It is acknowledged that on the 3 March 2026, the service submitted a copy of employment history for the adult to the Inspectorate - The service advised in writing that: - Corrective and Preventive Action An employment history was obtained for the adult. A checklist has been created for the staff folder, to ensure that all necessary documentation is received and kept on file for each member of staff in the future 7 August 2025 ##### [Bright Beginnings Childcare Centre](/creche/bright-beginnings-childcare-centre-westport.md), Westport · Regulation 16 — Record in relation to pre-school service - (k) Six of 12 accident/incident/injury reports sampled, from April 2025, were missing the following information which may compromise the health and well-being of the child should the relevant persons not be informed of the incident/accident/injury or if the care administered while the child is in the service is not documented: • One record was missing the care of the child • Three records were missing a parent/guardian signature • Two records were missing the manager’s signature - The following statements were received from the registered provider; (k) • The staff member responsible for the record of the care of the child is no longer employed by the service, therefore the record could not be completed. • One of 3 parent signatures is now documented. The remaining 2 children have since left the service and the parental signatures cannot be obtained. • The manager has signed the 2 children’s records for whom the manager’s signature was missing. All staff have been reminded of the importance of filling out the requirements of the children’s records correctly. (1) (i) The staff record of attendance/roster was incorrect for one staff member who was recorded as present in the service at 9:00 hours but did not arrive to the service until 9.30 hours during the inspection - The registered provider advised in a written response that: Corrective and Preventive Action (1) (g) and (3) The service has added the following policies: • fire safety, • outdoor play, • recruitment, • risk management, • staff management policy. The service will regularly review and update their policies as required. (i) There was no written record of daily staff rosters made available for inspection in the service and no evidence provided to show that the staff recorded their attendance in the service daily. These non- compliances were also found on the most recent focused inspection in the service on the 26 April 2023 - The registered provider advised in a written response that: Corrective and Preventive Action (h) A new attendance record book has been purchased for use by staff in the playroom. The use of the attendance record book will ensure this will not reoccur in the future. (i) The new attendance record book includes a staff attendance/roster section which will be completed daily going forward. The registered provider will sign off on the record book weekly 21 February 2023 ##### [Kilroe Pre-School](/creche/kilroe-pre-school-mayo.md), Mayo · Regulation 16 — Record in relation to pre-school service - (i) Details of the staff attendance on a daily basis were not accurately kept with staff attendance including the starting and finishing work not recorded for some days in the service - The registered provider advised in a written response that: Corrective and Preventive Action - (i) The staff attendance records have been revised to clearly indicate date and time of arrival and departure for every day in service. The staff roster will be checked on a daily basis, this will be implemented and monitored by the service provider 30 May 2022 ##### [Tir Na NÓg Preschool](/creche/tir-na-nog-preschool-westport.md), Westport · Regulation 16 — Record in relation to pre-school service - : A significant risk in relation to the safety of the children has been identified in relation to: - There were 17 children present on the day of inspection, however on inspection of the daily records of attendance for children were observed to be incomplete. On review of the roll book at 11:27 hours, 10 children were recorded to be in attendance in the service on the day. The name of one child in attendance in the service on the day was not in the roll book. A second document which was used for signing in at the entrance to the service was incomplete as only 15 children’s names were recorded for attendance on the day - submitted by the Registered Provider CORRECTIVE ACTION The service will ensure that both the sign in book and roll book are completed. PREVENTIVE ACTION Staff members will be reminded at the start of each week of the importance of signing in children as soon as they arrive. A sign will be placed as a reminder beside the door.
