#### Inspection of 13 May 2026 — Inspection Report

##### Regulation 16 — Record in relation to pre-school service

- Following a review of 18 Accident and Incident records available in the service. Three records did not have the required parental signatures and 12 did not have the date of when parental signatures were obtained

- We have got all unsigned accident/incident reports signed by the parents and backdated any accident/incident reports with no dates on them. We will address this issue in our next staff meeting which will take place next week and remind staff that it is important than accident/incident reports are signed and dated by parents

##### Regulation 23 — Safeguarding health, safety and welfare of child

- Infection Control: 1. Low level beds were stacked on top of one another with the children’s fitted sheets in place. This meant that the legs of the bed on top were resting on the sheet of the bed below., risking soiling the sheet the child would be sleeping on. This is not adequate for infection control purposes. 2. On the day of inspection, the nappy changing practices were not seen to be effective for infection control purposes. One staff member failed to remove their gloves until after a nappy change was completed and failed to complete cleaning the nappy changing area before engaging in another task in the care room. A second staff member did not wash their hands before or after the nappy change, and did not wash the hands of the child before returning them to their care room. The Nappy Changing Procedure was posted in a conspicuous place in each sanitary area and the Nappy Changing Procedure was robust to ensure good infection control practices. Both non-compliances were noted on the last inspection and the CAPA response failed to prevent the non- compliances from re-occurring. Action submitted by the Registered Provider

- Infection Control: 1.Beds are stacked as normal but no longer have fitted sheets in place. Each bed has the sheet and blanket resting on top of the bed. The room will only be prepared when it is empty and beds can be laid flat on the ground. At this point fitted sheets will be attached and blankets laid on top of each individual bed. .A meeting was held and all staff were informed of this new practice. Each staff member signed the memo. Staff will be reminded of this practice at our monthly staff meetings. 2.Nappy changing policy was printed off and placed above the nappy changing station to remind staff of the procedures. Staff attended a meeting which addressed this situation, and each staff member signed the memo. Gloves and aprons and all cleaning products are always available for use. A meeting was held to remind staff of the nappy changing policy and the importance of hygiene throughout the process. Staff will be reminded of this practice at our monthly staff meetings

#### Inspection of 25 June 2024 — Inspection Report

**Immediate action notice.** An Immediate Action Noticed for Garda Vetting was issued to the service on the 27th June 2024 and the manager responded on the same day. The response was accepted.

##### Regulation 9 — Management and recruitment

- The required Garda Vetting disclosure was not available for one adult employed in the service. An Immediate Action Notice was issued on the 27th of June and the manager responded to the notice on the same day and the response was accepted
- The required Garda Vetting referred to in paragraph (2) was not carried out prior to the adult being appointed to the pre-school service

- The staff member was immediately removed from the service and did not return until vetting was in place. We will ensure Garda Vetting is in place for all staff members. This will ensure no staff member will have access to working here without all correct documentation in place
- We have since received vetting for this staff member and she was allowed back on the premises. No start date will be given to new staff members until vetting has been received back

On further examination, the Nappy Changing Procedure of the was not robust enough to ensure good infection control practices and needs updating. Action submitted by the Registered Provider

- Infection Control: • All staff were spoken to immediately following the inspection. It was explained the reason why we will not be carrying out this practice again. One staff member will get the beds ready 10 minutes before sleep time and place them directly on the floor where they are going. To make sure that all staff members are aware of this new practice, it will be discussed at the next staff meeting. It will also be explained to any new staff joining the team • We have made changes to our nappy changing policy. This will also be discussed at the next staff meeting. All staff have also been issued with a copy of the new updated policy and they are also on display in all the nappy changing areas
