An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). 18 June 2026 ##### [R.O.S Early Years Ltd](/creche/r-o-s-early-years-ltd-arklow.md), Arklow · Regulation 16 — Record in relation to pre-school service - On the day of inspection, the staff roster did not reflect basic requirements as follows. • There were no surnames of the adults working in the service. • No identification of the roles of each staff member. • No identification of how breaks and lunches were to be covered • No identification of who was trained in First Aid Response - We have implemented a new roster to communicate the following, staff members full name, role in their room, the manager in charge and also to indicate who has FAR and first aid training. Three records did not have the required parental signatures and 12 did not have the date of when parental signatures were obtained - We have got all unsigned accident/incident reports signed by the parents and backdated any accident/incident reports with no dates on them. There was no record of the name, position, qualifications and experience of one other adult who was working directly with the children on the day of inspection - Records were not open to inspection as detailed under (1)(a) above - (1)(a)(3) All Certificates/References for all employees have been obtained and placed on file. All employee information/qualifications shall be on file going forward 29 January 2026 ##### [Enniskerry Montessori and Afterschool](/creche/enniskerry-montessori-and-afterschool-enniskerry.md), Enniskerry · Regulation 16 — Record in relation to pre-school service - (k) Eleven records were reviewed and while it is acknowledged that seven records had been completed as required by the regulation, the following information was missing on four of the records: • There was no evidence of a parent signature on four records, confirming that parents had not been informed of any such accident or incident. • Three records available for inspection had not been shared with parents as all triplicate pages relating to each of these incidents remained on file - All four unsigned accident and incident forms have been issued and signed by parents. The registered provider confirms that all accident and incident files are now fully up to date on the premises and parents have a copy. To prevent this non-compliance from reoccurring in the future, the registered provider has confirmed that the service has now implemented the following preventative measures to ensure all accident and incident forms are discussed, signed and a copy provided to parents along with compliant recording of same filed on premises. Management have discussed the importance of the requirement with all staff. The manager has implemented a new procedure for checking the accident and incident book before daily collections to ensure that any required actions are taken as appropriate. In the future, all accident and incident forms will be discussed, signed and a copy provided on the day to relevant parents and guardians 20 November 2025 ##### [Little Bunnies Montessori](/creche/little-bunnies-montessori-bray-3.md), Bray · Regulation 16 — Record in relation to pre-school service - (j) Two administration of medication records were available for review in a hardcopy book. One record had not been completed appropriately as there was no parent signature on the record. (k) A sample of five accident and incident records were available for review in a hardcopy book. While it is acknowledged that four of these records were completed appropriately, one record did not include the child’s full name, was not signed by the manager or parent and there was no evidence that this had been shared with the parent - To correct this action, the registered provider has stated that all records have been completed. To prevent this from re-occurring in the future, the registered provider has stated that a staff meeting was held to remind staff of their responsibilities and the requirements of this regulation 8 October 2025 ##### [Little Bunnies Montessori](/creche/little-bunnies-montessori-bray-2.md), Bray · Regulation 16 — Record in relation to pre-school service - (j) Eight files were reviewed and while it is acknowledged that three records had been completed as required by the regulation, the following information was missing on five of the records: • In four records, there was no parent signature recorded confirming that they had been informed that anti- febrile medication had been administered. • In one record, there was no information recorded in relation to the dose of medication administered - The registered provider has advised that a staff meeting was held and the outcomes of the inspection discussed. One to one support and supervision was engaged in with each member of staff to ensure they were familiar with the process 3 July 2025 ##### [Jolly Tots Creche](/creche/jolly-tots-creche-bray.md), Bray · Regulation 16 — Record in relation to pre-school service - Twelve accident and incident records were reviewed. Three of these records were not signed by parents at the time the accident/incident was recorded which is a risk to the health and safety of the children - Parents/guardians of the three children that the accident and incident records related, to have now signed them. In the future, it has been agreed following a team meeting that the staff rostered to work on the day of the accident and incident will ensure that these forms are signed on the day by the parent/guardian 17 June 2025 ##### [Little Harvard Child Care Ltd](/creche/little-harvard-child-care-ltd-wicklow.md), Wicklow · Regulation 16 — Record in relation to pre-school service - It is acknowledged that an adequate number of adults were working directly with the children during the inspection. However, the written roster which was available did not accurately reflect the staff present in the service. The area manager was recorded as working in the service from 09:00 to 18:00. This area manager was not present in the service on arrival of the inspectors at 9.50. but arrived at 11.20am - Local Management have been instructed to ensure that rosters accurately reflect the staffing at the time. They are now aware that they can also sign these reports.
