It is acknowledged that permission was present for the 12 children to administer anti febrile medication. • For one child, the record available was not complete and did not have the details for the (e) authorisation for the collection of the child

- (1) (i)Parental / guardian consent has been requested to give appropriate medical treatment of the child in the event of an emergency and will be sought and documented for the children going forward. (e) The service has updated the details for collection for the one child and will make sure going forward all children will have their Authorisation for collection completed on the form

(i) All 10 records reviewed did not have written parental consent for appropriate medical treatment of a child in the event of a medical emergency. This non-compliance was present on the previous inspection on 11 April 2024. The corrective and preventive actions as stated by the registered provider did not prevent recurrence of this non- compliance

- (f) I have reached out to our parents on the online application via the instant message feature. I asked that parents take the time over easter break to log in and update their child’s information regarding health, allergies and emergency contact details. I also called our online application customer care team and explained how the application is not performing great for us in this area, and they will be bringing it to their next feedback meeting. I will continue to check that the child record forms are completed and aim to have this section filled in face to face with parents at my initial registration meeting. (g) I have asked parents to fill in the doctor details section on our child record form over easter break, explain the importance of having this information on file in the case of emergency. I am happy to say to date a majority of Parents did comply over easter break and I will continue to check the remaining forms to have them completed, and keep them all up to date going forward. (h) I got on to our forms section on the online application, and I am creating a new form that can be used to obtain permission from Parents in the event of a medical emergency. This form will be signed digitally by Parents All forms will need to be signed by Parents prior to child’s start date in September. I will make this a mandatory field for parents while filling in their child’s details. (i) I have sent my medical emergency form to parents to be signed, and I am creating a new template form we can use going forward on the online application. I am working with child paths to add the medical emergency form to the forms section within the child record section to be signed by parents going forward

- The service completed the starting date for all the kids that were missing it. To do so, the service checked the starting date on three platforms: online application, attendance record book and pre-registration form to make sure there is no mistake. In future, the service will make sure that dates are mentioned straight away when the service add the name on the attendance register. A checklist was created to make sure all the information is filled in by the parents. The person in charge will go through the files once the parents bring them back and will make sure that all the information is filled. If there is some information missing, they will write the information missing on the checklist and get it corrected asap

This information is essential in the case that either child required emergency medical care

- Corrective Action Management introduced a checklist to make it easier identify any missing files. We have reverted to using the service’s old medication record sheet so that it has all the information required. Preventive Action Staff had retraining on required governance at our monthly meeting

Therefore, the service did not have sufficient documented details available for this child including a record of the person(s) authorised to collect the child from the service, information as to whether the child had allergies or not or written parental consent for the child to avail of medical treatment in the event of an emergency

- The registered provider stated the following corrective actions, and preventive actions have been undertaken: Corrective Action (1)(a) – (i) The registered provider obtained the child’s form from the child’s mother on the evening of the inspection. Preventive Action (1)(a) – (i) The registered provider will ensure to continue to request forms when parents haven’t sent them in , even if the parents are personal friends of the registered provider as was the case in this instance

Preventive Action A copy of the service Infectious Disease Policy will be included in the Parent Pack given to parents prior to children attending our service. This will also include information on immunisation along with it. We will continue to maintain a register of non-immunisations

Parental consent is required in the event of a medical emergency occurring

- Enrolment forms were checked and some forms where send home for completion. (a) The children’s start date has been added to the forms. (e) authorisation collection information for five children have been checked and completed. (f) details of any illness, disability, allergy or special need of the child have been checked and forms have been sent home for completion. (g) for one child, the registered medical practitioner information was completed. (h) record of immunisations will be reviewed with parents, and vaccination books have been sent by email. (i) written parental consent for appropriate medical treatment of the child in the event of an emergency was completed by parents

(f) Two of the records did not contain details of any illness, disability, allergy or special need a child may have. (i) Two of the records did not contain written parental consent for appropriate medical treatment of the child in the event of an emergency

- A ceased date has been added to the service forms so this can be recorded. The records have all been reviewed and now contain all of the necessary information

Parental consent is required in the event of a medical emergency

- Corrective Action The service has updated the Parental Consent form with the immunisation record part and given all parents a printed copy to sign. All Parental Consent forms are now signed and are in the children’s folder. Preventive Action Management will make sure that parents fill out all children's records and parents will not be allowed to join service until all relevant documentation has been fully completed

Evidenced by the following: o One child who began in the service in March 2024 did not have their children’s registration record completed. Part (1)(h) record of immunisations, if any, received by the child had been left blank. o There was no written parental consent part (1)(i) for appropriate medical treatment of the child in the event of an emergency on any of the ten new children’s registration records. It is acknowledged the service had recently moved all new children’s registrations on to a new electronic format system and the registered provider explained to the inspection team how this had been an oversight during the process

- Corrective Action (1)(h) The parent had verbally communicated to the Manager that the child had received all their immunisations however the child received them in a different country and was meant to bring the immunisations book in to preschool to record them. The parent has now completed the immunisation section of the enrolment form. (1)(i) The following morning all new parents who were in attendance completed a paper version of our Medical Consent Section. By Tuesday 10 September 2024 all Medical Consent sections were complete. Preventive Action (1)(h) Manager and Deputy Manager will sign off on enrolment forms to ensure we are not missing any information. (1)(i) The service has created an online Medical Consent section on the new electronic format system which the service will use from September 2025

This list is referred to at the beginning of every child staring in the crèche and now has been adjusted to include a form signed by parents giving permission for appropriate medical treatment of the child in the event of an emergency

The service have reviewed all record forms to make sure the start date is detailed, and a monthly review of hard copy record forms will take place to ensure all information is detailed

- (1) On review of the children’s records there were no written records available (a-i) in relation to four of the of the twenty-one children registered in the service. From discussion with the registered provider two of these record forms had been sent home with children’s parents to be updated with new information, one child had left the service, and one child did not have a registration record as the child was on a second week trial in the service. It is important that the registered provider ensures that a record in writing is kept in respect of each pre-school child attending the service. Corrective Action The service has received the forms in question back from the parents of the four children, that were not available on the premises on the day of the inspection. Preventive Action All children will have up to date Registration forms on the premises at all times. A record in writing for each child attending the service is kept on file and accessible at all times. Supporting documentation submitted No supporting documentation submitted. Summary Comment The registered provider through corrective and preventive action taken has addressed the non-compliance identified under Regulation 15
