used. 9. The roller cloth towel is now not in use and only disposable paper towels are now used. 10. Both nappy changing mats have now been replaced with new ones. 11. All mattresses in the sleep room now have a waterproof cover and these are cleaned daily. Safe Sleep: 12. New floor mattresses have now been ordered for all children that are 10cm in depth

Cleaning agents were observed to be accessible to children on a low-level shelf. Action was taken by the inspector to remove them from the area and brought to the attention of the adults present on the day. Infection Control: 3. Gloves and aprons were observed not to be removed after disposing of an unclean nappy, and prior to moving onto the next care activity of placing on a clean nappy, followed with redressing the child. This could lead to a risk of cross infection. Action submitted by the Registered Provider

- General Safety: 1. The service provider was informed by the inspector that two of the staff vetting confirmations were out of date. The service provider immediately renewed the vetting and has now renewed the vetting of all current staff. The service provider has set a reminder for the renewal of all staff vetting prior to the expiry date and this should avoid the non-compliance re-occurring. The reminder will trigger the service provider to renew all current vetting including any new staff at the time of renewal. The goal is to avoid the non-compliance re-occurring, so by having a reminder set, the non-compliance should not re-occur. The above corrective actions have been implemented immediately, and evidence of the vetting and reminder are attached. The service provider has now set a reminder to renew vetting before the existing vetting expires in three years. 2. The service provider was informed that some cleaning agents could be accessed by children due to the low-level location they were observed during the inspection. The service provider immediately removed any low-level cleaning products to high shelves to prevent the non-compliance reoccurring. The service provider has attached a photo of the cleaning agents on a higher shelf and informed staff not to store those items in the reach of children. The higher shelf storage area should prevent children being able to access the items. The goal is to avoid the non-compliance re-occurring, so by installing the higher shelf and storing the cleaning agents on this shelf the non-compliance should not re-occur. The above corrective actions have been implemented immediately, and evidence of the new shelf is attached. A new shelf was fitted at high level to ensure the children cannot reach the cleaning agents. Infection Control: 3. The service provider was informed during the inspection that staff were only wearing one pair of gloves during the nappy changing task. The inspector informed the staff and the service provider that two pairs of gloves should be worn at all times and one pair disposed of after each nappy is changed and a new pair put on to change the next nappy. The service provider and the inspector have informed the staff of the new regulation and immediately implemented the same. The new procedure is achievable and has been adopted immediately. The goal is to avoid the non-compliance re-occurring, so by having the inspector speak to the staff and the service provider ensuring the new procedure is followed the non- compliance should not re-occur. The above corrective actions have been implemented immediately

Tarpaulin which had originally been covered with soft play bark, had become uncovered and posed as a trip hazard in various parts throughout the outdoor play area. 5. A large tup of stagnant water was accessible to the children in the outdoor area and posed a risk to children. 6. Low hanging wires were accessible to children from IT equipment, a lamp and a radio. These posed as a risk to children. Infection Control: 7. The sand pit in the outdoor area was left uncovered when not in use, posing as a risk of infection should vermin access it. Infection Control: 2. Nappy changing was observed from 12:19, the following posed a risk for infection control, • Children’s hands were not routinely washed after each nappy change. • Adult’s hands were not routinely washed before or after applying gloves. • Soap was not available in the sink adjacent to the nappy changing unit. • Changing mats were not routinely wiped down between each use. • 2 nappy cream containers were not labelled with children’s names. 3. Changing mats located in the both the portacabin preschool and the main building were torn in parts and in need of replacing, this posed as a risk for infection control as they could not be cleaned effectively. 4. The mechanical ventilation system in the portacabin preschool required routine cleaning. A significant build-up of heavy dust had accumulated, causing the system not to operate efficiently. 5. An activity unit in the baby room was stained and in need of deep cleaning. Safe Sleep: 6. Individual sleep records were available for the children, however data on their breathing pattern during sleep was not recorded. 7. The baby sleep room was dark and did not allow for adequate visual checks to be carried out. Action submitted by the Registered Provider

- General Safety: 1. The two staff members have since had their vetting renewed. With the new legislation, we were in process of vetting all staff again, with only two to go on the day of inspection. A check list will be displayed on vetting which will indicate when vetting needs updated. Infection Control: 1. A copy of the nappy changing policy was given to each member of staff. Children wash their hands after being changed. The tubs of cream belonged to the managers child that wasn’t labelled. Nappy policy given to each staff member and discussed in the staff meeting. A staff member has been assigned to ensure all cream items are labelled and towels and soaps are refilled. 2. The changing mats have been replaced with new ones. Infection Control: 3. Children’s soothers were not managed in a hygienic manner in the service. One child was observed with a soother in their mouth which fell on the floor and was put back in their mouth again. It is acknowledged that staff encouraged the child to remove the soother shortly after this and placed it in their schoolbag for the reminder of the session. 4. Children’s hands were not always washed at appropriate intervals including before eating their afternoon snack and on return indoors from outdoor play. This poses a risk of cross infection. Cleaning equipment including a floor cleaner, brush and pan was stored in the general play area posing an infection control risk to children. There were also no records maintained in the service to demonstrate that routine cleaning took place. 4. Waste was not disposed of in line with infection control guidance. - General Safety: 1. Staff who were not covered have not been working directly with children and relief staff was brought in. Vetting was applied for. Garda vetting will be checked every august before reopening before new term to ensure it is valid. 2. A temporary safety catch has been put in place and a maintenance man is booked to fit new snip when arriving tomorrow. A permanent snip is being put on door and a sign to remind staff to ensure it is in place at all times. Infection Control: 3. While the medication administration records recorded parental consent for administration, there was no evidence that parents had been informed of medication administration on each occasion. It is acknowledged that a phone call is made to parents prior to administration and verbally informed on collection of the child however a written record is required. Safe Sleep: 5. The records of sleep checks carried out in the baby room did not provide sufficient detail on the colour, breathing pattern or position of each individual child. The records reviewed demonstrated that a tick was documented to confirm that both were checked, however the detail on the child’s pallor and breathing is required to be documented. - General Safety: 1. The practice in relation to the application of sunscreen was at variance with the service policy. The policy stated that sunscreen should be provided by the parents and individually labelled for each child. Adults informed inspectors that sunscreen is provided by the service and universally applied to all children. Adults stated that consent had been provided by parents however there was no evidence of this available on the day. Adults also stated that parents were not made aware of the brand and type of sunscreen used by the service. This practice poses a risk that a child may have an allergic reaction to the universal brand provided by the service. 2. The paint on the wooden surround of the wash hand basins in the sanitary accommodation off the senior preschool room was extensively chipped and therefore could not be adequately cleaned. Infection Control: 3. Self-sealing, leak proof, pedal bins were not used in the three nappy changing areas on the ground floor and may present as a risk of cross infection. 4. Pedal bins were broken in the junior and senior preschool rooms which may present as a risk of cross infection. 5. Sterilisers were not available in the baby and wobbler playrooms for the sterilisation of individual children’s soothers. Administration of Medication: 6. In conflict with the administration of medication policy of the service, adults stated that parents are not always contacted for the administration of unprescribed medication prior to each use. This may present as a safety hazard for a preschool child. Safe Sleep: 7. The temperature in the designated sleep room off the wobbler room was recorded at 24.7 °C which may present as a safety hazard for a preschool child. Action submitted by the Registered Provider

- General Safety: 1. The permission sheet was not kept on file once all parents signed. Another sheet was created with the added information (date bought/expiry date/store purchased at/etc.). The policy has since been updated and newer staff have been informed of this. 2. A gloss paint has been applied immediately after the inspection to rectify this. All staff are to inform room leaders and/or manager of resources/items that need attention. Infection Control: 3. Three nappy bins have been purchased to ensure the correct equipment is in all rooms. 4. New pedal bins have been purchased for both rooms. The air conditioning system was ordered several months ago and we were awaiting the tradesmen to install it. This was connected the evening after our inspection. Try and obtain a tradesman to install items to prevent this happening again

The mechanism for opening the gate to one of the outdoor play areas was on the outside which was accessible from the entry point to the building therefore posing a risk of unauthorised entry. Administration of Medication: 2. It is acknowledged that written consent was provided by parents prior to the administration of medication. However, individual records of medication administered to children were not signed by the parents on collection from the service which poses a risk that a parent may unintentionally administer additional doses of medication outside of the maximum safe limits. Safe Sleep: 3. The air temperature in the baby room at the time children slept in the room was recorded at 21.2°C which is outside of the safe range of 16-20°C. 4. One of the two cots located within the baby room was positioned beside a radiator. Cots are required to be positioned away from hazards. 5. There was no evidence available to demonstrate that five of the cot mattresses in the baby sleep room had met the required safety standard. Action submitted by the Registered Provider

- General Safety: 1. The service has purchased a coded lock to secure the gate. This use of this lock will impede any visitors from accessing the playground. The staff will all know the code the same as the keypad entry at main doors. Administration of Medication: 2. The medication form has been edited to include signature of parent to confirm they acknowledge the medication was given, volume and time. This will ensure the parent must sign upon collection of the child as well as giving prior permission for the administration of medication at each and every occurrence. Safe Sleep: 3. The heat going to the room has been reduced. An air conditioning unit was purchased which can be used in this space for further heat reduction. Staff have been reminded that the lower sleep temperature of 16 – 20°C applies in this area when children sleep and not the playroom limits of 18-22°C. The use of the air conditioning unit along with ventilation will further ensure the heat will be maintained at 16-20°C. 4. The radiator nearest to the cot has been turned off. Additionally, the cot has been repositioned 50cm away from the radiator and the other cot. 5. The mattresses have been replaced with new mattresses that meet the required safety standard
