#### Inspection of 20 April 2026 — Inspection Report

##### Regulation 19 — Health, welfare and development of child

- On the day of inspection children’s learning, development and well-being was not fully supported. 1. It was observed in the Discoveries classroom that a section of the classroom had been partitioned off and was being used for the children for extended periods of the day. The area contained very limited age-appropriate play and learning materials. This did not allow for sufficient opportunities for free play, exploration, creativity or developmental engagement. 2. In the Discoveries 1 room during the transition from play to lunch, children were observed to be waiting for extended periods before being served food. Throughout this time the children became disengaged and displayed unsafe and inappropriate behaviours due to lack of engagement

- 1. The registered provider confirmed that the staff were briefed on the ‘interactions and paedology policy’ and ‘Transitions policy’ and the importance of always keeping access to all the age -appropriate learning materials available. An order was placed containing new age -appropriate recourses and equipment and classroom was re-organised. The ‘transitions policy’, routine and ‘interactions and pedagogy policy’ were discussed at the staff meeting. The m anagement team will conduct regular spot checks on materials available to the children especially during transition times. 2. The registered provider confirmed that the staff were briefed on the ‘transitions policy’, ‘supporting children’s social behaviour, emotional regulation and well-being’ policy. The ‘transitions’, ‘interactions and pedagogy’, ‘supporting children’s social behaviour, emotional regulation and well-being’ policies were discussed at the staff meeting. ##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. Hazardous items were not always kept secure. A toolbox, painting materials and bleach were observed to be present in an unsecured cupboard within one of the children’s bathrooms. Children were observed to access the bathroom unsupervised, therefore posing a risk of harm to a child. It is acknowledged the items were removed immediately once brought to the attention of the person in charge. 2. In the Discoveries 1 nappy changing area cleaning agents were stored at the side of the nappy changing pad, which was accessible to the children despite being risk assessed through the daily risk checklist. Access to cleaning agents pose a safety risk to children. 3. A trip hazard was identified in the outdoor area. A large amount of tarpaulin was observed to be loose on the ground, where the bark needed to be replenished. Infection Control: 4. Foot operated pedal bins were available within the service for safe disposal of waste, however in one of the changing areas, the bin was observed to be full. As a result, the foot pedal could not be used effectively. Staff members were observed using their hands to lift the bin lid. This posed a risk of cross infection. Action submitted by the Registered Provider

- General Safety: 1. The registered provider confirmed that the staff were briefed on the ‘risk management’ policy. Signage was added to the area to remind colleagues to store cleaning equipment appropriately. During the staff meeting colleagues were re-trained on the ‘risk management’ policy. The management team will conduct regular spot checks to ensure compliance. 2. The registered provider confirmed that the staff were briefed on the ‘nappy changing’ policy. Signage was added beside hook in nappy area as a reminder to colleagues. Hooks were installed in nappy changing area for the spray bottles to be stored out of children’s reach. A reminder on the risk management during nappy changes were discussed at the staff meeting. 3. The registered provider confirmed that the staff were a new supply of play grade bark was ordered and replenished. It was confirmed the trip hazard was removed. The management team will keep the maintenance tracker updated and ensure all hazards are communicated and resolved in a timely manner. Infection Control: 4. The registered provider confirmed that the staff were briefed on the ‘infection control’ policy and the importance of the use of foot pedal operated bins and regularly changing the bin bag. The ‘Infection control’ policy was discussed at the staff meeting. The management team did a full check of foot pedal operated bins to ensure they were operating correctly. The management team will conduct regular spot checks of nappy changing bins being maintained to a significant standard

##### Regulation 27 — Supervision

- The inspector observed that following outdoor play within one of the care rooms, all children were brought indoors except for one child who remained in the outdoor area unsupervised. Staff were unaware that the child had been left outside. This was brought to the attention of staff by the inspector, and the child was subsequently brought back into the room. The service policy states a ‘count – move – count’ routine will occur when transitioning. The services policy and procedures were not followed in this instance resulting in the child not being supervised at all times

- The registered provider confirmed that the management have completed a thorough overview of the incident interviewing the colleague in question. It is conformed signage for reminders to complete ‘Count, Move, Count protocol was added to the outdoor areas. It is confirmed the staff member was re-trained in the ‘head count and documentation’ policy and ‘transitions’ policy and a risk assessment was completed

##### Regulation 9 — Management and recruitment

- (4) On review of documentation available one employee employed in the service since the last inspection, did not have documentation available to demonstrate that they held a major award in Early Childhood Care and Education at Level 5 or above on the National Framework of Qualifications. (7)(a) Documentation was not available to demonstrate the following. • A record of the mentoring system in place for all new staff members was not available on inspection. • It is acknowledged that monthly room team meetings were held but records did not include the date and time of the meeting, the staff in attendance and the nature or content of the meetings. • The service had not updated or retrained staff on their Supporting Children’s Social Behaviour Emotional Regulation and Wellbeing policy following an alleged incident that occurred within the service

- (4) In response to the non-compliance the service has stated that the colleague in question is a student who received confirmation from the third level institution that she attends, that completion of 2 years of the award in Early Childhood Care and Education is equivalent to QQI Level 6 on the National Framework of Qualifications. The employee provided transcripts to evidence that they successfully completed two years of the award. The above documents were submitted to the Department of Children, Equality, Disability, Integration and Youth (DCEDIY) requesting a letter of recognition to support their eligibility to work directly with children. The colleague is awaiting the issuance of a Letter of Recognition. Tigers Childcare Recruitment and Vetting policy was updated to provide clarity of the documentation required in such cases. (7)(a) In response the service has stated the following. • All new colleagues are mentored, and this is now documented on the Mentoring Shadow Practice Card. • The template for monthly team meetings were updated to reflect the date time of the meeting, the colleagues in attendance, and the content of the meeting. • All colleagues re-sat online training on Tigers Childcare Supporting Children’s Social Behaviour, Emotional Regulation and Wellbeing policy. In addition, in person training on High-scope 6 step conflict resolution was delivered via in person training to colleagues on 15 May 2025. • The Centre Management are responsible to assign a Mentor to carry out Shadow Practice Training to all new colleagues. Management will monitor and supervise that the shadow practice and team meetings are documented appropriately through regular supervisions with Room Leaders. • Tigers Childcare Accident, Incident and Injury policy was updated to include that, when reviewing a serious accident/incident, that re-training colleagues is carried out as a preventive measure where appropriate

##### Regulation 16 — Record in relation to pre-school service

- 1. On discussion with the registered provider and the designated person in charge it was confirmed that details regarding an alleged incident that occurred in the service was not completed by the staff member involved in the alleged incident. This is at variance with the service policy on accidents and incident. 2. Information relating to the alleged incident were available, however, the date, location and details recorded on the electronic application by the person in charge were not reflective of the events that occurred following the alleged incident

- (k) 1. In response the service has stated that following the inspection the details of the alleged incident was completed by the colleague involved in the alleged incident. A review was carried out on how the alleged incident was followed up and responded to. Tigers Childcare Accident, Incident and Injury policy was updated to further clarify the process and Centre Management were briefed on the process. 2. In response the incident report was amended to reflect the date, location and further details of the events that occurred following the alleged incident. A review was carried out on how the alleged incident was followed up and responded to. ##### Regulation 19 — Health, welfare and development of child

- Basic Needs: 1. Staff in the Senior Active Cubs room were not available to the children at lunch time to support and assist them in the preparation for lunch such as handwashing, handing out delph and cutlery and portioning out of food. This was due to the arrival of parents at 12midday to collect their children who had finished the ECCE sessional service. As a result, the children did not have a relaxed and sociable atmosphere at lunch time to enjoy their meal. Supporting Relationships: 2. A child in the Senior Active Cubs room was not facilitated by staff to complete an art activity that they were engaged in. As a result, the child became upset and failed to engage with further activities and refused to eat their lunch. Children should be provided with the opportunity to complete activities at their own pace and interest

- Basic Needs: 1. In response the service has stated that the Centre Manager briefed and retrained the colleagues in the Senior Active Cubs room on the standard practice in Tigers Childcare around preparation for mealtimes as per the Food, Nutrition and Health Promotion Policy. Delivery of lunch to the Senior Active Cubs room was moved to a later time to facilitate the handover for children going home. Supporting Relationships: 2. To address the non-compliance the Centre Manager briefed and retrained the colleagues in the Senior Active Cubs room on the standard practice on supporting children’s transitions within the daily routine to meet the needs of the children and accommodate children’s extended play. The policy on Interactions and Pedagogy was updated to include Transitions and added to the Shadow Practice Card

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. Documentation was not available to demonstrate that the service had undertaken a review and analysis of the policies and procedures in place following an alleged incident that occurred in the service. This is at variance with the service accident and incident policy. 2. At 11.09am, it was observed by the inspectors that children from the Explorers room while in the outdoor area, had access to the sun creams provided by parents. The children were observed covering their faces and hands with the creams without adult supervision until the inspectors and deputy person in charge requested that staff remove the creams from the children. Access to creams increased the potential risk of ingestion. 3. Children in the Junior Active Cubs room had access to a garden hose which was not secured. This increased the potential risk of strangulation. Action submitted by the Registered Provider

- General Safety: 1. To address the non-compliance the service has stated that the Accident, Incident Policy was reviewed and amended. A review was carried out on how the alleged incident was followed up and responded to. Tigers Childcare Accident, Incident and Injury policy was updated to further clarify the process and Centre Management were briefed on the process 2. In response the Management reminded all colleagues of the Sun Safety Policy, and that sun cream is never to be left in reach of children. A Risk Assessment was completed, and the Sun cream is now stored in a locked box. 3. The hose was removed and area cleared. Management reminded the colleagues that play equipment that could cause harm to be removed from area and to be stored out of reach from children after use. Staff reminded of our tidy as you go policy as per standard safety practices that are displayed in each room

##### Regulation 19 — Health, welfare and development of child

- Basic Needs: 1. Children in the Junior Discoverers room were not encouraged or promoted to remove their soothers during the inspection. As a result, two children were observed to have their soothers in their mouth during the morning from 11.07am to 12.15am while they played in the outdoor play area and in the care room. At 12.15pm the soothers were removed from the children for dinner time. The two children did not show signs of requiring the soother as a source of comfort. Prolonged use of soothers limits the opportunity for language development. Supporting Relationships: 2. Staff members did not support each other in the provision of care to the children to ensure that transitions between mealtime and sleep time in the Discoverers 1 room were smooth and relaxed to ensure the care needs of the children. For example, when the children from the Discoverers 1 room entered the sleep area after lunch their beds, sheets and blankets were not prepared. As a result, this created confusion for the children who lay on their friend’s beds and had to be moved by staff to their own bed and one child had to be brought to the cot room to sleep in a cot Physical and Material Environment 3. The outdoor play environments to the rear of the premises used by the Discoverers, Discoverers 1 and Junior Discoverers rooms were not equipped with materials and toys to fully engage the children and create opportunities for play and experiences. For example, o No supporting equipment present for the play kitchen in the playhouse and for the shape sorting push along toy. o No black paint on one of the blackboards. o Two broken sit on cars o Presence of plastic indoor toys with missing support play parts not suitable for outdoor play. o Toys in the outdoor area were dirty and therefore not appealing to play with

- Basic Needs: 1. In response to the non-compliance the service has stated that colleagues in this room were reminded of our practice to reserve the use of a soother for sleep time or if a child needs it to soothe for a limited time during the day. The importance of actively managing children’s soother time as per our Food, Nutrition and Health Promotion Policy will be addressed at our staff meeting on 15th May 2025. Staff members will use their professional judgement to appropriately balance the children’s need to soothe with the dangers of prolonged use. Supporting Relationships: 2. In response the registered provider has stated that following the inspection a new procedure was agreed among staff members whereby beds and cots that are located away from the playroom are pre-prepared in advance to ensure children’s smooth and relaxed transition from mealtime to sleep. The centre management will monitor the practice and offer support when required. Physical and Material Environment 3. In response to the non-compliances the registered provider has stated that. • An extensive order for additional play equipment was completed to include supporting materials for the play kitchen and playhouse. • The chalk boards were repainted • All unsuitable, broken toys or incomplete toys were removed from the area and the play equipment was cleaned The educators in the room will be required to complete a quality and support practice guide self-evaluation tool on environments regularly so that the environment is reviewed, updated and changing according to the likes, needs and development of the children

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. Staff did not ensure that the outdoor area used by the children in the Explorers and Senior Discoverers room was free of any hazards to reduce the potential risk of injury to a child. As a result, the following was observed. • No visibility strips were present on the door leading into the Explorers garden. • A rubber protective covering on the handlebar of a tricycle was missing and black ground matting was exposed and gaping which increased the potential risk of trips and falls in the Senior Discoverers room. 2. Staff attending to children in the two cots rooms located in the main reception area of the service did not have a means of contacting staff to request assistance while children slept. As a result, both inspectors were approached by staff to get assistance from their service colleagues. Infection Control: 3. Infection control practices were not completed according to the service policy on infection control, this increased the potential risk of cross infection. For example. In the Junior Discoverers room: • When dinners were ready for the children, they were placed directly onto the floor, in front of each child’s low highchair, while staff got organised for dinner time. • At dinner time three children, sitting in low highchairs, had their dinner of spaghetti bolognese spooned directly onto the tray in front of them to eat, the staff member then spoon-fed the children from the table. In the Discoverers room: • Three children were using soothers while having their nappy changed, in the nappy changing at 11.49am, 11.53am and 12.00pm. Their soothers were not sterilised on return to the care room. 4. During sleep time, the cot room of the Junior Discoverers room was observed to be congested with equipment, this is not an appropriate storage area. Safe Sleep: 5. The temperature of the cot rooms located in the main reception of the service were not maintained between the recommended temperature of 18-220C while children slept. For example. • Cot room 1, 24.40C at 1.47pm and 23.10C at 2.59pm. • Cot room 2, 23 0C at 1.40pm, 2.07pm and 3.00pm It is acknowledged that once staff were alerted to the temperature of the cot rooms by the inspectors, action was taken to reduce the temperature of the rooms. 6. While the inspectors recorded temperatures in the two cots rooms above the recommended 18 - 220C, the service thermometer read 210C. Fire Safety: 7. Staff did not ensure that an accurate record was maintained on the electronic application of the children present in the service. For example, a child that was present in the Senior Discoverers room was accidently signed out at 11.04am but was present in the service at 12.13pm. 8. A notice of the procedures to be followed in the event of a fire was conspicuously displayed on the premises and in the care rooms, however, staff were incorrectly requested to ensure their written attendance record sheet, and their register book accompanied them on exit from the service. The service maintains an electronic record of children in attendance and no longer uses a written record. Action submitted by the Registered Provider

- General Safety: 1. In response the service has stated that visibility strips for the door leading into the Explorers room were replaced and the bicycle was removed. Going forward a daily safety checklist for indoors and outdoors was updated to include the above hazards. Staff members will be briefed at our Centre Meeting scheduled for 15th May to ensure Daily Safety Checklists are completed and actioned with any outstanding risks escalated to Management without delay. 2. In response the service has stated that staff members were reminded to use the monitors that are in place in the cot rooms to call if extra support is required. Staff members will be briefed at our Centre Meeting scheduled 15th May to ensure that they use the monitors to call if extra support is required. Infection Control: 3. In response the staff members were reminded that we practice a family-style dining experience at Tigers Childcare where it is expected that staff members sit with children at mealtimes to encourage social chat and good eating habits and that food is served on plates and bowls for children to eat from. Staff members read our Food, Nutrition and Health Promotion policy to familiarise themselves of how to model appropriate etiquette and hygiene at mealtimes. The whole team will be briefed at our Centre Meeting on15 May 2025 to ensure that all mealtime practices as per our Food, Nutrition and Health Promotion policy are implemented at all times. Management will monitor mealtimes going forward. Staff members were also reminded of our Nappy Changing Policy which states that soothers are not permitted in the nappy changing area including before, during, and after the changing process. Management will monitor nappy changing going forward. 4. Following the inspection the equipment was removed from the cot room of the Junior Discoverers room. It was agreed with the staff members that the equipment would either remain available to use by the children in the room or removed to appropriate storage. The whole team will be briefed at our Centre Meeting on 15 May 2025 to ensure that materials and equipment are always stored appropriately. Safe Sleep: 5. In response air-conditioning units were installed in the cot rooms the following day. A staff member is now assigned to monitor the temperature of the rooms throughout the morning and to take appropriate action to ensure that the room temperature always remains within the recommended range when children are sleeping. The whole team will be briefed at our Centre Meeting on 15 May 2025 to ensure that sleep room and playroom temperatures are maintained within the required range. 6. A new digital thermometer was purchased to replace the existing thermometer. Management is responsible for checking existing thermometers periodically and replacing as necessary. Fire Safety: 7. Following the inspection staff members were reminded of our Arrivals and Collection procedures to ensure that children are signed in and out in real time and that ongoing headcounts take place. The Staff members read our Arrivals and Collections policy to familiarise themselves with the requirements of recording children’s attendance in real time and completing regular headcounts. The whole team will be briefed at our Centre Meeting on 15 May 2025 to ensure this procedure is always adhered to. Management will monitor attendance recording going forward. 8. The Fire Procedure notice was updated to reflect the electronic record of children’s attendance

##### Regulation 27 — Supervision

- 1. From 3.02-3.17pm three children aged between 1-2 years of age that slept on low floor beds were not always supervised by staff to ensure their safety. Three staff members were in an adjoining care room and entered the sleep area to complete sleep checks at 10-minute intervals

- 1. In response the service has stated that there is currently a vacancy for an extra staff member to support the room during this transitioning period. Until this vacancy is filled, staff members agreed an interim procedure whereby the last group of children to wake remain in the sleeping area and engage in story-time until the children are ready to transition with an educator to the Senior Discovers room. We are in the process of recruiting an extra support staff member
