On the day an external door leading to two of the care rooms was unsecure which posed a risk of unauthorised access to the service. 2. On the day of inspection, a suction-based anti-choking/airway clearance device was observed in the main hall, positioned directly beside the primary first aid box. These devices are not permitted to be used in preschool services. It is acknowledged that once brought to the attention of the person in charge it was removed. Infection Control: 3. There were two broken foot operated bins in the Little Smarties room, one in the sanitary room and one in the nappy changing room. This could pose a risk of infection control for preschool children - Corrective & Preventive Action General Safety: 1. The keypad on the door was reset and fixed that same evening. Weekly checks on all doors are now completed by our maintenance 2. The device was removed immediately. All First aid items are checked monthly, updates from TUSLA on “non permitted” items will also be checked during these checks to ensure future compliance Infection Control: 3. Bins were removed and replaced with new ones. Educators are reminded to check functionality of all bins during their weekly risk assessments and report any malfunctions to ensure compliance Found compliant: Regulation 9, 11, 15, 16, 19, 25, 31, 32. #### Inspection of 29 May 2025 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** 1. A written immediate action notice under ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1) On the day an external door leading to two of the care rooms was unsecure which posed a risk of unauthorised access to the service. An immediate action notice was issued on the day. 2) There was a trailing wire present in one of the sleep rooms which the preschool child could access. A tree in the stars Room had lights hanging down which the preschool children could access and posed a risk. It is acknowledged that when brought to the attention of staff they were removed immediately. 3) An internal fire door in a sleep room was being obstructed from closing fully due to door wedge placed below. Infection Control: 4) There were 10 mattresses in two of the sleep rooms that did not have water protective covers on them. This posed a risk of cross contamination. 5) A foot operated pedal bin in one of the sanitary areas had no lid, posing an infection control risk. 6) A child size couch in one of the care rooms which was made of cloth material was heavily soiled and was not cleaned effectively. 7) During nappy changing staff did not adhere to the service procedure. Hand washing of young children was inconsistent following the nappy changing process. This posed an infection control risk. Safe Sleep: 8) One of the sleep rooms did not have adequate lighting. The children were not easily visible when performing sleep checks. Sleep areas are required to have ambient lighting in accordance with best practice guidance. 9) On the day of inspection, a child under the age of 2 years was observed to be sleeping on a stackable bed. These beds are not suitable for children under two years. In accordance with best practice guidance a sleep plan is required for children under 2 years of age that do not sleep in cots. On discussion with staff no sleep plan was available for this child. Action submitted by the Registered Provider - General Safety: 1. We have installed keypad entries to all doors. The keypad entries will only allow permitted access even if the doors are unlocked 2. Both leads were removed immediately when notified by inspectors. Educators have all been re-informed of the risk of trailing leads. 3. Wedge has been removed so door can be fully closed. Educators have been reminded of the importance of ensuring fire doors remain closed and unobstructed Infection Control: 4. Mattresses protectors on the day where out for washing and have since been replaced. 50 new mattress protectors have been purchased to ensure a replacement is always available when washing is due. 5. Bin was removed immediately. New pedal bins were purchased on same day to replace any broken bins. 6. Childs sofa was removed and discarded on same day. A new washable sofa has since been purchased to replace the old one. 7. Educators were reminded of the importance of handwashing of all children after handwashing. New signage has been put up in all sanitary areas to remind educators. Safe Sleep: 8. All coverings have been removed from viewing panels on doors to allow more natural lighting in. Educators have been instructed to keep viewing panels clear at all times and turn on lamps during sleep time. 9. Cots have been added to the cottage sleep rooms for children who have not yet reached 2 years old. A sleep plan template has been provided to all rooms with children aged under 2 in the event a parent requesting the child be moved to a bed. All sleep rooms for the 2-year-old rooms have since had additional cots included Found compliant: Regulation 9, 11, 16, 19, 25, 28. #### Inspection of 17 October 2024 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** An immediate action notice was issued on the 16th of October under regulation 23 where sleep room temperatures exceeded the safe sleep temperatures for children under one year. A response to this immediate action notice was received and accepted on the 17th of October. ##### Regulation 9 — Management and recruitment - (2) (a) (b) One written and verified past employer reference or a reference from another source was not available for one adult in the service. (d) International police vetting was not available for one adult who had lived outside the state for a period of six consecutive months or more - One adult did not have the required certification of qualification of a major award in Early childhood care and education at level 5 or equivalent. It is acknowledged that a transcript of results was available on the day - In a written response from the registered provider: (2) (a) (b) A second written reference has since been provided. The onboarding protocol has been updated to ensure all requirements are complete before work placement or employment commencement (d) An international police check has now been applied for. We are now aware that the police vetting on file is not adequate. The employee applied for the incorrect vetting; she has since applied for the international vetting. This check has been highlighted in the onboarding process - Front cover for one adult who, on the inspection day, has only transcripts and has now received her certificate. While waiting for the final certificate, the transcripts will be accompanied by a letter from DCEDIY recognising the level of the course undertaken ##### Regulation 15 — Record of pre-school child - 1. 15(f) Three records did not adequately record details of any illness, disability, allergy or special need of the child. 2. 15(h) 12 records did not adequately record immunisations, if any, received by the child - In a written response from the registered provider: All files have been reviewed, and any gaps or missing information have now been updated. A list was sent to each room of missing information parents have responded with the requested information All forms will be accompanied by a sign-off form for the child's enrolment ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: (1) On the day an external door leading to the Tommy Toddler room and Tiny Tots room was unsecure which poses a risk of unauthorised access to the service. (2) A green mesh netting that was hanging on a pergola had become loose and was trailing resulting in a risk to the safety of children. Once brought to the attention of the person in charge this was removed immediately. (3) An electric fan placed in the Tommy Toddler sleep room was accessible to the young children sleeping in the room posing a safety risk. (4) It was unclear on the day from discussion staff how the outdoor sand pits are cleaned and maintained. Slaibh Beag 2 could not provide assurances that the sand pit is covered when not in use. Upon further discussion with staff in Tír na nÓg 1, it was explained the outdoor sand pit is not covered when not in use. Outdoor sand pits must be covered when not in use. (5) Garda vetting renewal for one adult had not been renewed within the required three year period. Infection Control: (6) One staff member was observed during a nappy change procedure touching the lidded pedal bin with their hand, on three occasions, instead of using the foot pedal for opening the bin, which was fully operational at the time. Safe Sleep: (7) The sleep room adjacent to Tommy toddlers remained in darkness. While it is acknowledged that flashlights were used on occasion check on sleeping children, the dark room did not allow for adequate sleep checks to be completed. (8) On the day the sleep room in ‘Tiny Tots’ exceeded the safe temperatures guidance for children at 21.9°c an immediate action notice was issued, and this was rectified by the installation of a portable cooling system on the 17th of October. Action submitted by the Registered Provider - General Safety: In a written response from the registered provider: 1. The door is now locked, and a bell has been installed. All team members have been informed of the importance of securing all doors. 2. The green mesh was removed on the day of inspection. This is now included in the playground risk assessment. 3. A new slimline fan has been purchased; it is on a high shelf out of the children’s reach. New slimline fans are used, and all old fans have been removed from these areas. 4. Covers are now on all exterior sand pits. Signs were erected to remind all educators to cover exterior sand pits each evening. 5. Vetting for one educator needed to be renewed, which was applied for the inspection. The front cover checklist for all staff folders has been updated. Infection Control: 6. Training was provided on operating a pedal bin. This has been included to the onboarding training. Safe Sleep: 7. A new night light is now in use in the sleep room. All educators have been made aware of the importance of visually monitoring sleeping children. Better light in the room will support visual checks without the use of handheld lights. 8. New digital thermometers for all rooms have been installed to provide more accurate temperature reading.
